COVID-19 has spiked worldwide, having multiple outbreaks even with the production of vaccines. Imaging exams, such as Computer Tomography (CT) and X-ray, are recommended by the World Health Organization after performing RT-PCR tests to enhance COVID-19 diagnosis for serious cases. This work proposes a deep learning methodology to evaluate whether a patient presents COVID-19-related findings in CT images as an auxiliary diagnostic tool. As a CT exam produces many images related to a patient, some are irrelevant for COVID-19 diagnosis (e.g., closed lungs), using the raw information may hinder the model. Hence, we provide a CT scan image selection algorithm to filter the most informative images with two versions: (a) non-sequential, and (b) sequential. Then, online data augmentation is applied before feeding these images to a Convolutional Neural Network (CNN). Moreover, we evaluate the performance of the model for both versions of the CT selection algorithm in different approaches: (i) 'per-image', (ii) 'per-patient majority voting', and (iii) 'per-patient conservative voting (30%)'. We applied the proposed methodology in a Brazilian university hospital, a reference for COVID-19 treatment. For the test set, approaches (a-i), (a-ii), and (a-iii) reached an accuracy of 84.6%, 70%, and 70%, respectively, while approaches (b-i), (b-ii), and (b-iii) reached 90.9%, 80%, and 80%, respectively. Hence, we consider the proposed sequential version the most suitable image selection algorithm for the analyzed data set and useful for supporting decisions in COVID-19 diagnosis. Background: Intraocular metastasis is most commonly found within the choroid because of its luxurious blood supply, whereas metastasis to the retina, from a separate blood supply, is very rare. We report a rare case of synchronous metastases from lung carcinoma to the choroid of the right eye and the retina of the left eye and a complaint of low vision in the right eye. Methods: We report a 63-year-old woman with progressive reduction of vision in her right eye for 1 year without previous history of cancer. The patient underwent comprehensive ophthalmic examination that included a multimodal evaluation: near-infrared reflectance scanning laser ophthalmoscopy, blue autofluorescence, and enhanced-depth imaging spectral domain optical coherence tomography (EDI SD-OCT; Heidelberg Engineering, Heidelberg, Germany). Results: Ophthalmoscopic examination revealed a yellow choroidal mass measuring 6 · 6 mm with overlying subretinal fluid and involving the macular area in the right eye. In the left eye, there was a superior juxtapapillary yellow-colored mass involving the retina and measuring 1 x 1 mm. Optical coherence tomography revealed the subretinal fluid contouring a dome-shaped choroidal mass in the right eye and intraretinal nodular tumor superior to the optic disk of the left eye. Chest computed tomography revealed a pulmonary nodule measuring 2.5 mm x 1.4 mm, and biopsy of the liver metastasis confirmed the diagnosis of mucinous lung carcinoma. Conclusion: The final diagnosis was presumed choroidal metastasis in the right eye and retinal metastasis in the left eye from underlying mucinous carcinoma of the lung. [ABSTRACT FROM AUTHOR]

Abstract: INTRODUCTION: Ectopic hepatic tissue is due to an uncommon failure of embryological liver development that is rarely described in the world medical literature. The incidence of ectopic liver (EL) has been reported to be anywhere from 0.24% to 0.47% as diagnosed at laparotomy or laparoscopy. We describe a case of EL adherent to the gallbladder, removed at laparoscopic cholecystectomy. PRESENTATION OF CASE: A 37-year-old female was admitted for elective cholecystectomy having had an episode of acute cholecystitis provoked by gallstones. During the procedure, a 30mm×10mm×5mm section of EL tissue attached to the anterior wall of the gallbladder was identified and removed by en-bloc excision during laparoscopic cholecystectomy. Histological examination confirmed the absence of malignant degeneration of the hepatic tissue. The patient recovered well postoperatively and was discharged the day after the operation. She was well when seen six months later. DISCUSSION: EL has been reported in several sites, such as the gallbladder, gastrohepatic ligament, adrenal glands, esophagus, and thoracic cavity. EL is often clinically silent and discovered incidentally during abdominal surgical procedures or autopsies. Because patients with ectopic liver may suffer complications such as torsion, peritoneal bleeding, fatty change, and evolution to cirrhosis or malignant degeneration to hepatocellular carcinoma, any ectopic liver tissue needs to be correctly identified and removed. CONCLUSION: Despite the rare occurrence of EL, it should be recognized and removed by the surgeon to prevent a higher risk of complications and malignant transformation. [Copyright &y& Elsevier]
