The article reexamines the differences between hydroxyethyl starches (HES) generations, with importance on the improved safety profile of the third generation products. It asserts that the improvements negate the assumption that the efficacy of HES solutions is associated with plasma concentration and are achieved with no loss of efficacy. Furthermore, it highlights the impact of source material on structure and pharmacokinetics and critically evaluates the role of the carrier solution. It concludes that the third generation of tetrastarches demonstrated an improved safety profile, unlike the previous generations that have effects on renal function and coagulation.
