Brain tumors comprise about 5–9% of all human neoplasms; and interestingly the central nervous system (CNS) neoplasms are ranked among the most prevalent neoplasms of childhood as well. Besides to the morphologic and histopathologic characteristics, and as each pathologic states first starts with molecular alterations, each tumor may have its own story in the matter of activating tumorigenesis pathways and having specific molecular characteristics. Importantly, the molecular classification of tumors has been highly considered in the past few decades for taking the most appropriate therapeutic approach. On the other hand, the tumors shall have tumor-scape mechanisms preventing the immunologic system to eliminate its invasion. The failure of innate and acquired immune system to defeat tumorigenesis mechanisms would consequently result in tumor development. Interestingly, the neuro-immunologic mechanism plays a role in development of psychiatric manifestations of brain tumors as well.Taking all these to account, the different arms of innate immunity, acquired immunity, and genetics have been approached to defeat development and/or progression of such tumors. Accordingly, the activation immunotherapeutic approaches focus on activating or strengthening the anti-tumor immunologic pathways in order to assist the weakened immune system to defeat the tumor (such as Dendritic cell vaccination, DNA vaccines, peptide vaccines, viral vector-based vaccines, monoclonal antibodies, and CAR T-cell therapy). In addition to immunologic components of brain and spinal cord tumors, numerous genes and genetic pathways have been recognized to take part in tumorigenesis. Taking these non-immune genetic pathways to account, some other therapeutic approaches such as stem cell therapy and gene therapy have been developed in the new era of cancer treatment. Moreover, and besides the biologic and medical aspects of these tumors, different physical/mathematical models have been proposed to either explain or predict tumor behavior. Such models would be advantageous in developing new therapeutic modalities in pre-clinical stages and enter new eras in cancer treatment. The first book of Human Brain and Spinal Cord Tumors, Neuro-immunology and Neuro-genetics, will mainly discuss the neuro-immunology and neurogenetic pathways associated with development of brain and spinal cord tumor. Psychoimmunology is a rapidly maturing area of scientific endeavor that provides a compelling integrative link between the immune system and its response to stress and psychiatric illness. Stress initiates pathological changes by activating the immune and endocrine systems. Inflammation is at the core of the complex and interactive systems that both contribute to and result from psychopathology. Consequently, inflammation research advances our knowledge of the pathology of depression, schizophrenia, chronic fatigue syndrome, posttraumatic stress disorder and a host of co-morbid conditions, notably diabetes, cardiovascular disease and cerebrovascular disease. The possible mechanisms underlying the bidirectionality of co-morbid medical and psychiatric disorders can be viewed as a consequence of inflammatory changes. These emerging novel concepts illustrate how the knowledge of inflammation can enable meaningful integration of psychopathology with physical co-morbidity. Neuroimmunology could be defined as the application of immunological methods to problems in neurobiology but such a definition is so all­ encompassing as to be unhelpful. It is not a precisely circumscribed discipline but it seems worthwhile at the outset to point to three of the major areas of activity. One rather early use of the term was in connection with studies on the immune response to antigens in the nervous system. This includes topics such as autoimmunity in the central and peripheral nervous sys­ tems, the response to neural tumors or viral infections, and the im­ munopathology of such processes. Although not at the forefront of the currently fashionable preoccupation with neuroimmunology, this area continues to be a vital and interesting one from both clinical and basic perspectives. A second very active area is the exploitation of antibodies to iden­ tified components of neural cells, and in particular to those molecules involved as neurotransmitters, in transmitter synthesis and breakdown, and as synaptic receptors. The immunohistochemical detection of these antigens has led to new insights into the functional organization of the nervous system, and reference to such studies is almost a sz'ne qua non for discussions of most central and peripheral synapses.

Up until approximately 20 years ago, the idea that the central nervous system (CNS) and components of the immune system were dynamically interactive was considered impossible (or at least highly unlikely) as the CNS was judged an immunosuppressive environment based upon experimental evidence highlighting the survival of tissue grafts within the brain. Additional evidence supporting this viewpoint included (i) the presence of the blood–brain barrier (BBB) which p- vides a physical and physiological obstruction that is difficult for cells and mac- molecules to cross, (ii) the relative absence of MHC class I and II expression on CNS cells like astrocytes and neurons, and (iii) lack of abundant antigen presenting cells (APC) which are required for the generation of an adaptive immune response. However, in spite of these obstacles, it is now well-accepted that the CNS is r- tinely subject to immune surveillance under both normal as well as diseased con- tions. Indeed, activated cells of the immune system such as T and B lymphocytes and monocyte/macrophages readily infiltrate and accumulate within the CNS f- lowing microbial infection, injury, or upon development of autoimmune responses directed toward resident antigens of the CNS.

It is commonly acknowledged that the nervous system and the immune system, those most complex of networks, share attributes beyond their intricacy. Elements common to the two systems include memory, connectivity, flexibility and developmental selection of cellular composition by a rigorous process involving widespread programmed cell death. There is one salient difference: the cells of the immune system are predominantly in constant motion, while post-mitotic neurons and glia are largely fixed in place. Therefore, chemokines, initially characterized as leukocyte chemoattractants, have for the last one and one-half decades been intensely and productively studied in the contexts of inflammation, immunity and hematopoietic development. Only recently have the two fields, neurobiology and immunology, displayed mutual interests in chemokines. This convergence of the two tribes of investigators was catalyzed by the finding that SDF-1 (now known as CXCL12) and its receptor, CXCR4, exerted significant and similar functions in development of both nervous and immune systems. Indeed CXCL12 and CXCR4 were required, in an uncannily similar fashion, for retention of pre-B lymphocytes at sites of maturation in the bone marrow and of neuronal progenitors in the external granule cell layer of the developing cerebellum. Recent reports indicate that chemoattraction of cerebellar granule cells through CXCR4 can be suppressed by reverse signaling initiated by binding of soluble eph receptors to transmembrane ephrin B, thereby establishing a link between chemokine action and a cardinal patterning system of the developing nervous system.
