Zusammenfassung: Das Dermatofibrosarcoma protuberans (DFSP) gehört zu den seltenen Tumoren der Haut, stellt aber gleichzeitig das häufigste kutane Sarkom dar. Es handelt sich um einen Tumor von fibroblastischer Differenzierung, der langsam, unterminierend und lokal destruierend wächst, aber nur selten metastasiert. Das klinische Bild ist insbesondere bei initialen Befunden oft uncharakteristisch, weshalb die Diagnose in der Regel erst spät und meist durch Biopsie und Histologie gestellt wird. Die Therapie der Wahl ist die komplette chirurgische Exzision des Tumors mit einem Sicherheitsabstand von 2–3 cm. Bei geringeren Sicherheitsabständen besteht ein erhöhtes Risiko für die Ausbildung von Lokalrezidiven. Inoperable oder metastasierte DFSP können bestrahlt werden; die Strahlensensitivität des DFSP gilt als hoch. Nachdem für ca. 90% aller DFSP eine charakteristische Chromosomentranslokation (17:22), einhergehend mit einer Genfusion, beschrieben wurde, die zur Ausbildung einer kontinuierlichen autokrinen Wachstumsstimulation über den PDGFβ-Signalweg führt, existiert die zusätzliche Möglichkeit einer molekular zielgerichteten Therapie. Die erste hierfür zugelassene Substanz ist der Multikinaseinhibitor Imatinib, der in klinischen Studien an ausgedehnten oder metastasierten DFSP eine Remissionsrate von ca. 70% zeigte.

Zusammenfassung: Molekular zielgerichtete Therapien nehmen zunehmend mehr Raum innerhalb der verschiedenen Therapieoptionen für schwerere Erkrankungen, insbesondere für maligne Tumoren, ein. Die zugehörigen Substanzen sind in der Regel hochspezifisch gegen eine oder mehrere molekulare Zielstrukturen gerichtet, unterbinden oder modifizieren auf diese Weise die an diese Zielstruktur angebundenen molekularen Signalwege und führen zu tief greifenden Veränderungen von Proliferation, Aktivierungsstatus und Interaktion der Zielzellen. Diese Eingriffe in hoch spezialisierte molekulare Signalwege haben in der Regel auch sehr spezifische Nebenwirkungen zur Folge, die sich häufig an der Haut der behandelten Patienten manifestieren und so für den Dermatologen ein gänzlich neues Spektrum von Arzneimittelnebenwirkungen aufzeigen. Dieser Übersichtsbeitrag behandelt die kutanen Nebenwirkungen von molekular zielgerichteten Substanzen im Rahmen der Psoriasistherapie sowie im Rahmen der antitumoralen Therapie. Beide Behandlungsfelder umfassen eine Vielzahl von zielgerichteten Therapeutika; behandelt werden für die Psoriasistherapie die Substanzen Alefacept, Efalizumab, Etanercept, Infliximab und Adalimumab; für die Therapie maligner Tumoren die Substanzen Imatinib, Sorafenib, Sunitinib, EGFR-Antagonisten sowie CTLA-4-Antagonisten.

Das Merkelzellkarzinom ist ein seltener, hoch aggressiver Hauttumor des älteren Menschen. Er besitzt neuroendokrine Eigenschaften. Die Pathogenese ist mit chronischer UV-Licht-Exposition und dem Merkelzell-Polyomavirus assoziiert. Klinisch fällt das Merkelzellkarzinom durch einen solitären, kutan oder subkutan gelegenen, roten bis bläulichen Knoten auf. Da der Tumor frühzeitig lymphogen metastasiert, sind zum Zeitpunkt der Diagnosestellung in etwa 30 % der Fälle bereits lokoregionäre Metastasen vorhanden. Die häufigen Lokalrezidive sowie die regionäre und ferne Metastasierung zeigen sich meist innerhalb der ersten 2 bis 3 Jahre nach Erstdiagnose. Die erste Therapie nach Diagnosestellung besteht in der vollständigen operativen Entfernung des Primärtumors mit weitem Sicherheitsabstand sowie einer Sentinellymphknotenbiopsie. Anschließend sollte eine adjuvante Radiatio des Tumorbetts erfolgen. Durch eine zusätzliche Bestrahlung der regionären Lymphknotenstationen kann die Rate lokoregionärer Rezidive gesenkt werden. Für die systemische Therapie des fortgeschrittenen Merkelzellkarzinoms haben sich gegen die PD-1/PD-L1-Achse gerichtete Checkpoint-Inhibitoren als gut und dauerhaft wirksam erwiesen. Die vormals häufig eingesetzte Chemotherapie zeigt hingegen zwar mäßige bis gute Ansprechraten, diese sind jedoch in der Regel von sehr kurzer Dauer. Merkel cell carcinoma is a rare, highly aggressive skin tumor with neuroendocrine features found in older people. The pathogenesis is associated with immunosuppression, chronic UV light exposure and the Merkel cell polyomavirus. Clinically, Merkel cell carcinoma presents as a solitary, cutaneous or subcutaneous, red to bluish node. Due to early lymphogenic metastasis, locoregional metastases are already present in approximately 30% of cases at the time of diagnosis. The frequent local recurrences as well as the regional and distant metastases usually appear within the first 2–3 years after the initial diagnosis. The first treatment after diagnosis consists of complete surgical removal of the primary tumor with wide safety margins as well as a sentinel lymph node biopsy. Subsequently, adjuvant irradiation of the primary site should be performed. By additional radiotherapy of the regional lymph node stations, the rate of locoregional recurrence can be reduced. For systemic therapy of advanced Merkel cell carcinoma checkpoint inhibitors targeted against the PD-1/PD-L1 axis have proven to be highly and durably effective. In contrast the formerly frequently used chemotherapy shows moderate to good response rates but they are as a rule very short-lived.

