Abstract: Sodium–glucose co-transporter-2 (SGLT2) inhibitors reduce progression of chronic kidney disease and the risk of cardiovascular morbidity and mortality in a wide range of patients. However, their effects on kidney disease progression in some patients with chronic kidney disease are unclear because few clinical kidney outcomes occurred among such patients in the completed trials. In particular, some guidelines stratify their level of recommendation about who should be treated with SGLT2 inhibitors based on diabetes status and albuminuria. We aimed to assess the effects of empagliflozin on progression of chronic kidney disease both overall and among specific types of participants in the EMPA-KIDNEY trial.

Abstract: Objective: An increase in coronal laxity is recognized as a risk factor for progression of knee osteoarthritis (OA). The purpose of this study was to evaluate coronal laxity, which was defined as the angular motion from the neutral, unloaded (baseline) position to the loaded position, in patients with advanced medial knee OA.Method: Preoperative coronal laxity was assessed using radiographs in patients with medial knee OA undergoing total knee arthroplasty by applying a force of 150 N with an arthrometer. A consecutive series of 211 knees with OA and 40 normal control knees were examined. A knee with OA was defined as clinically "balanced" when the difference between medial and lateral laxity was 3° or less. Values are expressed as median [25th, 75th percentile].Results: The laxity was 4° [3, 5] from the baseline on the medial side and 3° [2, 4] on the lateral side. The distribution of medial and lateral laxity indicated that 90% (189/211) of patients fell within 3°. The equivalence test showed that the medial and lateral laxity was similar, with an equivalence margin of 3° (P < 0.001). In the control knees, the laxity was 3° [2, 4] from the baseline on the medial side and 2° [2, 4] on the lateral side. The differences between the knees with advanced OA and the controls were significant (P = 0.005, medial; P = 0.006, lateral).Conclusion: This study showed that a clinically balanced knee was maintained even in patients with advanced medial knee OA. Abstract: Tumour hypoxia can be detected by 18 F-fluoromisonidazole positron emission tomography (FMISO-PET). Few studies have assessed the relationships of new PET parameters, including hypoxic volume (HV), metabolic tumour volume (MTV), and total lesion glycolysis (TLG), with 5-year survival of patients treated surgically for oral squamous cell carcinoma (OSCC). This study evaluated the relationships between these PET parameters and 5-year survival in OSCC patients. Twenty-three patients (age 42–84 years; 15 male, eight female) with OSCC underwent FMISO- and 18 F-fluoro-2-deoxyglucose (FDG)-PET computed tomography before surgery. All of them underwent radical surgery and were followed up for more than 5 years. The FDG-PET maximum standardized uptake value (SUV max ), HV, MTV, and TLG were measured. The ability of PET parameters to predict disease-free survival (DFS) and loco-regional recurrence (LR) was evaluated using receiver operating characteristic curve analysis. During the follow-up period, five of the 23 patients (22%) died and six (26%) experienced LR. Although FDG-PET SUV max was not significantly associated with DFS or LR, HV correlated significantly with both DFS and LR. TLG, but not MTV, was significantly associated with DFS; however neither MTV nor TLG was related significantly to LR. In conclusion, tumour HV may predict outcomes in patients with OSCC. Abstract: Permeation grouting has been developed as one of countermeasures against soil liquefaction during earthquakes. This study aims at examining the small strain properties and cyclic resistance of jellied sand improved by silicate-based permeation grouting. In the first part of the present study, the small strain properties and cyclic resistance of jellied sand were examined based on the measurements of propagation velocities of longitudinal and shear waves travelling through triaxial specimens and the results from subsequently conducted undrained cyclic triaxial tests. In the second part, straincontrolled small-amplitude cyclic triaxial tests are conducted using non-contact gap displacement transducers to examine the initial elastic moduli of silicate gel itself and intact sand as well as jellied sand. Abstract: Mouse embryonic stem (mES) cells and mouse induced pluripotent stem (miPS) cells are commonly maintained on inactivated mouse embryonic fibroblast feeder cells in medium supplemented with fetal bovine serum or proprietary replacements. An undefined medium containing unknown quantities of reagents has limited the development of applications for pluripotent cells because of the relative lack of knowledge regarding cell responses to differentiating growth factors. Therefore we developed a serum-free medium, designated ESF7, in which mES cells can be maintained in an undifferentiated state without feeder cells. The medium was tested for culturing miPS cells. The miPS cells have been maintained in ESF7 medium for more than 3 years with an undifferentiated phenotype manifested by the expression of pluripotency marker genes and alkaline phosphatase, and these cells exhibited largely normal karyotypes. Furthermore, we found that fibroblast growth factor-2 (FGF-2) with heparin induced miPS cell differentiation into neuronal cells, both in an adherent monolayer and in embryoid body suspension culture. Moreover, we found that FGF-2 with bone morphogenetic protein 2 induced miPS cell differentiation into cardiomyocytes in embryoid body suspension culture. Furthermore, we transplanted subcutaneously miPS cells maintained in ESF7 into the dorsal flanks of SCID mice; all of the transplants produced tumors with tissues derived from all three embryonic germ layers. As this simple serum-free adherent monoculture system supports the long-term propagation of pluripotent iPS cells in vitro, it will allow us to elucidate cell responses to growth factors under defined conditions, and it should provide useful information for differentiation protocols for human iPS cells. Abstract: Abstract: We present the case of a woman with congenital ADAMTS13 deficiency and discuss peripartum management of her fourth pregnancy. All four pregnancies were complicated by significant thrombocytopenia. Her first pregnancy ended with fetal demise ascribed to HELLP syndrome and placental abruption. During her second pregnancy, she was diagnosed with idiopathic thrombocytopenic purpura. Thrombotic thrombocytopenic purpura and congenital ADAMTS13 deficiency were diagnosed during the third pregnancy. She had recurrent thrombotic thrombocytopenic purpura during the fourth pregnancy and responded to treatment with fresh frozen plasma, with a successful outcome. Abstract: The target operating temperature of ultrasupercritical power plants is increasing and is planned to reach 700°C. Austenitic superalloys are promising materials for these applications to replace ferritic heat resistant steels, because of their high strength at 650–700°C. In general, austenitic nickel base superalloys show higher creep rupture strength than ferritic heat resistant steels; however, they have higher coefficients of thermal expansion, lower creep rupture ductilities, and higher costs. The effect of the Mo and Co content, amount of γ' phase, and Al/Ti ratio in the γ' phase on the thermal expansion behaviour of a Mo containing superalloy has been investigated by use of the conventional Mo containing Alloy 252 as a reference. Tensile and creep rupture properties were also measured. Following a modified heat treatment, the Co free superalloy developed on the basis of these tests showed higher creep rupture ductility than Alloy 252, while retaining comparable low thermal expansion and high creep rupture strength. Creep rupture properties at 700°C for up to 20 000 h were satisfactory, suggesting that the alloy is suitable for long term applications. Initial assessments of the weldability and mechanical properties of weld joints at 750°C are encouraging for boiler tube applications. [ABSTRACT FROM AUTHOR]

