Although the extant of injury after cervical spine fracture can be visualized by imaging, the deformations that occur in the spinal canal during injury are unknown.

The Collaborative Observations Regarding the Nightglow (CORN) campaign took place at the Urbana Atmospheric Observatory during September 1992. The instrumentation included, among others, the Aerospace Corporation narrowband nightglow CCD camera, which observes the OH Meinel (6–2) band (hereafter designated OH) and the O2atmospheric (0–1) band (hereafter designated O2) nightglow emissions; the University of Illinois Na density/temperature lidar; and the University of Illinois MF radar. Here we report on observations of small-scale (below 10-km horizontal wavelength) structures in the OH airglow images obtained with the CCD camera. These small-scale structures were aligned perpendicular to the motion of 30- to 50-km horizontal wavelength waves, which had observed periods of about 10–20 min. The small-scale structures were present for about 20 min and appear to be associated with an overturned or breaking atmospheric gravity wave as observed by the lidar. The breaking wave had a horizontal wavelength of between 500 and 1500 km, a vertical wavelength of about 6 km, and an observed period of between 4 and 6 hours. The motion of this larger-scale wave was in the same direction as the ˜30- to 50-km waves. While such small-scale structures have been observed before, and have been previously described as ripple-type wave structures [Taylor and Hapgood, 1990], these observations are the first which can associate their occurrence with independent evidence of wave breaking. The characteristics of the observed small-scale structures are similar to the vortices generated during wave breakdown in three dimensions in simulations described in Part 2 of this study [Fritts et al., this issue]. The results of this study support the idea that ripple type wave structures we observe are these vortices generated by convective instabilities rather than structures generated by dynamical instabilities.

We compared the effects of reamed versus unreamed locked intramedullary nailing on cortical bone blood flow in a fractured sheep tibia model. A standardized spiral fracture was created by three-point bending with torsion, and each tibia was stabilized by insertion of a locked intramedullary nail. Eleven animals were randomized into two groups one that had reaming before nail insertion and one that did not. Blood flow was measured in real time using laser Doppler flowmetry. Cortical bone perfusion measurements were made at three locations (proximal diaphysis, fracture site, distal diaphysis) and at eight time intervals (prefracture, postfracture, postreaming, postnail insertion, postlocking, and at 2-, 6-, and 12-week follow-up). All animals were killed at 12 weeks postoperatively. After reamed nail insertion, cortical bone perfusion was significantly decreased (p < 0.0009). After unreamed nail insertion, perfusion was decreased less (p < 0.003). Insertion of locking screws did not affect blood flow. Cortical bone perfusion was greater in the unreamed group at completion of the procedure (p < 0.011), at 2-week follow-up (p < 0.006) and at 6-week follow-up (p < 0.027). The findings suggest that cortical revascularization had occurred by 6 weeks in the unreamed group but not until 12 weeks in the reamed group. The study demonstrates that cortical circulation is spared to a greater degree by unreamed nailing. This may be advantageous in severe open tibial fractures where blood supply is already significantly compromised.

The chemistry and dynamics of the mesospheric sodium layer have been studied extensively with lidar (laser radar) techniques since the late 1960s. Meteoric ablation is generally regarded as the dominant source of all mesospheric alkali metals including sodium. The Na layer is typically confined to the region between 80 and 110 km with a peak near the mesopause at 90 km, where the density ranges from ∼103to 104cm−3. The Na column abundance at mid-latitudes in the Northern Hemisphere varies from a summer minimum of ∼ 3x 109cm−2to a winter maximum of ∼1010cm−2in December and January1. The seasonal and geographical variations in Na abundance are now believed to be related to changes in the mesopause temperature that affect the reaction rates of the main chemical loss processes for Na. In addition to chemical activity, the dynamic effects of the tides, gravity waves and the mean winds have a significant influence on the vertical structure of the layer. In June, the University of Illinois (UIUC) sodium lidar system was installed at Nordlysstasjonen, Svalbard (78°12' N, 15°50' E) on Spitsbergen Island and we conducted more than 27 h of observations during the period 10–15 July 1987. The average Na layer column abundance was ∼ 6 x 108cm−2, a value that is almost five times lower than typical summertime Na abundances measured at mid-latitudes. Additional observations were conducted during the period 7–11 September 1987 and the measured Na abundances varied between ∼ 5 x l09and 14 x 109cm−2.

The goals of internal fixation are an accurate reduction and stable fixation in the presence of adequate bony vascularity. This can be achieved by a variety of means including plate fixation. A certain amount of periosteal stripping is necessary for proper open reduction of a fracture and for proper plate application. With displaced diaphyseal fractures, cortical bone perfusion (CBP) is already compromised. Further damage, in terms of periosteal stripping for plate fixation, may not be acceptable. Little information is available as to what extent the periosteum contributes to cortical bone perfusion. The purpose of this study was to determine the acute effects of periosteal stripping on cortical bone perfusion in a sheep tibia model. Twenty-three sheep were operated on and had the medial aspect of their right tibia exposed. Cortical bone perfusion measurements were obtained using laser Doppler flowmetry prior to periosteal stripping and after periosteal stripping. The results of this study show that the cortical bone perfusion significantly decreased by 20% after periosteal stripping over the entire length of the tibia. We therefore conclude that the periosteum contributes to diaphyseal bone perfusion and that it is important to preserve this source with fractures where blood supply is already significantly compromised.
