Two patients were admitted to the Emergency Department with fever and altered consciousness. The diagnosis of Streptococcus pneumoniae meningitis was rapidly obtained from cerebrospinal fluid (CSF) culture. Their neurological condition rapidly improved after the introduction of antimicrobial therapy plus dexamethasone. The brain's computed tomography on admission did not reveal any specific lesion. By the second week of treatment, while CSF was sterile, their neurological condition gradually worsened and both died from extensive brain ischaemic lesions. While persisting inflammation could be the underlying mechanism, these observations illustrate that such complications are not fully prevented by a short course of high dose steroids.

Objective: To review the clinical and experimental data concerning the serious neurologic adverse events, and more particularly seizures, which could be related to the administration of recent antibiotics, with special reference to cephalosporins, monobactams, carbapenems, and fluoroquinolones., Data Sources: We have searched in the MEDLINE database over the years 1966-1998 the pertinent publications dealing with antibiotics related neurotoxicity. We used the thesaurus function and the following key words: antibiotics, neurotoxicity, seizures. Additional references were found in the articles sorted by the MEDLINE search., Data Synthesis: Neurotoxic manifestations following antibiotics administration are infrequently encountered under usual conditions. Experimental studies are helpful to demonstrate that these compounds might interact with a major component of the neurotransmission, the gamma aminobutyric acid (GABA) receptor complex. Structure-toxicity relationships can be described. For the clinician, the recognition of some predisposing factors related either to the patient (age, previous central nervous system disorder ...) or to the drug metabolism (reduced renal clearance, drug interactions ...) may help to minimise the risk of adverse neurologic manifestations. Several factors have to be taken into account before assessing causality: delay from administration, evolution, origin of the adverse event (risk factors, other drugs, non pharmacological origin), possibility of rechallenge, confirmation by biological testing or in vitro experiments....

The authors describe the main features of acute paraquat poisoning. The clinical picture together with severity grading is presented. The pathophysiological mechanisms responsible for multiple organ dysfunction are discussed. Despite a better understanding of pathophysiological pathways and numerous experimental data, the treatment of the most severe forms of poisoning remains limited. The efficacy of more recently developed therapeutic approaches is still to be evaluated.

We report two cases of severe acetaminophen poisoning with favorable outcome after delayed medical treatment with N-acetylcysteine, despite severe hepatic disturbances. We briefly review the most commonly accepted prognostic criteria and the role of liver transplantation.

A 29-year-old woman who developed fulminant pulmonary edema several minutes after intravenous administration of radiocontrast media (RCM) is reported. The low pulmonary-capillary wedge pressure and the high protein concentration in pulmonary edema fluid indicated a noncardiogenic pulmonary edema related to increased lung vascular permeability. Laboratory studies showed a slight decrease of several complement components (C3, C4 and factor B) and a transient consumption coagulopathy. Potential pathophysiologic mechanisms involved in RCM- induced pulmonary edema are discussed.

About two consecutive cases of acute intoxication by trichloroethylene, the authors want to point out the possible occurrence of serious disturbances in cardiac conduction which are less frequently described in the literature than disturbances in myocardial excitability.