Abstract: Recently, aerostatic porous bearings have been used in ultra precision machine tools and precision-measuring equipment because they have higher stiffness and larger damping coefficients than other types of aerostatic bearings. However, an aerostatic porous bearing has a disadvantage in that it is prone to cause pneumatic hammer instability. Therefore, it is considered that reducing the permeability of the porous material only near the bearing surface in an aerostatic porous bearing is a very effective means of avoiding pneumatic hammer instability. In this article, the whirling instability of a rotor supported by aerostatic porous journal bearings with a surface-restricted layer is investigated theoretically and experimentally. 18. Concentrations and activity ratios of 228Ra and 226Ra in surface seawater along the Pacific coast of Japan

Abstract: We measured the 228Ra/226Ra activity ratios in surface seawater along the Pacific coast of Japan at five ports around the island of Izu-Oshima (n= 29), at Atami in Sagami Bay (n= 13), and at Umizuri Park in Tokyo Bay (n= 14). We also conducted these measurements along a transect from the open Pacific Ocean across the Kuroshio to the mouth of Tokyo Bay (n= 7). The activity ratios decreased with increasing salinity of the sampling sites. The 228Ra/226Ra activity ratios in surface seawater along the coast gradually decreased after at the end of autumn and were lowest in winter and the beginning of spring. The surface salinity along the coast decreased from summer into autumn and increased from winter to the beginning of spring. The activity ratios decreased with the increase of salinity.

