Purpose: Pulsed electrical field (PEF) ablation is an energy-based technique used to treat a range of cancers by irreversible electroporation (IRE). Our objective was to use computational and plant-based models to characterize the electric field distribution and ablation zones induced with a commercial 8-needle array-based applicator intended for treatment of skin cancer when high-frequency IRE (H-FIRE) pulses are applied. Electric field characterisation of this device was not previously assessed. Methods: Vegetable experimental were conducted using parallel plate electrodes to obtain the lethal threshold for H-FIRE pulses. Then a 3D computational model of the applicator was built mimicking the experimental conditions. The computational ablation zones were validated with the experiments for different voltage levels ranging from 220 to 525 V. Results: A threshold of 453 V/cm was estimated for H-FIRE pulses, which was used to predict computationally the ablation zones. It was found that the model showed a low prediction error, ranging from 2% for the minor diameter to 4.5% for the depth compared with experiments. Voltages higher than 370 V created an ablation volume with a rectangular prism shape determined by the positions of the needles, whereas lower voltages provoked the appearance of untreated areas (gaps). Conclusions: Our computer model predicts reasonably well the ablation zone induced by H-FIRE pulses, suggesting that a sufficiently large voltage must be applied to avoid the appearance of gaps. The validated model with vegetable experiments could serve as the basis for future computer studies to predict the behaviour of this device on heterogeneous tissues.

Multipathological patients are a vulnerable population with high comorbidity, functional impairment, and nutritional risk. Almost 50% of these hospitalized patients have dysphagia. There is no consensus on whether placement of a percutaneous endoscopic gastrostomy (PEG) tube provides greater clinical benefit. The purpose of this study was to know and compare 2 groups of multipathological patients with dysphagia according to the mode of feeding: PEG vs. oral.

Los pacientes pluripatológicos constituyen una población vulnerable con alta comorbilidad, deterioro funcional y riesgo nutricional. Casi un 50% de estos pacientes hospitalizados presentan disfagia. No existe consenso sobre si la colocación de una sonda de gastrostomía percutánea endoscópica (PEG) aporta mayor beneficio clínico. El propósito de este estudio fue conocer y comparar 2 grupos de pacientes pluripatológicos con disfagia según el modo de alimentación: PEG vs. vía oral.

Throughout the twentieth century, Catholic institutions in Argentina experienced a period of consolidation, followed by evidence of decline and crisis. The reinforcement of the Romanization process at the beginning of the twentieth century, combined with the integralist emphasis of Catholicism at the beginning of the century, led to the strengthening of the Catholic presence in the country. The number of institutions and consecrated agents responsible for them continued to consolidate, as measured by the Statistical Yearbooks of the Church. However, this consolidation was followed by the beginning of the decline. This article focuses on the Catholic institutions that the Church itself monitors over time through its Statistical Yearbooks. It analyzes the number of dioceses and parishes, the number of priests and religious congregations, and the administration of the sacraments. The article examines the challenges currently facing the Catholic Church in Argentina, including the decline in the number of priests, religious people, and participation in the sacraments. Overall, this article offers a comprehensive view of Catholic institutions in Argentina from the twentieth century to the present. It highlights the consolidation, evidence of decline, and crisis that the Church has faced over the years and provides insight into the challenges it continues to face today.

In the current context of mandatory growth of renewable energy sources for electrical supply, in order to reduce CO2 emissions in the process of electricity generation, it is worth interest to evaluate the feasibility of the deployment of new large solar photovoltaic production facilities. This study seeks to analyze the costs and benefits of this type of infrastructures, considering the region of León (Spain) as case study due to its high potential. The Levelized Cost of Energy is used in this study as main decision tool, so a sensibility study on this indicator has been carried out, including the financial interest rates, the capital and operation expenditures, the facilities performance and the expected life span of the installations. Three main locations have been selected in the region, close to the main power demand sites, in order to minimize power distribution losses between the power plants and end-users. Results show that an LCOE in the range between 0.21 and 0.26 €/kWh can be achieved. The simplified calculation presented allows a preliminary and quick assessment of the grid parity of a photovoltaic plant in locations where they are not known in detail.

2 billion peripheral intravenous catheters (PIVC) are inserted into inpatients worldwide each year. Almost one in two PIVCs fail before completion of intravenous therapy. We aimed to determine the efficacy and costs of a multimodal intervention to reduce PIVC failure among hospitalised patients.

