Data on P homeostasis in insulin resistance (IR) are still conflicting. We investigated calcium-phosphate homeostasis parameters in men with/without IR. 177 volunteers (aged 61.62 ± 12.11), whose body mass index (BMI) was 29.97 ± 6.35, were studied. On fasting blood and spot urine samples, we measured serum creatinine, sodium (sNa), potassium (sK), chloride (sCl), calcium (sCa), phosphate (sP), alkaline phosphatase total activity (ALP), glucose, insulin, parathyroid hormone (PTH), 25-hydroxy-vitamin D [25(OH)D], and urinary electrolytes corrected for creatinine (uNa/Cr, uK/Cr, uCl/Cr, uCa/Cr, and uP/Cr). Through the QUICKI index, we separated subjects with (IR+, n = 68) or without (IR-, n = 109) IR, and their parameters were compared. Associations were assessed by age-adjusted partial correlation, whose coefficients were compared by Fisher's transform. IR + had higher sP (3.54 ± 0.65 vs. 3.35 ± 0.47, p = 0.044) and lower uCa/Cr levels (0.073 ± 0.056 vs. 0.095 ± 0.072, p = 0.047) than IR-. BMI correlated with sP (r = 0.21, p < 0.05) and PTH (r = 0.29, p < 0.01). QUICKI negatively correlated with sCa (r = −0.22, p < 0.05) and positively with uCa/Cr (r = 0.21, p < 0.05), in turn correlating with uNa/Cr (r = 0.45, p < 0.001). In both groups, uCa/Cr correlated with eGFR and uNa/Cr (p < 0.05 to p < 0.001). In IR + only, sP correlated with BMI, PTH with insulin, and uP/Cr (p < 0.05 for all). IR+ and IR-coefficients differed (p < 0.05 to p < 0.001) for the correlation of sP with BMI and of PTH with insulin and uP/Cr. The higher sP and lower uCa/Cr levels found in men with IR + suggest that IR could modulate calcium-phosphate homeostasis, likely by affecting their renal handling. • Men with insulin resistance (IR+) had higher sP and lower uCa/Cr than IR- ones. • In IR+ only, sP correlated with BMI, and PTH with insulin and uP/Cr. • QUICKI (index of insulin sensitivity) positively correlated with uCa/Cr. • The renal clearances of Ca and P significantly differed between IR+ and IR-. OBJECTIVE: SARS-CoV-2 causes acute respiratory disease, interstitial and alveolar pneumonia, and involves numerous organs and systems such as the kidney, heart, digestive tract, blood, and nervous system. We aimed to evaluate the incidence of renal manifestations in patients diagnosed with COVID-19 infection. PATIENTS AND METHODS: We performed a monocentric, cross-sectional, observational study, conducted on 114 patients with SARSCoV-2. Clinical and laboratory parameters [renal function, serum electrolytes, inflammatory state, blood gas analysis, Interleukin 6 (IL-6) and urinalysis] were evaluated. The same values were checked out after two months (T1), however after negativization. RESULTS: We enrolled 114 patients (59 males) with a mean age of 63.8 ± 13.9 years. We found hematuria in 48 patients (55.8%), proteinuria in 33 patients (38.4%), leukocyturia in 61 patients (70.9%), acute kidney injury (AKI) in 28 patients (24.6%), AKI in chronic kidney disease (CKD) in 24 patients (21.1%). Moreover, we found a significant increase of inflammatory indexes as C Reactive Protein (CRP), lactic dehydrogenase (LDH), alpha 1 and alpha 2 globulins with a subsequent reduction at T1 (p = 0.016, p < 0.001, p = 0.005, p = 0.007; respectively). Hemoglobin and erythrocyte values significantly decreased (p < 0.001, p = 0.003, respectively), and we found lymphopenia (p < 0.001). Also, we found elevated levels of the D-Dimer (p < 0.001) and a significant increase in the International Normalized Ratio (INR) (p = 0.038). We also showed a significant improvement after negativization in oxygen partial pressure (p = 0.001) and oxygen saturation (p < 0.001) and a significant increase in pH (p = 0.018) and bicarbonate concentration (p = 0.042). Moreover, we found a significant increase in IL-6 (p = 0.004). Also, we reported mild hyponatremia and hypokalemia with subsequent significant recovery (p < 0.001, p < 0.001, respectively) and mild hypochloremia with a recovery to the limits of statistical significance (p = 0.053). At the entrance, we found an increase in serum glucose with a significant reduction during recovery (p < 0.001). CONCLUSIONS: The prevalence of AKI and/or CKD and/or abnormal urinalysis in patients diagnosed with COVID-19 on admission seems to be high and appears as a negative prognostic factor. Urinalysis appears to be very useful in unveiling the potential kidney impairment of COVID-19 patients; therefore, urinalysis could be used to reflect and predict the disease severity. The paper explores the politics of scale associated with the top-down planning of large hydraulic infrastructures in the Tigris--Euphrates basin. Against the backdrop of a worsening water crisis and the lack of cooperation between riparian countries, dams and reservoirs across the transboundary river system are sites of contestation between competing claims over dwindling and disputed resources. Drawing on post-structuralist insights from human geography on the politics of scale and the literature on megaprojects, it is argued here that hydraulic infrastructures are materially and discursively implicated in the construction of waterscapes at different scales to sustain broader political imaginaries. Based on ethnographic fieldwork and with a focus on the autonomous Region