19. Radioactive equilibria and disequilibria of U-series nuclides in the products from Izu arc volcanoes, Japan

Abstract: Activity ratios among 238U-230Th-226Ra in the products from Izu arc volcanoes, Japan, were observed in order to estimate the time scale of magmatic processes and the magma generation for Izu arc volcanism. Activity ratios of 238U/230Th and 226Ra/230Th in the basaltic and andesitic products from Izu arc volcanoes were greater than unity, being enriched in 238U and 226Ra relative to 230Th. The 226Ra/230Th activity ratio versus 238U/230Th activity ratio diagram for these products showed positive correlation, suggesting that the 238U-230Th-226Ra disequilibria occurred during the magma genesis by the additions of U- and Ra-rich fluids derived from the subducting slab by dehydration to the mantle wedge. The 230Th-226Ra radioactive disequilibria observed in the basaltic and andesitic products imply a short period of time (<8000 years) between the magma genesis and the eruption. The majority of rhyolitic products was considered to be almost in equilibrium of 238U=230Th=226Ra. The observation that 238U-230Th-226Ra for the rhyolite are in radioactive equilibrium suggested that the rhyolitic magma from Izu arc was generated in the partial melting of the earth crust heated by the basaltic magma of high temperature.

20. Leaf Gas Exchange Activity in Soybean as Related to Leaf Traits and Stem Growth Habit

Abstract: Understanding the relationships between leaf photosynthetic activity and physiological and morphological traits of soybeans [Glycine max. (L.) Merr.] with different stem growth habits could be exploited for yield improvement. Leaf photosynthetic rate (Pn), stomatal conductance (gs), leaf mass per unit area (LMA), and leaf nitrogen content per unit area (LNC) were measured for field‐grown progeny of a cross between an indeterminate type, ‘Stressland’, and a determinate type, ‘Tachinagaha’, soybean. The Pnof the uppermost fully expanded leaves of Stressland was 19% greater than Tachinagaha during the first 20 d after seed filling (growth stage R5) in 2005. Among 18 F3lines, Pnat 14 d after R5 in 2005 varied from 22.1 to 34.2 μmol CO2m−2s−1and was more strongly and significantly correlated with gs(P< 0.01) than with LMA (P< 0.05) and LNC (P= 0.11). In 2006, Pnof eight selected F4genotypes and the parents varied from 27.9 to 35.0 μmol CO2m−2s−1Leaf gas exchange activity of the selected lines was represented by two attributes; the maximum of gs(gmax), and functional leaf lifespan (LL). Values of gmaxwere positively correlated with stomatal density (r= 0.91, P< 0.01), and LL was positively correlated with LMA (r= 0.89, P< 0.01). Some indeterminate types had very high gmaxbut exhibited shorter LL. Genetic improvement of maximum leaf photosynthesis appears possible by increasing stomatal density, and the Dt1locus for stem habit has a positive effect on soybean leaf photosynthesis, both in its maximum and duration.

21. Ovarian Mucinous Cystadenoma with Smooth Muscle Proliferation in a Cynomolgus Monkey (Macaca fascicularis)

Abstract: An ovarian mucinous cystadenoma was found in a 5-year-old female cynomolgus monkey (Macaca fascicularis). The tumor was composed of various sizes of multilocular cystic glands lined by a single layer of mucin-filled epithelium. Each of these cystic glands was surrounded by a large amount of solid fibrous stroma resembling smooth muscle. The ovarian surface epithelium showed partial invagination into the ovarian cortex, and a transition was observed between the surface epithelium and the mucinous cyst-forming epithelium. Immunohistochemically, the stromal cells were positive for α-smooth muscle actin and proliferating cell nuclear antigen. Ultrastructurally, the glandular epithelium had numerous mucinous secretory granules and microvilli. The stromal cells had numerous parallel microfibrils with focal density. It is rare to encounter evidence of a transition from the surface epithelium to the mucinous tumor epithelium and to show stromal smooth muscle proliferation in a mucinous cystadenoma.