Onychomycosis is a frequent and underdiagnosed condition. Approximately 90% of toenail onychomycosis infections are caused by dermatophytes, but classical diagnosis based on culture and microscopy observation is slow and has low sensitivity. Both limitations can be solved incorporating molecular techniques to routine diagnosis of onychomycosis.

Background: Breast and ovarian carcinomas represent major health problems in women worldwide. Chemotherapy constitutes the main treatment strategy, and the use of nanocarriers, a good tool to improve it. Several nanoformulations have already been approved, and others are under clinical trials for the treatment of both types of cancers. Objective: This review focuses on the analysis of the nanoformulations that are under clinical research in the treatment of these neoplasms. Results: Currently, there are 6 nanoformulations in clinical trials for breast and ovarian carcinomas, most of them in phase II and phase III. In the case of breast cancer treatment, these nanomedicines contain paclitaxel; and, for ovarian cancer, nanoformulations containing paclitaxel or camptothecin analogs are being evaluated. The nanoencapsulation of these antineoplastics facilitates their administration and reduces their systemic toxicity. Nevertheless, the final approval and commercialization of nanoformulations may be limited by other aspects like lack of correlation between the efficacy results evaluated at in vitro and in vivo levels, difficulty in producing large batches of nanoformulations in a reproducible manner and high production costs compared to conventional formulations of antineoplastics. However, these challenges are not insurmountable and the number of approved nanoformulations for cancer therapy is growing. Conclusion: Reviewed nanoformulations have shown, in general, excellent results, demonstrating a good safety profile, a higher maximum tolerated dose and a similar or even slightly better antitumor efficacy compared to the administration of free drugs, reinforcing the use of nano-chemotherapy in both breast and ovarian tumors.

Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) is the pathogen agent causing coronavirus disease (COVID)-19, which was declared a global pandemic in 2020. The spike protein of this virus and the angiotensin-converter enzyme (ACE)-2 in host cells in humans play a vital role in infection and in COVID-19 pathogenesis. Estradiol is known to modulate the actions of immune cells, and, therefore, the antiviral mechanisms of these cells could also be modified by this hormone stimulus. Even though estradiol is not considered a protective factor, evidence shows that women with high levels of this hormone have a lower risk of developing severe symptoms and an even a lower incidence of death. Understanding the mechanism of action of estradiol with regard to viral infections and COVID-19 is essential for the improvement of therapeutic strategies. This review aims to describe the effects that estradiol exerts on immune cells during viral infections and COVID-19.

The writing analyzes trends in the life and ministry of women in Latin America. It provides a quantitative sketch identifying religious sisters’ trends from 1970 to 2017 and compares them both with other regions and with Latin American priests. It exams three patterns, all of them concluding with declining numbers at the beginning of the 2000s.The analysis is grounded on a database that was built using the Statistical Yearbook of the Church. The article also examines the social space women religious have occupied in the Latin American countries over time; the construction of their social and ecclesial identity; and their relationship with Catholic consecrated men. Finally, it addresses their current challenges deepening in three issues that account for their crisis: first, the persistent clerical culture that permeates the Church’s dynamics; second, the disputes stirred up around decisions on their works and expressions of the past but not of their present; and third, the building of their present identity through the complex process of finding inspiration in the confrontation of their foundational sources with present pastoral theological approaches, as those framed within feminist theologies. Unearthing clues to better understanding the Latin American sisters’ crisis is a thread that runs throughout the analysis.

Fracking is a technology used for the extraction of shale gas contained in rocks on the Earth’s surface. The main characteristic of this method is that it consists of injecting pressurized water into the ground, thus creating a series of fractures through which the gas comes out to be collected later. Fracking has a number of both socio-economic and environmental implications that can be both beneficial (including increased energy security, economic growth, or reduced emissions of pollutants and climate change) and harmful (induced seismicity, increased global temperature, and potential greenhouse emissions if not properly implemented). The realization of a systematic review of the literature classifying the articles found according to the type of evidence they present; that it gathers all the impacts has allowed to group them and to give a general vision about the topic; that no articles have been found that have this same objective in the existing literature, thus contributing to the increase of the existing knowledge in this field. It is concluded that environmental risks, including those that could affect human health, should be integrated into the cost structure of fracking, as a risk premium or provision of funds to remedy possible negative effects.

