This book explores the transdisciplinary approach to general surgery for the frail patient, promoting the use of a geriatric model of care in general surgical settings and thus proposing a “gerosurgery” approach in frail persons beyond chronological age. Aging, frailty, and surgical needs are increasing dramatically worldwide while surgeons are daily facing with the trouble of the trade-off between short-term outcomes and efficacy of surgery in older persons. However, recent studies have clearly shown that age itself is not a prognostic risk factor for complications after elective surgery in older patients, whereas cognitive or functional frailty is. Neither a referring physician nor an assessing surgeon should deny patients surgery purely based on chronological age. Instead, decisions should be based on a CGA (comprehensive geriatric assessment) with a precise picture of the patient considering the cognitive, functional, nutritional, socioeconomic, and affective status. Recent studies examining the use of preparative CGA in surgical patients showed encouraging results on postoperative outcomes in old and/or frail patients. The most compelling current evidence comes from the orthopaedic community, which has embraced the idea of multidisciplinary team care. Preoperative evaluation, postoperative care, pain control, nutritional support, delirium prevention, mobilization, and rehabilitation are necessary also in general surgery. An expert multidisciplinary team, including geriatricians, anaesthesiologists, specialist nurses, physiotherapists, and nutritionists should mandatorily support surgeons.With this book, the reader will have an exhaustive overview and will gain essential, practical knowledge on how best to manage the surgical and frail patient and how to develop clinical systems that do so reliably.

This open access book explores a new conceptual framework for the sustainable management of the boreal forest in the face of climate change. The boreal forest is the second-largest terrestrial biome on Earth and covers a 14 million km2 belt, representing about 25% of the Earth's forest area. Two-thirds of this forest biome is managed and supplies 37% of global wood production. These forests also provide a range of natural resources and ecosystem services essential to humanity. However, climate change is altering species distributions, natural disturbance regimes, and forest ecosystem structure and functioning. Although sustainable management is the main goal across the boreal biome, a novel framework is required to adapt forest strategies and practices to climate change. This collaborative effort draws upon 148 authors in summarizing the sustainable management of these forests and detailing the most recent experimental and observational results collected from across the boreal biome. It presents the state of sustainable management in boreal forests and highlights the critical importance of this biome in a context of global change because of these forests'key role in a range of natural processes, including carbon sequestration, nutrient cycling, and the maintaining of biodiversity. 46. Maintenance of Plant Genetic Resources : Towards an Understanding of Seed Deterioration

Conservation of biodiversity including plant genetic resources are fundamental for the future life on Earth. To safe-guard crop wild relatives, locally adapted landraces and varieties, ex situ genebanks were established at the beginning of the 20th century; primarily to store seeds. However, as any other material on Earth, seeds age and lose viability when stored for prolonged periods. The main factors determining deterioration processes are the genotype, the environmental conditions during seed development and the storage conditions including relative humidity, temperature, gas composition and pressure. To understand physiological, biochemical and genetic changes during seed deterioration in genetic resources of wheat, barley and oilseed rape, fundamental processes of germination, dormancy and seed viability loss are discussed in this book based on 13 scientific publications. Here, the loss of seed viability was investigated depending on seed moisture contents (MCs) between 5% and >25%, storage temperatures between 0°C and 45°C and a modified atmosphere with increased O2 concentration (75%) or increased atmospheric pressure (18 MPa). Although, the response to the different conditions varied among species, overall, elevating seed MCs and storage temperatures led to a gradual change of biochemical mechanisms during seed deterioration. Under dry seed storage conditions, glutathione (GSH) and tocochromanols functioned as low-molecular-weight antioxidants and were degraded. Lipids were oxidised or hydrolysed, the pH decreased, whereas organic radicals accumulated over time and correlated negatively with seed viability. At high seed MCs (>13%) and storage temperatures (>40 °C), membrane damages, changes of pH or organic radicals were absent. Furthermore, GSH and GSSG depleted whereas tocochromanols remained stable or increased. Therefore, seeds stored under high MCs and temperatures were exposed to a different environment than seeds exposed to lower MCs and temperatures where water activity and deterioration rate were reduced and the cytoplasm was assumed to be glassy. As a consequence, quantitative trait loci (QTLs) varied between ageing treatments; thus to understand genetic mechanisms of seeds deterioration in seedbanks, studies must be carried out on dry-stored seeds. Overall, our plant genetic resources are safely preserved in genebanks. However, to avoid genetic drift and to prolong the life of the seed, the conditions for pre-storage and storage should be further optimised and improved for each individual species.