22. Direct introduction of mated and virgin queens using smoke: a method that gives almost 100% acceptance when hives have been queenless for 2 days or more

Abstract: SummaryWe compared the acceptance of virgin and mated queens introduced into queenless hives using either artificial queen cells or direct introduction accompanied by smoke. In Experiment 1, virgin queens aged 3–4 days were introduced into 5-frame hives than had been dequeened 1, 2, 3, 4, 5, or 6 days previously. Acceptance increased significantly with the length of time a colony had been queenless, and direct introduction gave significantly greater success than artificial queen cells (between 31% and 100% acceptance vs. 8% to 92% for direct and cell introduction respectively, depending on the period of queenlessness). In Experiment 2, virgin and mated queens were introduced into 2-frame observation hives that had been dequeened 1, 2, 3 and 4 days previously. The probability of acceptance was significantly higher for mated queens than virgins, for direct introduction versus artificial queen cells, and for longer queenless periods. Accordingly, the probability of a queen being balled by the workers declined significantly with the duration of the queenless period, and was significantly less for mated versus virgin queens. Finally, in Experiment 3, we introduced mated queens into medium-sized hives (10 medium Langstroth frames) that had been queenless for 2 days using both the direct introduction and artificial cells. 23. Application of underwater shock wave and laser-induced liquid jet to neurosurgery

Abstract: Paper deals with applications of underwater shock waves to medicine. A historical development of underwater shock wave generation by using pulsed Ho:YAG laser beam irradiation in water is briefly described and an overview is given regarding potential applications of shock waves to neuro-surgery. The laser beam irradiation in a liquid-filled catheter produces water vapor bubble and shock waves intermittently produces micro-liquid jets in a controlled fashion from the exit of the catheter. Correlations between shock dynamics and bubble dynamics are emphasized. To optimize the jet motion, results of basic parametric studies are briefly presented. The liquid jet discharged from the catheter exit has an impulse high enough to clearly exhibit effectiveness for various medical purposes. In liquid jets we observed reasonably strong shock waves and hence invented a compact shock generator aiming to apply to microsurgery. We applied it to a rat's bone window and developed an effective method of brain protection against shock loading. The insertion of Gore-Tex® sheet is found to attenuate shock waves drastically even for very short stand off distance and its physical mechanism is clarified. The laser-induced liquid jet (LILJ) is successfully applied to soft tissue dissection. Animal experiments were performed and results of histological observations are presented in details. Results of animal experiments revealed that LILJ can sharply dissect soft tissue with a minimum amount of liquid consumption, while blood vessels larger than 0.2 mm in diameter are preserved. Shock waves and LILJ have a potential to be indispensable tools in neuro-surgery.

24. Osteoid Osteoma Treated with Computed Tomography—Guided Percutaneous Radiofrequency Ablation: A Case Series

Abstract: The nidi of osteoid osteoma are small and difficult to identify precisely; this can necessitate substantial resection of the surrounding normal bone. We applied percutaneous, image-guided radiofrequency ablation to treat 5 patients with osteoid osteoma. The precise location of each nidus was determined using a 3-mm–thick spiral computed tomographic scan. The most appropriate needle pathway was selected to avoid major neural and vascular structures. A 14-gauge core needle was introduced into the nidus and then exchanged for a 17-gauge straight rigid electrode with a 1-cm exposed tip to heat the lesion to 90 degrees centigrade for 5 minutes. Four of the 5 patients were discharged on the day after surgery without any external supports, and were pain-free after a few days. The remaining patient had a pes equinus contracture induced by the penetration of the calf muscles through a posterior approach but was fully recovered 6 days later. The mean follow-up period was 20.6 months. No recurrences or late complications were observed in this series. This case series shows that the computed tomography–guided percutaneous radiofrequency ablation is a simple, minimally invasive, and highly effective technique for the treatment of osteoid osteoma.

