1. Academic-Practice Partnership to Strengthen Nursing Care of Older Adults.

Abstract

A long-standing academic-practice partnership was leveraged to facilitate student learning opportunities pertaining to care provision for older adults living with multiple chronic conditions and complex medical problems. Students from a gerontological nursing course in an accelerated baccalaureate nursing program were partnered with gerontology-educated population health nurses in primary care settings. Students observed how population health nurses integrated the Institute for Healthcare Improvement Age-Friendly 4Ms framework into clinical practice as they performed behavioral, psychosocial, and biometric health risks assessments for older adults during their Medicare annual wellness visit. The population health nurses served as role models for professional delivery of age-friendly care including preventative health and wellness care. Student confidence and perception of their understanding of age-friendly and gerontological nursing care improved. Post clinical experience debrief sessions and clinical reflection assignments demonstrated students' admiration of the expansive role and person-centered approach that population health nurses undertake to ensure comprehensive assessment and wellness promotion. Students appreciated the fluidity of population health nurses' conversation regarding the things that matter most to older adults with complex medical conditions. • An academic practice partnership provided a clinical experience for ABSN students. • ABSN students partnered with gerontological educated population health nurses for a day. • The students and mentors completed annual Medicare wellness exams for older adults. 3. Relative motion orthoses for early active motion after finger extensor and flexor tendon repairs: A systematic review.

Abstract

• Systematic review of RM approach for finger tendon repair rehabilitation. • Outcomes of over 500 extensor tendon (ET) repairs, predominantly in zone V and VI. • ET evidence suggests good functional outcomes; 1 tendon rupture (non-adherence). • Limited flexor tendon repair evidence with only eight patient outcomes reported. • Further research required for ET zones IV and VII and flexor tendon repairs. The relative motion (RM) orthosis was introduced over 40 years ago for extensor tendon rehabilitation and more recently applied to flexor tendon repairs. We systematically reviewed the evidence for RM orthoses following surgical repair of finger extensor and flexor tendon injuries including indications for use, configuration and schedule of orthosis wear, and clinical outcomes. Systematic review. A PRISMA-compliant systematic review searched eight databases and five trial registries, from database inception to January 7, 2022. The protocol was registered prospectively (CRD42020211579). We identified studies describing patients undergoing rehabilitation using RM orthoses after surgical repair of acute tendon injuries of the finger and hand. For extensor tendon repairs, ten studies, one trial registry and five conference abstracts met inclusion criteria, reporting outcomes of 521 patients with injuries in zones IV-VII. Miller's criteria were predominantly used to report range of motion; with 89.6% and 86.9% reporting good or excellent outcomes for extension lag and flexion deficit, respectively. For flexor tendon repairs, one retrospective case series was included reporting outcomes in eight patients following zones I-II repairs. Mean total active motion was 86%. No tendon ruptures were reported due to the orthosis not protecting the repair for either the RME or RMF approaches. Variation was seen in use of RME plus or only , use of night orthoses and orthotic wear schedules, which may be the result of evolution of the RM approach. Since Hirth et al's 2016 scoping review, there are five additional studies, including two RCTs reporting the use of the RM orthosis in extensor tendon rehabilitation. There is now good evidence that the RM approach is safe in zones V-VI extensor tendon repairs. Limited evidence currently exists for zones IV and VII extensor and for flexor tendon repairs. Abstract

Understanding counseling students' preferences, competence, and perceived training related to presenting concerns is important information for counselor training programs. Given the association between poverty and mental health concerns, we compared counseling students' reactions to presenting concerns often linked to poverty (e.g., financial concerns/assistance) to their reactions toward other clinical issues (e.g., gender identity development). Students' provided ratings of clinical preference for working with various presenting concerns, and concerns that may be prevalent among clients living in poverty ranked last. Additionally, we utilized a repeated measure design to examine differences in students' perceived competence and perceived training across four case vignettes depicting various presenting concerns. Our findings revealed that counseling students felt most competent and most trained to address self-growth issues as compared to poverty, substance use, or Posttraumatic Stress Disorder. 9. SIRT4 loss reprograms intestinal nucleotide metabolism to support proliferation following perturbation of homeostasis

Abstract

The intestine is a highly metabolic tissue, but the metabolic programs that influence intestinal crypt proliferation, differentiation, and regeneration are still emerging. Here, we investigate how mitochondrial sirtuin 4 (SIRT4) affects intestinal homeostasis. Intestinal SIRT4 loss promotes cell proliferation in the intestine following ionizing radiation (IR). SIRT4 functions as a tumor suppressor in a mouse model of intestinal cancer, and SIRT4 loss drives dysregulated glutamine and nucleotide metabolism in intestinal adenomas. Intestinal organoids lacking SIRT4 display increased proliferation after IR stress, along with increased glutamine uptake and a shift toward de novonucleotide biosynthesis over salvage pathways. Inhibition of de novonucleotide biosynthesis diminishes the growth advantage of SIRT4-deficient organoids after IR stress. This work establishes SIRT4 as a modulator of intestinal metabolism and homeostasis in the setting of DNA-damaging stress.

