Thyreologie – interdisziplinär! so lautete das Thema der 23. Konferenz über die menschliche Schilddrüse, die vom 12. bis 14. Oktober 2017 in Heidelberg stattfand. Die Behandlung von Patienten mit Schilddrüsen-Erkrankungen steht weltweit wieder einmal vor einem durchgreifenden Wandel. Strumen und Schilddrüsenknoten gehörten über lange Zeit in Iodmangelgebieten wie Deutschland fast zum Normalbefund. Dieser „Epidemie“ folgte ein Anstieg der Operationsfrequenz mit all ihren Komplikationen. Ähnliche Entwicklungen zeigen sich auch in der histologischen Einschätzung von Schilddrüsentumoren. Papilläre Tumore wurden – unabhängig von ihrer biologischen Bedeutung – prinzipiell als papilläre Karzinome eingestuft und entsprechend therapiert. Seit diesem Jahr wird jedoch z. B. die follikuläre Variante des papillären Mikrokarzinoms der Schilddrüse nicht mehr den bösartigen Tumoren zugeordnet und soll zukünftig die inadäquat aggressive Therapie dieser kleinen Tumore verhindern. Leitlinien zur Diagnostik und Therapie von Schilddrüsenerkrankungen versuchen diesen Anforderungen Rechnung zu tragen.
