1. Association between incremental gains in the objective response rate and survival improvement in phase III trials of first-line chemotherapy for extensive disease small-cell lung cancer

Abstract

Background The duration of, resources required for and cost of clinical trials could be reduced if a surrogate end point was to be used in place of survival. We assessed the extent to which the objective response rate (ORR) is predictive of mortality, how much difference in the ORR is needed to predict an obvious survival difference and what factors could affect the association between the two parameters during the first-line treatment of extensive disease (ED)-small-cell lung cancer (SCLC). Methods We used the ORRs and median survival times (MSTs) from 48 phase III trials of first-line chemotherapy involving 8779 randomised patients with ED-SCLC in a linear regression analysis. The MST difference was calculated as the difference in MST between the investigational and reference arms; the ORR difference was similarly defined. Results ORR difference between the treatment arms was modestly associated with the MST difference in the overall trials (R2 = 0.3314). In contrast, the relationship was stronger among only trials in which prophylactic cranial irradiation was given to those having an objective response to the initial chemotherapy (R2 = 0.6279). In this trial setting, large differences in ORR were needed to predict a survival advantage (1.2-day survival advantage per 2% increase in ORR). Conclusions In the first-line treatment of ED-SCLC, a favourable relationship was detected between the two parameters in the selected trial setting. Large ORR differences were needed to predict a survival benefit, clearly suggesting the need for new chemotherapeutic agents.

Abstract

Background: This study aimed to investigate the survival outcome of patients with non-small-cell lung cancer (NSCLC) who had obtained disease stabilisation with gefitinib treatment and to clarify the effect of continued treatment with gefitinib on prognosis. Patients and methods: We reviewed the clinical records of 365 Japanese patients with NSCLC who received gefitinib (250 mg/day). Results: Of 324 (89%) patients assessable for response, 147 (45%) obtained disease stabilisation and 71 (22%) patients achieved an objective response. Abstract

Aim: The ROS1 inhibitor crizotinib produces a remarkable response in patients with non-small cell lung cancer (NSCLC) harboring the ROS1 fusion gene (J Clin Oncol 2013 abstr 8032); however, acquired crizotinib resistance is inevitable, but the mechanism is unclear. Methods: HCC78 cells harboring the SLC34A2-ROS1 gene alteration were used to model acquired crizotinib resistance to explore alternative molecular mechanisms. The resistant cell line (HCC78R) was established from HCC78 cells after continuous exposure to crizotinib for 4 months in vitro. Results: The 50% inhibitory concentration (IC50) of crizotinib in HCC78R cells (4085.9 nM) was 47-fold larger than the IC50 in parental HCC78 cells (85.8 nM) (p = 0.0145). The expression of ROS1 protein was decreased significantly in HCC78R cells, as compared to HCC78 cells. Crizotinib inhibited the phosphorylation of AKT or ERK and downstream ROS1 signaling in HCC78 cells, but did not inhibit this in HCC78R. A receptor tyrosine kinas array showed that there was no increased tyrosine kinase activation in HCC78R cells, as compared to HCC78 cells. Interestingly, however, the phosphorylation of EGFR was maintained under 2,000 nM crizotinib exposure in HCC78R cells, but inhibited in parental HCC78 cells. Therefore, we hypothesize that the EGFR signaling pathway plays a role in HCC78R cells. Expectedly, HCC78R cells were very sensitive to EGFR-TKI in a cell proliferation assay (IC50: 117 nM vs. > 10,000 nM for HCC78R vs. HCC78 cells). Consistently, the addition of EGF to the culture medium rendered the parental HCC78 cells resistant to crizotinib. No activation of EGFR mutations were found in HCC78R cells. Finally, foretinib, a multi-kinase inhibitor of ROS1 and EGFR, effectively inhibited the proliferation of both HCC78R and HCC78 cells. Conclusions: These studies suggest that EGFR signaling plays a role in crizotinib resistance in lung cancer cells harboring ROS1 fusion genes. Abstract