25. Rheology of Block Copolypeptide Solutions: Hydrogels with Tunable Properties

Abstract: Amphiphilic block copolypeptides were prepared through transition-metal-mediated polymerization of amino acid N-carboxyanhydrides. In aqueous solution these materials form strong hydrogels at low concentrations. The self-assembly process that is responsible for gelation was investigated by measuring the rheological properties of the gels for a variety of molecular architectures: poly-l-lysine-b-poly-l-leucine diblock and poly-l-lysine-b-poly-l-leucine-b-poly-l-lysine triblock copolypeptides. Experiments showed that the rodlike helical secondary structure of enantiomerically pure poly-l-leucine blocks was instrumental for gelation at polypeptide concentrations as low as 0.25 wt %. The hydrophilic polyelectrolyte segments have stretched coil configurations and stabilize the twisted fibril assemblies by forming a corona around the hydrophobic core. The self-assembly of hydrophobic blocks is highly specific and sensitive to the chirality of the helices. It was found that mechanical properties of the gels can be tuned through the molecular architecture of the block copolypeptides and also by carefully mixing different polypeptides in solution.

26. Correlation and Linear Regression between Blood Pressure Decreases after a Test Dose Injection of Propofol and that following Anaesthesia Induction

Abstract: Propofol reduces systemic vascular resistance and suppresses cardiac function when injected rapidly. In this study we investigated whether blood pressure decrease after a minimal dose (test-dose) injection of propofol correlates with that after an induction-dose injection.Patients were randomly divided into two groups; anaesthesia was induced in group A (n=60) using 1.5 mg/kg propofol and in group B (n=61) using 2.0 mg/kg. Blood pressure reduction after a minimal dose injection (0.4 mg/kg) was examined non-invasively prior to anaesthetic induction. Bispectral Index monitoring was measured and sedation level scored to evaluate anaesthetic depth.After the minimal dose injection, 18 of 121 patients showed behaviour suggesting minor disinhibition, five patients were sedated and seven were drowsy. Oxygen saturation was not significantly changed after test-dose injection. Reduction in systolic blood pressure (mean±SD) was 17±11 mmHg after the minimal dose injection, 42±20 mmHg after a 1.5 mg/kg induction dose injection, and 42±22 mmHg after a 2.0 mg/kg induction-dose injection. In both groups, blood pressure after induction was significantly lower than the control value (P<0.05). In both groups, a positive correlation was observed between blood pressure reduction after the minimal dose injection and that after the induction-dose injection [P<0.01, R value for systolic blood pressure correlation in group A 0.712 (P<0.01) and in group B 0.758 (P<0.01)].We concluded there was a positive correlation between blood pressure reduction after a minimal (test-dose) injection and that after an induction-dose injection.

Abstract: Charcot-Marie-Tooth disease (CMT) is the most commonly inherited peripheral neuropathy. DNA duplication and point mutation of the gene encoding peripheral myelin protein 22 (PMP22) have been found in CMT type 1A dominants. To investigate the influence of the point mutation of PMP22 on the secondary structure, protected partial peptides in the putative first transmembrane domain, wild type Boc-IVLH(Bom)VAVLVLLFVSTIV-OMe (1) and its Pro¹⁶ mutant Boc-IVLH(Bom)VAVPVLLFVSTIV-OMe (2) were synthesized. Circular dichorism (CD)-spectral analysis suggested that peptide 1 adopts a stable α-helical conformation in membrane-mimetic solvent,1-BuOH/1,1,1,3,3,3-hexafluoro-2-propanol (HFIP) system. On the contrary, the mutant 2 favors β-sheet conformation in the same solvent system. Interestingly, α-helix to β-sheet transition of 2 was observed at higher contents of 1-BuOH than 70%.

Abstract: The Reuven Ramaty High-Energy Solar Spectroscopic Imager (RHESSI) observes solar hard X-rays and gamma-rays from 3 keV to 17 MeV with spatial resolution as high as 2.3 arc sec. Instead of focusing optics, imaging is based on nine rotating modulation collimators that time-modulate the incident flux as the spacecraft rotates. Starting from the arrival time of individual photons, ground-based software then uses the modulated signals to reconstruct images of the source. The purpose of this paper is to convey both an intuitive feel and the mathematical basis for this imaging process. Following a review of the relevant hardware, the imaging principles and the basic back-projection method are described, along with their relation to Fourier transforms. Several specific algorithms (Clean, MEM, Pixons and Forward-Fitting) applicable to RHESSI imaging are briefly described. The characteristic strengths and weaknesses of this type of imaging are summarized.

