Abstract: In recent decades, anaesthesia and surgery have undergone major scientific and technical developments. However, these improvements have not solved a recurring problem, communication deficiencies within teams in charge of surgical patients. Current figures show that 21% to 65% of accidents and errors in patient management during the perioperative period are related to communication problems. These problems occur when gaps arise in the continuity and coordination of care within teams. Some of the contributing factors to these gaps are emergency status of patients, staff shifts and handovers following patient transfers. To minimize the impact of these phenomena, it is important to improve standardization of information flow within operating theatres and to improve teamwork between anaesthetists and surgeons. This can be done through crew resource management training programs or simulation. This should ultimately contribute to minimise medical error and improve the overall quality of care provided to patients in operating theatres and during all the perioperative period. Abstract: The law number 2005-370 of April 22, 2005 concerning the patients’ rights at the end of life imposes from now the refusal of futility of care, a shared decision-making in unconscious patients and the duty of a palliative strategy. Our observation underlines the feasibility to withhold or withdraw life-sustaining treatments in prehospital area, in following the different steps of the legal decision-making process. However, this case also shows the major difficulty to decide, because of a lack of traceability. Finally, this case illustrates the current difficulty to identify or to join the consultant, provided by the law. [Copyright &y& Elsevier]

Abstract: The law number 2005-370 of April 22, 2005 concerning the patients’ rights at the end of life imposes from now the refusal of futility of care, a shared decision-making in unconscious patients and the duty of a palliative strategy. We describe a case of polytrauma, for which the shared decision-making process led to a palliative strategy after initial aggressive life-support treatments. This case underlines the need for a two-step model of decision-making process, which distinguishes between goals of care and withdrawal or withholding of life support. It suggests that implementation of written procedures could improve the quality of management at the end of life and traceability of decisions. [Copyright &y& Elsevier]

Abstract: Unanticipated difficult tracheal intubation may be challenging to anaesthesiologists. It is still associated with morbidity or mortality. Previous difficult intubation is vital information to organize appropriate airway management. Unfortunately, previous studies in the literature have shown that there is poor communication of this information. We propose in this article an airway alert form, following several criteria published in the literature. This is, in our opinion, an important step to improve the efficiency of guidelines on airway management and the overall quality of the anaesthesia process. Its efficacy to decrease morbidity related to difficult tracheal intubation remains to be demonstrated. [Copyright &y& Elsevier]

Abstract: The objectives of this review are (1) to explain the goals of a medico-legal appraisement and describe the various jurisdictions able to order it; (2) to analyze the actions to be taken from the medico-legal point of view during and after the accident; (3) to give advise concerning communication with the patient, its family and the colleagues involved in the case; (4) to describe of the role of the medical advisor before (constitution of the file to communicate), during (assistance to the blamed practitioner) and after the expertise (to learn the practical lessons); (5) to recall the importance of the quality of the anaesthetic file, the keystone of the expertise, and the rules governing the responsibility for the doctor.
