Objective: The purpose of the present study was to identify the 100 top cited systematic reviews and meta-analyses on dental journals so as to gain insight into the influential publications in dentistry.Material and methods: The Web of Science was used to comprehensively identify the 100 most cited papers without year and language restriction. Specific parameters regarding the title, journal, publication year, authors, country of origin, institution and university, collaborations, keyword analysis and field of study of each manuscript were retrieved.Results: The citations ranged from 642 to 140. The most productive years were 2008 and 2009. The majority of top cited papers were published in Clinical Oral Implants Research and Journal of Clinical Periodontology. The leading countries were United States, followed by Switzerland. The University of Zurich was the most productive institution with 8 articles. Major topics of interest in the top 100 most-cited papers were dental implants and periodontology. The most frequently occurring keywords were systematic review, dental implants and meta-analyses.Conclusions: Systematic reviews published in high impact factor Dental journals focused on implantology and periodontology had the highest citation rates. Objective: A trend towards the state governance of healthcare through quality indicators and national clinical guidelines has been observed, and it is argued that this trend can be a challenge to the autonomy of healthcare professionals. In Sweden, these regulatory tools have been implemented in combination with subsidies for adult dental care that are based on guideline recommendations which serve to ensure that dental care is evidence-based and cost-effective. This paper aims to analyse the implications of these changes regarding dentists' autonomy and whether the government's political intentions can be fulfilled.Material and Methods: The paper is based on documents from government authorities and professional theories.Results: The financial control over Swedish dental care has been strengthened, and it can be argued that this is a step in the right direction from a societal point of view, as public resources are limited. Dentists' professional autonomy with their patients is not affected, which is appropriate, as patients should be treated according to their individual needs and expectations.Conclusions: This article shows that the state's governance does not directly detail dentists' work, which indicates a balance between state governance and dentists' autonomy. Background: Academic detailing (AD) is a defined form of educational outreach that can be deployed to intrinsically motivate practitioners towards improving quality of care. This paper describes the design of the ADVOCATE Field Studies. This proof of concept study aims to evaluate the feasibility, acceptability and usefulness of AD, reinforced with feedback information to promote prevention-oriented, patient-centred and evidence-based oral healthcare delivery by general dental practitioners (GDPs). Methods: Six groups of GDPs will be recruited; two groups of six to eight GDPs in each of three countries - the Netherlands, Germany and Denmark. GDPs will meet for four Academic Detailing Group (ADG) meetings for open discussions using comparative feedback data to stimulate debate about their dental practice performance and care delivery. Group meetings will be moderated using the AD methodology. Qualitative data will be collected through focus group interviews, an online discussion forum, field notes and debriefs of ADG meetings and analysed by conventional content analysis using MaxQDA software. Discussion: The results of the study will provide novel information on the feasibility, perceived acceptability and usefulness of AD and feedback data for GDPs to improve oral healthcare delivery. Objectives. The aims of this literature review are to provide answers to questions on how to improve bonding between titanium and dental porcelain and how to further implement, in clinical practice, ceramic-veneered titanium as an alternative to conventional metal–ceramic systems. Material and methods. A literature search of PubMed and also among referenced published scientific papers was performed and 24 fulfilled the search criteria, namely mentions of titanium, ceramics and bond strength. These papers were compiled for comparison and evaluated regarding the bond strength achieved with different methods. Results. The results strongly indicate that there are possibilities to improve both the present materials and methods for titanium–ceramic veneering. Conclusions. The results indicate that present knowledge is sufficient to conclude that veneering titanium with low-fused porcelain for crowns and fixed partial dentures can be recommended for routine clinical use. The aim of this case-control study was to investigate whether cognitively impaired individuals have a higher burden of calcified carotid artery atheroma (CCAA) than controls without cognitive impairment. The study included 154 cases with Alzheimer's disease (n = 52), mild cognitive impairment (n = 51), or subjective cognitive decline (n = 51) diagnosed at a university memory clinic. Seventy-six cognitively healthy controls were sampled through the Swedish population register. All participants underwent clinical oral and panoramic radiographic examinations. Two oral and maxillofacial radiologists performed blinded analyses of the panoramic radiographs for signs of CCAA, which was registered as absent or present and, if present, unilateral or bilateral. Consensus assessment was used for all statistical analyses. CCAA was common (40%) in this middle-aged and older Swedish population. We found no differences in the prevalence of CCAA between cases and controls (40% vs. 42%). The global COVID-19 pandemic has led to an increase in remote teaching and online assessment in higher education. The examination performance of undergraduate students of dentistry was evaluated to assess any possible association between the altered learning environment and learners' performance. Using administrative data from the University of Helsinki, the examination performance during the years 2018 and 2019 was compared with that of 2020. In 16 out of 22 courses (73%) taught remotely during the pandemic, a statistically significant difference (p <.05) between the awarded grades was observed, as compared to the two previous years. Annual variation in examination grades was common even before the onset of remote teaching, but more so after it. The grade variation during the pandemic was nearly as prevalent towards falling grades (26%) as towards rising grades (36%), implying a multifactorial cause possibly unrelated to remote teaching. Our results imply that online teaching did not systematically improve or worsen the examination performance of undergraduate students of dentistry. Our findings have important policy implications for educational practices in the future. Objective. To compare two methods used to examine the cement interface homogeneity of adhesively luted glass fiber posts (GFPs). Material and methods. GFPs were divided into four groups ( n = 5 in each) and inserted into artificial root canals under standardized conditions: Group I == RelyX Unicem, application with application aid; Group II == RelyX Unicem; Group III == Panavia F 2.0; and Group IV == Variolink II. Posts in Groups II--IV were cemented without using an appliance. All specimens were sectioned at three levels (cervical, middle and apical) perpendicularly to the post's long axis and examined and photographed ( n = 60) using scanning electron microscopy (SEM). Cement interface inhomogeneities were (A) measured by means of SEM software and (B) estimated using a graphics program with SEM images being divided into 72 equal circle segments to calculate a percentage value of inhomogeneities of the 360°° circumference. Results. Median values of inhomogeneities (A/B; %) within the cement interface for the cervical, middle and apical levels of analysis, respectively were 1.4/2.1, 2.2/4.2 and 1.9/2.1 for Group I; 21.0/20.1, 24.8/23.6 and 27.0/24.3 for Group II; 1.5/1.7, 5.5/6.3 and 19.4/20.8 for Group III; and 18.1/16.7, 16.1/15.3 and 27.2/25.7 for Group IV. The two methods correlated very well (0.994), with a value of one indicating a 100% correlation. Conclusion. The aims of this systematic review were to identify the study designs and topics of Swedish orthodontic articles, to elucidate their international