Background The prognosis of gastric cancer with peritoneal metastasis is extremely poor. Neither systemic chemotherapy nor surgery alone prolongs survival of patients significantly. Methods Patients diagnosed with advanced gastric cancer underwent staging laparoscopy and received chemotherapy when peritoneal dissemination and/or cancer cells on peritoneal cytology were confirmed. The chemotherapy regimen consisted of S-1, weekly intravenous and intraperitoneal paclitaxel, which was verified in our phase II trial (Ann Oncol 2009). S-1 was administered at 80 mg/m2/day for 14 consecutive days, followed by 7 days rest. Paclitaxel was administered intravenously at 50 mg/m2 and intraperitoneally at 20 mg/m2 on days 1 and 8. Clinical response of chemotherapy was assessed by computed tomography, gastroendoscopy, peritoneal cytology and second-look laparoscopy. Radical gastrectomy was carried out when macroscopic curative resection was made achievable by chemotherapy. Chemotherapy was restarted after operation as soon as possible. Overall survival, relapse free survival, morbidity and mortality of gastrectomy were evaluated. Results Out of 100 patients with peritoneal metastasis who received chemotherapy, 60 patients underwent gastrectomy after response to chemotherapy, including 54 with macroscopic metastasis and 6 with positive peritoneal cytology only. A median of three courses were administered preoperatively (range 1–16). Total or distal gastrectomy with lymphnode dissection was carried out in 54 or 6 patients, respectively. The median survival time was 34.5 months. The median relapse-free survival was 16.7 months. The first site of relapse was the peritoneum in 24 patients and the other organ site in 17 patients. Postoperative complications included anastomotic leakage and pancreatic fistula in two patients each, which were healed conservatively. There were no treatment-related deaths. Conclusions Gastrectomy combined with S-1, intravenous and intraperitoneal paclitaxel is safe and active for gastric cancer patients with peritoneal metastasis.

Abstract

SRPX2 (Sushi repeat-containing protein, X-linked 2) has recently emerged as a multifunctional protein that is involved in seizure disorders, angiogenesis and cellular adhesion. Here, we analyzed this protein biochemically. SRPX2 protein was secreted with a highly post-translational modification. Chondroitinase ABC treatment completely decreased the molecular mass of purified SRPX2 protein to its predicted size, whereas heparitinase, keratanase and hyaluroinidase did not. Secreted SRPX2 protein was also detected using an anti-chondroitin sulfate antibody. These results indicate that SRPX2 is a novel chondroitin sulfate proteoglycan (CSPG). Furthermore, a binding assay revealed that hepatocyte growth factor dose-dependently binds to SRPX2 protein, and a ligand–glycosaminoglycans interaction was speculated to be likely in proteoglycans. Regarding its molecular architecture, SRPX2 has sushi repeat modules similar to four other CSPGs/lecticans; however, the molecular architecture of SRPX2 seems to be quite different from that of the lecticans. Taken together, we found that SRPX2 is a novel CSPG that is overexpressed in gastrointestinal cancer cells. Our findings provide key glycobiological insight into SRPX2 in cancer cells and demonstrate that SRPX2 is a new member of the cancer-related proteoglycan family.

Abstract

Background EGFR-TKI (E) therapy yielded a better PFS than standard cytotoxic chemotherapy (C) therapy and a comparable OS in untreated patients with EGFR-mt tumors, suggesting that each of the treatments is now crucial for such subpopulation. But, it has not been fully evaluated yet which of each should be initiated first, and to what degree both of two are actually administered in early line setting in the treatment course. We here investigated a potential difference in incidence and pattern of delivery of subsequent crossover therapy after failure to each of the treatments in patients with EGFR-mt tumors. Methods Consecutive 79 patients with advanced EGFR-mt NSCLC were retrospectively assessed who underwent E therapy (n = 39) or standard C therapy (n = 40) in the 1st-line setting between 2007 and 2011. Results In the E group, 16 (41%) of 39 patients were still on 1st-line E therapy. Nine (39%) of the remaining 23 could not receive standard C therapy after failure to E therapy due to symptomatic CNS metastasis (mets) in six, skeletal events in two, and patient refusal in one, whilst in C group only one (3%) of 40 failed to receive subsequent E therapy because of relapse (carcinomatous lymphangitis) (chi-square test; P Conclusions It seems that patients with EGFR-mt tumors who were treated with 1st-line C had better opportunity to receive post-progression E therapy than those treated with 1st-line E had chance to receive subsequent C therapy, possibly due to the difference in PS deterioration rate and relapse pattern.