of Kurdistan in Iraq, the analysis juxtaposes the narratives deployed by state- and non-state actors to support or counter the development of additional dams. While, on the one hand, the Kurdistan Regional Government portrays hydraulic infrastructures as a vital source of security and well-being within the overarching nationalist narrative of Kurdish self-determination, on the other hand, transnational civil society groups under the umbrella of the Save the Tigris and Iraqi Marshes Campaign have mobilized against the adverse impact of megaprojects and appealed to the common Mesopotamian heritage in order to de-escalate political tensions within the transboundary river basin. In both cases, hydraulic infrastructures provide a framework for political action to secure recognition of rights and assert the appropriate scale of governance. Furthermore, bottom-up resistance is accompanied by the promotion of a participatory and inclusive approach to shared waters. OBJECTIVE: Chronic kidney disease (CKD) and ocular disease share several cardiovascular risk factors as well as pathogenetic mechanisms having Renin-Angiotensin-Aldosterone System (RAAS) as main actor. Moreover, kidney and eyes have common genetic and embryonic origin. In this literature review, we present main evidence supporting this association for early identifying diseases affecting both systems and evaluating potential multi-target therapeutic strategies. MATERIALS AND METHODS: We performed a literature review of the current peer-reviewed English-language randomized controlled studies (RCTs), reference lists of nephrology or ophthalmology textbooks, review articles and relevant studies with ocular or eye and kidney or renal diseases as keywords until March 2020. Prospective and retrospective studies as well as meta-analyses and latest systematic reviews were included. RESULTS: We evaluated a total of 683 records, finally selecting 119 articles related to ocular and renal diseases. Records were divided into two areas: chronic and acute kidney disease and ocular or eye diseases. Some of the examined studies were discarded for population biases/intervention or deemed unfit. CONCLUSIONS: Based on our results, we conclude that there is evidence of a clear association between kidney and eye diseases, being this cross-link mainly based on RAAS dysregulation. Our review suggests that it may be useful to screen CKD patients for associated ocular diseases, such as cataract, glaucoma, diabetic retinopathy and age-related macular degeneration. A comprehensive study of CKD and proteinuric patients should include careful eye examination. Renal impairment in young patients should prompt a search for ocular disease, such as TUNA syndrome or oculo-renal syndrome, in particular if family history of concurrent ocular and renal disease is present. Objective: to evaluate the effect of therapeutic listening on people's anxiety in the immediate preoperative period. Method: a quasi-experimental and intervention research study carried out from July to October 2018 with 150 people in a Hospital from Minas Gerais/Brazil. Anxiety was assessed using the Hospital Anxiety and Depression Scale and the physiological measures. Descriptive and inferential statistical analysis and Chi-square, Spearman and Wilcoxon tests were performed. Results: of the 31 people with anxiety, 20 had a reduction in the anxiety levels and in the physiological measures mean values after the intervention. A significant relationship was found between the gender, monthly family income, previous surgical complications and significant life events variables and the anxiety measure. A weak positive correlation was found between anxiety and respiratory rate and a weak negative correlation between anxiety and age group. Conclusion: when identifying patients with anxiety, interventions such as therapeutic listening can be implemented to make the perioperative period healthier. [ABSTRACT FROM AUTHOR]

This article aims to analyze Brazilian research on statistics education involving teachers to highlight methodological trends and training contexts mobilized by such research. To this end, a survey was carried out in the Dissertation and Thesis Bank of CAPES, considering the descriptor "Statistical Education" and the 2013-2018 timeframe. A total of ninety-nine studies were found, twenty of which met the proposed objective. This is a documentary and bibliographic study, in which an interpretative approach was adopted using the technique of content analysis. The data analysis verified that nine of those studies did not lead to training processes with teachers. In addition, the analysis of the other eleven studies, which led to training, revealed an intrinsic relationship between the methodological path and the training contexts reported. This demonstrated the expressiveness of training contexts based on epistemology of collaboration, that is, those contexts that both give voice and listen to teachers and recognize them as producers of knowledge. Conversely, we identified two research studies that shaped the training considering the paradigm of technical rationality, and designed training courses to address certain content/concepts that teachers supposedly do not master. It was evidenced that the conception of teacher training has a direct and explicit relationship with the organization of the training spaces outlined. Finally, we identified one research study based on epistemology of practice, which focuses on the understanding of knowledge mobilized in teaching practice. [ABSTRACT FROM AUTHOR]