position, and to verify in which scientific journals the articles had been published in the past decade. A search of the Medline database for papers published between 1992 and 2002 was made using the Medical Search Heading terms 'orthodontics', 'malocclusion', 'cephalometry', and 'facial bones and growth'. Two independent reviewers selected the articles of Swedish origin and categorized each article according to research design and principal topic. Overall, 15,571 articles in orthodontic research were found, and the Swedish contribution was 1.9% with the majority of these (71.5%) being submitted by universities. Most of the Swedish articles (84.5%) had been published in 10 journals and many high-quality studies with orthodontic interest were published in non-orthodontic journals with higher impact factor scores than the orthodontic journals. Every second study was prospective, and of these, 15 (5.2% of all Swedish articles) were randomized clinical trials (RCTs). It was found that nearly every third study, prospective as well as retrospective, was uncontrolled. The main classification was treatment studies (51.9%), followed by development (18.6%) and diagnostic information (10.7%) studies. Thus, the majority of the articles evaluated therapeutic interventions; however, although the RCT is the preferred study design in evaluation studies, few used this method. Delamination or fracture of composite veneers can occur as a result of improper design of the fiber-reinforced composite (FRC) framework. This in vitro study tested the repair bond strength of restorative composite to aged FRC. The substrate was multiphase polymer matrix FRC (everStick) aged by boiling for 8 h and storing at 37 degrees C in water for 6 weeks. The aged substrate surfaces were wet-ground flat with 1200-grit silicon carbide paper and subjected randomly to 5 different surface treatments: 1) An adhesion primer (Composite Activator) and resin (CA), 2) Silane (EspeSil) and resin (SIL-MP), 3) Silane, adhesive primer, and resin (Clearfil Repair) (CF), 4) Air particle-abrading (CoJet), silane, and resin (CJ-SIL-MP), 5) Resin (Scotchbond Multipurpose Resin) only as control (MP). Restorative composite resin (Z250) was added to the substrate in 2 mm layer increments and light-cured. Subsequently, every surface treatment group was divided into 2 subgroups of 12 specimens each. The specimens were either 48 h water-stored or thermocycled (6000 x 5-55 degrees C). The shear bond strengths of composite resin to FRC were measured at a crosshead speed of 1.0 mm/min. The data were analyzed by ANOVA for factors 'treatment type' and 'storage condition'; Tukey's post-hoc tests and Weibull analysis were performed. ANOVA showed a significant difference as a function of surface treatment (P<0.05) and storage condition (P<0.05). The CJ-SIL-MP group showed highest bond strength and Weibull modulus after thermocycling. The purpose of the present study was to investigate the survival rate of root filled teeth in general dentistry in a Swedish county and to identify risk predictors with a significant influence on the survival rate. This is a retrospective 6-year follow-up study on 1642 recall patients with 1720 teeth root filled in general dentistry in the Public Dental Service in the county of Stockholm, Sweden. Background variables were collected from the database at baseline as potential predictors of tooth loss. The outcome variables were extraction during the 6-year follow-up period and the reason for the extraction. Stepwise Cox regression analysis was adopted in order to investigate the influence of the potential risk predictors on the risk for tooth extraction. Nine percent of the root filled teeth were lost after 6 years. The most frequent reason for tooth loss was fracture and/or cracks (58%). The survival rate of the root filled teeth increased significantly for younger patients, root filled teeth with metal crowns (96%) and high quality of the root filling (93%). The survival rate differed significantly between tooth groups with the lowest survival for molars (83%). Composite fillings were significantly associated with lower quality of the root fillings. Ninety-one percent of the root filled teeth survived after 6 years. The survival rate was significantly higher for teeth with root-fillings of high quality and metal crowns as well as for root filled teeth in younger patients. The aim of this study was to analyse the survival rate of cracked teeth after endodontic treatment. The secondary aim was to compare the survival rate of cracked teeth restored with composite filling/crown and those restored with a full crown. The study was conducted retrospectively from three general dental clinics in Stockholm, which are all part of the national dental service organisation. Two-hundred patients with teeth receiving endodontic treatment due to symptomatic cracks were included. The patient data range from year 2001 to 2016. The mean age of the patients was 48 years (range 29–69). Fifty-five per cent had cracks located above the pulpal cavity, 11% within the pulpal cavity and 3% located in the root canal. The cracks were located most commonly on the proximal surfaces. The survival rate for teeth with cracks was 68% and 54% after 5 and 10 years, respectively. The survival rate was significantly higher (97%) for cracked teeth receiving a full crown after endodontic treatment compared to teeth restored with either a composite filling or composite crown. The overall survival rate for cracked teeth was 68% after 5 years, while it was significantly higher for cracked teeth restored with a full crown. The global pandemic of coronavirus disease-19, caused by the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), is the latest hazard facing healthcare workers (HCW) including dental care workers (DCW). It is clear that the major mode of SARS-CoV-2 transmission is the airborne route, through inhalation of virus-infested aerosols and droplets. Several respiratory protection equipment (RPE), including masks, face shields/visors, and respirators, are available to obviate facial and conjunctival contamination by microbes. However, as their barrier value against microbial inhalation has not been evaluated, we systematically reviewed the data on the effectiveness and efficacy of facemasks and respirators, including protective eyewear, with particular emphasis on dental healthcare. PubMed, MEDLINE, the Cochrane Library, and Embase databases were searched between 01January 1990 and 15 May 2020. Of 310 identified English language records, 21 were included as per eligibility criteria. In clinical terms, wearing layered, face-fitting masks/respirators and protective-eyewear can limit the spread of infection among HCWs. Specifically, combined interventions such as a face mask and a face shield, better resist bioaerosol inhalation than either alone. The prolonged and over-extended use of surgical masks compromise their effectiveness. In general, RPE is effective as a barrier protection against aerosolized microbes in healthcare settings. But their filtration efficacy is compromised by the (i) inhalant particle size, (ii) airflow dynamics, (iii) mask-fit factor, (iv) period of wear, (v) 'wetness' of the masks, and (vi) their fabrication quality. Objective: Are patients with symptomatic unilateral maxillary sinusitis sent for a dental evaluation of odontogenic maxillary sinusitis (OMS)?Patients and Methods: The medical records of all patients diagnosed with sinusitis at a university Ear-nose and throat (ENT) clinic from 2010 to 2015 were scrutinized for symptomatic unilateral maxillary sinusitis verified on CT. Any dental evaluation was recorded, as well as symptoms of OMS, duration to dental evaluation, diagnosis of OMS and dental treatment.Results: A total of 1338 patients diagnosed with sinusitis were screened; 172 had unilateral maxillary sinusitis and 48% (82/172) OMS. 34% did not have any dental evaluation at all. Patients referred to local specialist dental care waited a median of 11 weeks for evaluation and a median of 15 weeks further for dental treatment. Symptoms more frequently reported in OMS patients were foul smell or taste (43.9% vs 11.1%, p < .0001), purulent rhinorrhea (40% vs 19.1%, p = .015) than non-OMS patients. The OMS group also reported more frequently that they were active smokers (31.7% vs 10%, p = .0005).Conclusions: OMS was found in 48% of unilateral maxillary sinusitis and one-third of the patients with unilateral maxillary sinusitis had no dental