Abstract

Background We previously reported an efficacy and safety of fractionated schedule of P and docetaxel (D) (days 1, 8, 29 and 36, each) and concurrent TRT (DP-TRT) for LA-NSCLC (JCO 2010). Although the median survival time (MST: 26.3 months) was excellent, grade 3 or greater pneumonitis (10%) and esophagitis (14%) were observed and treatment-related death was 3%. Thus, further improvement in the safety as well as efficacy is strongly warranted. S, an oral fluoropyrimidine, is a new active agent possessing a radio-sensitizing effect. Additionally, combining S and P offered an active and safe regimen for metastatic NSCLC. The objective of this study was to assess the efficacy and safety of S plus P with concurrent TRT for LA-NSCLC. Methods: Patients with stage IIIA/IIIB, aged under 76 years and PS 0-1, and without any prior chemotherapy were eligible for this study. Patients were treated with P (40 mg/m2 on day 1, 8, 29 and 36) and S (40 mg/m2/dose b.i.d. on days 1–14 and 29–42) and TRT (60 Gy/30 fr over 6 weeks starting on day 1). Primary end point was response rate (RR), and required sample size was 48 patients. Results Between 2006 and 2009, 48 patients were enrolled (37 men; median age, 66 years; PS 0/1, 36/14; IIIA/IIIB, 23/25; sq/non-sq, 22/26). Partial response was observed in 37 patients (77%; 95% confidence interval: 63–88%). The response rate was higher in older patients (65 years or older) than younger ones (under 66 years) (89% versus 64%, P = 0.041). At a median follow-up of 40 months, median progression-free survival and MST were 9.3 and 31.3 months, respectively. No difference in efficacy (response and survivals) was observed stratified by histology (sq versus non-sq). Toxic effects were generally mild, including G3/4 neutropenia (44%), G3/4 thrombocytopenia (13%), G3 febrile neutropenia (8%) and G3 pneumonitis (4%). No one developed Gr3/4 esophagitis. No toxic deaths occurred. Conclusions This chemoradiotherapy regimen yielded a favorable overall survival data. It was also well-tolerated in patients with LA-NSCLC as compared with concurrent DP-TRT therapy especially in term of TRT-related toxic effects. A phase III trial of this regimen versus DP-TRT is now planning.

Abstract

Background There are no reported data on the management of the central vein access port (CV-port) system when the CV-port system can be flushed, but no blood aspirated, mainly because a small thrombus occludes the catheter tip (catheter tip thrombosis). Moreover, it remains unclear whether catheter tip thrombosis needs to be treated. Methods From April 2008 to January 2012, 199 patients who were treated with outpatient chemotherapy via a CV-port in the Center for Clinical Oncology, Okayama University Hospital, were analyzed retrospectively. Results Of the patients, no blood could be aspirated in 21 (17.6 %) when treated via a CV-port. The median time between CV-port placement and the failure of blood aspiration was 273 (range 7–1641) days. Among the patients with no blood aspiration from their CV-port, thirteen cases (61.9 %) had no complications other than failure of blood aspiration, and anticancer agents have been successfully administered. However, the other eight patients had their CV-port system replaced due to complications, including three catheter ruptures, two thrombotic events around the catheter, two infections, and one occlusion due to catheter bending. Conclusions Approximately 60% of patients might not require any treatment or intervention for the failure of blood aspiration from their CV-port.
