A persistent cavum septum pellucidum (CSP) is present in approximately 0.73% of adults, although its incidence ranges from 0.14 to 18.9% depending on the detection method. Cystic CSP is even rarer. A cyst causing mass within the CSP can obstruct the intraventricular foramen, leading to blockage of CSF flow and possible hydrocephalus, often justifying surgical intervention. We describe spontaneous decompression of a cystic CSP in a 36-year-old man. Initial MRI showed a cystic CSP with lateral bowing of the septal walls to 1.9 cm. Follow-up MRI 15 months later demonstrated no lateral bowing, and the septal wall width was 1.0 cm. This spontaneous decompression was not explained by the one previously described case report of cystic CSP regression.

Objective and importance. We present a complication of Ommaya reservoir placement that has not been previously reported. Following injection of a seemingly appropriately placed catheter, the patient developed seizures. Imaging studies showed the development and resolution of a cavum septi pellucidi. This case illustrates that the septum pellucidum is made of two layers and that a potential space exists between these layers. Caution is recommended when injecting a single-hole ventricular catheter if the tip is against the septum pellucidum.

The authors report a case of a huge colloid cyst of the septum pellucidum with acute worsening. A 42-year-old man was admitted for unexplained lethargy. A brain CT scan disclosed a huge intra-ventricular lesion. He experienced a sudden deterioration with coma, and was managed with bilateral ventricular external shunting. Total removal was performed a few days later using a right frontal trans-ventricular approach. The discussion focuses on colloid cyst potential to mimic other intracranial lesions, rendering accurate diagnosis difficult and leading to possibly inappropriate management in similar cases.

Background: We report a rare case of germinoma mainly located in the septum pellucidum., Case Description: A 19-year-old man presented with germinoma located in the septum pellucidum manifesting as amnesia and right hemiparesis. Magnetic resonance imaging demonstrated a heterogeneously enhanced mass in the septum pellucidum and abnormal T2-weighted signals in the left basal ganglia. An anterior transcallosal approach was performed to remove the mass in the septum pellucidum. The histological diagnosis was germinoma. Radiotherapy and chemotherapy were given. He was discharged with slight amnesia and right hemipareis., Interpretation: Intracranial germinoma can arise from the septum pellucidum.

The authors report the case of a 29-year-old female who presented with symptoms of shunt dysfunction 11 years after first being shunted for an aqueductal stenosis. After numerous revisions she developed an isolated third ventricle, necessitating triventricular shunting to obtain a new equilibrium. An isolated third ventricle is a very rare phenomenon, usually seen in very complex hydrocephalus and only reported on twice before (Filler et al., 1995). Etiological factors postulated in the development of an isolated lateral or fourth ventricle, all seem to contribute also to the development of an isolated third ventricle.

The cavum septi pellucidi (CSP) and cavum Vergae (CV) are persistent, primitive, or acquired, midline structures of adult human brain. It is customary to distinguish between the non-communicating and the communicating cava, depending on whether the cavum communicates with the cerebral ventricular system or not. Only a few cases of symptomatic non-communicating cava, called septum pellucidum and cavum Vergae cysts, have been described in the literature. In this study, the authors describe the morphological, histological and histo-immunological characteristics of an additional case of septum pellucidum-cavum Vergae cyst in a forty-year-old man who died the day following a ventriculo-peritoneal shunt. We have found a communication between the CSP and the leptomeningeal space of the anterior interhemispheric fissure, in the absence of subarachnoid haemorrhage. The authors discuss the origin of the intracystic fluid and the classification of the CSP.
