To assess the clinical importance of hemopericardium after cardiac surgery, serial blood pool scintigrams were performed in 13 random patients throughout the initial hours after coronary artery bypass graft surgery. Scintigraphic measurements of pericardial fluid accumulation and left ventricular ejection fraction were made. Hemodynamics, cardiac, output, and chest tube drainage were monitored; and symptoms of postpericardiotomy syndrome were recorded for a mean of 7.4 months after surgery. Seven of the 13 patients had no scintigraphic evidence of bloody pericardial effusion. Six patients had scintigraphic evidence of bloody pericardial effusion; three of these effusions were small, localized posteriorly, and evident throughout the study. In two other patients large collections of fluid (over 100 ml) developed. In one of these patients increased mediastinal drainage required reoperation. The other patient remained stable although mediastinal drainage decreased. The sixth patient showed a moderate effusion (95 ml) that decreased without evident effusion or drainage when the last image was taken. Two patients (one with evidence of a postoperative bloody effusion), had symptoms of postpericardiotomy syndrome in the follow-up period. This study reports the generally benign occurrence of bloody postoperative mediastinal effusions, the frequent accumulation of substantial amounts of undrained sanguineous fluid, and the lack of connection between the presence and or amount of pericardial blood and the postpericardiotomy syndrome. The importance of these scintigraphic findings can be interpreted only with knowledge of associated mediastinal drainage.

The syndrome of fever and pericarditis is reported following implantation of a transvenous pacemaker in a 72-year-old man. The pacemaker was placed for prophylactic reasons (i.e., presence of bifascicular block). The syndrome resolved spontaneously after over four weeks of fever and a pericardial friction rub. Perforation of the right ventricle, although not recognized in this patient, is a complication which occurs with passage of a transvenous pacemaker. There were no other antecedent events to explain the syndrome such as myocardial infarction or trauma to the chest.

To investigate the incidence and clinical significance of postoperative pericardial effusion (PE), the presence of PE was evaluated by echocardiography, 1 and 2 weeks postoperatively, in 50 patients after insertion of a valve prosthesis and in 100 patients after coronary bypass surgery (50 patients receiving a combination of aspirin and dipyridamole and 50 receiving warfarin). PE was found during either procedure in 77% of patients and was marked in 29%. Symptoms of postpericardiotomy syndrome (p less than 0.05), pericardial friction rub (p less than 0.01), atrial arrhythmias (p less than 0.05), cardiac enlargement (p less than 0.01), and pleural effusion (p less than 0.05) were detected more frequently in patients with PE than in those without PE. PE was not related to the type of antithrombotic therapy, the rate of coronary bypass graft occlusion, or the type of cardiac surgery. However, the use of the left internal mammary artery as a coronary bypass graft was associated with a slightly higher incidence of PE (p less than 0.05). One patient (0.7%) required surgical drainage of PE. It was concluded that PE is a common and benign finding after cardiac surgery and usually disappears without specific therapy.

PPS is a major cause of morbidity after cardiac surgery and may cause bypass graft closure and fatal cardiac tamponade. Little is known about its incidence and cause. To better define this syndrome characterized by postoperative fever, pericardial friction rub, and pericardial pain, we used two out of three of the preceding criteria to diagnose PPS. In a prospective epidemiologic study we followed 944 consecutive patients undergoing open-heart surgery between November 1984 and November 1985. The overall incidence was 17.8%. The incidence was increased in younger patients, in those with a history of prednisone use in the past, in patients with a past history of pericarditis, those with aortic valve replacement, and in patients who received enflurane or halothane anesthesia. PPS is a common syndrome. Knowledge of risk factors associated with PPS may allow its prevention and identification of patients who warrant early and aggressive treatment.

Two cases of penetrating stab wounds of the chest are described, complicated by pyrexia, pericarditis, and a tendency to relapse. The clinical picture is compared with that seen in the postcommissurotomy syndrome. Current views on the pathogenesis of the postcommissurotomy syndrome are discussed. The striking similarity of traumatic pericarditis to the postcommissurotomy syndrome as well as to the syndrome following surgery of nonrheumatic heart disease suggests acceptance of the more generic term “postpericardiotomy syndrome”.

Four cases of constrictive pericarditis that followed blunt trauma are reported. Documentation of a postpericardiotomy syndrome occurring 5 months after trauma in one patient was noted, and the time relationships of trauma, postpericardiotomy syndrome, and constrictive pericarditis are outlined.

Forty-five patients were identified as having constrictive pericarditis after cardiac surgery. The mean patient age was 61 years (range, 40 to 77 years). Twenty-three of 37 patients with adequate clinical information were reported to have had a diagnosis of postpericardiotomy syndrome after the original surgery. The mean interval from original surgery to presentation with constriction was 23.4 months (range, 1 to 204 months). Computerized tomography was helpful in establishing a diagnosis of constriction in 23 of 29 patients (79%). Bypass graft patency was 93% (85 of 91 grafts). Severe pulmonary hypertension (pulmonary artery systolic pressure greater than or equal to 60 mm Hg) was present in nine patients; 8 had coexistent valvular disease (seven cases of mitral valve disease, and aortic valve disease in one). Thirty-seven of the 45 patients underwent pericardial stripping, 28 of whom experienced marked symptomatic improvement. One patient had persistent right heart failure, which was not documented to be secondary to constriction. Four patients had persistent constrictive physiologic conditions. Three of these patients had more extensive pericardial stripping and showed clinical improvement. Four patients (11%) died within 30 days of stripping. Eight patients received medical therapy alone. The decision to treat patients medically was based either on favorable response to medical therapy (five patients), or poor general clinical status.

Postpericardiotomy syndrome has been recognized as a frequent complication following coronary artery bypass graft (CABG) surgery. We observed five cases of postpericardiotomy syndrome resulting in exudative pleural effusions with white blood cell differential counts greater than 80% lymphocytes. Tuberculosis, lymphoma, and other neoplasma have been major diseases associated with lymphocytic exudative pleural effusions. We feel postpericardiotomy syndrome is another important etiology that should be considered in post-CABG patients with lymphocytic pleural effusion.

Attention is drawn to the occurrence of a postcardiotomy syndrome after implantation of a cardiac pacemaker. The complication was observed in 2 out of 11 surviving patients in whom recurrent Adams-Stokes attacks necessitated this operation.
