ObjectiveThe Mexican government has pursued multiple initiatives to improve healthcare coverage and financial protection. Yet, out-of-pocket health spending and use of private sector providers in Mexico remains high. In this paper, we sought to describe the characteristics of public and private healthcare users, describe recent visit quality across provider types, and to assess whether perceiving the public healthcare sector as poor quality is associated with private health sector use.Methods and findingsWe analyzed the cross-sectional People's Voice Survey conducted from December 2022 to January 2023. We used Chi-square tests to compare contextual, individual, and need-for-care factors and ratings of most recent visits between users of public (social security and other public providers) and private sector providers (stand-alone private providers and providers adjacent to pharmacies). We used a multivariable Poisson regression model to assess associations between low ratings of public healthcare sources and the use of private care. Among the 811 respondents with a healthcare visit in the past year, 31.2% used private sources. Private healthcare users were more educated and had higher incomes than public healthcare users. Quality of most recent visit was rated more highly in private providers (70.2% rating the visit as excellent or very good for stand-alone private providers and 54.3% for pharmacy-adjacent doctors) compared to social security (41.6%) and other public providers (46.6%). Those who perceived public health institutions as low quality had a higher probability of seeking private healthcare.ConclusionUsers rated public care visits poorly relative to private care; at the population level, perceptions of poor quality care may drive private care use and hence out-of-pocket costs. Improving public healthcare quality is necessary to ensure universal health coverage.

IntroductionNearly 30 000 Mexican women develop breast cancer annually, frequently presenting unmet supportive care needs. In high-income countries, incorporating electronic patient-reported outcomes (ePROs) into cancer care has demonstrated potential for increasing patient-centred care and reducing unmet needs. No such ePRO interventions have been implemented in Mexico. This paper presents the study protocol for designing and evaluating an ePRO digital health application combined with proactive follow-up by nurses.Methods and analysisWe designed a two-component intervention for women receiving breast cancer treatment: a responsive web application for monitoring ePROs and clinical algorithms guiding proactive follow-up by nurses. We will conduct a pilot test of the intervention with 50 patients with breast cancer for 6 weeks to assess feasibility and adjust the application. We will conduct a parallel arm randomised controlled trial assigning 205 patients each to intervention and control in one of Mexico's largest public oncology hospitals. The intervention will be provided for 6 months, with additional 3 months of post-intervention observation. The control group will receive usual healthcare and a list of breast cancer information sources. Women diagnosed with stages I, II or III breast cancer who initiate chemotherapy and/or radiotherapy will be invited to participate. The primary study outcome will be supportive care needs; secondary outcomes include global quality of life and breast symptoms. Information on the outcomes will be obtained through web-based self-administered questionnaires collected at baseline, 1, 3, 6 and 9 months.Ethics and disseminationThe National Research and Ethics Committees of the Mexican Institute of Social Security approved the study (R-2021-785-059). Participants will sign an informed consent form prior to their inclusion. Findings will be disseminated through a policy brief to the local authorities, a webinar for patients, publications in peer-reviewed journals and presentations at national and international conferences.Trial registration numberNCT05925257.

BackgroundMexican migrants traveling across the Mexico-United States (U.S.) border region represent a large, highly mobile, and socially vulnerable subset of Mexican nationals. Population-level health data for this group is hard to obtain given their geographic dispersion, mobility, and largely unauthorized status in the U.S. Over the last 14 years, the Migrante Project has implemented a unique migration framework and novel methodological approach to generate population-level estimates of disease burden and healthcare access for migrants traversing the Mexico-U.S. border. This paper describes the rationale and history of the Migrante Project and the protocol for the next phases of the project.Methods/designIn the next phases, two probability, face-to-face surveys of Mexican migrant flows will be conducted at key crossing points in Tijuana, Ciudad Juarez, and Matamoros (N = 1,200 each). Both survey waves will obtain data on demographics, migration history, health status, health care access, COVID-19 history, and from biometric tests. In addition, the first survey will focus on non-communicable disease (NCD), while the second will dive deeper into mental health and substance use. The project will also pilot test the feasibility of a longitudinal dimension with 90 survey respondents that will be re-interviewed by phone 6 months after completing the face-to-face baseline survey.DiscussionInterview and biometric data from the Migrante project will help to characterize health care access and health status and identify variations in NCD-related outcomes, mental health, and substance use across migration phases. The results will also set the basis for a future longitudinal extension of this migrant health observatory. Analyses of previous Migrante data, paired with data from these upcoming phases, can shed light on the impact of health care and immigration policies on migrants' health and inform policy and programmatic responses to improve migrant health in sending, transit, and receiving communities.

