Exposure to nickel-containing orthodontic appliances may cause intraor extraoral allergic reactions. Nickel is the most typical antigen implicated in causing allergic contact dermatitis, which is a Type IV delayed hypersensitivity immune response. This report presents an unusual reaction to nickel during the orthodontic treatment of an adult female patient. The patient had no previous history of allergy and had been wearing fixed metal upper appliances while in orthodontic treatment to assist the eruption of her impacted teeth. The adverse hypersensitivity reactions appeared only after the surgical exposure and included severe signs of eczematic and urticarial reactions of the face with redness, irritation, itching, eczema, soreness, fissuring, and desquamation as well as intraoral diffuse red zones. Cleidocranial dysostosis is a rare congenital skeletal disorder, associated with clavicular hypoplasia or aplasia, delayed closure of cranial fontanels, brachycephalic skull, delayed exfoliation of primary dentition, eruption of permanent teeth, and multiple supernumerary and morphologic abnormalities of the maxilla and mandible. The disorder is caused by mutation in the CBFA1 gene, on the short arm of chromosome 6p21. The prevalence of cleidocranial dysostosis is estimated one per million, without sex or ethnic group predilection. The purpose of this paper is to describe the orthodontic treatment in a patient with cleidocranial dysostosis. Objective: To test the null hypothesis that American blacks do not have a higher frequency of extra permanent teeth than whites. Materials and Methods: Panoramic radiographs of adolescent orthodontic patients, either American whites (n = 1100) or American blacks (n = 600), were reviewed systematically. Results: The frequencies of supernumerary incisors, premolars, and molars were each significantly more common in blacks. While incisors are the most common extra teeth in whites (and extra molars are least common), just the opposite ranking occurs in blacks. Overall, the odds ratio was 8.8 (95% confidence limits = 3.9, 20.0), confirming that American blacks are significantly more likely (almost 9 times more likely) to possess extra permanent teeth than American whites. Conclusions: The hypothesis is rejected. Both the frequencies and the patterns of extra permanent teeth are significantly different in blacks and whites, suggesting different frequencies of the relevant (but unidentified) factors governing the developmental mechanisms that result in hyperdontia. The occurrence of multiple supernumerary teeth is a rare phenomenon and is often found in association with syndromes such as cleidocranial dysplasia, Gardners syndrome, or cleft lip and palate. Only a few examples of nonsyndromal multiple supernumerary teeth have been reported in the literature. There is a predilection for the mandible and in the premolar area followed by the molar and anterior regions. This article reports the presence of nonsyndromal multiple supernumerary teeth involving both jaws in children and in their mothers. This study describes the use of personal computer (PC)-based three-dimensional computed tomographic (3D CT) images in the evaluation of supernumerary and ectopically impacted teeth. Three selected cases were presented as examples of the more complex cases in which 3D CT imaging added information not readily available from periapical, occlusal, or panoramic radiographs. Patients were CT scanned from the occlusal plane to the periapical region of the impacted teeth. Digital Image and Communications in Medicine CT data were transferred to a personal laptop computer using a network line. 3D volume rendering was performed using PC-based volumetric analysis software. 3D CT-reformatted imaging of the teeth is a useful way to investigate and localize supernumerary or impacted teeth. Newer software that enables this investigation using a PC provides a relatively inexpensive way to carry out such investigations, making it easier for dental practitioners to request such investigations and to view the results in real time in their own offices. Transmigrated teeth are rare, though reports have increased with the advent of panoramic radiography. Ectopic eruption of canines and other dental anomalies as retained deciduous teeth and supernumerary teeth have a genetic etiology and may be linked. A case report is presented which exhibits transmigration of both a mandibular canine and lateral incisor as well as the presence of a supplemental premolar and retained deciduous molar roots.