evaluation. Background: The prescription rate for antibiotics in dental clinics is not declining despite the increase in the antibiotic resistance problem. In this study, we observed the change in antibiotic prescription rates by dentists in a Korean dental hospital for various treatments after conducting interventions targeting dentists.Methods: The first intervention was to distribute guidelines. The second intervention was to remove the bundled prescription button containing antibiotics from the Electronic Medical Record system. A total of 22,098 treatment records were divided into 12 main treatment categories, and Chi-square tests and logistic regression analyses were performed.Results: After the interventions were applied, the overall prescription rate for antibiotics dropped. The antibiotic prescription rate decreased by an odds ratio of 0.774 (95% CI: 0.686-0.873) after intervention 1 and by an odds ratio of 0.574 (95% CI: 0.501-0.658) after intervention 2. The treatments with significantly reduced antibiotic prescription rates were extraction for orthodontic treatment, dental implant surgery, extraction of an impacted tooth and general extraction. These treatments are typically performed in patients without an active infection. The prescription rate did not change for periodontal treatments or endodontic treatments, which are usually performed in patients with an infection. The prescription rate also remained constant for minor operations and other basic treatments.Conclusion: The interventions induced behavioural changes in the dentists and were effective in lowering the antibiotic prescription rates in a dental hospital. Objective: The present study aims to gain knowledge about the dentist's use and choice of digital intraoral imaging methods.Materials and Methods: A questionnaire sent to 2481 dentists within the Swedish Dental Society contained questions about the type of X-ray technique used, problems experienced with digital radiography, and reasons for choosing digital technology, and about indications, clinic size and type of service. Response rate was 53%.Results: Ninety-eight percent of the dentists had made the transition to digital radiography; only 2% used film technique, and solid-state detector (SSD) was the most used digital technique. More years in service decreases the likelihood of applying individual indications for performing a full mouth examination. More retakes were done with SSDs compared to storage phosphor plates. Reasons for choosing digital techniques were that work was easier and communication with the patients improved. However, dentists also experienced problems with digital techniques, such as exposure and projection errors and inadequate image quality. The Swedish Radiation Safety Authority states that all radiological examinations should be justified, something not always followed.Conclusions: This study showed that 98% of the respondents, Swedish dentists within the Swedish Dental Society, used digital techniques, and the most used was the solid-state technique. Odontogenic sinusitis (OS) is a highly prevalent, underappreciated and underdiagnosed disease that has been known for over 100 years. Apical periodontitis, periodontal disease and iatrogenic extrusion of foreign bodies into the sinus are the main causes of OS. Although the prevalence of sinus pathosis of dental origin is still controversial, otolaryngologists recognize that in the presence of recalcitrant sinusitis, a dental origin should be considered and properly treated. Currently, cone-beam computed tomography is the gold-standard imaging technique to assess the relationship between dental conditions, especially apical periodontitis and sinus diseases, and whenever this association is detected, patients should be seen by both a dentist and an otolaryngologist in order to achieve complete recovery. Objective: To describe a population-based organization of dentoalveolar surgical service for 0 to 18-year old subjects in a Danish municipal dental service, and analyze the type of dentoalveolar surgical interventions needed.Material and Methods: The study was conducted in the Municipality of Aarhus, Denmark during five consecutive school-years. An internal referral system was established within the municipality where patients could be referred to colleagues with a higher level of competencies and more experiences with paediatric dentoalveolar surgery. The analysis includes a total of 1812 children and a total of 2854 surgical interventions.Results: Almost 80% of the patients, representing more than 80% of the dentoalveolar surgical interventions needed, were referred internally. Denudations were the most frequent treatment type (40.3%) carried out, followed by removal of third molars (18.0%). Furthermore, 22 odontomas and 100 supernumerary teeth were removed.Conclusions: The need of dentoalveolar surgery in children and adolescents is relatively low, but includes a wide range of interventions. An organizational system, where dentists can refer to colleagues who have developed special competencies in this field, results in most of these surgical patients being referred and treated internally. ObjectiveThe aim of the study was to examine the prevalence of malocclusion traits and the extent of orthodontic treatment in a Finnish adult population.Materials and methodsThe study population comprised subjects (n = 1964) from the Northern Finland Birth Cohort 1966 living in the city of Oulu and within 100 km of it. A clinical oral and dental examination with registration of occlusion was carried out in 2012 in connection with a 46-year follow-up survey. Data on previous orthodontic treatment were collected based on a questionnaire.ResultsIn the clinical examination, 39.5% of the subjects had at least one malocclusion trait. The most common malocclusion traits were lateral crossbite (17.9%), overbite ≥ 6 mm (11.7%) and overjet ≥ 6 mm (9.7%). Crossbite on the left premolars, negative overjet and increased overbite were found more frequently in men. The prevalence of malocclusion traits was at the same level in treated and untreated groups. Overall, 18.6% of the subjects had undergone orthodontic treatment. Women showed a significantly higher prevalence of orthodontic treatment.ConclusionsThe most common malocclusion trait in the present study was lateral crossbite. Significant male dominance in the prevalence of malocclusion was observed, which has not been reported earlier in Finland. Orthodontic treatment of malocclusion traits was more common among females in Northern Finland. This study indicates that orthodontic treatment provided in childhood was, on average, adequate in reducing malocclusion traits to the level observed in the general population. Objectives: To assess leakage of class-I restorations using a gas-enhanced permeation test (GEPT) as compared with conventional SEM or dye analysis.Materials and Methods: Pressure differences over time and penetrating water volumes were measured simultaneously in a two-chamber system (GEPT) before and after class I cavity preparation in 30 molars. Ten teeth were restored with a composite restoration without bonding (A1), a composite restoration with bonding (A2) or a ceramic indirect restoration (B). Five intact teeth served as controls (C). Another GEPT measurement was performed and impressions were taken. Teeth were subjected to thermodynamic loading (1 200 000 cycles) and final GEPT measurements and impressions were made. SEM evaluation of the marginal continuity was performed and teeth were subjected to a Fuchsin dye penetration test. Spearman's rank test was used to compare results from different tests.Results: The GEPT and SEM values did not correlate before loading (0.359, p = 0.051), but significantly correlated afterwards (0.662, p < 0.0001). The correlations between the Fuchsin dye penetration test and GEPT and SEM surface marginal analysis were significant (0.777 and 0.534, p-values < 0.0001 and 0.002, respectively).Conclusions: SEM marginal analysis was mainly limited in reflecting the surface restoration integrity. GEPT evaluation may, therefore, serve as a tool to non-destructively assess restoration sub-surface integrity over time.Clinical Relevance: The current study provided proof that restoration margin quality does not necessarily reflect its leakage behaviour. Objective: The objective of the present study was to carry out a systematic review to analyse the effectiveness of pulp revascularization in the root formation of necrotic immature permanent teeth, as