This paper focuses on an analysis of the current situation of distillation and reverse osmosis technologies and their relationship with renewable energies, to the extent that there is a strong trend in the use of distillation and reverse osmosis technologies. Large-scale seawater desalination using renewable energy is not developed, so a detailed study of the combination of renewable energy facilities, reverse osmosis, and distillation systems are needed to be effective and competitive compared to the use nonrenewable energy. Solar multiple is a very important parameter in the solar thermal industry. This is in the range of 1.5 – 3 and depends on the relationship between storage and plant power. This parameter is important because it reflects a balance between the cost of the parabolic trough collectors and the total of the solar thermal plant. If the same criterion is followed, with a thermal gap of 100 °C, chosen for the parabolic trough collectors sizing, the solar multiple in the system object of this article, has a value higher than 3. The proposed system employs renewable energies for reducing the downtime of the desalination plants, doing possible that the desalination plants being competitive in sites with poor quality solar radiation.

In pregnancy, maternal monocytes and macrophages acquire a specific phenotype that enables them to maintain immune tolerance and facilitate hormone–immune cell interactions, which are necessary for gestational progression. The aim of this study was to determine the effect of pregnancy hormone mixtures of the first and third trimesters on both resting and activated monocytes and macrophages. Pregnancy hormone levels (cortisol, estradiol, progesterone, and prolactin) were quantified at the first and third trimesters. The average of the levels obtained was used to prepare two mixtures of synthetic hormones: low and high. These mixtures were then used to stimulate THP-1 monocytes and macrophages, resting or activated with LPS. Cytokine production in the culture supernatants and surface marker expression (CD14, CD86, and CD163) were evaluated by ELISA and flow cytometry, respectively. We found that the hormones modulated the pro-inflammatory response of THP-1 cells, LPS-activated monocytes, and macrophages, inducing high levels of IL-10 and low levels of IL-8, IL-1-β, and IL-6. All hormone stimulation increased the CD163 receptor in both resting and LPS-activated monocytes and macrophages in a dose-independent manner, unlike CD14 and CD86. Pregnancy hormones promote the expression of the markers associated with the M2-like phenotype, modulating their pro-inflammatory response. This phenotype regulation by hormones could be a determinant in pregnancy.

Las mucinosis cutáneas son un grupo de condiciones caracterizadas por el depósito anormal de mucina en la piel. Pueden ser primarias, que a su vez pueden ser inflamatorias-degenerativas (dérmicas o foliculares) y hamartomatosas-neoplásicas; o secundarias. La mucinosis papulonodular forma parte de las mucinosis primarias inflamatorias-degenerativas dérmicas. Se ha descrito su asociación con enfermedades autoinmunes del tejido conectivo, especialmente con el lupus eritematoso sistémico, pero se considera una manifestación inusual de esta enfermedad.

Cutaneous mucinosis is a group of conditions characterized by the abnormal deposition of mucin in the skin. They can be primary, which in turn can be inflammatory-degenerative, and hamartomatous-neoplastic; or secondary. Papulonodular mucinosis is part of the group of dermal inflammatory-degenerative primary mucinosis. Its association with autoimmune connective tissue diseases has been described, especially with systemic lupus erythematosus, but it is considered an unusual manifestation of this disease.

To analyze adverse reactions in patients with nonmetastatic colorectal cancer due to treatment with either innovative or generic capecitabine and/or to the chemotherapeutic regimen employed, to the capecitabine alone, or in combination with oxaliplatin (XELOX).

Diseases affecting the central nervous system (CNS) should be regarded as a major health challenge due to the current lack of effective treatments given the hindrance to brain drug delivery imposed by the blood–brain barrier (BBB). Since efficient brain drug delivery should not solely rely on passive targeting, active targeting of nanomedicines into the CNS is being explored. The present study is devoted to the development of lipid nanocapsules (LNCs) decorated with nonpsychotropic cannabinoids as pioneering nonimmunogenic brain-targeting molecules and to the evaluation of their brain-targeting ability both in vitro and in vivo. Noticeably, both the permeability experiments across the hCMEC/D3 cell-based in vitro BBB model and the biodistribution experiments in mice consistently demonstrated that the highest brain-targeting ability was achieved with the smallest-sized cannabinoid-decorated LNCs. Importantly, the enhancement in brain targeting achieved with the conjugation of cannabidiol to LNCs outperformed by 6-fold the enhancement observed for the G-Technology (the main brain active strategy that has already entered clinical trials for the treatment of CNS diseases). As the transport efficiency across the BBB certainly determines the efficacy of the treatments for brain disorders, small cannabinoid-decorated LNCs represent auspicious platforms for the design and development of novel therapies for CNS diseases.