BackgroundTimely monitoring of SARS-CoV-2 variants is crucial to effectively managing both prevention and treatment efforts. In this paper, we aim to describe demographic and clinical patterns of individuals with COVID-19-like symptoms during the first three epidemic waves in Mexico to identify changes in those patterns that may reflect differences determined by virus variants.MethodsWe conducted a descriptive analysis of a large database containing records for all individuals who sought care at the Mexican Social Security Institute (IMSS) due to COVID-19-like symptoms from March 2020 to October 2021 (4.48 million records). We described the clinical and demographic profile of individuals tested (3.38 million, 32% with PCR and 68% with rapid test) by test result (positives and negatives) and untested, and among those tested, and the changes in those profiles across the first three epidemic waves.ResultsIndividuals with COVID-19-like symptoms were older in the first wave and younger in the third one (the mean age for those positive was 46.6 in the first wave and 36.1 in the third wave; for negatives and not-tested, the mean age was 41 and 38.5 in the first wave and 34.3 and 33.5 in the third wave). As the pandemic progressed, an increasing number of individuals sought care for suspected COVID-19. The positivity rate decreased over time but remained well over the recommended 5%. The pattern of presenting symptoms changed over time, with some of those symptoms decreasing over time (dyspnea 40.6 to 14.0%, cough 80.4 to 76.2%, fever 77.5 to 65.2%, headache 80.3 to 78.5%), and some increasing (odynophagia 48.7 to 58.5%, rhinorrhea 28.6 to 47.5%, anosmia 11.8 to 23.2%, dysgeusia 11.2 to 23.2%).ConclusionDuring epidemic surges, the general consensus was that any individual presenting with respiratory symptoms was a suspected COVID-19 case. However, symptoms and signs are dynamic, with clinical patterns changing not only with the evolution of the virus but also with demographic changes in the affected population. A better understanding of these changing patterns is needed to improve preparedness for future surges and pandemics.

In the hot dry spring of monsoon-driven environments, keeping an accurate calendar to regulate the annual planting of crops is of critical importance. Before the Spanish conquest, the Basin of Mexico had a highly productive farming system able to feed its very large population. However, how they managed to keep their farming dates in synchrony with the solar year is not known. In this paper, we show that the observation of sunrise against the Basin's eastern horizon could have provided an accurate solar calendar and that some important sunrise landmarks coincide well with the themes of seasonal festivities described in early codices. We also show that a long stone causeway in the summit of Mount Tlaloc aligns perfectly with the rising sun on February 23 to 24, in coincidence with the Basin's new year in the Mexica calendar. Third, we demonstrate that, when viewed from the sacred Mount Tepeyac in the bottom of the Basin, sunrise aligns with Mount Tlaloc also on February 24. The importance of Mount Tlaloc as a calendric landmark seems to be corroborated by illustrations and texts in ancient Mexica codices. Our findings demonstrate that by using carefully developed alignments with the rugged eastern horizon, the inhabitants of the Basin of Mexico were able to adjust their calendar to keep in synchrony with the solar year and successfully plan their corn harvests.

The success of market deregulation in low- and middle-income countries depends on the strength of price and non-price competition between firms. In this paper, we study the recently deregulated retail gasoline market in Mexico. During our sample period, nearly 650 new gasoline stations entered the market. We estimate the causal effect of entry on the prices and quality of incumbent firms. We find that the entry of a nearby station decreases markups by nearly 4% for regular gasoline and about 2% for premium gasoline and diesel. We validate these results using the structure of ownership in the market, showing near zero impacts when the incumbent and entrant have the same owner. In addition, we show that the effect of competition on markups attenuates with distance and driving time. We find no evidence that entry affects the quality of existing stations, as measured by online ratings and regulatory inspections.

Spain adopted the Constitution of Cádiz in 1812 as a response to the regime of Joseph Bonaparte, which deposed King Ferdinand VII and inspired dissent throughout Spain. Fondly known as La Pepa, the new Spanish constitution would prove short lived—but long influence the course of history and political theory. Indeed, the Constitution of Cádiz was the first truly liberal European document of the kind—drawing on Rousseau, Locke, and Voltaire, it enumerated universal male suffrage, a constitutional monarchy and democratic parliamentary body, and certain social rights previously restricted in largely closed European states. Though the Constitution of Cádiz would crumble by 1814, the immediate influence of this document was felt by New Spain, which would draft its own document and declare the Mexican Federation in 1824. This paper explores the causal link between these two events, applying theory from Rawls, Polanyi, Mill, and The Federalist Papers to determine how each document differs, where parallels emerge, why each failed to last, and how the lessons from Spain and colonial dissent encouraged Mexico to federate. By offering a textual comparison of each document and weaving in anecdotes from history, this paper provides a robust assessment of two quintessential documents for modern political theory and liberal thought in both Europe and Latin America.