12. Relationship between Adverse Childhood Experiences, Social-Emotional Competencies, and Problem Behaviors among Elementary-Aged Children

Abstract

ABSTRACTAdverse childhood experiences (ACEs) impact both physical and emotional health throughout the lifespan and contribute to negative effects in childhood such as learning difficulties, internalizing and externalizing behavior problems, and social development. The purpose of this study was to explore the effect of the number of ACEs on children's current social-emotional assets and behavioral problems when controlling for age and gender. Participants were children (N = 58) ranging in age from 5 to 12 years old recruited from four public elementary schools in the southwestern United States. Parents of participants completed Social Emotional Assets and Resilience Scales-Parent (SEARS-P), Strengths and Difficulties Questionnaire (SDQ), and extended version of the ACEs Questionnaire developed for the current study. Results revealed that the number of ACEs was able to predict 100% of the variance in social-emotional competencies and 98% of variance in behavioral problems with age and gender having a negligible influence on both models. Children in this study who experienced a higher number of ACEs also experienced lower social-emotional competencies and a higher number of behavioral problems as reported by parents. These findings highlight the impact of ACEs on children and the opportunity to facilitate the development of children's social-emotional competencies which serve as protective factors.

13. Exploring wearable technology use and importance of health monitoring in the hazardous occupations of first responders and professional drivers

Abstract

Objectives:Hazardous materials (HAZMAT) pose risks to the health and safety of professionals involved with transportation and emergency responses. Two distinct occupational groups that encounter HAZMAT events are first responders and professional drivers. Wearable technology is a tool that can assist with monitoring the health of professionals involved in HAZMAT events. The aim of this study was to compare and evaluate the perceptions of first responders and professional drivers on wearable technology and attitudes toward health monitoring.Methods:A survey was administered to first responders (n= 112) and professional drivers (n= 218). Statistical approaches included bivariate analysis, latent class analysis, logistic regression analysis, and path analysis for the variables of interest.Results:There were significant differences between the groups in perceptions of the benefits of monitoring certain health indicators. Professional drivers were more likely to have a history of wearable technology use compared with first responders (odds ratio [OR] = 10.1; 95% CI, 4.42-22.9), reported greater exposure to HAZMAT (OR = 4.32; 95% CI, 2.24-8.32), and were more willing to have their health data monitored by someone other than themselves (OR = 9.27; 95% CI, 3.67-23.4). A multinomial regression model revealed that occupation was not a significant predictor of class preference for acceptance of monitoring specific health indicators.Conclusions:Occupation appeared to be important but further analysis uncovered that characteristics of individuals within the occupations were more salient to the use of wearable technology. HAZMAT exposure, someone else monitoring health data, and experience with wearable technology use were found to be important factors for perceptions about benefits of health monitoring with wearable technology.Key points:Wearable technology can be useful to protect health and safety of high-risk occupations. Before integrating real-time monitoring of occupational groups, perceptions and acceptability of wearable technology must be assessed. The perceptions of wearable technology for real-time monitoring differed between characteristics of individuals within occupations rather than the occupation itself. Key variables identified in this study can guide integration of wearable technology to protect the health and safety of high-risk occupations potentially exposed to HAZMAT.

Abstract

This report presents a case where vibration training to the arm alone, as opposed to whole-body vibration, was used to aid rehabilitation to a serious traumatic injury. An improvement in soft tissue extensibility to a major traumatic wound to the wrist and forearm has been noted in a therapy plan including vibration under stretch. After 12 weeks of intensive therapy, a considerable improvement was seen in both the active extension of the wrist and the composite extension of all fingers. 17. Genomic and life-history discontinuity reveals a precinctive lineage for a deep-water grouper with gene flow from tropical to temperate waters on the west coast of Australia