well as the level of scientific evidence regarding this theme.Materials and Methods: The methodology was based on searching electronic databases such as Web of Science, Pubmed, BVS (Medline, Scielo, Lilacs and BBO), Scopus and Cochrane, including manual searches for the references listed in the studies found. The terms used for the literature search were pulp revascularization and endodontics.Results: Initially, 277 articles were identified from the electronic databases; 17 studies remained after analysis and exclusion of duplicates; exclusion criteria also eliminated six articles; 11 remained for evaluation.Conclusions: Although the results found in the present systematic review are relevant, the scientific evidence should be interpreted with caution as the articles report different methods and evaluation parameters. Despite the capacity of the pulp revascularization technique to stimulate the development of the apical closure and thickening of radicular dentin, several aspects still remain unknown, like the key factors of this repair, the type of tissue formed and the long-term prognosis. Objectives: To evaluate the effect of thermal cycling and inner surface treatment with aluminum oxide at different granulations on the tensile strength of Ni-Cr copings cemented with different cementing agents.Materials and Methods: Ninety-six metal copings were manufactured and divided into two groups: before and after thermal cycling (n = 48). The copings of both groups were internally treated by sandblasting with aluminum oxide particles of 100 (n = 24) and 320 (n = 24) mesh. The copings were cemented on previously manufactured metal cores using zinc phosphate (n = 8), conventional glass ionomer (CGIC) (n = 8) and resin-modified glass ionomer (RMGIC) (n = 8) cements. The tensile strength before and after thermal cycling was then determined (Newtons).Results: The tensile strength before and after thermal cycling was significantly higher in copings cemented with RMGIC compared to CGIC (p < 0.05) and was similar to that for zinc phosphate (p > 0.05). Thermal cycling and sandblasting of the inner surface of the metal copings with different granulations did not influence retention (p > 0.05).Conclusions: Zinc phosphate cements and RMGIC showed similar retention. Objectives: The aim of this study was to evaluate the fracture resistance of endodontically-treated teeth restored with nano-hybrid composite resin, bulk-fill flowable and short fibre-reinforced-composite in the absence/presence of retention slots.Materials and Methods: One hundred and ten extracted non-carious human mandibular molars received endodontic treatment followed by mesio-occlusodistal (MOD) cavities with 3 ± 0.2 mm thicknesses of buccal and lingual walls. Teeth were divided into two main groups according to the retention slot preparation. The dove-tail retention slots were prepared on the middle of opposite buccal and lingual walls to create mechanical interlocking. Each group was further divided into four sub-groups according to restorative material types: control (no restoration), nano-hybrid composite resin (Filtek™ Z550), bulk-fill flowable (Filtek™ Bulk Fill) and short fibre-reinforced-composite (everX Posterior™). Restored teeth were subjected to compressive load at a strain rate of 1 mm/min. The data were statistically analysed using two-way ANOVA and Tukey's test for multiple comparisons.Results: The fracture resistance was significantly affected by the presence of the retentive slots and restorative material (p < 0.05). Restored teeth with retentive slots significantly increased the fracture resistance compared with restored teeth without retentive slots (p < 0.05). Short fibre-reinforced-composite with retentive slot cavities had significantly higher fracture resistance values compared with the other test groups (p < 0.05).Conclusions: The preparation of retention slots may increase the fracture resistance of endodontically-treated teeth, especially, when restored with short fibre-reinforced composite. Objective: Few studies of patient harm and harm-prevention methods in dentistry exist. This study aimed to identify and characterize dental patient safety incidents (PSIs) in a national sample of closed dental cases reported to the Regional State Administrative Agencies (AVIs) and the National Supervisory Authority for Welfare and Health (Valvira) in Finland.Materials and Methods: The sample included all available fully resolved dental cases (n = 948) during 2000-2012 (initiated by the end of 2011). Cases included both patient and next of kin complaints and notifications from other authorities, employers, pharmacies, etc. The cases analyzed concerned both public and private dentistry and included incident reports lodged against dentists and other dental-care professionals. Data also include the most severe cases since these are reported to Valvira. PSIs were categorized according to common incident types and preventability and severity assessments were based on expert opinions in the decisions from closed cases.Results: Most alleged PSIs were proven valid and evaluated as potentially preventable. PSIs were most often related to different dental treatment procedures or diagnostics. More than half of all PSIs were assessed as severe, posing severe risk or as causing permanent or long-lasting harm to patients. The risk for PSI was highest among male general dental practitioners with recurring complaints and notifications.Conclusions: Despite some limitations, this register-based study identifies new perspectives on improving safety in dental care. Objective. A previous cross-sectional investigation showed that the mandibular foramen location depends on the age and the vertical facial pattern of growing individuals. The aim of the present longitudinal study was to explain how these factors influence the distance between the foramen and the occlusal plane. It is known that a certain distance is necessary for a successful inferior alveolar nerve block in clinical dentistry. Materials and methods. This distance, as well as another four cephalometric variables, were measured on both pre-treatment and 10-year post-treatment lateral cephalometric radiographs collected from 50 patients who underwent orthodontic treatment. The changes between these two sets of measurements were also calculated. Results. A multiple regression analysis was performed using the pre-treatment age, the pre-treatment inter-maxillary angle, the rotation of the occlusal plane and the change in mandibular ramus height as independent variables and the change of foramen-occlusal plane distance as a dependent variable. The independent variables under investigation were found to account for more than half of the variability of the foramen-occlusal plane distance ( r = 0.732; p < 0.001). Conclusion. In very young individuals the mandibular foramen is located approximately at the level of the occlusal plane. With age it moves upwards relative to the occlusal plane and more so for those individuals with a low anterior facial height (short-face vertical facial type). These observations are, at least, partially explained by the differential growth of the various elements of the maxillo-mandibular complex and the change of the inclination of the occlusal plane. Objective. The purpose was to study associations between tooth brushing frequency, use of fluoride lozenges and consumption of sugary drinks at 1.5 years of age and having caries experience at 5 years of age. Methods. This study was based on data from the Norwegian Mother and Child Cohort Study conducted by the Norwegian Institute of Public Health and by the Public Dental Services. A total of 1095 children were followed from pregnancy to the age of 5 years. Questionnaires regarding oral health behavior were completed by the parents at 1.5 and 5 years of age. Clinical and radiographic examination of the children was performed at the age of 5 years. Results. In multiple logistic regression, having caries experience at 5 years of age was associated with; at 1.5 years of age having the teeth brushed less than twice daily (OR = 2.1, CI = 1.3-3.6) and being offered sugary drinks at least once a week (OR = 1.8, CI = 1.1-2.9) when controlled for family characteristics and oral health behavior at 5 years of age. Conclusions. Tooth brushing frequency and consumption of sugary drinks in early childhood were related to caries development during pre-school age, independent of family characteristics and oral health behavior at 5 years of