OBJECTIVE: Arterial hypertension (AH) represents a major risk factor for cardiovascular disease and is associated to several complications, such as prolonged corrected QT (QTc) interval and impaired heart rate variability (HRV). Secondary causes of AH include autosomal dominant polycystic kidney disease (ADPKD) and atherosclerotic renal artery stenosis (ARAS), both known to be related to arrhythmic risk and autonomic imbalance. The aim of the study is to evaluate whether global autonomic activity and QTc interval differently affect ADPKD and ARAS hypertensive patients. PATIENTS AND METHODS: An observational study was performed on 59 patients: 16 ADPKD patients and 19 diagnosed with ARAS, compared to 24 healthy controls (HC). All patients underwent clinical evaluation, biochemical lab tests, 24-hour electrocardiogram (ECG) and renal Doppler ultrasound. HRV was assessed through the analysis of 24-hour ECG to detect standard deviation of normal-to-normal RR intervals (SDNN). QTc interval was defined as prolonged when > 440 msec. RESULTS: SDNN was significantly lower in ADPKD and ARAS patients than HC (p < 0.0001) and no significant differences were found between ADPKD and ARAS patients (p > 0.05). QTc was found significantly higher in ARAS patients than HC (p = 0.001) and in ARAS patients than ADPKD patients (p = 0.004). CONCLUSIONS: The pathogenesis of hypertension in ADPKD and ARAS patients is related to the activation of the renin angiotensin aldosterone system (RAAS). In ADPKD, cyst enlargement leads to kidney ischemia and renin release, associated to endothelial dysfunction, low nitric oxide and sympathetic tone activation. Differently, reduction in renal perfusion pressure activates RAAS and renal adrenergic nerves in ARAS patients. We can speculate that prolonged QTc interval is more present in ARAS vs. ADPKD hypertensive patients due to a greater activation of RAAS. We suggest adding 24-hour HRV evaluation in association with traditional risk factors in course of ADPKD and ARAS hypertension to better stratify cardiovascular risk in these groups of patients. Dyspnea can be assessed using self-rating scales but, as death approaches, self-reporting becomes difficult. The validated Respiratory Distress Observation Scale measures dyspnea distress. The aim of this study was to develop the Italian version of the Respiratory Distress Observation Scale and to examine its psychometric properties. This was a cross-sectional study, analyzing cultural and linguistic validation, content validity, and psychometric properties. Eighty-nine palliative care subjects were enrolled to validate the Italian version of the Respiratory Distress Observation Scale. Patients had an average age of 74.5 (SD, 11.6) years, and 52% (n = 46) were female. Fourteen experts in palliative care evaluated the Italian Respiratory Distress Observation Scale in terms of the content validity ratio and the content validity index (CVI). The Italian Respiratory Distress Observation Scale was reported with an internal consistency (Cronbach α value) of.72 and an overall substantial interrater reliability (Cohen κ method). The Italian Respiratory Distress Observation Scale achieved a scale-level CVI of 93%, an items-level CVI of greater than 86%, and a minimum content validity ratio value of 0.71. A weak positive correlation was found between the Respiratory Distress Observation Scale and the Dyspnea Visual Analog Scale scores (0.374; P <.001). The Italian Respiratory Distress Observation Scale showed good reliability and validity for patients in palliative care. OBJECTIVE: Non-invasive positive pressure ventilation (NIV) is now an indispensable safeguard in the management of many pathologies. However, sometimes the positive end-expiratory pressure (PEEP) showed harmful effects on renal function, although effects on renal hemodynamic are unclear. We aimed at evaluating the effects of NIV on renal and endothelial function, in patients with chronic or acute respiratory failure. PATIENTS AND METHODS: We performed a longitudinal, prospective, interventional study. We enrolled 17 hospitalized and non-hospitalized patients (11 males) with indication to NIV and stable hemodynamic parameters. Patients were treated with NIV and followed up at T0, at T1 (at the end of the NIV cycle) and at T2 (fifteen days after). RESULTS: 17 patients (11 males) with a mean age of 71.94 ± 14.89 years were enrolled. A significant increase in flow mediated dilation (FMD) was found (p = 0.004). We showed a significant improvement, after NIV, in the values of pH (p = 0.0002), pCO2 (p = 0.0001), pO2 (p = 0.04), lactates (p = 0.04), sO2 (p = 0.02) and in the P/F Ratio (p = 0.004). We also showed a significant reduction of serum glucose (p = 0.01) and a significant increase of serum chlorine (p = 0.047), while we did not report a significant increase of creatinine (p = 0.297) or a significant change in diuresis. CONCLUSIONS: In our study NIV has no significant effects on renal function in patients with