This research focuses on the analysis of onomasiological variation in Old English texts written by Ælfric; more specifically, I am interested in the study of the different motifs that shape the linguistic expressions of shame and guilt used by this Anglo-Saxon monk across different textual genres. Through the fine-grained analysis of the whole set of shame and guilt expressions recorded in the entire corpus of Ælfrician texts, a network of literal and figurative conceptualizations for each emotion is proposed here. Based on this network, I have reconstructed and analysed patterns of conceptual variation in Ælfric’s English in order to show the existing tension between literal, metonymic and metaphoric expressions for these two emotions. As shall be seen here, the introduction in Anglo-Saxon England of Augustinian psychology by Ælfric and other highly educated authors favoured (i) the progressive neglect of the Germanic concept of shame and guilt as instruments of social control, (ii) the dissemination of new shame-related values, and (iii) the growing use of a new set of embodied conceptualizations for the two emotions under scrutiny here, most of which have become common figurative expressions of shame and guilt in later varieties of English. The new expressions (e.g., SHAME IS SOMETHING COVERING A PERSON, GUILT IS A BURDEN) illustrate the shift towards a progressive embodiment of the new emotional standards brought by Christianization. According to these standards, rather than an external judgment or reproach, shame and guilt involve a negative evaluation of oneself.

Currently, there is a growing interest in exploring the underlying processes that explain how and why therapeutic approaches work. This study aims to investigate whether Mindfulness-Based Cognitive Therapy (MBCT) can enhance direct experience compared to a control intervention. 72 women diagnosed with eating disorders were assigned to either MBCT or a control treatment. Pre- and post-intervention, participants were asked to eat an orange slice and write down thoughts about the eating experience, classifying the focus of each experience as either experiential or analytical. Compared with the control group, participants who underwent MBCT showed a statistically significant increase in direct experience.

The aim of colonic stenting with self-expandable metallic stents in neoplastic colon obstruction is to avoid emergency surgery and thus potentially reduce morbidity, mortality, and need for a stoma. Concern has been raised, however, about the effect of colonic stenting on short-term complications and long-term survival. We compared morbidity rates after colonic stenting as a bridge to surgery (SBTS) versus emergency surgery (ES) in the management of left-sided malignant large-bowel obstruction. This multicentre randomised controlled trial was designed with the endorsement of the European Association for Endoscopic Surgery. The study population was consecutive patients with acute, symptomatic malignant left-sided large-bowel obstruction localised between the splenic flexure and 15 cm from the anal margin. The primary outcome was overall morbidity within 60 days after surgery. Between March 2008 and November 2015, 144 patients were randomly assigned to undergo either SBTS or ES; 29/144 (13.9%) were excluded post-randomisation mainly because of wrong diagnosis at computed tomography examination. The remaining 115 patients (SBTS n= 56, ES n= 59) were deemed eligible for analysis. The complications rate within 60 days was 51.8% in the SBTS group and 57.6% in the ES group (p= 0.529). Although long-term follow-up is still ongoing, no statistically significant difference in 3-year overall survival (p= 0.998) and progression-free survival rates between the groups has been observed (p= 0.893). Eleven patients in the SBTS group and 23 in the ES group received a stoma (p= 0.031), with a reversal rate of 30% so far. Our findings indicate that the two treatment strategies are equivalent. No difference in oncologic outcome was found at a median follow-up of 36 months. The significantly lower stoma rate noted in the SBTS group argues in favour of the SBTS procedure when performed in expert hands.

Although language-family specific traits which do not find direct counterparts outside a given language family are usually ignored in quantitative phylogenetic studies, scholars have made ample use of them in qualitative investigations, revealing their potential for identifying language relationships. An example of such a family specific trait are body-part expressions in Pano languages, which are often lexicalized forms, composed of bound roots (also called body-part prefixes in the literature) and non-productive derivative morphemes (called here body-part formatives). We use various statistical methods to demonstrate that whereas body-part roots are generally conservative, body-part formatives exhibit diverse chronologies and are often the result of recent and parallel innovations. In line with this, the phylogenetic structure of body-part roots projects the major branches of the family, while formatives are highly non-tree-like. Beyond its contribution to the phylogenetic analysis of Pano languages, this study provides significative insights into the role of grammatical innovations for language classification, the origin of morphological complexity in the Amazon and the phylogenetic signal of specific grammatical traits in language families.

INTRODUCTION: The Colonoscopy Satisfaction and Safety Questionnaire based on Patient experience (CSSQP) was recently developed and validated within a Bowel Cancer Screening Program. We aimed to identify factor related to patient experience through the CSSQP, including all indications for colonoscopy. Indicators of satisfaction and perceived safety with colonoscopy were also assessed to compare the different centers.

In clear contrast with this view of shame and guilt as instruments of social control, the Christianization of England implied the spread of new shame-related values and the growing use of a new set of embodied conceptualizations for the two emotions under scrutiny here, most of which have become common figurative expressions of shame and guilt in later varieties of English. The new expressions (e.g. SHAME IS REDNESS IN THE FACE and SHAME IS SOMETHING COVERING A PERSON) illustrate the shift towards a progressive embodiment of the new emotional standards brought by Christianization. According to these standards, rather than an external judgment or reproach, shame and guilt involve a negative evaluation of oneself. Furthermore, I argue here that these onomasiological changes are informing us on the lexical choices of Old English speakers and on the sociolinguistic factors that conditioned the development of new emotional styles (i.e., the different ways feelings were expressed and, surely, felt) in Anglo-Saxon England.

ABSTRACTThe aim of this study was to determine the diagnostic usefulness of quantification of the H. pylorigenome in detection of infection in patients with upper gastrointestinal bleeding (UGB). A total of 158 consecutive patients with digestive disorders, 80 of whom had clinical presentation of UGB, were studied. The number of microorganisms was quantified using a real-time PCR system which amplifies the urease gene with an internal control for eliminating the false negatives. A biopsy sample from the antrum and corpus of each patient was processed. The rapid urease test, culture, histological study, stool antigen test, and breath test were done. The gold standard was a positive culture or positive results in at least two of the other techniques. When a positive result was defined as any number of microorganisms/human cell, the sensitivity of real-time PCR was greater in bleeding patients, especially in the gastric corpus: 68.4% (95% confidence interval [CI], 52.3 to 84.5%) in non-UGB patients versus 91.5% (95% CI, 79.6 to 97.6%) in UGB patients. When a positive result was defined as a number of microorganisms/human cell above the optimal value that maximizes the Youden index (>3.56 microorganisms/human cell in the antrum and >2.69 in the corpus), the sensitivity and specificity in UGB patients were over 80% in both antrum and corpus. Our findings suggest that some bleeding patients with infection caused by H. pylorimay not be correctly diagnosed by classical methods, and such patients could benefit from the improved diagnosis provided by real-time PCR. However, the clinical significance of a small number of microorganisms in patients with negative results in classical tests should be evaluated.

There was a need for a solid asthma guideline in Mexico to update and unify asthma management. Because high-quality asthma guidelines exist worldwide, in which the latest evidence on asthma management is summarized, the ADAPTE approach allows for the development of a national asthma guideline based on evidence from already existing guidelines, adapted to national needs.