As the fourth contribution in the 'Land' section, this paper forms a research 'diptych' with the next paper by Levy. Whereas she focuses on the notarial institution in mid-nineteenth century Mexico, this contribution examines it in a contemporary context. The notary is one of the chief components of property rights protection in civil-law systems, performing various public functions such as writing deeds for real estate property. Yet notaries are considered an 'inefficient' institution by many, due to the perception of rent-seeking behavior enabled by their near-monopoly over validating property rights claims. This study examines notaries in Mexico to unpack the apparent contradiction in the role of notaries in economic development. I use a combination of interviews with notaries and clients, and data on notarial practice and bureaucratic outcomes across the country, to examine notaries' social function. The theoretical lens of endogenous development and institutional functionalism reveals an alternate explanation for their seemingly high-cost services, as well as their role in economic development. Mexican notaries have a dual social function: public representative and private service provider. They perform diverse and essential activities, which in other countries are performed by multiple actors such as real estate agents, escrow offices and title insurance companies. Thus, what is perceived as inefficiency by some can be interpreted as an efficient response to the context in which they operate, and their semi-privatized nature can overcome problems found in other bureaucratic arrangements.

Social life in Mexico’s state of Michoacán is consumed by a crisis of violence. Foregrounding critical planning, this paper presents a grounded local history of the municipality of Tancítaro, Michoacán, which has the largest concentration of avocado production globally, and analyzes violence there in light of the production of space, uneven development, and the spatial politics of land. This quantitative and archival research, coupled with theoretical explanations on violence, suggests that considerations of crises and planning require situated analyses with ethnographic methods and embedded fieldwork that cross geographic scales and disciplinary boundaries as they foreground perspectives of affected community residents.

This paper analyses the construction of the hegemonic methods for the evaluation and representation of sustainable development and their translation into non-Western habitats. The concept Sustainable Orientalism pursues to examine the adaptation and translation of contemporary dominant discourses, methods and representations that shape the idea of a sustainable development in cities and regions around the world, and their translation to growing economies of non-Western societies. A correlation of Orientalism and sustainable development determines that the study and knowledge of non-Western environments by advanced assessment frameworks do not merely reproduce the outlying territories: it works them out, or animate them, using narrative techniques, and historical and exploratory attitudes of scientific ideas generated in the West. The paper questions the pursuit of environmental justice in the 21st century based on the distortion and degradation of knowledge that is implied in the exercise of a Sustainable Orientalism.

ObjectiveThe present paper describes the methodology and sample characteristics of the Neuropsychological Norms for the U.S.-Mexico Border Region in Spanish (NP-NUMBRS) Project, which aimed to generate demographically-adjusted norms for a battery of neuropsychological tests in this population.MethodsThe sample consisted of 254 healthy Spanish-speakers, ages 19-60 years, recruited from the U.S.-Mexico border regions of Arizona (n = 102) and California (n = 152). Participants completed a comprehensive neuropsychological test battery assessing multiple domains (verbal fluency, speed of information processing, attention/working memory, executive function, learning and memory, visual-spatial skills and fine motor skills). Fluency in both Spanish and English was assessed with performance-based measures. Other culturally-relevant data on educational, social, and language background were obtained via self-report. Demographic influences on test performances were modeled using fractional polynomial equations that allow consideration of linear and non-linear effects.ResultsThere were no significant demographic differences between participants tested in Arizona and California. Age and gender were similar across education ranges. Two thirds of the sample were Spanish dominant and the remainder were considered bilingual. Individual articles in this Special Issue detail the generation of demographically adjusted T-scores for the various tests in the battery as well as an exploration of bilingualism effects.DiscussionNorms developed through the NP-NUMBRS project stand to improve the diagnostic accuracy of neuropsychological assessment in Spanish-speaking young-to-middle-aged adults living in the U.S.-Mexico border region. Application of the present norms to other groups should be done with caution.

ObjectiveThis paper summarizes the findings of the Neuropsychological Norms for the U.S.-Mexico Border Region in Spanish (NP-NUMBRS) Project and offers a roadmap for future research.MethodsThe NP-NUMBRS project represents the largest and most comprehensive co-normed neuropsychological battery to date for native Spanish-speaking healthy adults from the U.S. (California/Arizona)-Mexico borderland region (N = 254; ages 19-60 years). These norms provide demographic adjustments for tests across numerous domains (i.e., verbal fluency, processing speed, attention/working memory, executive function, episodic memory [learning and delayed recall], visuospatial, and fine motor skills).ConclusionsThis project: 1) shows that the NP-NUMBRS norms consistently outperformed previously published norms for English-speaking non-Hispanic (White and African-American) adults in identifying impairment; 2) explores the role of Spanish-English bilingualism in test performance; and 3) provides support for the diagnostic validity of these norms in detecting HIV-associated neurocognitive impairment. Study limitations include the limited assessment of sociocultural variables and generalizability (e.g., other Latina/o populations, age limit [19 - 60 years]). Future research is needed to: 1) investigate these norms with U.S.-dwelling Spanish-speakers of non-Mexican heritage and other clinical subpopulations; 2) expand coverage of cognitive domains (e.g. language, visuospatial); 3) develop large normative datasets for children and older Latina/o populations; 4) examine how sociocultural factors impact performance (e.g., bilingualism, acculturation); 5) investigate these norms' diagnostic and ecological validity; and 6) develop norms for neurocognitive change across time. It is hoped that the NP-NUMBRS norms will aid researchers and clinicians working with U.S.-dwelling Spanish-speakers from the U.S.-Mexico borderland to conduct research and evidence-based neuropsychological evaluations in a more culturally responsive and ethical manner.