age. The results indicate that early established habits regarding tooth brushing and consumption of sugary drinks have long-term effects on caries development. Objective. To analyse the prevalence of temporomandibular disorders and related pain (TMD-pain) among adult recall patients in general dental practice. Materials and methods. From November 2006 to September 2008, all adults attending a Swedish Public Dental Service (PDS) clinic for recall examination were asked two standardized questions about temporomandibular pain and dysfunction. Mouth-opening capacity was measured. The responses to the questions and mouth-opening capacity were combined to give a TMD-pain score, on a scale of 0-3. The patients' acceptance of their TMD condition was also noted. Results. The subjects comprised 2837 adults (53% females, 47% men). Of the total sample, 4.9% reported a TMD-pain score of 1-3. The gender difference was significant: women predominated ( p < 0.003). Forty-three per cent of those with TMD-pain scores of 1-3 (36% men, 47% women) considered that the condition warranted treatment, especially those registering a pain score (significant difference between pain and dysfunction groups, p < 0.000). Conclusions. The TMD-pain score shows promise as a useful instrument for detecting and recording TMD-pain. The prevalence of TMD disclosed in the study is high enough to be considered a public health concern. Background. Few data are available on dental patient safety (PS), as most PS studies have focused on other activities in health care. Objective. To detect types and causes of dental PS incidents (PSIs), including adverse events (AEs) and near misses (NMs), in Finnish dental care. Material and methods. Altogether 1041 privately or publicly employed dentists in southern Finland completed a structured questionnaire using an internet-based system (Webropol) in 2010. Results. Nearly one third of the dentists reported some PSI in the previous 12 months. Of the 872 reported events, 53% were classified as AEs, 45% as NMs and 2% remained unclassified. Nearly half of the PSIs had occurred during some form of dental treatment. One third of the AEs were related to dental equipment, devices and supplies. Most of the reported AEs resulted in little or no permanent harm to patients. However, 13% of AEs were considered as serious enough to potentially cause severe harm or did in fact cause permanent harm. Conclusions. Reported dental PSIs in Finland are in many respects similar to those reported in other countries. Compared to all annual dental visits in Finland, severe dental AEs seem to be relatively rare. Less severe AEs and NMs are not uncommon, especially in dental surgery, endodontic and restorative treatment. Objective. The aim of this study was to describe the oral health in the adult population of Skåne by clinical factors with special reference to age, gender, ethnicity and education. Materials and methods. Clinical examinations were performed on 451 randomly selected individuals, 20-89 years old, living in Skåne, Sweden. The clinical examination included a radiographic examination and a questionnaire. Clinical variables were put into cross-tabulations along with age, gender, educational level and ethnicity. Results. For all of the described clinical variables, except caries, the frequency increased with age. There were no differences in the clinical variables due to gender. The frequencies of missing teeth, caries, periodontal conditions and DMFT were higher among those with a lower educational level. More missing teeth, worse periodontal conditions, more apical destructions and less dental fillings were found in individuals who were not born in Sweden. Conclusions. The oral health in the adult population of Skåne was overall good, with low frequencies of oral disease and a large number of remaining teeth up to a high age. Objective. The purpose of the present survey was to evaluate the implementation and experience of a new method for posterior vertical bite reconstruction using direct resin composite restorations by private practitioners who attended a hands-on continuing education course on this technique. Materials and methods. In the years 2007 and 2008, 17 1-day continuing education courses on vertical bite reconstruction in the worn dentition by using direct resin composite restorations were attended by 310 participants. A 13-item questionnaire was posted to all course participants in April 2009 seeking information on the acceptance, implementation and experience of the presented technique in the private practice. Results. A total of 97 (31%) questionnaires were returned, whereas 67% of the respondents had used the presented technique. Analysis of the overall experience and satisfaction with the placed resin composite restorations using visual analog scale (VAS) revealed a mean VAS score of 7.2 ± 1.7 (0 = maximal unsatisfied, 10 = maximal satisfied). The direct resin composite restorations were predominantly rated 'good' within the assessed criteria surface texture, anatomical form, marginal integrity, marginal discoloration and color match. Ninety-eight percent of the private practitioners stated that they would continue to carry out vertical bite reconstructions according to this technique. The three most frequently observed clinical problems were related to marginal discolorations, bulk fractures and adhesive failures. Conclusion. The presented restoration method was well accepted by private practitioners who used this treatment approach at least once after course participation, even though some clinical problems were observed. Statement of the problem. An important aspect of dental education is teaching and learning shade matching. Objective. To assess the success of two different strategies for learning shade matching. Materials and methods. Seventy-one pre-clinical students (mean age 23; 33.3% men) were divided into two groups. One group (TTB) formed teams of two and they matched three pre-determined teeth on each other using the 3D-Master shade guide. After this exercise, they learned shade matching using a standardized device (Tooth Guide Training Box, TTB) by matching shade tabs in an artificial environment. The other group (GL) matched tooth color in a pre-defined clinical setting in groups of four students. After this, they matched the same teeth as before training, again in groups of two, similarly to group TTB. The reference tooth color was determined by two experienced prosthodontic clinicians. The L**a**b** values for the tabs were provided by the manufacturer and the color difference (Δ Eab) between the chosen tab and the reference was calculated. Linear general estimation equation models were used for statistical evaluation ( p == 0.05). Results. In the TTB group, the difference between Δ Eab values before and after training was 0.03, which was not significant ( p == 0.927; 95% CI: −0.70/0.77). In the GL group, Δ Eab was 0.98 smaller after training, which was significant ( p < 0.001; 95% CI: 0.50/1.50). Conclusion. The ability to match tooth color could be improved by using a group-learning approach in a clinical setting and implementation of such a training session should be considered in undergraduate and postgraduate dental education. Objective. To analyze correlations between the inclination of the permanent maxillary lateral incisors and the intraosseous position of the erupting permanent maxillary canines on panoramic radiographs. Material and methods. The inclination of the lateral incisor to the midline, the mesiodistal position of the crown of the canine, and the inclination of the canine to the midline and to the long axis of the adjacent incisor were measured on panoramic radiographs of 1013 subjects aged 8--11 years. Results. The distal inclination of the lateral incisor crown increased until 10 years, after which no significant changes occurred. The canine erupted with increasing mesial inclination of the crown until 9 years, after which it began to progressively right itself. As a result, inclination of the canine to the adjacent incisor increased between 8 and 9 years and decreased between 10 and 11 years. Until the children reached 9 years of age, the more mesial the crown of the unerupted canine, the greater the decrease in the distal inclination of the lateral incisor crown. Conclusions. The inclination of the crown of the lateral incisor varies during eruption of the canine, increasing distally until 10 years as a physiological stage in the mixed dentition. The inclination of the lateral incisor is also associated with the mesiodistal position of the canine crown. Objective. To compare handmade and computer-aided design--computer-aided manufacturing (CAD-CAM)-fabricated fixed dental prostheses (FDPs) composed of a particulate filler composite. Material and methods. Handmade FDPs were made of restorative composite (Z 100) and CAD-CAM-fabricated FDPs were made of commercial CAD-CAM blocks (VITA Temp) and two experimental CAD-CAM blocks of particulate filler composite. Experimental CAD composite A was prepared by mixing 31.2 wt.