respiratory failure. Probably these patients required low PEEP values, which were less harmful to lung parenchyma and not effective on systemic hemodynamic. Furthermore, NIV has improved endothelial function in the short term, likely by reducing oxidative stress, as improvements of the gas-analysis parameters showed. Background and Aims: Glucagon-like Peptide 1 Receptor Agonists (GLP1-RA) has been associated with a reduction of major cardiovascular events (MACE) and mortality on the basis of the results of cardiovascular outcome trials (CVOT). Several meta-analyses on this issue have been recently published; however, they were all restricted to CVOT, with the exclusion of all studies designed for other endpoints; moreover, other cardiovascular endpoints, such as atrial fibrillation and heart failure have not been fully explored.Methods and Results: A Medline search for GLP-1 receptor agonists (exenatide, liraglutide, lixisenatide, albiglutide, dulaglutide, or semaglutide) was performed, collecting all randomized clinical trials with a duration ≥52 weeks, enrolling patients with type 2 diabetes, and comparing a GLP-1 receptor agonist with placebo or any other non-GLP-1 receptor agonist drug. We included 43 trials, enrolling 63,134 patients. A significant reduction of MACE (MH-OR 0.87 [0.83, 0.92]), all-cause mortality (MH-OR 0.89 [0.83, 0.96]), and a nonstatistical trend toward reduction of heart failure (MH-OR 0.93 [0.85, 1.01]) was observed - GLP1-RA did not increase the risk of atrial fibrillation (MH-OR 0.94 [0.84, 1.04]).Conclusion: The present meta-analysis confirms the favorable effects of glucagon-like peptide-1 receptor agonists on major cardiovascular events, cardiovascular and all-cause mortality, stroke, and possibly myocardial infarction. Conversely, the effects on heart failure remain uncertain. The genus Fusarium is noted for including important plant pathogens and mycotoxin producers. Furthermore, many Fusarium lineages have been reported to be efficient in controlling insects and to exhibit promising characteristics for agricultural pest control such as causing high mortality rates and having fast action and abundant sporulation. In this review we present a survey of peer-reviewed papers published from 2000 to 2019, demonstrating the widespread pathogenicity of Fusarium to insects. This survey was made using search strings in a number of databases. We list the major complexes and species of Fusarium reported as entomopathogenic and highlight the features of interest for insect control as well as the advantages and implications of this practice. Out of a total of forty papers, at least 30 species and 273 isolates of Fusarium were reported as pathogenic to at least one species of insect. Ten complexes of Fusarium species harbor entomopathogenic fungi, of which F. incarnatum-equiseti , F. fujikuroi , F. oxysporum and F. solani (= Neocosmospora solani) species complexes represented the most abundant number of entomopathogenic strains. The entomopathogenic interactions of these fungi have received greater attention in recent years, but much remains to be explored, especially regarding the specificity of these fungi for the host insect, the production of undesirable secondary metabolites, and side-effect and safety tests organisms not targets. Detailed investigations in this regard will be crucial if we are to fully exploit the potential of Fusarium for controlling insect pests. • Fusarium isolates can be effective insect pest control agents. • At least ten Fusarium species complexes harbor entomopathogenic fungi. • A better characterization of these strains will enable their effective application as biocontrol agents. The green areas can produce social, environmental and economic benefits. In an urban and suburban environment, green areas can work as a buffer against the detrimental impacts of lifestyle stresses, mitigate the impact of air pollution and hot waves, as well as contribute to the conservation of biodiversity. Current studies have demonstrated that people living closer to green areas receive positive effects on mental and physical well-being. The aim of this study was to gain information on healthy living and well-being in relation with green space, among urban park goers in the city of Rome. The research was performed, by administering a questionnaire. Questionnaire-based interviews include 20 questions designed to collect information on physical and recreational activities performed and their frequencies, personal feelings about the benefit and protection of the green area. Moreover, park user characteristics (sex and age) were also recorded and how much green spaces influence their life choices. A total of 102 park goers were interviewed during summer 2015: most park users were adults (18-30 years old) males (55%). The results of this study indicate that the green area has healthy results on park users in terms of daily stress reduction and well-being perception, as well as awareness of the positive effects on air quality and mitigation of events related to climate change as the heat waves. The resulting interviews provided us useful information on perception and awareness of the positive effects of green area on mental and physical health of park users. [ABSTRACT FROM AUTHOR]