Abstract: The photocatalytic activity for water decomposition of RuO2-dispersed ZnGa2O4 with an octahedrally coordinated Ga³⁺ ion was studied in order to examine the photocatalytic properties of p-block metal oxides with d¹⁰ configuration. ZnGa2O4 was impregnated with Ru3(CO)12 or Ru(C5H7O2)3 in THF and oxidized to convert them to RuO2. For RuO2-loaded ZnGa2O4, hydrogen and oxygen were stably produced from the initial stage of reaction under Hg−Xe lamp light irradiation. The activity dependence on the calcination temperature of ZnGa2O4, the amount of RuO2 loaded, and the oxidation temperature of RuO2 showed that the dispersion of small RuO2 particles leads to high photocatalytic activity. The electronic structure of ZnGa2O4 was calculated by a plane-wave-based density function theory (DFT). The valence bands were mainly composed of the O 2p orbitals, whereas the conduction bands were formed by hybridzation of Ga 4s4p and Zn 4s4p orbitals. Large dispersion observed in the conduction band indicated the large mobility of photoexcited electrons, which was considered to be responsible for high photocatalytic performance. The feature of electronic structures relating to photocatalysis was discussed in comparison with those of transition metal oxides.

31. Photothermal characterization of highly porous, polycrystalline TiO2electrodes fabricated by chemical synthesis

Abstract: We report the optical absorption characteristics of highly porous, polycrystalline TiO2electrodes and the influence of hydrolysis period for the preparation processes by photoacoustic (PA) spectroscopy together with photoelectrochemical (PEC) current ones. The PA spectra show peaks which are attributed to the lowest transition energy due to the quantum confinement effect. The peak intensity decreases with the increase of hydrolysis periods, indicating the possibilities of the changes in the thermal properties and the densities due to hydrolysis processes. The PEC spectra indicate that the photocurrent intensity also show peak and that of the longer hydrolysis periods is somewhat smaller than others, indicating the increase of interface states due to the formation of grain boundaries with the increase of hydrolysis processes.

32. Importance of the changes in joint effusion shown by magnetic resonance imaging before and after arthroscopic lysis and lavage of the temporomandibular joint

Abstract: We investigated the changes in the amount of joint effusion estimated from T2-weighted magnetic resonance imaging (MRI) before and after arthroscopic lysis and lavage of the temporomandibular joint (TMJ). We studied 29 consecutive patients, each with internal derangement and osteoarthritis in one TMJ. Before operation, the MRI showed joint effusions in 22 of the patients (76%). After operation, the amount of the effusion decreased in 16 and increased in 2 patients. Effusions developed postoperatively in four of the seven patients who had no effusion before operation. In four of the six patients in whom the effusion increased, the symptoms had almost resolved by the time the MRI was taken. There was no significant correlation between changes in the amount of joint effusion and the clinical condition of the patients before and after the operation. In conclusion, changes in the amount of joint effusion in the TMJ are not related to the patient's clinical condition.

33. Structural Change of the Surroundings of K + in K 2 ZnBr 4 Crystal on the Monoclinic-Orthorhombic Phase Transition

Abstract: In K 2 ZnBr 4 crystal, the monoclinic phase with the Sr 2 GeS 4 -type structure ( P 2 1 / m , Z =2) is transformed into the orthorhombic phase with the g -K 2 SO 4 -type structure ( Pmcn , Z =4) above 470K (an f - g transition). This f - g transition is known to be a reconstructive one. The crystal structure of the orthorhombic phase is refined in order to investigate in detail the structural change on the phase transition. It is found that the structure of the surroundings of one of two independent K + 's in the monoclinic phase is almost the same as the rotated structure of the surroundings of one of two independent K + 's in the orthorhombic one. We also discuss the difference between the crystal structures of K 2 ZnCl 4 (orthorhombic) and K 2 ZnI 4 (monoclinic) and the corresponding structures of K 2 ZnBr 4 .