% of dimethacrylate resin with 68.7 wt.% of filler particles of barium oxide silicate (BaSiO2). Experimental CAD composite B was prepared by mixing 25.6 wt.% of dimethacrylate resin with 74.3 wt.% of filler particles of BaSiO2. Six groups were fabricated ( n == 6 in each); FDPs were statically loaded until final fracture. Results. Experimental CAD composites A and B revealed the highest load-bearing capacity of the FDPs, while Z 100 showed the lowest. Conclusion. Objective. To investigate the prevalence, characteristics (malocclusion, location, type) and gender distribution of supernumerary teeth (ST) in an Iranian orthodontic population. Material and methods. A retrospective study was carried out using radiographs (periapicals, panoramic) and study models of 1751 subjects (870 girls, 881 boys; age range 9--27 years). The Pearson chi-square test was used to determine differences in the distribution of ST when stratified by gender and malocclusion type. Results. Fourteen ST were found in 13 patients (six males, seven females), giving a prevalence rate of 0.74%. The prevalence rate of ST was higher in Class III malocclusions (5.2%) and was a rare finding among Class II malocclusions (0.1%). The occurrence rate of ST was higher in maxilla (78.5%) than in mandible (21.5%). The most common type of ST was the conical form (43%). We did not observe any late-developing paramolar or distomolar ST. Conclusion. The prevalence rate of ST in our study was 0.74% and ST were more common among Class III malocclusions. Objective. To evaluate the effect of air abrasion with different particles of different sizes and forms on the shear bond strength of adhesive resin cement to zirconia core. Material and methods. Sixty zirconia core disks were produced and sintered. The specimens were divided into six equal groups for application of air abrasion procedures. The surfaces of the specimens were treated with one of five air abrasion particles: 30 μμm silica-coated aluminum oxide particles; 1--3 μμm synthetic diamond particles; 110 μμm aluminum oxide particles; 30--50 μμm synthetic diamond particles; and 60--80 μμm cubic boron nitride particles. The remaining 10 specimens were untreated and served as controls. Composite resin disks were cemented to each of the zirconia core specimens. All specimens were stored in distilled water at 37°°C for 24 h and thermocycled for 6000 cycles. The shear bond strength was measured using a universal testing machine at a crosshead speed of 1 mm/min. Data were statistically analyzed by one-way ANOVA with Tamhane tests (αα == 0.05). The effect of the air abrasion procedures was examined using scanning electron microscopy. Results. Air abrasion with different materials affected the bond strength ( P < 0.001). The highest bond strengths were obtained by air abrasion with 30--50 μμm synthetic diamond particles; the lowest bond strengths were obtained in the control group ( P < 0.001). Conclusion. Air abrasion with 30--50 μμm synthetic diamond particles, 60--80 μμm cubic boron nitride particles and 110 μμm aluminum oxide particles showed higher bond strength values than other methods. Objective. There is a gap in the education of dental undergraduate students between theoretical knowledge and the practical procedure of tooth extraction. It was the aim of this study to implement and validate an undergraduate course for oral extraction using mannequin models, in order to enhance practical competence in this field of dentistry. Material and methods. A one-term oral extraction course was implemented as part of the oral surgery curriculum for undergraduate dental students, including 4 h of theoretical education, followed by 4 h of practical training in tooth extraction on mannequin models, with the students divided into small groups. Forty-nine students attending this extraction course were asked to validate their training in dental extraction using a questionnaire (TRIL-mod; University of Trier, Trier, Germany). The students' practical competence in oral extraction was assessed by a senior supervisor at the end of the course, using a standardized checklist. Results. All questionnaires were returned. Overall, the course was rated with an average score of 4.7 (rating range: 1--6; 1 = worst/6 = best). The assessment of practical skills yielded a successful and complete performance of tooth extraction by 94% of the students. Conclusions. A tooth extraction course using a mannequin model is appreciated by dental students and can achieve a sufficient level of competence in tooth extraction with reasonable educational efforts. Objective. To assess any differences in psychological and endocrine variables between homogeneous core groups of young women with well-defined muscle- or disk-related temporomandibular disorders (TMDs) and matched controls. Material and methods. Fifteen women, aged 18–24 years, fulfilling the TMD Research Diagnostic Criteria I a/b (but not II or III) and 15 fulfilling the II a/b and III criteria were consecutively selected from referrals to an orofacial pain/TMD clinic. Thirty consecutive healthy age-matched women attending yearly routine check-ups at a general dental clinic served as controls. All 60 subjects answered a questionnaire, the Profile of Fatigue Related Symptoms (PFRS), and salivary samples on waking were collected for analysis of cortisol levels. Results. Symptom duration and pain levels were similar irrespective of muscle- or disk-related symptoms. Both diagnostic groups obtained scores that were similar and significantly higher than those of controls in all four scales of the PFRS: fatigue, emotional distress, cognitive difficulties and somatic symptoms. Salivary cortisol levels on waking did not differ between patients and controls. Conclusions. Patients with TMDs, irrespective of diagnosis, appeared to be more psychologically distressed than controls evaluated psychometrically, which is in line with earlier findings. A corresponding difference was not reflected in a single measurement of morning salivary cortisol. A more comprehensive evaluation of endocrine variables is probably necessary to reveal whether any differences actually exist in this respect. [ABSTRACT FROM AUTHOR]

Objective. Controversy exists concerning the etiological factors behind degenerative changes in the temporomandibular joints (TMJs). Tooth attrition, occlusal support, food consistency, ageing, gender and genetics are some possible causative factors that have been discussed in the literature. The aim of this study was to examine contemporary human skull material for possible relations between degenerative form and surface changes in the TMJs in relation to occlusal support. Material and methods. The material consisted of 259 human skulls from 170 males and 89 females, with an age range of 18–100 years. Results. Dental status was in general poor, and 22% of skulls were edentulous. Form and surface changes of both the condyles and the temporal components were more common in the present material compared to that in most previous studies. In males, irrespective of age, only weak and clinically insignificant correlations could be found between degenerative TMJ changes and occlusal support. In women, however, the correlations between these variables were in general much stronger, especially at higher ages. Conclusions. The present findings do not lend support to the hypothesis that loss of occlusal support is a causative factor for degenerative changes in the TMJs in male subjects. In women, such a correlation was obvious in the present sample, at least at higher ages. It can be speculated that hormonal factors play a role in the sex difference found. [ABSTRACT FROM AUTHOR]

