- Abstract
Understanding the health consequences associated with exposure to risk factors is necessary to inform public health policy and practice. To systematically quantify the contributions of risk factor exposures to specific health outcomes, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 aims to provide comprehensive estimates of exposure levels, relative health risks, and attributable burden of disease for 88 risk factors in 204 countries and territories and 811 subnational locations, from 1990 to 2021.

- Abstract
Detailed, comprehensive, and timely reporting on population health by underlying causes of disability and premature death is crucial to understanding and responding to complex patterns of disease and injury burden over time and across age groups, sexes, and locations. The availability of disease burden estimates can promote evidence-based interventions that enable public health researchers, policy makers, and other professionals to implement strategies that can mitigate diseases. It can also facilitate more rigorous monitoring of progress towards national and international health targets, such as the Sustainable Development Goals. For three decades, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) has filled that need. A global network of collaborators contributed to the production of GBD 2021 by providing, reviewing, and analysing all available data. GBD estimates are updated routinely with additional data and refined analytical methods. GBD 2021 presents, for the first time, estimates of health loss due to the COVID-19 pandemic.

- Abstract
Accurate assessments of current and future fertility—including overall trends and changing population age structures across countries and regions—are essential to help plan for the profound social, economic, environmental, and geopolitical challenges that these changes will bring. Estimates and projections of fertility are necessary to inform policies involving resource and health-care needs, labour supply, education, gender equality, and family planning and support. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 produced up-to-date and comprehensive demographic assessments of key fertility indicators at global, regional, and national levels from 1950 to 2021 and forecast fertility metrics to 2100 based on a reference scenario and key policy-dependent alternative scenarios.

Regular, detailed reporting on population health by underlying cause of death is fundamental for public health decision making. Cause-specific estimates of mortality and the subsequent effects on life expectancy worldwide are valuable metrics to gauge progress in reducing mortality rates. These estimates are particularly important following large-scale mortality spikes, such as the COVID-19 pandemic. When systematically analysed, mortality rates and life expectancy allow comparisons of the consequences of causes of death globally and over time, providing a nuanced understanding of the effect of these causes on global populations.

Nanotechnology has emerged as a transformative pathway in vaccine research and delivery. Nanovaccines, encompassing lipid and nonlipid formulations, exhibit considerable advantages over traditional vaccine techniques, including enhanced antigen stability, heightened immunogenicity, targeted distribution, and the potential for codelivery with adjuvants or immune modulators. This review provides a comprehensive overview of the latest advancements and applications of lipid and non-lipid-based nanovaccines in current vaccination strategies for immunization. The review commences by outlining the fundamental concepts underlying lipid and nonlipid nanovaccine design before delving into the diverse components and production processes employed in their development. Subsequently, a comparative analysis of various nanocarriers is presented, elucidating their distinct physicochemical characteristics and impact on the immune response, along with preclinical and clinical studies. The discussion also highlights how nanotechnology enables the possibility of personalized and combined vaccination techniques, facilitating the creation of tailored nanovaccines to meet the individual patient needs. Serious consequences including septicemia and amputations can result from complex wounds, which is a serious healthcare concern. In addition, there are currently only a few choices for management, which justifies the search for novel, highly effective wound-healing medications. This research work was aimed at fabricating chitosan-based Alkanna tinctoriaand Mupirocin nanoparticles by ionic gelation technique for burn wound management. Preliminary studies were conducted, and the prepared nanoparticles were characterized by various techniques that involve, high-performance liquid chromatography for the detection of components in A. tinctoriaroot extract, ATR-FTIR, particle size, zeta potential, percent drug content (DC%), percent entrapment efficiency (EE%), scanning electron microscopy, and transmission electron microscopy (TEM) for surface morphology. The optimized formulation CS-AT-MU-NPs3 shows a particle size of 340.8 ± 34.46 nm and positive zeta potential 27.3 ± 3.10 mV. In vitrodrug release study was also performed, which demonstrated improved and controlled release of the drug from the nanoparticles. The CS-AT-MU-NPs3 exhibited a maximum release up to 92.61% (AT) and 88.35% (MU). Antibacterial and antifungal activities of the formulation were also accessed by utilizing the agar well diffusion technique. The combination of AT and MU in chitosan-based nanoparticles was significantly effective against bacterial and fungal strains like Staphylococcus aureus, Pseudomonas aeruginosa, and Candida albicansas compared to other formulations. The skin irritation study was also conducted, which shows that the prepared formulation did not cause any observable changes to the skin in terms of inflammation, erythema, edema, or any other symptoms associated with skin irritation. All the chitosan-based nanoparticles showed almost 75% reduction in wound contraction, while the optimized formulation CS-AT-MU-NPs3 showed complete wound healing on the 15th day. Based on the results, it can be assumed that chitosan-based nanoparticles containing A. tinctoriaand Mupirocin demonstrated good wound healing and could be used to effectively manage burn wounds of any description.