The monopoly of violence in the hands of the state is conceived as the principal vehicle to generate order. A problem with this vision is that parts of the state and its law enforcement apparatus often become extensions of criminality rather than solutions to it. We argue that one solution to this dilemma is to “opt out from the state.” Using a multi-method strategy combining extensive qualitative research, quasi-experimental statistical analyses, and survey data, the paper demonstrates that indigenous communities in Mexico are better able to escape predatory criminal rule when they are legally allowed to carve a space of autonomy from the state through the institution of “usos y costumbres.” We demonstrate that these municipalities are more immune to violence than similar localities where regular police forces and local judiciaries are in charge of law enforcement and where mayors are elected through multiparty elections rather than customary practices.

This paper is a checklist of the fishes that have been documented, through both published and unpublished sources, in marine and estuarine waters, and out 200 miles, from the United States-Canadian border on the Beaufort Sea to Cabo San Lucas, Mexico. A minimum of 241 families and 1,644 species are known within this range, including both native and nonnative species. For each of these species, we include maximum size, geographic and depth ranges, whether it is native or nonnative, as well as a brief mention of any taxonomic issues.

Purpose: This paper summarizes the findings of a research project aimed at benchmarking the environmental sustainability practices of the top 500 Mexican companies. Design/methodology/approach: The paper surveyed the firms with regard to various aspects of their adoption of environmental sustainability practices, including who or what prompted adoption, future adoption plans, decision-making responsibility, and internal/external challenges. The survey also explored how the adoption of environmental sustainability practices relates to the competitiveness of these firms. Findings: The results suggest that Mexican companies are very active in the various areas of business where environmental sustainability is relevant. Not surprisingly, however, the Mexican companies are seen to be at an early stage of development along the sustainability "learning curve". Research limitations/implications: The sample consisted of 103 self-selected firms representing the six primary business sectors in the Mexican economy. Because the manufacturing sector is significantly overrepresented in the sample and because of its importance in addressing issues of environmental sustainability, when appropriate, specific results for this sector are reported and contrasted to the overall sample. Practical implications: The vast majority of these firms see adopting environmental sustainability practices as being profitable and think this will be even more important in the future. Originality/value: Improving the environmental performance of business firms through the adoption of sustainability practices is compatible with competitiveness and improved financial performance. In Mexico, one might expect that the same would be true, but only anecdotal evidence was heretofore available. © Emerald Group Publishing Limited.

This paper evaluates correlates of trichomoniasis among female sex workers who inject drugs (FSWIDs) in two Mexico-US border cities. HIV-negative FSWIDs aged 18 years or older were enrolled in a study between 2008 and 2010 in Tijuana and Ciudad Juarez (Cd.), Mexico. All participants underwent a baseline interviewer-administered survey and did a rapid test for trichomoniasis. Using regression to estimate prevalence ratios, we examined sociodemographics, sex work characteristics, sexual health and behavior, substance use, and police and violence exposures as potential correlates of trichomoniasis. Of 584 women (284 in Tijuana, 300 in Cd. Juarez), prevalence of trichomoniasis was 33.6%. Factors associated with trichomoniasis in multivariable analysis were having money stolen by police in the past six months (adjusted prevalence ratio [aPR] =1.448, 95% confidence interval [CI] = 1.152-1.821), recent methamphetamine use (aPR = 1.432, CI = 1.055-1.944), lifetime syphilis infection (aPR = 1.360, CI = 1.061-1.743), ever use of a home remedy to treat vaginal symptoms (aPR = 1.301, CI = 1.027-1.649), and number of regular clients in the past month (aPR = 1.006 per client, CI = 1.004-1.009), while controlling for age and city of interview. Alongside the need for trichomoniasis surveillance and treatment programs, findings indicate that both structural and behavioral factors serve as primary correlates of trichomoniasis among FSWIDs in these cities.

This paper utilizes eighteen months of ethnographic and interview research undertaken in 2003 and 2004 as well as follow-up fieldwork from 2005 to 2007 to explore the sociocultural factors affecting the interactions and barriers between U.S. biomedical professionals and their unauthorized Mexican migrant patients. The participants include unauthorized indigenous Triqui migrants along a transnational circuit from the mountains of Oaxaca, Mexico, to central California, to northwest Washington State and the physicians and nurses staffing the clinics serving Triqui people in these locations. The data show that social and economic structures in health care and subtle cultural factors in biomedicine keep medical professionals from seeing the social determinants of suffering of their unauthorized migrant patients. These barriers lead clinicians inadvertently to blame their patients--specifically their biology or behavior--for their suffering. This paper challenges the focus of mainstream cultural competency training by showing that it is not the culture of the patient, but rather the structure and culture of biomedicine that form the primary barriers to effective multicultural health care.

