AbstractPeople living in rural areas of Australia are significantly disadvantaged, compared to metropolitan dwelling peers, in accessing basic support services. Australians with intellectual disabilities living in rural areas are at specific risk of further disadvantage. This article examines the findings and recommendations of the Disability Royal Commission through the lens of rurality. A frequency analysis of key words relating to rurality indicated an interesting trend in relation to the term “remote” which had a higher count rate than either “regional” or “rural”. This finding may indicate that issues from non-remote rural locations were less well represented. The Final Report included many examples from people with intellectual disabilities or their carers regarding the lack of vital services in rural areas, and specifically highlighted the need to introduce a “provider of last resort”. Problems with support were also identified following domestic violence and rape, and concerns noted regarding the provision of public transport in rural areas. The Final Report contains 222 recommendations, of which 11 contained a specific focus addressing a rural issue, and these are briefly discussed. It is unfortunate that, in some key support areas, there appear to be an implicit expectation that the remaining 200+ recommendations will all apply evenly across Australia, irrespective of location. Nonetheless, the Disability Royal Commission should be commended for their inclusive approach to involving people with disabilities living in rural areas which led to the reporting from rural people with intellectual disabilities and their carers regarding the issues that they have faced.

AbstractChanging disease patterns and improved life expectancy have resulted in a growing cohort of older Australians with an intellectual disability, with the provision of end-of-life care to this group only recently emerging as a priority area. Particularly in rural settings, where the availability of both specialist and general local services may be limited, end-of-life care support remains under-explored. The current project aimed to specifically examine end-of-life care provision in rural areas, the focus of this article being individual quality-of-life during the end-of-life stage. The study used a focus group interview model with 22 rural disability support workers from either New South Wales or Queensland, who had direct experience in providing end-of-life care. An advisory committee, composed of people with intellectual disabilities, carers, and service providers, offered overall project guidance. Participants elaborated on factors that contributed to quality-of-life during end-of-life care. Verbatim transcripts of the focus groups were thematically analysed by the team, and three thematic categories identified: availability of services; individual needs; and untreated pain. Specifically, participants noted concerns about the unavailability of health services, inflexibility of funding support, artificial government barriers, and a widespread lack of pain relief for individuals. We conclude that end-of-life support people with intellectual disabilities in rural areas has to trade off the ongoing delivery of quality-of-life activities against the increasing need for health care, which itself is subject to accessibility issues, and which is exacerbated by the more general funding challenges seen across all locations with the National Disability Health Insurance implementation.

Background: Research has identified that rurally located persons face health inequities when compared to their urban peers. While health policies and practice frameworks to address the needs of rural people are largely developed on the principles of evidence-based medicine, it is not clear whether this evidence base appropriately considers geographic factors that may impact participant outcomes. The purpose of this project was to examine whether research within leading health and medical journals identified the geographic classification (location) of their participants, thus enabling the original authors and subsequent readers to appropriately consider this factor in the making of policy and practice recommendations. Methods: A retrospective analysis of study participants’ geographic identifiers within articles from medical and health journals was carried out in this project. Eleven journals were chosen for evaluation, with 300 consecutive eligible papers from each journal retrospectively reviewed. All 3300 papers were analyzed to determine whether the research participants’ geographic location was identified. This classification was then stratified into varying categories of rural or urban, as appropriate. Results: It was not possible to identify participants’ geographic location in 2193 (66%) of the 3300 reviewed articles. A total of 121 papers (4%) had sole focus on rural residents, with another 95 articles (3%) comparing outcomes between rural and urban locations. Slightly more than a quarter (27%) of the articles either just featured urban participants or made no distinction between rural and urban locations. Conclusions: These findings indicate that insufficient attention is given to the geographic location of participants. This failure means that the outcomes and recommendations of the research base may be invalid for a large section of the world’s population and result in inequities in healthcare services. There is an emerging concern about using this evidence base for developing health guidelines and policy frameworks, as the needs of a substantial proportion of the population may not have been appropriately captured.

ABSTRACTPrecision medicine approaches to depression argue for the assessment of individual client profiles of the two major diagnostic criteria of depressed mood and anhedonia. To explore this suggestion, 1,513 males and 1,348 females aged between 55 and 87 years from Newcastle, Australia, completed the Center for Epidemiological Studies-Depression Scale (CESD). Total CESD score and the two sets of scores gradually increased with age; females had significantly higher scores on total CESD and the two subsets of scores. Anhedonia showed greater prevalence than depressed mood and also increased from age 70 years, whereas depressed mood remained fairly stable until age 80 years. Treatment efficacy for depression may benefit from consideration of individual client profiles on these two aspects of depression.

Social support is proposed as a coping mechanism against anxiety and depression amongst older persons, but few data have examined how this occurs. This study assessed the contributions of two sub-components of social support as mediators against psychological distress—broadly defined as anxiety and depression. 1,560 men and 1,758 women from the Hunter Community Study (Australia) completed the Duke Social Support Scale and the Kessler Psychological Distress Scale. The Duke Social Support Scale examined the amount of social interaction and satisfaction with social interactions. Significant mediating effects of social support were found in the Kessler Psychological Distress Scale items measuring depression but not anxiety. Satisfaction with social support was a significant predictor of Kessler Psychological Distress Scale total score and Sadness items, but the amount of social support was not a predictor of stress. Social support may assist with symptoms of depression, i.e., specific sadness/depressed mood, but not necessarily with anxiety. Implications for policy and service delivery were discussed.

This article uses a descriptive case study design to examine the potential of narrative therapy as a direct intervention for adults with moderate-to-severe intellectual disabilities, autism and/or severe communication limitations. Archival clinical data on four individuals who received a form of social constructionist narrative therapy are examined for goal attainment. The data were analysed qualitatively with specific input from individuals, their families and carers. Findings indicate improvements in quality of life through reductions in situational and environmental anxieties, and in coping with grief and loss. The results suggest that narrative therapy techniques can be beneficial in assisting individuals with severe intellectual disability to achieve meaningful and persistent improvements in their life.