Diabetes mellitus, notably type 2, is a rising global health challenge, prompting the need for effective management strategies. Common medications such as metformin, insulin, repaglinide and sitagliptin can induce side effects like gastrointestinal disturbances, hypoglycemia, weight gain and specific organ risks. Plant‐derived therapies like Karanjin from Pongamia pinnatapresent promising alternatives due to their historical use, holistic health benefits and potentially fewer adverse effects. This study employs in silico analysis to explore Karanjin's interactions with diabetes‐associated receptors, aiming to unveil its therapeutic potential while addressing the limitations and side effects associated with conventional medications. The research encompassed the selection of proteins from the Protein Data Bank (PDB), followed by structural refinement processes and optimization. Ligands such as Karanjin and standard drugs were retrieved from PubChem, followed by a comprehensive analysis of their ADMET profiling and pharmacokinetic properties. Protein–ligand interactions were evaluated through molecular docking using AutoDockTools 1.5.7, followed by the analysis of structural stability using coarse‐grained simulations with CABS Flex 2.0. Molecular dynamics simulations were performed using Desmond 7.2 and the OPLS4 force field to explore how Karanjin interacts with proteins over 100 nanoseconds, focusing on the dynamics and structural stability. Karanjin, a phytochemical from Pongamia pinnata, shows superior drug candidate potential compared to common medications, offering advantages in efficacy and reduced side effects. It adheres to drug‐likeness criteria and exhibits optimal ADMET properties, including moderate solubility, high gastrointestinal absorption and blood–brain barrier penetration. Molecular docking revealed Karanjin's highest binding energy against receptor 3L2M (Pig pancreatic alpha‐amylase) at −9.1 kcal/mol, indicating strong efficacy potential. Molecular dynamics simulations confirmed stable ligand–protein complexes with minor fluctuations in RMSD and RMSF, suggesting robust interactions with receptors 3L2M. Karanjin demonstrates potential in pharmaceutical expansion for treating metabolic disorders such as diabetes, as supported by computational analysis. Prospects for Karanjin in pharmaceutical development include structural modifications for enhanced efficacy and safety. Nanoencapsulation may improve bioavailability and targeted delivery to pancreatic cells, while combination therapies could optimize treatment outcomes in diabetes management. Clinical trials and experimental studies are crucial to validate its potential as a novel therapeutic agent. The study evaluates Karanjin from Pongamia pinnataas a potential therapeutic for diabetes through in silico analysis. Karanjin showed promising ADMET properties, strong binding energies and stability in molecular dynamics simulation, especially with pancreatic alpha‐amylase (3L2M), suggesting its potential for diabetes management.