This paper utilizes eighteen months of ethnographic and interview research undertaken in 2003 and 2004 as well as follow-up fieldwork from 2005 to 2007 to explore the sociocultural factors affecting the interactions and barriers between U.S. biomedical professionals and their unauthorized Mexican migrant patients. The participants include unauthorized indigenous Triqui migrants along a transnational circuit from the mountains of Oaxaca, Mexico, to central California, to northwest Washington State and the physicians and nurses staffing the clinics serving Triqui people in these locations. The data show that social and economic structures in health care and subtle cultural factors in biomedicine keep medical professionals from seeing the social determinants of suffering of their unauthorized migrant patients. In the spring of 2006, for the first time ever, tens of thousands of Mexican citizens living in the U.S. received absentee ballots to vote for the next president of Mexico, crowning a major transformation in Mexican policy. This paper, published in advance of the 2006 election, explores this form of transnational migrant empowerment as a major step in the redefinition of the terms of Mexican nationality and citizenship which allows for permanent residence abroad and even de facto dual citizenship and binational civic participation—and explores how the new policy may open a new dimension of minority politics in the United States. The arrival of distance voting in Mexico’s elections is part of a larger response to the rise of a new network of Mexican immigrant leaders, activists, and organizations in the U.S.—a network that the Mexican government helped develop and which is distinct from established U.S. Latino leadership. Both the U.S. and Mexico have repeatedly altered their basic policies toward the migration of Mexicans to the United States. This background paper places Mexico’s policy today against the backdrop of the various ends it has pursued, the means it has employed, and the results it has achieved over time. It examines the evolving civic orientation of both Mexican immigrants and U.S.-born Mexican Americans, touches on the role of issues such as guest worker programs, and pays special attention to the role of the Institute of Mexicans Abroad (Instituto de los Mexicanos en el Exterior—IME), a major new government agency.

BackgroundType 2 Diabetes (T2D) is now a massive epidemic in both California and Mexico, with serious consequences for social and economic well-being. A large proportion of these populations share common ethnic backgrounds. Yet diverse environmental and social conditions across regions create unique opportunities to explore the ways that T2D risk, incidence, management and outcomes manifest.Main textAn action-oriented research consortium headed up by the University of California and Universidad Nacional Autónoma de Mexico was constituted to set priorities for bi-national translational research, in an attempt to implement and evaluate clinical, public health and policy actions to decrease the burden of T2D for people of Mexican origin. In this paper, we describe the epidemiology of T2D in Mexico and California, review current efforts to combat the epidemic, highlight gaps in knowledge and identify urgent areas of opportunity for collaboration. The group has developed a common research agenda and funding has been obtained to evaluate biological samples from the 2016 Mexican Health Survey, collaborate in a telemedicine-based retinopathy project, implement interventions in food banks, promote a communications campaign, and design a large-scale diabetes prevention effectiveness trial.ConclusionsT2D has caused a state of emergency in Mexico and is a major health problem among Mexican populations on both sides of the border. Understanding the commonalities and differences between California and Mexico for those of Mexican origin with respect to T2D, when combined with a sharing of knowledge and advances, can produce a bi-national translational research agenda to inform relevant policy and practice. Amidst economic and political uncertainty and limited healthcare budgets, this collaboration can contribute to the development of scientific evidence to inform policies and interventions. This may provide a promising collaborative model that could be expanded to other health conditions and regions of the world.

This paper is the result of discussions at the 2003 Meeting of the Latin American Studies Association, in Dallas, Texas, March 27-29, 2003, where we organized and presented papers for a panel entitled “Women, Ethnicity, and Medical Authority: Reproductive and Children’s Health in Latin America since 1780.” While the papers focus on distinct regions and historical moments in Latin America, they all engage common themes. Each examines how “officials” representing various elite institutions—the Church, formal medicine, and the state—used cultural explanations of race and difference to construct new notions of “reproductive health” and “mothering” as measures of “civilization.” Moreover, these papers show how such constructions, which took place during moments of sweeping transformation in colonial and post-colonial Latin America, were used to justify a variety of forms of social discrimination, exclusion, and inclusion.Adam Warren discusses ways in which Catholic priests used different types of gender ideals based on ethnic differences, notions of maternal intuition, and religious and moral sensibilities to advocate the practice of cesarean sections on indigenous Andean women in Peru. Alexandra Puerto examines the material causes of poor public health, focusing on how the organization and structure of plantation economies and labor creates high infant mortality rates in rural communities, specifically a henequen plantation in Yucatán, Mexico. Tamera Marko examines ways white upper class slave-owner parents, black slave wet nurses and black male folkhealers in Rio de Janeiro, Brazil, used the same ideals of motherhood and maternal instincts, as well as images of barbaric and “unfit” mothers, to negotiate their “fitness” and thus, their rights to be a mother, to be “free”, to be a citizen.This work also contributes to broader historiographies about Catholic pastoral campaigns, rural insurrection, independence, abolition, family, childhood, labor, and rural capitalist transition. It also sheds light on the historical origins of many debates about reproductive health, race, and citizenship, which continue to shape public policy in Latin America today.

