Melanoma differentiation-associated protein 5 (MDA-5) antibodies have been linked to a distinct clinical phenotype in patients with inflammatory myopathy. The spectrum of MDA-5 associated connective tissue disease is still not well characterized, and it may extend beyond classically defined inflammatory myopathies such as polymyositis and dermatomyositis. We report a case of an 11-year-old boy who presented with inflammatory polyarthritis and persistent fasciitis. He was found to have positive MDA-5 antibodies. The patient did not meet the criteria for diagnosis of inflammatory myositis as he had no evidence of muscle disease and lacked the characteristic skin manifestations seen in juvenile dermatomyositis. This case demonstrates the wide clinical spectrum of MDA-5 associated disease.
