Health systems across the world face multiple pressures. Input costs are soaring, systems are struggling to keep up with increasing demand for their services and areas of the world still lack universal health coverage. All of this whilst health inequalities between the best and worst-off within countries persist and, in some countries, are even widening. There is a need to think of new initiatives in response to these global health challenges. One such response is social finance. Social finance is about creating social returns. This innovative and rapidly growing sector promotes new ways of banking and funding social and public services. However, social finance has an under-recognised, and potentially underexploited, role in responding to specific aspects of global health challenges: funding and facilitating access to health(care) services and acting on health. The objectives of this book are to conceptualise and evidence different forms of social finance - microfinance and impact bonds - acting in these ways and to critically engage with current debates and challenges. With such evidence to hand, we can either avoid adoption of new trends in financing public services or, more hopefully, attract greater policy support and resources for new tools for public health and in supporting more precarious, but potentially essential, parts of the finance sector. The firsthand pandemic experiences of rural health-care providers—who were already burdened when COVID-19 hit—raise questions about the future of public health and health-care delivery. This book explores the ethical problems of algorithmic bias and its potential impact on populations that experience health disparities by examining the historical underpinnings of explicit and implicit bias, the influence of the social determinants of health, and the inclusion of racial and ethnic minorities in data. Over the last twenty-five years, the diagnosis and treatment of disease have advanced at breakneck speeds. Currently, we have technologies that have revolutionized the practice of medicine, such as telemedicine, precision medicine, big data, and AI. Seth Holmes, an anthropologist and MD in the mold of Paul Farmer and Didier Fassin, shows how market forces, anti-immigrant sentiment, and racism undermine health and health care. Holmes was invited to trek with his companions clandestinely through the desert into Arizona and was jailed with them before they were deported. He lived with Indigenous families in the mountains of Oaxaca and in farm labor camps in the United States, planted and harvested corn, picked strawberries, and accompanied sick workers to clinics and hospitals. This “embodied anthropology” deepens our theoretical understanding of the ways in which social inequities come to be perceived as normal and natural in society and in health care. In a new epilogue, Holmes and Indigenous Oaxacan scholar Jorge Ramirez-Lopez provide a substantive update about the protagonists in the book, focusing on the ways in which they have been involved individually and collectively in movements for Indigenous immigrant rights, farmworker rights, and the right to health over the last decade.

This book highlights novel and pragmatic health promotion efforts being adopted with boys and young men of colour (BYMOC) globally that apply a strengths-based approach. Men's adoption of risky health practices and reluctance to seek help and engage in preventive health behaviours have frequently been used to explain their poorer health outcomes, particularly among adolescent boys and young men, and disproportionately affecting BYMOC. Emerging literature on equity and men's health has conveyed that intersections among age, race, sexuality, socioeconomic status and geography contribute to a complex array of health and social inequities. There is growing evidence to suggest these inequities shape the health practices of BYMOC. Unfortunately, these health and social inequities can have negative lifelong consequences. Bioethics needs an expanded moral vision. Born in the ferment of the 1970s, the field responded to rapid developments in biomedical technology and injustices in clinical care and research. Since then, bioethics has predominantly focused on respect for autonomy, beneficence and nonmaleficence, and the zero-sum'lifeboat'ethics of distributive justice, applying these principles almost exclusively within the walls of medical institutions. It is now time for bioethics to take full account of the problems of health disparities and structural injustice that are made newly urgent by the COVID-19 pandemic and the effects of climate change. Approximately 7.4 million people in the United States live with an intellectual or developmental disability (IDD), defined by the Centers for Disease Control and Prevention as'a group of conditions due to an impairment in physical, learning, language, or behavior areas. These conditions begin during the developmental period, may impact day-to-day functioning, and usually last throughout a person's lifetime.'Individuals with IDD and their caretakers face exceptional barriers to staying healthy and accessing appropriate health services. Among these barriers are difficulty finding care providers that are adequately trained in meeting their specialized needs, unwieldy payment structures, and a lack of coordination between the various systems of care with which patients with IDD may interact (e.g., education, social work, various segments of the health care system). The National Academies of Sciences, Engineering, and Medicine hosted a workshop to discuss promising innovations in (1) workforce development, (2) financing and payment, and (3) care coordination; and to share visions for improved systems of care. Participants noted that while many existing approaches could serve as models for improving care, large changes will need to be made in these 3 facets of the care system in order to make them accessible to all IDD patients. This publication summarizes the presentations and discussions of the workshop.

Each year, the individuals and organizations in the U.S. organ donation, procurement, allocation, and distribution system work together to provide transplants to many thousands of people, but thousands more die before getting a transplant due to the ongoing shortage of deceased donor organs and inequitable access to transplant waiting lists. Realizing the Promise of Equity in the Organ Transplantation System, a new consensus study report from the National Academies of Sciences, Engineering, and Medicine's Committee on A Fairer and More Equitable, Cost-Effective, and Transparent System of Donor Organ Procurement, Allocation, and Distribution, provides expert recommendations to improve fairness, equity, transparency, and cost-effectiveness in the donor organ system.

An estimated 10 percent of children in the United States are living with disabilities, including a disproportionate number of children living in poverty and children of marginalized racial and ethnic groups. During the pandemic, children with disabilities suffered disproportionately compared to their peers without disabilities. To learn more about what policies and practices might be sustained or implemented beyond the pandemic to support children with disabilities and their families, the Board on Children, Youth, and Families hosted a workshop on June 13-15, 2022. Workshop presenters included service providers, researchers, government leaders, youth with disabilities, and caregivers of children and youth with disabilities. In this workshop, practices were identified that could improve the system of care for children with disabilities as well as improve access to services for underserved and marginalized populations.

The persistent trends in suicide necessitate action among mental health care providers and payers, researchers, and community leaders. Health care settings provide an important opportunity for suicide intervention and prevention, but they cannot yet fully manage suicide risk because of a lack of training, knowledge gaps, and reimbursement challenges. School, workplace, and community-based interventions can help reduce the incidence of suicidal behavior, as can better access to care and reduced access to lethal means of suicide. To better understand the strategies to improve access to effective interventions to prevent suicide, the Forum on Mental Health and Substance Use Disorders at the National Academies of Sciences, Engineering, and Medicine hosted a two-part virtual public workshop, Strategies and Interventions to Reduce Suicide, on June 22, 2021, and July 28, 2021. The first webinar examined the scope of the public health problem, discussed implementation of effective approaches for suicide prevention care, and addressed known barriers to health care access. The second webinar focused on building 9-8-8, the new nationwide emergency number designated to the National Suicide Prevention Lifeline. Participants discussed current crisis systems, gaps, challenges, and needs for marginalized populations. This Proceedings of a Workshop summarizes the presentations and discussions that occurred during the workshops.