Inflammatory arthritis is a growing worldwide concern, afflicting millions of individuals and leading to a depreciating quality of life, significant morbidity, and subsequent healthcare burden. Conventional therapies for inflammatory arthritis often hold limitations, including systemic cross-reactivities, side effects, and inadequate management of inflammation. Therefore, “Smart” stimuli-responsive biomaterials are materialized as promising theranostic approaches for inflammatory arthritis management. These biomaterials are designed for specific external and internal stimulation, enabling the sustained release of bioactive therapeutic agents during local or systemic administration. Stimuli-responsive biomaterials allow precise spatiotemporal control over drug release, minimizing systemic exposure, reducing off-target effects, and real-time monitoring of pathological conditions, driving a potential breakthrough in managing inflammatory arthritis. Owing to this prominence, this review article expounds on the emerging theranostic prospects of stimuli-responsive “Smart” biomaterials targeting the inflammatory molecular signatures associated with intricately inflammatory arthritis to elicit minimal or no contradictory side effects. In addition, the application of these biomaterials for real-time disease progression and multi-stimuli-responsive biomaterials is comprehended to show the progress in inflammatory arthritis diagnosis and treatment. This “single-platted” comprehensive acquaintance of molecular and clinical-based outcomes of stimuli-responsive biomaterials navigates the inflammatory microenvironment, proposing an optimistic avenue for personalized and effective management of inflammatory arthritis with minimized side effects.