Using ethnographic data collected in three agricultural communities in the states of Tlaxcala and Puebla, this paper explores the intersection of institutional change and climatic uncertainty in the production process and livelihood decisions of small-scale farm households. The research focuses on household response to the neoliberal reforms in Mexico’s agricultural sector of the 1990s and to the series of climatic events that were experienced in central Mexico in 1997, 1998 and 1999. These latter events have been associated with an observed increase in the occurrence of the El Niño-Southern Oscillation phenomenon. The experience of smallholder farmers with these two parallel processes of exogenous change serves as a rough analogue of the more abstract processes of economic globalization and climatic change. The paper argues that ultimately a farm household’s vulnerability to climatic changes will depend on the series of livelihood decisions the household takes, from season to season, over the course of decades. Those decisions, in turn, are not in any way a simple product of climatic factors, but rather are the result of what Kelly and Adger have called “the architecture of entitlements, the social, economic and institutional factors that influence levels of vulnerability within a community or nation and promote or constrain options for adaptation” (Kelly and Adger 2000: 326). Documenting adaptation in practice to concrete events allows us to see the difficult competition between non-climatic and climatic risks in decision-making and the important role of institutions in structuring adaptation capacity.

This paper examines health care utilization among indigenous immigrants from Oaxaca, Mexico, who have settled in a farmworker community in southern California. In 2016, two trained Spanish-Mixteco and Spanish-Zapoteco bi-lingual interviewers conducted in-depth interviews with 44 indigenous women residing in Oxnard, California on issues that affect health care utilization. Interviews were conducted in Mixteco, Zapoteco and Spanish and were coded to identify structural, cultural, and provider-related barriers to health care utilization. Five bi-lingual Spanish-Mixteco indigenous interpreters employed at local clinics were also interviewed. Many women reported lack of health insurance, inability to pay, language barriers, long waiting times, rushed encounters with providers, and seeking western medical care only after home remedies did not work. However, several women were able to access routine health care services, often with support from indigenous interpreters employed at clinics. Interviews with five interpreters found that they provided assistance with interpretation during medical encounters and appointment making. They also educated patients about upcoming exams, identified low-cost services and insurance programs available to patients, assisted with paperwork and occasionally educated physicians on behalf of their patients. In addition to addressing barriers to health care access our findings suggest the importance of identifying and leveraging community assets, such as indigenous navigators, when developing programs for such underserved communities. Our findings can inform best practice in settings that provide health care to indigenous populations and may also apply to settings that provide health care to other immigrant communities that have very limited familiarity and contact with western health care.

Mexico's recent energy reform (2013) has provided the foundations for increased private participation in attempts to offset or reverse the country's continued decline in fossil fuel production. This country is currently on path to becoming a net energy importer by 2020. Conversely, in 2015, and for the first time in over 20 years, the United States (US) became a net oil exporter to Mexico. One of the strategies being pursued by Mexico to prevent an impending supply–demand energy imbalance is the development of shale resources using horizontal drilling and hydraulic fracturing techniques. Hence, an evaluation of the inherent risks associated with hydraulic fracturing is crucial for Mexico's energy planning and decision-making process. This paper draws lessons from the recent 'shale boom’ in the US, and it analyzes and summarizes the environmental, social, economic, and community impacts that Mexico should be aware of as its nascent shale industry develops. The analysis seeks to inform mainly Mexican policy makers, but also academics, nongovernmental organizations, and the public in general, about the main concerns regarding hydraulic fracturing activities, and the importance of regulatory enforcement and community engagement in advancing sustainability. We highlight that Mexico should only develop its unconventional resources after careful evaluation of all potential impacts and after the formulation of regulation intended for their mitigation. Furthermore, using the US as a case study, we argue that development of unconventional oil and gas resources in Mexico could lead to a short-term boom rather than to a dependable and sustainable long-term energy supply. Our analysis concludes with a set of recommendations for Mexico, featuring best practices that could be used to attenuate and address some of the impacts likely to emerge from shale oil and gas development.

Microbialites are modern analogs of ancient microbial consortia that date as far back as the Archaean Eon. Microbialites have contributed to the geochemical history of our planet through their diverse metabolic capacities that mediate mineral precipitation. These mineral-forming microbial assemblages accumulate major ions, trace elements and biomass from their ambient aquatic environments; their role in the resulting chemical structure of these lithifications needs clarification. We studied the biogeochemistry and microbial structure of microbialites collected from diverse locations in Mexico and in a previously undescribed microbialite in Cuba. We examined their structure, chemistry and mineralogy at different scales using an array of nested methods including 16S rRNA gene high-throughput sequencing, elemental analysis, X-Ray fluorescence (XRF), X-Ray diffraction (XRD), Scanning Electron Microscopy-Energy Dispersive Spectroscopy (SEM-EDS), Fourier Transformed Infrared (FTIR) spectroscopy and Synchrotron Radiation-based Fourier Transformed Infrared (SR-FTIR) spectromicroscopy. The resulting data revealed high biological and chemical diversity among microbialites and specific microbe to chemical correlations. Regardless of the sampling site, Proteobacteria had the most significant correlations with biogeochemical parameters such as organic carbon (Corg), nitrogen and Corg:Ca ratio. Biogeochemically relevant bacterial groups (dominant phototrophs and heterotrophs) showed significant correlations with major ion composition, mineral type and transition element content, such as cadmium, cobalt, chromium, copper and nickel. Microbial-chemical relationships were discussed in reference to microbialite formation, microbial metabolic capacities and the role of transition elements as enzyme cofactors. This paper provides an analytical baseline to drive our understanding of the links between microbial diversity with the chemistry of their lithified precipitations.