Objective. To examine the link between tooth loss and multilevel factors in a national sample of middle-aged adults in Brazil. Material and methods. Analyses were based on the 2003 cross-sectional national epidemiological survey of the oral health of the Brazilian population, which covered 13 431 individuals (age 35–44 years). Multistage cluster sampling was used. The dependent variable was tooth loss and the independent variables were classified according to the individual or contextual level. A multilevel negative binomial regression model was adopted. Results. The average tooth loss was 14 (standard deviation 9.5) teeth. Half of the individuals had lost 12 teeth. The contextual variables showed independent effects on tooth loss. It was found that having 9 years or more of schooling was associated with protection against tooth loss (means ratio range 0.68–0.76). Not having visited the dentist and not having visited in the last ≥3 years accounted for increases of 33.5% and 21.3%, respectively, in the risk of tooth loss ( P < 0.05). The increase in tooth extraction ratio showed a strong contextual effect on increased risk of tooth loss, besides changing the effect of protective variables. Conclusions. Tooth loss in middle-aged adults has important associations with social determinants of health. This study points to the importance of the social context as the main cause of oral health injuries suffered by most middle-aged Brazilian adults. [ABSTRACT FROM AUTHOR]

Objective. To describe the periodontal condition of the elderly in Finland and its association with sociodemographic factors. Material and methods. The study was based on a subpopulation of 1460 persons (748 dentate persons) aged ≥65 years who participated in a nationally representative Health 2000 Survey in Finland. The data for this study were collected by means of an interview and a clinical oral examination. Results. Seventy-three percent of the participants in this study had gingival bleeding in at least one sextant. The proportion of participants with no teeth with deepened periodontal pockets ≥4 mm deep was 31%. Twenty-eight percent of the participants had one to three teeth with deepened periodontal pockets ≥4 mm deep, 15% had four to six and 26% had ≥7. At the individual level, a high number of teeth with deepened periodontal pockets was associated with sociodemographic factors such as high age, intermediate or higher education, and living in a rural area. Conclusion. At a national level, the greatest need for periodontal treatment presently derives from persons aged 65–74 years and/or those with low education, due to their high representation in the elderly population. Objective. This study compared wound healing following incisions with either a scalpel, a diode laser or an Er,Cr:YSGG laser in guinea pig oral mucosa. Material and methods. Three types of wound were made randomly with either a stainless-steel scalpel, a diode laser or an Er,Cr:YSGG laser in the buccal mucosa of 24 guinea pigs. Five guinea pigs were sacrificed on each of Days 1, 3, 5 and 7 post-surgery. Four guinea pigs were sacrificed on Day 14 post-surgery. Biopsy samples from each oral mucosa wound were examined using light microscopy and the expression of tumor necrosis factor (TNF)-α and transforming growth factor (TGF)-β1 was determined by immunohistochemical staining. The expression of TNF-α and TGF-β1 was evaluated by calculating the percentage of positively stained cells and immunostaining intensity was evaluated using a scale ranging from 0 to 3. Results. Infiltration of inflammatory cells decreased rapidly at Day 5 post-surgery in all three groups of animals. The highest level of TNF-α expression was found at Day 1 post-surgery for the diode laser wounds. The intensity of TNF-α immunostaining was highest at Day 3 post-surgery and lowest at Day 7 post-surgery for all three groups of animals. For the scalpel wounds, a lower level of TGF-β1 expression was seen until Day 3 post-surgery and a higher level from Day 7 post-surgery compared to laser wounds. The intensity of TGF-β1 immunostaining was highest at Day 1 post-surgery for the diode laser wounds. Conclusions. The diode laser is considered a good cutting device for oral mucosa; however, more tissue damage occurs than with the use of a scalpel or an Er,Cr:YSGG laser. Objective. The microstructure of hypomineralized enamel in permanent teeth has been described in several studies as less distinct prism sheaths and disorganized enamel with lack of organization of the enamel crystals. The mechanical properties, hardness and modulus of elasticity of the hypomineralized enamel have lower values compared with normal. The aim of this study was to examine normal and hypomineralized enamel using scanning electron microscopy (SEM), hardness measurements and X-ray microanalysis (XRMA). Material and methods. Four extracted hypomineralized permanent first molars, sectioned and cut in half, were analyzed with SEM, XRMA and hardness measurements. Results. An inverse relation was found between the micro hardness and the Ca:C ratio in hypomineralized and normal enamel. The acid-etched hypomineralized enamel appeared on SEM to be covered with a structureless layer and the prisms appeared disorganized, with thick prism sheaths and loosely packed crystallites. Furthermore, bacteria were found deep in porous hypomineralized enamel close to the enamel–dentin junction. Conclusions. Teeth diagnosed with molar incisor hypomineralization have significantly lower hardness values in the hypomineralized enamel compared with normal enamel. Objective. To assess the bond strength of Rely-X Unicem (RX), in comparison with Cement-Post (CP), to the cervical, middle and apical thirds of root canal dentin irradiated with a 980-nm diode laser. Material and methods. Forty maxillary canines were separated into four equal groups according to the cement used and the laser surface treatment: RX/laser irradiation (LI) group; RX/no irradiation (NI) group; CP/LI group; and CP/NI group. Two slices (2-mm thick) of each root third were submitted to a push-out test to assess the bond strength of the cement to the root canal dentin. The data obtained were submitted to two-way ANOVA and fracture analysis was performed by scanning electron microscopy. Results. The type of cement as well as 980-nm LI at the different root canal thirds significantly affected the bond strength values ( p < 0.05). LI promoted an increase in bond strength of RX in all root thirds ( p < 0.05). For all groups, bond strength to the cervical third was highest, followed by the middle and apical thirds ( p < 0.05). When the cements were compared, RX always presented the highest bond strength in comparison with CP, irrespective of the laser application ( p < 0.05). Fracture analysis showed a predominance of mixed failures for RX and of adhesive failure between dentin and cement for CP, irrespective of the laser application. Conclusion. Objective. The purpose of this study was to make a clinical comparison of the chemomechanical method for caries removal and the conventional rotary instruments technique when used in children and adolescents. Material and Methods . The study comprised 120 patients aged 3-17 years randomized into two groups: caries were removed chemomechanically in 60 patients and 60 patients received conventional treatment with rotary instruments. The outcome variables were: clinically complete caries removal, pain during caries removal, need for local anesthesia, treatment time, preferences of patients, and clinical success of the restorations during the 12-month evaluation period. Results. Complete caries removal was achieved in 92% of chemomechanically treated teeth and in all teeth treated with rotary instruments (p>0.05). The chemomechanical method significantly reduced the need for local anesthesia (p<0.001). Eighty-five percent of patients treated with Carisolv and 47% treated with rotary instruments were satisfied with the treatment (p<0.05). The mean time for chemomechanical caries removal was 11.2±3.3 min and 5.2±2.8 min for caries removal with rotary instruments (p<0.001). At the end of the 12-month evaluation period, there was no observed influence of the caries removal method on the survival of the restorations. Conclusions. Objective . The aim of this study was to assess the prevalence, extent, and risk indicators of tooth loss in an isolated population of Brazil. Material and methods . Two-hundred-and-forty-two subjects, ranging in age from 14 to 82 years (mean 36.2 years), were identified by census in an isolated population of Brazil. All consenting subjects received a full-mouth clinical (DFT index and information about missing teeth) and periodontal examination of 6 sites per tooth. Furthermore, they were interviewed using a