The transmission of various infectious diseases, by Aedes aegypti and Anopheles stephensi has poses a menace to human well-being. To combat the vector of this serious disease, environmentally friendly prevention strategies are essential. The fight against malaria and dengue is greatly aided by nanotechnology and pharmacology. The aqueous leaf extract of Zaleya decandra was used in the present study to synthesise palladium nanoparticles (Zd -PdNPs) by simple and sustainable processes. Numerous physiochemical methods were used to identify and classify bio-synthesized Zd -PdNPs. Additionally, Ae. aegypti and An. stephensi IV instar larvae were tested toxicity using Z. decandra extract and Zd -PdNPs. Further research was conducted to determine how the NPs-based treatment affected the histology and morphology of mosquito larvae. The LC 50 values of ZD-PdNPs against Ae. aegypti larvae were 18.071 and 14.124 µg/mL; An. stephensi had 12.140 and 10.118 µg/mL at 12 and 24 hr exposure. Furthermore, zebrafish embryo developmental toxicity and the freshwater sludge worm Tubifex tubifex have both been utilised as test models for non-target eco-toxicological investigations in the typical laboratory setting. Gas chromatography-mass spectrometry was used to examine the chemical constituents of Z. decandra. n-hexadecanoic acid, 6-octadecenoic acid, phthalic acid, butyl undecyl ester, squalene, nonanoic acid, methyl ester, and butyl undecyl ester 2-methyl-4-phenylthiolane were identified as potential compounds for bio reduction and synthesis of nanoparticle. The current study reveals that natural molecules bonded nanomaterial that are effective against human disease spreading mosquito vectors larvae and highlights the potential value of Nano-larvicides as well as environmental safety. There is a swing in research developments concerning the utilization of natural products as effective pharmacotherapeutic agents due to their comparatively lower toxicities than synthetic compounds. Among natural products, mangiferin is a natural C-glucosyl xanthonoid polyphenol with remarkable pharmacological activities. Emerging evidence indicates the therapeutic benefits of mangiferin against various kidney disorders, including renal injury, diabetic nephropathy, renal fibrosis, hyperuricemic nephropathy, and lupus nephritis, in experimental animal models. The mangiferin induced antioxidant response resulting in vital functions, such as protection against renal inflammation, inhibits renal cell apoptosis, activates autophagy, causes immunomodulation, regulates renal urate transporters and modulates cell signalling pathways. The purpose of this review provide a brief overview of the in vitro/in vivo reno-protective effect of mangiferin and the underlying mechanism(s) in protecting against kidney disorders. Understanding the pharmacological actions of mangiferin is prominence due to its excellent therapeutic potential in managing kidney disorders. Thus, in addition to this review, in-silico molecular docking is performed against nuclear factor kappa B (NF-κB) and soluble epoxide hydrolase (sEH) to study the mechanism of action of mangiferin. It is believed that mangiferin is a safe reno-protective molecule. The observed positive effects are attributed to the inhibition of inflammation caused by NF-κB and sEH upregulation and oxidative stress activation. Studies on the efficacy and safety of mangiferin in clinical trials are further warranted to confirm its medicinal potential as therapeutic agent for kidney disorders in humans. Breast cancer (BC) has high incidence and mortality rates, making it a major global health issue. BC treatment has been challenging due to the presence of drug resistance and the limited availability of therapeutic options for triple-negative and metastatic BC, thereby urging the exploration of more effective anti-cancer agents. Hesperidin and its aglycone hesperetin, two flavonoids from citrus species, have been extensively evaluated for their anti-cancer potentials. In this review, available literatures on the chemotherapeutic and chemosensitising activities of hesperidin and hesperetin in preclinical BC models are reported. The safety and bioavailability of hesperidin and hesperetin as well as the strategies to enhance their bioavailability are also discussed. Overall, hesperidin and hesperetin can inhibit cell proliferation, migration and BC stem cells as well as induce apoptosis and cell cycle arrest in vitro. They can also inhibit tumour growth, metastasis and neoplastic changes in tissue architecture in vivo. Moreover, the co-administration of hesperidin or hesperetin with doxorubicin, letrozole or tamoxifen can enhance the efficacies of these clinically available agents. These chemotherapeutic and chemosensitising activities of hesperidin and hesperetin have been linked to several mechanisms, including the modulation of signalling pathways, glucose uptake, enzymes, miRNA expression, oxidative status, cell cycle regulatory proteins, tumour suppressor p53, plasma and liver lipid profiles as well as DNA repair mechanisms. However, poor water solubility, extensive phase II metabolism and apical efflux have posed limitations to the bioavailability of hesperidin and hesperetin. Various strategies for bioavailability enhancement have been studied, including the utilisation of nano-based drug delivery systems and the co-administration of hesperetin with other flavonoids. In particular, nanoformulated hesperidin and hesperetin possess greater chemotherapeutic and chemosensitising activities than free compounds. Despite promising preclinical results, further safety and efficacy evaluation of hesperidin and hesperetin as well as their nanoformulations in clinical trials is required to ascertain their potentials to be developed as clinically useful agents for BC treatment. In the current pandemic, COVID-19 patients with predisposing factors are at an increased risk of mucormycosis, an uncommon angioinvasive infection that is caused by fungi with Mucor genus which is mainly found in plants and soil. Mucormycosis development in COVID-19 patient is related to various factors, such as diabetes, immunocompromise and neutropenia. Excessive use of glucocorticoids for the treatment of critically ill COVID-19 patients also leads to opportunistic infections, such as pulmonary aspergillosis. COVID-19 patients with mucormycosis have a very high mortality rate. This review describes the pathogenesis and various treatment approaches for mucormycosis in COVID-19 patients, including medicinal plants, conventional therapies, adjunct and combination therapies. 13 C-NMR, ¹H-NMR, COSY, DEPT and Mass spectrometry). The isolation and characterization experiment offered two phytometabolites: An ester (Ag-1) and tetrahydronapthalene type lactone (Ag-2). Present study concludes and reports the two phytometabolites, benzyl myristate (Ag-1) and 3-Methyl-6α, 8β-diol-7-carboxylic acid tetralin-11, 9β-olide (Ag-2) for the first time in Alpinia galanga rhizome. The study recommends that these phytometabolites Ag-1 and Ag-2 can be utilized as effective analytical biomarkers for identification, purity and quality control of this plant in future. The LC50values of ZD-PdNPs against Ae. aegyptilarvae were 18.071 and 14.124 µg/mL; An. stephensihad 12.140 and 10.118 µg/mL at 12 and 24 hr exposure. Furthermore, zebrafish embryo developmental toxicity and the freshwater sludge worm Tubifex tubifexhave both been utilised as test models for non-target eco-toxicological investigations in the typical laboratory setting. Gas chromatography-mass spectrometry was used to examine the chemical constituents of Z. decandra. n-hexadecanoic acid, 6-octadecenoic acid, phthalic acid, butyl undecyl ester, squalene, nonanoic acid, methyl ester, and butyl undecyl ester 2-methyl-4-phenylthiolane were identified as potential compounds for bio reduction and synthesis of nanoparticle. The current study reveals that natural molecules bonded nanomaterial that are effective against human disease spreading mosquito vectors larvae and highlights the potential value of Nano-larvicides as well as environmental safety.