Background:This randomized phase III trial was designed to demonstrate the superiority of autologous peptide-loaded dendritic cell (DC) vaccination over standard dacarbazine (DTIC) chemotherapy in stage IV melanoma patients.

Background:The onset and persistence of cutaneous lichen sclerosus et atrophicus (LSA) are linked to the presence of an inflammatory infiltrate of CD3+ T cells that includes CD4+ and CD8+ cells. The functional relevance of the presence of these cells is unknown. Objective:The study intended to quantify resting and activated cytotoxic T cells in LSA lesions. Methods:Twenty patients with active LSA were studied. Skin-infiltrating T cells were immunohistologically characterized with antibodies against CD3, CD8, T-cell-restricted intracellular antigen (TIA-1) and granzyme B (GrB). TIA-1 labels cytotoxic granules of resting and activated T cells, whereas GrB designates activated cytotoxic T lymphocytes (CTL). Results:In all cases, numerous T cells were consistently found expressing cytotoxic granules. The results indicated a high number of infiltrating CD8+ TIA+ T cells. Furthermore, a notable number of GrB+ activated CTL associated with hydropic degeneration of the basal cell layer were found within the dermal infiltrate and at the dermoepidermal interface. Conclusion:This study shows that a high proportion of skin-infiltrating T cells in LSA has a potential cytotoxic function. The results indicate that hydropic degeneration of basal keratinocytes may at least partially be mediated by CTL-dependent mechanisms. Our data also indicate that a cell-mediated immune response may play an important role in the pathogenesis of the disease.

It has been suggested that extracellular mRNA might be released from tumor cells and might be protected from serum RNase by proteins or proteolipid complexes. Our group has recently assessed the potential value of extracellular RNA detection by RT-PCR in the peripheral blood of patients with metastatic melanoma.

We report a 67-year-old woman with mycosis fungoides (MF) who was receiving subcutaneous injections of recombinant interferon α-2b (IFN-α). After 2 months of IFN-α treatment, bullous lesions appeared on her trunk and extremities. A skin biopsy specimen from the trunk revealed histopathologic features of bullous MF. Bullous lesions with specific infiltrates of MF are very rare; to the best of our knowledge, this is the first case showing specific bullous lesions of MF developed under IFN-α treatment.

Background: Bioflavonoids and ascorbic acid have been shown to increase capillary resistance and to mediate potent antioxidative radical scavenging activities. Objective: We evaluated the clinical effect of oral bioflavonoids and ascorbic acid in patients with chronic progressive pigmented purpura (PPP). Methods: In an open pilot study, oral rutoside (50 mg twice a day) and ascorbic acid (500 mg twice a day) were administered to 3 patients with chronic PPP. Results: At the end of the 4-week treatment period, complete clearance of the skin lesions was achieved in all 3 patients. No adverse reactions were noted. All patients remained free of lesions at the end of 3 months after treatment. Conclusion: Our results suggest a beneficial effect of bioflavonoids in combination with ascorbic acid on PPP. Because the disease is mostly resistant to other treatment modalities, placebo-controlled studies are necessary to determine the usefulness of this therapy in PPP. We report on the successful therapy of rapid progressive generalized vitiligo in a 24-year-old woman. After intravenous injection of 500 mg of methylprednisolone on 3 consecutive days, the disease progression was stopped and repigmentation was induced. The treatment was repeated monthly and after 6 cycles, repigmentation was achieved on vast parts of most vitiligo lesions. The treatment was well tolerated by the patient, and no side effects were noted. We conclude that high-dose glucocorticoid pulse therapy may represent a therapeutic alternative in selected patients with generalized rapid progressive vitiligo.