Abstract

We address a critical knowledge gap for the Eightbar Grouper, Hyporthodus octofasciatus, one of the largest groupers targeted in mostly tropical, deep-water fisheries, using genetic analyses of mitochondrial DNA (mtDNA; N = 248) and age and growth information (N = 727) from specimens collected from the eastern Indian Ocean to the western central Pacific Ocean. We additionally used a genotype-by-sequencing approach on a subset of these samples (N = 94) to determine the extent of egg and larval dispersal between tropical and temperate regions along the west coast of Australia (WA). Discontinuity in mtDNA and nuclear genomes, and marked variation in growth trajectories and longevity revealed a distinct, non-interbreeding cryptic lineage in WA. The WA lineage appears to be geographically isolated from the oceanic Indo-West Pacific (IP) lineage and precinctive to a single continuous coastline from tropical to temperate waters (covering > 2200 km and ∼20° of latitude). The Hyporthodusphylogeny reveals a possible allopatric speciation scenario with sister lineages endemic to the east (i.e. H. ergastularius) and west (i.e. WA lineage of H. octofasciatus) coasts of Australia, each diverging from a broader IP distributed ancestor (i.e. IP lineage of H. octofasciatus) in the early Pleistocene Epoch. Sustainable management of these species therefore needs to consider their evolutionary distinction, in addition to aspects of their life-history strategies and limited geographic distribution that infers high vulnerability to fishing at relatively low levels of exploitation.

20. Principles of tendon repair.

Abstract

Tendon injuries in the hand are common; they are usually open injuries requiring surgical intervention. In this article we discuss tendon injuries in terms of approach to repair beginning at the time of diagnosis through to the rehabilitation programmes and outcome measures commonly used in the UK. In general tendon injuries should be considered using Verdan's zones for both extensor and flexor injuries. Flexor tendons require a high strength repair and usually warrant a core suture with epitendinous reinforcement bearing in mind the importance of not disrupting glide with unnecessary suture bulk. Extensor tendons more proximally can be treated in the same way but distally require only a running suture in the flattened tendon ends. All tendon injuries require a period of protected mobilization with splinting aiming to protect the repair but reduce stiffness in other joints. [ABSTRACT FROM AUTHOR]

An improvement in soft tissue extensibility to a major traumatic wound to the wrist and forearm has been noted in a therapy plan including vibration under stretch. After 12 weeks of intensive therapy, a considerable improvement was seen in both the active extension of the wrist and the composite extension of all fingers. This may highlight the use of vibration, as an adjunct to therapy, to specific areas of the human body for improving outcome from traumatic injury.

Purpose: The purpose of this paper is to highlight theoretical and clinical similarities between therapeutic communities (TCs) and group analysis (GA). Design/methodology/approach: Literature review shows comparison of TC and group-analytic concepts with illustrative case material. Findings: Findings reveal many similarities between TCs and GA, but also significant divergences, particularly in practice. Practical implications: This paper provides theoretical basis for TC practice, and highlights the need for greater theorising of TC practice. Social implications: This paper highlights the importance of group-based treatment approaches in mental health. Originality/value: This is the first paper to review the relevant literature and compare theory and practice in TCs and GA, highlighting their common roots in the Northfields Experiments in the Second World War.

Abstract

Upon nutrient stimulation, pre-adipocytes undergo differentiation to transform into mature adipocytes capable of storing nutrients as fat. We profiled cellular metabolite consumption to identify early metabolic drivers of adipocyte differentiation. We find that adipocyte differentiation raises the uptake and consumption of numerous amino acids. In particular, branched-chain amino acid (BCAA) catabolism precedes and promotes peroxisome proliferator-activated receptor gamma (PPARγ), a key regulator of adipogenesis. In early adipogenesis, the mitochondrial sirtuin SIRT4 elevates BCAA catabolism through the activation of methylcrotonyl-coenzyme A (CoA) carboxylase (MCCC). MCCC supports leucine oxidation by catalyzing the carboxylation of 3-methylcrotonyl-CoA to 3-methylglutaconyl-CoA. Sirtuin 4 (SIRT4) expression is decreased in adipose tissue of numerous diabetic mouse models, and its expression is most correlated with BCAA enzymes, suggesting a potential role for SIRT4 in adipose pathology through the alteration of BCAA metabolism. In summary, this work provides a temporal analysis of adipocyte differentiation and uncovers early metabolic events that stimulate transcriptional reprogramming.

Abstract

The coronavirus disease 2019 (COVID-19) pandemic requires the continued development of safe, long-lasting, and efficacious vaccines for preventive responses to major outbreaks around the world, and especially in isolated and developing countries. To combat severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), we characterize a temperature-stable vaccine candidate (TOH-Vac1) that uses a replication-competent, attenuated vaccinia virus as a vector to express a membrane-tethered spike receptor binding domain (RBD) antigen. We evaluate the effects of dose escalation and administration routes on vaccine safety, efficacy, and immunogenicity in animal models. Our vaccine induces high levels of SARS-CoV-2 neutralizing antibodies and favorable T cell responses, while maintaining an optimal safety profile in mice and cynomolgus macaques. We demonstrate robust immune responses and protective immunity against SARS-CoV-2 variants after only a single dose. Together, these findings support further development of our novel and versatile vaccine platform as an alternative or complementary approach to current vaccines.
