For protection purposes, the biological effects of radiation are separated into stochastic effects (cancer, hereditary effects) presumed to be unicellular in origin, and tissue reactions due to injury in populations of cells. The latter are deterministic effects, renamed ‘tissue reactions’ in the 2007 Recommendations of the International Commission on Radiological Protection because of the increasing evidence of the ability to modify responses after irradiation. Tissue reactions become manifest either early or late after doses above a threshold dose, which is the basis for recommended dose limits for avoiding such effects. Latency time before manifestation is related to cell turnover rates, and tissue proliferative and structural organisation. Threshold doses have been defined for practical purposes at 1% incidence of an effect. In general, threshold doses are lower for longer follow-up times because of the slow progression of injury before manifestation. Radiosensitive individuals in the population may contribute to low threshold doses, and in the future, threshold doses may be increased by the use of various biological response modifiers post irradiation for reducing injury. Threshold doses would be expected to be higher for fractionated or protracted doses, unless doses below the threshold dose only cause single-hit-type events that are not modified by repair/recovery phenomena, or if different mechanisms of injury are involved at low and high doses.

This report provides a review of early and late effects of radiation in normal tissues and organs with respect to radiation protection. It was instigated following a recommendation in Publication 103 (ICRP, 2007), and it provides updated estimates of ‘practical’ threshold doses for tissue injury defined at the level of 1% incidence. Estimates are given for morbidity and mortality endpoints in all organ systems following acute, fractionated, or chronic exposure. The organ systems comprise the haematopoietic, immune, reproductive, circulatory, respiratory, musculoskeletal, endocrine, and nervous systems; the digestive and urinary tracts; the skin; and the eye.Particular attention is paid to circulatory disease and cataracts because of recent evidence of higher incidences of injury than expected after lower doses; hence, threshold doses appear to be lower than previously considered. This is largely because of the increasing incidences with increasing times after exposure. In the context of protection, it is the threshold doses for very long follow-up times that are the most relevant for workers and the public; for example, the atomic bomb survivors with 40–50 years of follow-up. Radiotherapy data generally apply for shorter follow-up times because of competing causes of death in cancer patients, and hence the risks of radiation-induced circulatory disease at those earlier times are lower.A variety of biological response modifiers have been used to help reduce late reactions in many tissues. These include antioxidants, radical scavengers, inhibitors of apoptosis, anti-inflammatory drugs, angiotensin-converting enzyme inhibitors, growth factors, and cytokines. In many cases, these give dose modification factors of 1.1–1.2, and in a few cases 1.5–2, indicating the potential for increasing threshold doses in known exposure cases. In contrast, there are agents that enhance radiation responses, notably other cytotoxic agents such as antimetabolites, alkylating agents, anti-angiogenic drugs, and antibiotics, as well as genetic and comorbidity factors.Most tissues show a sparing effect of dose fractionation, so that total doses for a given endpoint are higher if the dose is fractionated rather than when given as a single dose. However, for reactions manifesting very late after low total doses, particularly for cataracts and circulatory disease, it appears that the rate of dose delivery does not modify the low incidence. This implies that the injury in these cases and at these low dose levels is caused by single-hit irreparable-type events. For these two tissues, a threshold dose of 0.5 Gy is proposed herein for practical purposes, irrespective of the rate of dose delivery, and future studies may elucidate this judgement further.

This report presents detailed information on age- and gender-related differences in the anatomical and physiological characteristics of reference individuals. These reference values provide needed input to prospective dosimetry calculations for radiation protection purposes for both workers and members of the general public. The purpose of this report is to consolidate and unify in one publication, important new information on reference anatomical and physiological values that has become available since Publication 23 was published by the ICRP in 1975. There are two aspects of this work. The first is to revise and extend the information in Publication 23 as appropriate. The second is to provide additional information on individual variation among grossly normal individuals resulting from differences in age, gender, race, or other factors. This publication collects, unifies, and expands the updated ICRP reference values for the purpose of providing a comprehensive and consistent set of age- and gender-specific reference values for anatomical and physiological features of the human body pertinent to radiation dosimetry. The reference values given in this report are based on: (a) anatomical and physiological information not published before by the ICRP; (b) recent ICRP publications containing reference value information; and (c) information in Publication 23 that is still considered valid and appropriate for radiation protection purposes. Moving from the past emphasis on 'Reference Man', the new report presents a series of reference values for both male and female subjects of six different ages: newborn, 1 year, 5 years, 10 years, 15 years, and adult. In selecting reference values, the Commission has used data on Western Europeans and North Americans because these populations have been well studied with respect to antomy, body composition, and physiology. When appropriate, comparisons are made between the chosen reference values and data from several Asian populations. The first section of the report provides summary tables of all the anatomical and physiological parameters given as reference values in this publication. These results give a comprehensive view of reference values for an individual as influenced by age and gender. The second section describes characteristics of dosimetric importance for the embryo and fetus. Information is provided on the development of the total body and the timing of appearance and development of the various organ systems. Reference values are provided on the mass of the total body and selected organs and tissues, as well as a number of physiological parameters. The third section deals with reference values of important anatomical and physiological characteristics of reference individuals from birth to adulthood. This section begins with details on the growth and composition of the total body in males and females. It then describes and quantifies anatomical and physiological characteristics of various organ systems and changes in these characteristics during growth, maturity, and pregnancy. Reference values are specified for characteristics of dosimetric importance. The final section gives a brief summary of the elemental composition of individuals. Focusing on the elements of dosimetric importance, information is presented on the body content of 13 elements: calcium, carbon, chloride, hydrogen, iodine, iron, magnesium, nitrogen, oxygen, potassium, sodium, sulphur, and phosphorus.

In its Publications 56, 67, 69, 71, and 72, ICRP has provided age-specific biokinetic models and, using those models, compiled dose coefficients (doses per unit intake) for intakes of radionuclides by members of the public. Committed effective doses for the inhalation or ingestion of radionuclides by workers who are occupationally exposed were given in Publication 68 based on the most recent dosimetric and biokinetic models for adults. Additionally, a compilation of dose coefficients for both workers and members of the public has been issued on a CD-ROM. The present report complements that series by addressing doses to the embryo/fetus after intakes of radionuclides by a female member of the public or a female worker, before or during pregnancy. Ingestion and inhalation of selected radionuclides of the 31 elements for which age-dependent biokinetic models were provided in the previous reports are considered. These elements are H, C, S, Ca, Fe, Co, Ni, Zn, Se, Sr, Zr, Nb, Mo, Tc, Ru, Ag, Sb, Te, I, Cs, Ba, Ce, Pb, Po, Ra, Th, U, Np, Pu, Am, and Cm. New biokinetic and dosimetric models for calculating doses to the developing embryo and fetus are developed and used in conjunction with the models for infants, children, and adults presented in the previous reports. The models used take account of transfer of radionuclides across the placenta, distribution and retention of radionuclides in fetal tissues, growth of the fetus, and photon irradiation from radionuclides in the placenta and maternal tissues. Human and animal data are used as available in the development of these models. Intake scenarios comprising single or continuous maternal intakes are taken into account in the compilation of effective dose coefficients following ingestion or inhalation of the radionuclides considered. A CD-ROM with more comprehensive information on doses from inhalation of different particle sizes, tissue doses, and doses at various times after birth has been developed concurrently with the report and will be available shortly. The report does not consider doses to the offspring due to intakes of radionuclides in maternal milk and external irradiation from the mother's body after birth. The radiation sensitivity of the offspring is not discussed.
