Background: Degenerative disease of the lumbar spine is one of the most prevalent pathologies worldwide, and MRI is the gold standard imaging modality that helps to assess soft tissue and bony abnormalities and elicit causes of neural compression. It is not uncommon in the daily practice to have patients presenting with neurological symptoms during standing or walking while MRI fails to detect lesion that explains their clinical picture. The aim of this study was to detect changes that appear on dynamic weight-bearing MRI of the lumbar spine that was hidden on conventional supine MRI and to correlate them with the clinical situation. Methodology: Ninety patients with back pain were enrolled in the study, they did conventional and dynamic MRI of the lumbar spine. We compared findings in both modalities as regard alignment, ligamentum flavum buckling, foraminal narrowing and neural compression. Results: Dynamic MRI showed neural compression in 87% of patients in comparison to supine MRI, ligamentum flavum buckling was reported in 80%, neural foraminal narrowing was seen in 24%, exaggeration of the lumbar lordoisis in 10% and exaggeration of the already existing disc protrusion was documented in 60% of the included patients. Conclusions: Upright dynamic MRI has added to supine MRI in problem solving cases with clinical radiological mismatch. The results indicated changes in the spine alignment, neural compression and spinal canal stenosis. Since the examination is non-invasive, it could be used in the preoperative planning of patients with degenerative lumbar spine disease. [ABSTRACT FROM AUTHOR]

Objectives: Endotracheal intubation is most commonly taught and performed in the supine position. Recent literature suggests that elevating the patient's head to a more upright position may decrease peri-intubation complications. However, there is little data on the feasibility of upright intubation in the emergency department. The goal of this study was to measure the success rate of emergency medicine residents performing intubation in supine and non-supine, including upright positions.Methods: This was a prospective observational study. Residents performing intubation recorded the angle of the head of the bed. The number of attempts required for successful intubation was recorded by faculty and espiratory therapists. The primary outcome of first past success was calculated with respect to three groups: 0-10° (supine), 11-44° (inclined), and ≥45° (upright); first past success was also analyzed in 5 degree angle increments.Results: A total of 231 intubations performed by 58 residents were analyzed. First pass success was 65.8% for the supine group, 77.9% for the inclined group, and 85.6% for the upright group (p=0.024). For every 5 degree increase in angle, there was increased likelihood of first pass success (AOR=1.11; 95% CI=1.01-1.22, p=0.043).Conclusions: In our study emergency medicine residents had a high rate of success intubating in the upright position. While this does not demonstrate causation, it correlates with recent literature challenging the traditional supine approach to intubation and indicates that further investigation into optimal positioning during emergency department intubations is warranted. The authors discuss the findings of their research into the assessment of the cause of lateral hindfoot pain in flatfoot deformity. They compared assessments of patients with flatfoot deformity who either had no pain or experienced lateral hindfoot pain. They determined that weightbearing multiplanar imaging was a reliable method of assessing arthrosis and impingement in patients with flatfoot deformity and lateral pain.

PURPOSE: The purpose of this study is to investigate the outcome and stability of combined surgical anti orthodontic treatment of severe maxillary anterior teeth and dentoalveolar bone protrusion. METHODS: Twenty-eight adults with severe bimaxillary protrusion were included. Presurgical orthodontic treatment coordinated axis of upper anteriors and dentoalveolar bone. Maxilla was impacted and set back by Le Fort I osteotomy and maxillary anterior segmental osteotomy, while upper anteriors and dentoalveolar bone were uprighted and retracted. Post-surgical orthedontic treatment detailed the occlusion. Each patient underwent X ray examination at five time points including pre-treatment, pre-surgery, one month after surgery, three months after surgery and post-treatment. Hard tissue and soft tissue changes were investigated by cephalometric analysis. The data were analyzed by paired t tests with SPSS12.0 software package. RESULTS: It was observed that upper anteriors were uprighted and retracted (11.77±9.9)° (P<0.001) after treatment while dentoalveolar bone was uprighted (6.24±3.9)°(P<0.001). UI-LI angle increased (19.4±8.6)°(P<0.001), UI-Apog distance decreased (6.62±2.2)mm (P<0.001). The thickness of alveolus on anterior teeth labial side showed no change (P>0.05). The treatment outcome demonstrated a significant improvement in soft tissue profile. Nasolabial angle increased (11.06±11.8)° (P<0.001), upper and lower lips moved backword, and pog' moved forward. Most measurements showed no significant difference from surgery time to post treatment. CONCLUSIONS: Combined surgical and orthodontic treatment is a very effective procedure in treatment of severe maxillary anterior teeth and dentoalveolar bone protrusion and gummy smile. It remains the thickness of alveolus on anterior teeth labial side. It shows big improvement in soft tissue profile and great stability. Chest radiography is the first line of thoracic imaging performed in patients with thoracic diseases. It is probably the most frequently performed type of X-ray examination. It is recommended to be performed in the full upright position except where the patient's condition will not permit. This is because the erect technique allows full expansion of the lungs, prevents engorgement of pulmonary vessels and also helps in fluid level evaluation.