35. Effects of minor surgery and endotracheal intubation on postoperative breathing patterns in patients anaesthetized with isoflurane or sevoflurane.

Abstract: We studied the effects of minor surgery and endotracheal intubation on postoperative breathing patterns. We measured breathing patterns and laryngeal resistance during the periods immediately before intubation (preoperative) and immediately after extubation following minor surgery (postoperative) in eight patients anaesthetized with sevoflurane and eight patients anaesthetized with isoflurane, breathing spontaneously through a laryngeal mask airway at a constant end-tidal anaesthetic concentration (1.0 MAC). In both sevoflurane-anaesthetized and isoflurane-anaesthetized patients, expiratory time was reduced and inspiratory and expiratory laryngeal resistance increased after surgery. In sevoflurane-anaesthetized patients, occlusion pressure (P0.1) increased without changes in inspiratory time (T(I)). Occlusion pressure did not change and T(I) was greater in isoflurane-anaesthetized patients after surgery. Minor surgery may have a small but significant influence on breathing and increased laryngeal resistance following endotracheal intubation may modulate these changes. The difference in breathing pattern between sevoflurane and isoflurane may be a result of different responses of the central nervous system to different anaesthetics in the presence of increased laryngeal resistance.

36. Direct identification of a major autophosphorylation site on vascular endothelial growth factor receptor Flt-1 that mediates phosphatidylinositol 3′-kinase binding

Abstract: Progress has been made in our understanding of the mechanism by which the binding of vascular endothelial growth factor (VEGF) to cognate receptors induces a range of biological responses, but it is far from complete. Identification of receptor autophosphorylation sites will allow us to determine how activated VEGF receptors are coupled to specific downstream signalling proteins. In the present study, we have expressed human VEGF receptors in insect cells using the baculovirus expression system, identified a major autophosphorylation site on the VEGF receptor fms-like tyrosine kinase-1 (Flt-1) by HPLC–electrospray ionization (ESI)–MS, and characterized in vitro interactions between Flt-1 and phosphatidylinositol 3′-kinase (PI3-kinase). Infection of High 5 insect cells with Flt-1 recombinant virus resulted in the expression of a 170kDa glycoprotein, which bound VEGF with a Kd of 2×10−10M in intact insect cells. The overexpressed recombinant Flt-1 receptors exhibited tyrosine kinase activity and were constitutively phosphorylated. Analysis of Flt-1 tryptic peptides by HPLC–ESI–MS with selective phosphate ion monitoring identified a hexapeptide (YVNAFK; where single-letter amino-acid code has been used) containing a phosphotyrosine (pTyr) residue at position 1213. Using synthetic phosphopeptides, this pTyr residue was found to be directly involved in the binding of PI3-kinase in vitro even though it did not fall within a consensus pYM/VXM PI3-kinase binding motif. These results suggest that phosphorylated Flt-1 associates with PI3-kinase at pTyr1213 to mediate the activation of this pathway in VEGF signalling.

Abstract: Perlecan, a widespread heparan sulfate proteoglycan, functions as a bioactive reservoir for growth factors by stabilizing them against misfolding or proteolysis. These factors, chiefly members of the fibroblast growth factor (FGF) gene family, are coupled to the N-terminal heparan sulfate chains, which augment high affinity binding and receptor activation. However, rather little is known about biological partners of the protein core. The major goal of this study was to identify novel proteins that interact with the protein core of perlecan. Using the yeast two-hybrid system and domain III of perlecan as bait, we screened ∼0.5 106cDNA clones from a keratinocyte library and identified a strongly interactive clone. This cDNA corresponded to FGF-binding protein (FGF-BP), a secreted protein previously shown to bind acidic and basic FGF and to modulate their activities. Using a panel of deletion mutants, FGF-BP binding was localized to the second EGF repeat of domain III, a region very close to the binding site for FGF7. FGF-BP could be coimmunoprecipitated with an antibody against perlecan and bound in solution to recombinant domain III-alkaline phosphatase fusion protein. Immunohistochemical analyses revealed colocalization of FGF-BP and perlecan in the pericellular stroma of various squamous cell carcinomas suggesting a potential in vivointeraction. Thus, FGF-BP should be considered a novel biological ligand for perlecan, an interaction that could influence cancer growth and tissue remodeling.

