Background: The HeartMate XVE left ventricular assist device is a valuable treatment option for patients with end-stage heart failure. During the past several years, the XVE has undergone a series of design enhancements to improve reliability. We compared the reliability of the two most recent design iterations of the XVE pump (stitch modification to the inflow valve assembly and new inflow valve housing redesign) to the earlier VE version., Methods: A retrospective evaluation of device reliability was performed for 268 devices implanted in 245 patients (VE: n = 167 devices, 147 patients, implant dates October 16, 1998, to December 19, 2003; XVE: n = 101 devices, 98 patients, implant dates August 1, 2002, to April 14, 2004)., Results: Median duration of device support for the VE and XVE was 159 days (range, 0 to 1,206 days) and 229 days (range, 0 to 693 days), respectively (p = 0.495). Significantly fewer major device malfunctions occurred within the XVE group as compared with the VE group (6 versus 36, respectively; p = 0.0003). The number of major device malfunctions per patient-year of support for inflow valve dysfunction, bearing wear, and other failures for the VE and XVE were 0.2 versus 0.04 (p = 0.006), 0.16 versus 0.01 (p = 0.005), and 0.06 versus 0.04 (p = 1.000), respectively. The freedom from major device malfunction at 1 year was 76% +/- 6% for the VE and 97% +/- 2% for the XVE device (p < 0.001). The freedom from death as a result of major device malfunction at 1 year was 97% +/- 2% for the VE and 98% +/- 2% for the XVE (p = 0.698)., Conclusions: Design enhancements to the HeartMate XVE have significantly reduced the incidence of major device malfunctions compared with the earlier VE model because of a reduction in failure modes from bearing wear and inlet valve dysfunction. Further follow-up is necessary to establish the long-term durability of the most recent XVE pump version.

Background: Left ventricular assist device (LVAD) failure and malfunction rates are critical gauges for establishing LVADs as a long-term therapy for end-stage heart failure patients. These device performance measures, however, have been inadequately characterized in the bridge-to-transplantation literature., Methods: REMATCH is a randomized trial that compares optimal medical management with LVAD implantation for patients with end-stage heart failure. An independent committee adjudicated patient outcomes. The primary endpoint--survival--was analyzed by intention to treat using the log-rank statistic. Frequency of event occurrence was analyzed by Poisson regression. The time to first event was analyzed by the product limit method. Device performance was disaggregated into confirmed malfunctions and system failures. The latter were events in which patients could not be rescued with backup circulatory support measures., Results: The 1-year survival rate was 52% (95% confidence limit [CL]; 40%-63%) for LVAD patients versus 28% (95% CL; 17%-39%) for medical patients and the 2-year survival rate was 29% (95% CL; 19%-40%) for LVAD patients versus 13% (95% CL; 5%-22%) for medical patients. System failure was 0.13 per patient per year and the confirmed LVAD malfunction rate was 0.90. Freedom from device replacement was 87% at 1 year and 37% at 2 years., Conclusions: Despite the observed rates of device malfunction and replacement, LVAD implantation confers clinically significant improvement with regard to survival as compared with medical management. Device modifications and innovations for infection management exhibit great promise of improving device performance in the near future.

Mechanical circulatory support used as a bridge to chronic transplantation is now a firmly established treatment modality. Trials evaluating circulatory assist for permanent circulatory assist device implications are underway. A population of patients is beginning to emerge who are resuscitated using readily available "acute" devices and need long-term supporting "chronic" device to bridge either cardiac transplantation or permanent device implantation. Careful management of this new acute-to-chronic bridge group will maximize the chances of a successful long-term outcome. The best strategies for management may only unfold by later analysis of larger populations. This article summarizes the current utilization of acute and chronic devices, as well as the small but growing population of acute-to-chronic bridge patients, and suggests what seem today to be sound management principles for them.

Background: Myocardial hypothermia of less than 10 degrees C is an essential component of preservation of donor hearts before implantation. Here we report temperature changes and comparison of methods for maintenance of myocardial hypothermia during implantation., Methods: Twenty patients were prospectively randomized into two equal groups. In one cohort myocardial hypothermia was maintained by the "Stanford method" of continuous lavage of the pericardium and left atrium with refrigerated saline solution. In the other a cooling jacket was used without saline lavage. Temperatures at multiple sites were measured at 30-second intervals from initiation of cardiac suturing until aortic cross-clamp removal. Comparisons were made between groups at each temperature site., Results: The cooling jacket group temperatures were significantly lower at the left ventricular epicardium and endocardium than those of the Stanford method group., Conclusions: During cardiac implantation maintenance of myocardial hypothermia with a cooling jacket resulted in significantly deeper and more consistent hypothermia of the left ventricle than pericardial and left atrial lavage with refrigerated saline solution. Blood loss from aspirated saline lavage and perfusate dilution by the saline solution were eliminated.

A new technique using the Thompson self-retaining retractor system (Thompson Surgical Instruments, Inc, Traverse City, MI) to harvest lesser saphenous veins is presented. This modification, used in 10 patients undergoing redo myocardial revascularization, provided a rapid, comfortable, and convenient method for harvesting lesser saphenous veins.

Manual cardiopulmonary resuscitation is currently the standard treatment for cardiac arrest patients both in and out of the hospital. Accumulated experimental and anecdotal clinical evidence suggests enhanced survival in patients with extreme circulatory decompensation who have been emergently supported with portable cardiopulmonary bypass. Long-term survival is possible even when application is delayed, but early institution of support after cardiac arrest in selected patients offers the best survival advantages. In our hospital this has been achieved by training a team of in-house personnel to emergently prepare, apply, and temporarily manage cardiopulmonary bypass until personnel with greater specialty training arrive. Machinery needed to perform emergency cardiopulmonary bypass is currently available in all hospitals with open heart surgery programs. Simple support is often therapeutic but can also serve as a bridge to definitive diagnostic and other therapeutic procedures. Commercial units are becoming more biocompatible and easier to use, making both wider application and more prolonged support likely in the future.