ObjectiveThe overall aim of this paper is to examine effectiveness of an evidence-based intervention in community settings, and the factors associated with effectiveness. Limited research in the area of HIV prevention has focused on evaluating intervention program effectiveness in real-world settings.MethodsWe implemented an efficacious theory-based sexual risk reduction intervention for female sex workers (FSW) called Mujer Segura across 13 different clinics in 13 sites across Mexico. The overall design was a cluster randomized Type I design simultaneously testing intervention program effectiveness with an observational study of implementation factors. We aimed to examine the effectiveness of Mujer Segura at reducing HIV/STI incidence among FSW participants at each site, and to examine the client-, provider-, organization-, and structure-related factors associated with program effectiveness.ResultsWe found lower HIV/STI incidence density in the intervention relative to the control group in 5 sites we labeled as "program effective sites," but not in 8 sites we labeled as "program ineffective sites." Using generalized estimating equations controlling for site and computed mean difference effect sizes, we examined statistically and practically significant differences, respectively, between the two groups of sites along various client-, provider-, organization-, and structure-related characteristics. Results indicated that client-level HIV/AIDS related knowledge, and proficiency and engagement in the organizational social context were associated with program effectiveness.ConclusionsEnormous resources are required to systematically and adequately test the role of multilevel factors on program effectiveness. We successfully implemented Mujer Segura in 13 sites in Mexico. Results suggest that other measures may need to be included in future implementation studies than the ones included here. We were able to point to a few specific factors that should be targeted to increase effectiveness of similar evidence-based programs in low- and other middle-income countries like Mexico.Trial registrationClinicalTrials.gov NCT01465607.

This working paper examines the relationship between the transformation of labor markets and the role of immigrant workers in a regional context. It analyzes the participation of Mexican immigrants in the labor markets of the San Francisco Bay Area during the 1980s, using data from the Bureau of the Census (Public Use Microdata Samples). In order to analyze employment trends, the paper compares the performance of three groups: native-born, Asian immigrants, and Mexican immigrants.The study focuses on tow Bay Area counties that experienced a very high influx of immigrants during the 1980s: Santa Clara and Contra Costa counties. Santa Clara County is home to the Silicon Valley, the most successful high-technology region in the world. The labor demands of Silicon Valley have attracted both highly educated and unskilled immigrants. Contra Costa County, with a history of using Mexican-origin agricultural labor, has recently seen the rapid growth of such sectors as finance, insurance, and real estate that have produced new labor demands.The results that the constant demand for immigrant labor, the formation of “daughter communities” , and the implementation of immigration policies have led to the consolidation of the Bay Area as an ensemble of fragmented ethnic communities that have secured access to distinct niches in the labor market. Asian immigrants, who seem to follow the employment patterns of native-born workers, find jobs in the most dynamic sectors of the regional economy, while most Mexican immigrants obtain jobs in more traditional sectors such as construction, agriculture, and personal services. Some highly educated Mexican immigrants have also responded to the labor demands created by the development of the high technology sector in Silicon Valley.

Compared to HIV research on men who have sex with men, less is known about the risks and vulnerabilities for HIV among Male to Female (MTF) transgender persons, particularly in different geographic regions like Mexico. In Tijuana, Mexico, a border city experiencing a dynamic HIV epidemic, no precedent data exists on the MTF transgender population. Our aims were to estimate HIV prevalence and examine the behaviors and characteristics of the population. We conducted a cross-sectional study of 100 MTF transgender persons recruited through time location sampling in 2012. Participants underwent interviewer-administered (paper and pen) surveys and rapid tests for HIV. Descriptive univariate analyses were conducted on various factors, including sociodemographics, substance use, accessing social services (requested vs. received), stigma, and sex behaviors. A total of 22% tested positive for HIV, a prevalence higher than other key populations at risk for HIV in Tijuana.