Abstract: Tumor-derived adhesion factor/angiomodulin (AGM) is accumulated in tumor blood vessels and on the endothelial cell surface (Akaogi, K., Okabe, Y., Sato, J., Nagashima, Y., Yasumitsu, H., Sugahara, K., and Miyazaki, K. (1996) Proc. Natl. Acad. Sci. U. S. A. 93, 8384-8389). In cell culture, it promotes cell adhesion and morphological changes to form cord-like structures of the human bladder carcinoma cell line ECV-304. The cord formation is prevented by heparin, which inhibits the binding of AGM to ECV-304 cells. This observation suggests that AGM interacts with cell surface heparan sulfate (HS) proteoglycans. In this study, HS glycosaminoglycans and core proteins of integral transmembrane proteoglycans, syndecan-1 and -4, were identified by immunocytochemistry on ECV-304 cells, and the structural requirements for the interaction of HS with AGM were characterized. Inhibition experiments with sulfated polysaccharides and chemically modified heparin derivatives indicated that sulfate groups were essential for both AGM-HS binding and cord-like structure formation and that the rank order of the different sulfate groups in terms of their contribution was N-sulfate > 6-O-sulfate > 2-O-sulfate. The minimum size of heparin, a chemical analog of HS, required for the binding to AGM was a dodecasaccharide as determined by competition experiments using size-defined heparin oligosaccharides. Thus, a specific sulfation pattern in the HS of cell surface syndecans of ECV-304 cells is required for AGM binding and the morphological changes.

49. Anti-idiotypic antibodies in patients with different clinical forms of paracoccidioidomycosis.

Abstract: Paracoccidioidomycosis (PCM) is the most prevalent systemic mycosis in Latin America. Patients with PCM show a wide spectrum of clinical and pathological manifestations depending on both host and pathogen factors. Two clinical forms of the disease are recognized: the acute or juvenile form and the chronic or adult form. The major antigenic component of the parasite is a glycoprotein of 43 kDa (gp43). All patient sera present antibodies against gp43 (anti-gp43) and, as demonstrated before by our group, spontaneous anti-idiotypic (anti-Id) antibodies (Ab2) can be detected in patient sera with high titers of anti-gp43. Since it has been postulated that anti-Id antibodies may have a modulating function, we decided to purify and characterize anti-Id antibodies in this system. The possible correlation of Ab2 titers with different clinical forms of disease was also verified. Results showed that purified human anti-Id antibodies (human Ab2) recognized specifically the idiotype of some murine monoclonal anti-gp43 (17c and 3e) but not others (40.d7, 27a, and 8a). Spontaneous anti-Id antibodies were found in all clinical forms of disease. The majority of patients (88%, n = 8) with the acute form of PCM had high titers of Ab2. However, among patients with the multifocal chronic form of the disease, only 29% (n = 14) had high titers of Ab2; 70% (n = 10) of patients with the unifocal chronic form had low titers of Ab2. A correlation between Ab2 titers and anti-gp43 titers was observed before and during antimycotic treatment. Our results suggest that titers of anti-Id antibodies correlate with the severity of PCM in humans.

50. Subacute hepatic failure associated with a new antidiabetic agent, troglitazone: A case report with autopsy examination

Abstract: An autopsy case of fatal subacute hepatic failure after administration of troglitazone is described. The liver dysfunction developed about five months after the patient, a sixty-three-year-old woman, had been initially treated with troglitazone. The patient developed hepatic failure and died despite various hepatic auxiliary treatments such as plasmapheresis. Autopsy findings revealed focal liver cell necrosis, cholestasis and steatosis with infiltration of lymphocytes and neutrophils and lack of regenerative activity. The causative mechanism of liver dysfunction may be metabolite aberration, as a result of accumulation of hepatotoxic metabolite(s), in a category of idiosyncratic liver injury. It is proposed to monitor liver function strictly and periodically for the diabetic patients prescribed troglitazone.