structured written questionnaire in order to gather information about demographic, environmental, and biological variables. Results . Of the 200 subjects (80% response rate), 19 (9.5%) were edentulous, 90% had lost at least one tooth, and 39% had lost more than 8 teeth. The mean number of teeth lost was 9.5 (95% CI = 8.2-10.8). First mandibular molars were the most commonly missing teeth. In a multiple logistic regression analysis based on a theoretical hierarchical model of tooth loss, having more than 8 teeth lost was strongly associated with adult age (OR = 18.3-17.3, 95% CIs = 4.8-69.7 and 4.0-75.1) and female gender (OR = 5.9, 95% CI = 1.9-18.2) in the final model. Conclusions . Tooth loss was highly prevalent and extensive in this isolated population. Demographic and behavioral factors played an important role in tooth loss prevalence in this population. Objective. The prevalence of caries and various risk factors was studied cross-sectionally in two cohorts of 72-year-olds over the course of 20 years. Incidence was also studied over a period of 6 years. Material and Methods. Samples of two dentate subjects from cohorts of 70 to 72-year-olds were included; cohort III (n=135), born 1911-12, and cohort VI (n=139), born 1930. Cohort III was also examined at 70 years of age and re-examined after 6 years. An interview and clinical examination, including DMFS, plaque and gingival bleeding indices, pocket depth, and secretion rate for stimulated whole saliva, were carried out. Results. The mean numbers of teeth in cohorts III and VI were 14 and 21, respectively. On average, 2.1 tooth surfaces were decayed (DS) in cohort III compared with 1.3 in cohort VI (p<0.05). 49% of the individuals in cohort III did not develop any new DS, while 19% developed ≥3 DS during the 6-year follow-up period. The prevalence of caries was related to plaque and gingival bleeding indices, as well as a pocket depth of ≥4 mm, at ages 70 and 72. Moreover, the incidence of caries was related to the same factors. Conclusions. The numbers of remaining teeth increased and the prevalence of caries decreased over the 20-year period. The incidence of caries over the follow-up period of 6 years was moderate. The results indicate that the prevalence of plaque, gingival bleeding, and a pocket depth of ≥4 mm are predictive factors for both prevalence and incidence of caries. [ABSTRACT FROM AUTHOR]

Objectives. To study occlusal wear of anterior teeth in orthodontic patients retained with different retainers until 5 years post-treatment, and to investigate whether type of retention influences occlusal wear. Material and methods. Orthodontic patients (n=222), aged 15 years maximally at the start of treatment, were followed until 5 years post-treatment. In the maxilla, a retainer bonded on all six teeth or a removable retainer was used; in the mandible, a lingual retainer was bonded on all anterior teeth or on canines only. Dental casts were analyzed before treatment (T0), after treatment (T1), and 5 years post-treatment (T5). Incisal and canine wear were scored by applying a grading scale. Intercanine width, overjet, and overbite were measured with an electronic caliper. Statistics used were: Paired samples t-test for differences over time; Pearson correlation coefficients for associations between wear and retention type; and backward linear regression for influence of retention type on wear. Results. There was an increase in wear during all time periods and for all teeth. From T0 to T5 an increase in maxillary intercanine width and maxillary retention had an effect on changes in canine wear. Incisal wear was associated with an increase in upper intercanine width (T1-T5). For both arches, an increase in maxillary intercanine width during treatment was associated with less progression of canine and incisal wear, but the explained variance was low, 13.4% and 19.3%, respectively. Conclusions. Retention type and, occasionally, an increase in intercanine width influence anterior teeth wear post-treatment. Objective. Periodontal regeneration is histologically defined as regeneration of the tooth supporting structures, including alveolar bone, periodontal ligament, and cementum. Cells in the remaining periodontal tissues need optimal conditions if they are to perform their functions in the regeneration process. The present study is an investigation of the molecular effects of ABM/P-15 on human periodontal ligament cells (PDL) in vitro. Material and methods. PDL cells obtained from healthy subjects were used for in vitro experiments. Cell proliferation, morphology, and mineralization using Von kossa staining were evaluated. mRNA expressions for transforming growth factor-β (TGF-β), insulin-like growth factor-I (IGF-I), basic fibroblast growth factor (b-FGF), vascular endothelial growth factor (VEGF), bone morphogenic protein-2 (BMP-2), platelet-derived growth factor (PDGF), and type 1 collagen (COL1) were assessed on days 3 and 7 using RT-PCR. Results. ABM/P-15 enhanced proliferation of cultured PDL cells. It increased the mRNA expression of TGF-β and BMP-2 in cultured PDL cells on days 3 and 7. IGF-I and b-FGF mRNA expressions showed a slight decrease, while PDGF expression was observed to have increased on day 3. VEGF and COL1 mRNA expressions were found not to be different on days 3 and 7. No differences were observed in the mineralization properties of cultured PDL cells treated with or without ABM/P-15. Conclusions. Based on the results of this in vitro study, it may be concluded that ABM/P-15 enhanced the regenerative capacity of PDL by regulating specific gene expressions of cells during early wound healing. Objective. To identify factors associated with dentists' decisions on choice of restorative material in children and adolescents. Material and methods. In the period 2001-2004, 27 dentists in the Public Dental Health Service in Norway placed 4030 Class II restorations in 1912 patients. The reason for placement, previous caries experience (DMFT), oral hygiene, and characteristics of the cavity were recorded. Results. The most frequently used material was resin composite (81.5%), followed by compomer (12.7%), amalgam (4.6%), and glass ionomer cement (1.2%). Tooth-colored restorations were more frequently placed than amalgam in younger patients (p=0.017). Female patients received fewer amalgam restorations than male patients (p=0.006). Amalgam was more often used in patients with high DMFT (p<0.001) and more commonly in treatment of deeper dentine caries than caries near the dentino-enamel border (p=0.021). Amalgam was more frequently placed in molars than in premolars (p<0.001). In a logistic regression model, gender, DMFT, caries severity, and tooth type were significantly related to choice of material. Conclusion. Dentists' choices of restorative material indicate that the majority prefer amalgam in more challenging restorations with respect to caries activity, lesion depth, and tooth type. The findings indicate that in a period when the use of amalgam was phasing out, resin composite was the predominant material of choice for Class II restorations in children and adolescents. Objective. To evaluate dental age in 6-year-old children with submucous cleft palate (SMCP) and to compare this in age-matched and sex-matched children with clefts of the soft palate (CPs). Material and Methods. The dental maturity of 73 children (39 girls) with SMCP was evaluated from panoramic radiographs and compared in age-matched and sex-matched controls with CPs. Mean biological age of the children was 6.1, ranging from 5.5 to 6.8 (boys 6.2 years, range 5.7-6.8; girls 6.1 years, range 5.5-6.7). Dental stages were assessed following the method of Demirjian, and dental age was calculated in accordance with the Finnish dental maturity reference values. Student's paired t-test was used in the statistical analysis. Results. There were no significant differences in dental age between the sexes, such that boys and girls were combined in the further analyses. Dental age of the children with SMCP (6.2 years, range 4.9-7.4) was similar to their biological age (NS). Dental age of those with CPs was 0.2 years delayed (dental age 5.9 years, range 4.6-7.5; p<0.001) compared to their biological age. Children with CPs had a lower dental age than children with SMCP (p<0.001). Conclusion. Dental maturation in 6-year-old children with submucous cleft palate is not delayed, whereas in children with clefts of the soft palate dental age is slightly delayed. [ABSTRACT FROM AUTHOR]