This paper examines the relationship between regional development and labor migration to the United States in the context of NAFTA. To this end, mainly through ethnographic work, the migration experience of people from Tlacuitapa, Jalisco is analyzed to see whether or this flow can be reduced through the implementation of NAFTA.The paper develops two principal arguments. First, the current migration process between Mexico and the Untied states is not only the result of push-pull economic factors, as is generally assumed, but also the result of well-developed social networks and the implementation of government policies in both Mexico and the United States as manifested by the formation of a number of “transnational communities,” like Tlacuitapa. The term transnational community describes rural Mexican communities that specialize in the production and reproduction of international migrant workers. This observation leads to a second and related argument: the additional job creation resulting from NAFTA would not necessarily stem the international migration flows in regions with a long tradition of migration to the United States.Although manufacturing jobs have been created in a city near Tlacuitapa, the migration flows has not been affected in this community. Tlacuitapa is compared to other communities in the Los Altos de Jalisco region that have successfully stemmed migration flows to the United States. These cases reveal that international migration can be reduced in transnational communities by facilitating the establishment of small business and cooperatives. However, NAFT might cause an apposite effect by affecting these small-scale enterprises negatively.The case of Tlacuitapa suggests that the creation of transnational communities is an important aspect of the integration between Mexico and the United States. Since labor mobility across national borders like international trade is part of the consolidation of a global economy, migration from Mexico should be part of the agenda in negotiations between the two countries. Since NAFTA by itself will not play a significant role in deterring emigration from traditional sending areas, labor mobility should be addressed in the mutual interests of people from both countries; otherwise NAFTA will become another pipe dream of immigration deterrence like the Immigration Reform and Control Act of 1986.

Drawing on ethnographic fieldwork between 2013 and 2016, this paper investigates emerging forms of hacking and entrepreneurial development in Mexico. I show how research participants attend hackathons and hone their coding skills at co-working spaces in Mexico City and in Xalapa, as they hack away to build solidarity and find the “coding bliss,” the affective dimension one encounters when creating beautiful code. As hacker-entrepreneurs tease out the tensions between self-making and being-made, they fill an overarching neoliberal agenda with substance, meaning, and materiality. For young people in Mexico, “hacking” emerges as a way to make sense of their future livelihoods in a precarious state and economy, as a way to exist in a system where things just don’t seem to work, and as a way to let the “code work” intervene in narratives that have only delivered false hopes. As hackathons continue to proliferate across the globe, I conclude by examining how the underlying logics of software design, such as “loose coupling,” become fundamental for the re-organizing of social relations in Mexico.

The broader purpose of this paper is to contribute to an awareness of the cultural and social role of African descent peoples in the history of late colonial and nineteenth century Mexico. I specifically focus on the ethnic relations between African descent peoples and Mayans in communities of the Yucatán peninsula during the 1847 to 1901 guerras de casta or Caste Wars. This period offers unique insights to interethnic relations in a region and time where Indigenous peoples maintained autonomy from western powers, which influenced the parameters of the dynamic interactions between African descent and Native American peoples. I utilize quantitative and qualitative sources such as census data, court documents, newspaper articles, and ethnographic interviews that depict the social, political, and cultural context of the Yucatán peninsula. This essay reveals that Mayan and Afro-descendent trends originating in the colonial era continued into the nineteenth and early twentieth centuries. Thus, at no point in Mexican history did the influence of Afro-Latin American culture ever end. Instead, Afro-descendent peoples continue to contribute to Mexican cultures in coeval and interrelated courses of development with Native American and European cultural influence. Moreover, nineteenth century global migration trends contributed to even more diverse influences on Mexican cultural history.

This paper examines the variation in the value of property rights to housing in Mexico, focusing specifically on differences between urban housing markets. Roughly 30% of owner-occupied houses in Mexico do not have a proper deed. Houses with no deed are estimated to be five percent less valuable than otherwise similar houses with a full deed, yet this premium varies widely across cities. I match data from the 2012 and 2014 National Survey of Household Incomes and Expenditures to different sources of city-level data in order to examine hypotheses explaining this variation in a multilevel regression framework. I find that deeds are valued more in cities with more highly educated residents, more political competition, and more voting. Measures of local economic activity, degree of informality, and the regulatory bureaucracy are not associated with higher value to full property rights. Additionally, I find that more educated households value deeds more, and having a deed is more valuable for larger houses in neighborhoods with less vacancy and higher infrastructure quality. Based on these results, I suggest funds to subsidize titling should be redirected to places where titles are worth more. More broadly, I suggest policymakers reconsider framing property-titling programs as poverty alleviation. Low-income households would benefit more from subsidies for improvements to housing and residential infrastructure. At the same time, the federal government should further push to reduce the costs of transferring property.

BackgroundAs geospatial data have become increasingly integral to health and human rights research, their collection using formal address designations or paper maps has been complicated by numerous factors, including poor cartographic literacy, nomenclature imprecision, and human error. As part of a longitudinal study of people who inject drugs in Tijuana, Mexico, respondents were prompted to georeference specific experiences.ResultsAt baseline, only about one third of the 737 participants were native to Tijuana, underscoring prevalence of migration/deportation experience. Areas frequented typically represented locations with no street address (e.g. informal encampments). Through web-based cartographic technology and participatory mapping, this study was able to overcome the use of vernacular names and difficulties mapping liminal spaces in generating georeferenced data points that were subsequently analyzed in other research.ConclusionIntegrating low-threshold virtual navigation as part of data collection can enhance investigations of mobile populations, informal settlements, and other locations in research into structural production of health at low- or no cost. However, further research into user experience is warranted.