Even with circulatory arrest during pulmonary thromboendarterectomy, continued back-bleeding obscures the operative field. This necessitates frequent interruptions of dissection to allow aspiration with a second instrument. Therefore, pulmonary thromboendarterectomy dissectors are described that allow simultaneous suction of blood from the operative field. Before the use of these dissectors, circulatory arrest time was 59 +/- 23 minutes in 124 consecutive patients. In the last 10 consecutive patients using these dissectors, the mean circulatory arrest time has been 52 +/- 22 minutes (p = not significant). It is anticipated that with a larger database, the ability to aspirate blood simultaneously during the dissecting process will result in a significant shortening of circulatory arrest time.

The stenotic aortic valve was surgically repaired in 48 adults, 21 women and 27 men, aged 38 to 83 years. Five had congenital aortic stenosis (AS), with a mean aortic valve gradient and area of 58 +/- 23 mm Hg (standard deviation) and 0.54 +/- 0.13 cm2, respectively; 32 had senile AS with a mean aortic valve gradient and area of 43 +/- 20 mm Hg and 0.98 +/- 0.41 cm2; and 11 had rheumatic AS with a mean aortic valve gradient and area of 59 +/- 24 mm Hg and 0.47 +/- 0.15 cm2. Only 6 patients underwent isolated aortic valvoplasty, 11 underwent concomitant mitral valve procedure, and 34 underwent concomitant coronary revascularization. Repair consisted of decalcification in 33 patients and decalcification as well as commissurotomy in 15 patients. There were three hospital deaths, none related to the aortic valve. Only 2 patients (both rheumatic) did not improve clinically. During follow-up (mean, 64 +/- 41 months) aortic valve restenosis developed in 24% (10 patients, 3/5 congenital, 4/11 rheumatic, and 3/32 senile) at a mean of 64 +/- 28 months. Postoperative Doppler echocardiographic assessment of 21 patients with senile AS at 1.1 +/- 2.7 and 18.1 +/- 1.4 months showed significantly lower aortic valve gradient and improved area in comparison with preoperative values. At 36 +/- 2.7 months, aortic valve gradient and area were not significantly different than preoperative values, and at 58.5 +/- 2.6 months aortic valve gradient was 1.41 (p = 0.07) times the preoperative value. At 7 years, actuarial freedom from aortic valve-related symptoms of the patients with senile AS was 87%. We conclude that in select patients aortic valve repair results in excellent relief of AS. Late restenosis is expected and more likely to occur in the valves with congenital and rheumatic disease than in those with senile disease.

A portable cardiopulmonary bypass system that can be rapidly deployed in a nonsurgical setting using nursing staff was used in 38 patients with cardiovascular collapse refractory to ACLS protocol. Percutaneous or cutdown cannulation sites were: femoral vein-femoral artery (n = 18), right internal jugular vein-femoral artery (n = 2), right atrium-ascending aorta (n = 12), or a combination approach (n = 4). Two patients could not be cannulated. Patient diagnoses were pulmonary emboli (n = 3), failed coronary angioplasty (n = 7), myocardial infarction with cardiogenic shock (n = 5), trauma (n = 7), aortic stenosis (n = 2), postcardiotomy deterioration (n = 10), deterioration after cardiac transplantation (n = 2), cardiomyopathy with shock (n = 1), and ruptured ascending aortic dissection (n = 1). Ninety-five percent of patients (36 of 38) were successfully resuscitated to a stable rhythm. Eight diagnostic procedures (coronary angiography, n = 4; pulmonary angiography, n = 3; and aortography, n = 1) were performed while patients were on cardiopulmonary support. Early deaths resulted from massive hemorrhage (n = 8), inability to cannulate (n = 2), and irreversible myocardial injury (n = 10). Sixty-six percent (24 of 36) of patients successfully cannulated underwent conversion to standard cardiopulmonary bypass with attendant operative procedure or placement of ventricular assist device or total artificial heart. Fifty percent (18 of 36) of patients cannulated were successfully weaned from cardiopulmonary support, and 17% (6/36) are long-term survivors.(ABSTRACT TRUNCATED AT 250 WORDS)

The effects of cardiopulmonary bypass using hemodilution on interstitial fluid pressure were measured using the Scholander wick technique. In 10 mongrel dogs, interstitial fluid pressure was measured in subcutaneous tissue, skeletal muscle, stomach, and left ventricle before and during 2 hours of cardiopulmonary bypass. Changes in interstitial fluid pressure were correlated with plasma colloidal osmotic pressure and duration of bypass. In subcutaneous tissue and skeletal muscle, interstitial fluid pressure increased during bypass; it did not change in the stomach. End-diastolic interstitial fluid pressure in the left ventricle increased significantly. These increases in pressure were presumably due to an increase in interstitial water. The rise in interstitial fluid pressure acts to partially neutralize the fall in plasma colloidal osmotic pressure.

A patient with an acute dissection of the thoracic aorta is described. Intraoperative evoked-response evaluation suggested the presence of a critical intercostal vessel in the excluded segment, but no major vessel could be identified. Expedient grafting and reestablishment of flow led to prompt recovery of the evoked response, suggesting that mechanical distortion of the aorta by the distal cross-clamp led to a compromised flow through a subadjacent intercostal artery that was an important source of cord perfusion. Recognition of this possibility is important because it may prevent time-consuming attempts to implant small vessels in the excluded segment, which are not major contributors to spinal cord blood flow.