Breast cancer (BC) remains an enigmatic fatal modality ubiquitously prevalent in different parts of the world. Contemporary medicines face severe challenges in remediating and healing breast cancer. Due to its spatial specificity and nominal invasive therapeutic regime, photothermal therapy (PTT) has attracted much scientific attention down the lane. PTT utilizes a near-infrared (NIR) light source to irradiate the tumor target intravenously or non-invasively, which is converted into heat energy over an optical fibre. Dynamic progress in nanomaterial synthesis was achieved with specialized visual, physicochemical, biological, and pharmacological features to make up for the inadequacies and expand the horizon of PTT. Numerous nanomaterials have substantial NIR absorption and can function as efficient photothermal transducers. It is achievable to limit the wavelength range of an absorbance peak for specific nanomaterials by manipulating their synthesis, enhancing the precision and quality of PTT. Along the same lines, various nanomaterials are conjugated with a wide range of surface-modifying chemicals, including polymers and antibodies, which may modify the persistence of the nanomaterial and diminish toxicity concerns. In this article, we tend to put forth specific insights and fundamental conceptualizations on pre-existing PTT and its advances upon conjugation with different biocompatible nanomaterials working in synergy to combat breast cancer, encompassing several strategies like immunotherapy, chemotherapy, photodynamic therapy, and radiotherapy coupled with PTT. Stem cells can multiply into more cells with similar types in an undifferentiated form and differentiate into other types of cells. The great success and key essence of stem cell technology is the isolation of high-quality Mesenchymal Stem Cells (MSCs) with high potency, either with multipotent or pluripotent property. In this line, Stem cells from Human Exfoliated Deciduous teeth (SHEDs) are highly proliferative stem cells from dental pulp and have multipoint differentiation capacity. These cells play a pivotal role in regenerative medicine, such as cell repair associated with neurodegenerative, hepatobiliary, and pancreatic diseases. In addition, stem cell therapy has been widely used to regulate immune response and repair of tissue lesions. This overview captured the differential biological characteristics, and the potential role of stem cell technology and paid special attention to human welfare SHEDs in eliminating the above-mentioned diseases. This review provides further insights into stem cell technology by expanding the therapeutic potential of SHEDs in tissue engineering and cell organ repairs.

Green nanoparticles represent a sustainable and eco-friendly approach to developing larvicides targeted at controlling mosquito populations. These nanoparticles are synthesized using natural, non-toxic materials and environmentally benign processes, emphasizing their potential to combat mosquito larvae while minimizing adverse effects on the ecosystem. This approach aligns with the growing need for sustainable solutions in vector control, providing an effective alternative to traditional larvicides with reduced environmental impact. The aim of the present investigation we have utilized the leaf from Laureliopsis philippiana (LP) to synthesize silver (Ag) and palladium (Pd) nanoparticles (NPs) and studied toxicity on deadly disease vectors larvae of Aedes aegypti and Anopheles stephensi. Biosynthesized Ag and Pd NPs were extensively characterized through physical and chemical methods. Gas chromatography–mass spectrometry analysis of L. philippiana assisted in identifying the secondary metabolites of the extract's total flavonoid and phenolic compounds, which are in the reactants of the bio-reduction process during the production of NPs. The in-silico analysis was conducted on mosquito sterol carrier protein-2 (1PZ4) and mosquito odorant binding protein (2L2C). The biosynthesized Ag and Pd NPs revealed the UV absorption peak at 417 and 370 nm, and X-ray diffraction results showed the crystalline structure of NPs. At the same time, in transmission electron microscopy representation, the Ag and Pd NPs were spherical, clustered, and exhibited a noticeable average size of Ag NPs: 13.25 ± 2.53 nm and Pd NPs: 4.07 ± 1.64 nm. The biogenic Ag and Pd NPs showed concentration-dependent larvicidal mortality at 12 and 24-hour exposure with LC 50 values of 29.596, 17.804, and 23.915, 14.878 µg/mL on Ae. aegypti and 23.946, 13.938, 19.457, and 12.876 µg/mL against An. stephensi , correspondingly. In-silico studies proved that Laureliopsine A and isoterpinolene showed significant docking scores compared to permethrin on mosquito proteins (1PZ4 and 2L2C). Besides, the LP-Ag and Pd NPs non-toxicity analysis was carried out on zebrafish (Danio rerio) embryos at 72 h post-fertilization. Most antibiotics used in topical formulations are not absorbed by deeper skin layers and subdermal tissues. Hence the skin infections are treated with large doses of oral or parenteral antibiotics which lead to unpleasant side effects with drug resistance. This could be the necessity for the preparation of transdermal drug delivery systems, which addressed the issues with administration. The present study designed to develop cephalexin loaded transferosomal (CPX-TFs) gel for an effective treatment of skin infection by improving transdermal delivery. The transferosomes (TFs) were prepared by thin film hydration method by employing QbD based Box-Behnken design. The major objectives of the study was to prepare CPX-TFs with higher entrapment and lower particle size, to enhance the penetration deeper into the skin layers. The seventeen different batches considering concentrations of phospholipon® 90H, sodium deoxycholate at different temperature condition were prepared and the formula was optimized. These variables showed positive effect on its entrapment efficiency and on particle size of transferosomes. The TFs with lower vesicle size showing better stability whereas in vitrodiffusion study demonstrated cumulative release (92.96 ± 2%) up to 24 h. The optimized CPX-TFs were incorporated in carbopol 934 gel (0.25%, 0.5%, 1%, 1.5%, and 2% w/v) by dispersion method. This gel with 96.52 ± 3% (2.5% w/v) drug content demonstrated about 91.26 ± 2% release of CPX over a period of 24 h, showing biphasic drug release pattern (initial burst release in first 6 h then sustaining the release over 24 h). It shows excellent anti-bacterial action against the E coli, S aureusand K pneumonaie.The optimized CPX‒TFs gel presented maximum flux value than pure CPX suspension. The increased in permeation with gradual increased in surfactant concentration was observed. The in vivopharmacokinetic study shows steady state plasma concentration and Cmaxfor CPX-TFs gel was found to be 2.3-fold which was comparatively greater than oral CPX suspension. The histopathological study confirms the safety of transdermal application of CPX-TFs gel. It was well tolerated by rat skin. Hence CPX-TFs gel could be the potential delivery system for transdermal administration of CPX for treatment of skin infections.

To determine the ameliorative potential of the active fraction from different extracts of Rumex vesicarius against potassium dichromate and gentamicin induced nephrotoxicity in experimental rats and its possible mechanism of action. Both sex wistar rats were divided into 6 groups (n=6/group) were fed with a control, potassium dichromate and gentamicin supplemented with different extracts at the doses of 200 and 400mg/kg respectively. Oral administration of EERV offered a significant (p<0.01 and p<0.001) dose dependent protection against PD and GN induced nephrotoxicity. Potassium dichromate and gentamicin nephrotoxicity assessed in terms of body weight, kidney weight, creatinine, urea, uric acid, BUN, albumin and total protein. Thus the present study revealed that EERV phytochemical constituents play an important role in protection against kidney damage. [ABSTRACT FROM AUTHOR]

Blastocystis sp. is a unicellular parasitic microorganism commonly found in the gastrointestinal tracts of humans and animals. It causes symptomatic or asymptomatic infection and its route of transmission is via fecal-oral. High prevalence of Blastocystis infection in developing countries is usually due to poor hygiene practices, exposure to animals infected with the parasite and intake of contaminated water or food. Blastocystis infected individuals often suffer from diarrhea, abdominal pain, nausea, and stomach bloating. Even though pathogenicity of Blastocystis is unclear, it is commonly associated with irritable bowel syndrome. In this review, we have analysed the evidence that shows the association between this microorganism and gastrointestinal disorders. There have been a number of studies which showed that the pathogenicity of Blastocystis is related to its different STs. The pathogenicity is speculated to be due to cysteine proteases formation which stimulates mucosal cells to release interleukin-8 which has been associated with extreme dehydration and gut inflammation. In vitro studies on human colonic epithelial cells revealed that incubation of Blastocystis modulated the host immune response by stimulating the formation of pro-inflammatory cytokines and granulocyte macrophage colonystimulating factor. Metronidazole is found to be the first-line drug of choice. Another treatment option is the combination therapy with trimethoprim/sulfamethoxazole.
