We present an automatic lung lobe segmentation algorithm for COPD patients. The method enhances fissures, removes unlikely fissure candidates, after which a B-spline is fitted iteratively through the remaining candidate objects. The iterative fitting approach circumvents the need to classify each object as being part of the fissure or being noise, and allows the fissure to be detected in multiple disconnected parts. This property is beneficial for good performance in patient data, containing incomplete and disease-affected fissures. The proposed algorithm is tested on 22 COPD patients, resulting in accurate lobe-based densitometry, and a median overlap of the fissure (defined 3 voxels wide) with an expert ground truth of 0.65, 0.54 and 0.44 for the three main fissures. This compares to complete lobe overlaps of 0.99, 0.98, 0.98, 0.97 and 0.87 for the five main lobes, showing promise for lobe segmentation on data of patients with moderate to severe COPD.

Intravascular optical coherence tomography (IVOCT) provides very high resolution cross-sectional image sequences of vessels. It has been rapidly accepted for stent implantation and its follow up evaluation. Given the large amount of stent struts in a single image sequence, only automated detection methods are feasible. In this paper, we present an automated stent strut detection technique which requires neither lumen nor vessel wall segmentation. To detect strut-pixel candidates, both global intensity histograms and local intensity profiles of the raw polar images are used. Gaussian smoothing is applied followed by specified Prewitt compass filters to detect the trailing shadow of each strut. The shadow edge positions assist the strut-pixel candidates clustering. In the end, a 3D guide wire filter is applied to remove the guide wire from the detection results. For validation, two experts marked 6738 struts in 1021 frames in 10 IVOCT image sequences from a one-year follow up study. The struts were labeled as malapposed, apposed or covered together with the image quality (high, medium, low). The inter-observer agreement was 96%. The algorithm was validated for different combinations of strut status and image quality. Compared to the manual results, 93% of the struts were correctly detected by the new method. For each combination, the lowest accuracy was 88%, which shows the robustness towards different situations. The presented method can detect struts automatically regardless of the strut status or the image quality, which can be used for quantitative measurement, 3D reconstruction and visualization of the implanted stents.

With fluoroscopic analysis of knee implant kinematics the implant contour must be detected in each image frame, followed by estimation of the implant pose. With a large number of possibly low-quality images, the contour detection is a time-consuming bottleneck. The present paper proposes an automated contour detection method, which is integrated in the pose estimation.In a phantom experiment the automated method was compared with a standard method, which uses manual selection of correct contour parts. Both methods demonstrated comparable precision, with a minor difference in the Y-position (0.08 mm versus 0.06 mm). The precision of each method was so small (below 0.2 mm and 0.3°) that both are sufficiently accurate for clinical research purposes.The efficiency of both methods was assessed on six clinical datasets. With the automated method the observer spent 1.5 min per image, significantly less than 3.9 min with the standard method. A Bland–Altman analysis between the methods demonstrated no discernible trends in the relative femoral poses.The threefold increase in efficiency demonstrates that a pose estimation approach with integrated contour detection is more intuitive than a standard method. It eliminates most of the manual work in fluoroscopic analysis, with sufficient precision for clinical research purposes.

Abstract: To qualitatively assess the accuracy of automated cardiovascular magnetic resonance planning procedures devised from scout acquisitions in free-breathing and breath-holding modes, to quantitatively evaluate the accuracy of the derived left ventricular volumes, mass and function and compare these parameters with the ones obtained from the manually planned acquisitions. Ten healthy volunteers underwent cardiovascular MR (CMR) acquisitions for ventricular function assessment. Short-axis data sets of the left ventricle (LV) were manually planned and generated twice in an automatic fashion. Automated planning parameters were derived from gated scout acquisitions in free-breathing and breath-holding modes. End-diastolic volume (EDV), end-systolic volume (ESV), ejection fraction (EF), and left ventricular mass (LVM) were measured. The agreement between the manual and automatic planning methods, as well as the variability of the aforementioned measurements were assessed. The differences between two automated planning methods were also compared. The mean differences between the manual and automated CMR planning derived from gated scouts in free-breathing mode were 8.05 ml (EDV), 1.84 ml (ESV), 0.69% (EF), and 4.72 g (LVM). The comparison between manual and automated CMR planning derived from gated scouts in breath-holding mode yielded the following differences: 4.22 ml (EDV), 0.34 ml (ESV), 0.3% (EF), and –0.72 mg (LVM). The variability coefficients were 3.72 and 3.66 (EDV), 5.6 and 8.19 (ESV), 3.46 and 4.31 (EF), 6.49 and 5.20 (LVM) for the automated CMR planning methods derived from scouts in free-breathing and breath-holding modes, respectively. Automated CMR planning methods can provide accurate measurements of LV dimensions in normal subjects, and therefore may be utilized in the clinical environment to provide a cost-effective solution for functional assessment of the human cardiovascular system.

Abstract: In vivo MRI provides a means to non-invasively image and assess the morphological features of atherosclerotic carotid arteries. To assess quantitatively the degree of vulnerability and the type of plaque, the contours of the lumen, outer boundary of the vessel wall and plaque components, need to be traced. Currently this is done manually, which is time-consuming and sensitive to inter- and intra-observer variability. The goal of this work was to develop an automated contour detection technique for tracing the lumen, outer boundary and plaque contours in carotid MR short-axis black-blood images. Seventeen patients with carotid atherosclerosis were imaged using high-resolution in vivo MRI, generating a total of 50 PD- and T1-weighted MR images. These images were automatically segmented using the algorithm presented in this work, which combines model-based segmentation and fuzzy clustering to detect the vessel wall, lumen and lipid core boundaries. The results demonstrate excellent correspondence between automatic and manual area measurements for lumen (r=0.92) and outer (r=0.91), and acceptable correspondence for fibrous cap thickness (r=0.71). Though further optimization is required, our algorithm is a powerful tool for automatic detection of lumen and outer boundaries, and characterization of plaque in atherosclerotic vessels.

Summary The goal of this investigation was to establish which anatomical parameters of stenotic lesions correlate best with its functional severity. Therefore, thirty-eight patients with single vessel disease underwent coronary cineangiography and exercise/redistribution thallium-201 scintigraphy.

Summary Current evidence with regard to the possible association between clinical expression of coronary disease prior to the time of angioplasty, and the subsequent risk of restenosis following successful dilatation, remains inconclusive.

Summary It is the objective of the INTACT-study to test in man, whether a significant retardation of the progression of coronary artery disease is attainable with the Ca-antagonist nifedipine; this may be possible on the basis of numerous animal experiments. INTACT is a prospective, randomized, double blind, placebo controlled investigation in 423 patients with preferably early stages of coronary sclerosis in whom a progression of the disease seems likely. A proper coronary angiogram led to inclusion of the patients in the study (October 1983–June 1985). Over the following 3-years-period patients received either nifedipine 80 mg/day or placebo. The study is concluded by a control coronary angiogram with angiographic projections which are identical to those of the first coronary angiography. The extent of coronary sclerosis is objectivated by computer-assisted quantitative measurement of the entire coronary arterial system with the CAAS-system (Rotterdam). For definition purposes the coronary artery system subdivided into 25 segments. Parameters for progression assessment will be mean segment diameter, minimal obstruction diameter, percentage severity of obstruction, length of obstruction and plaque area.

After myocardial infarction, left ventricular volume and ejection fraction can be assessed by echocardiography, magnetic resonance imaging and radionuclide angiography to guide therapy and determine prognosis. Whether a measured parameter gives the same results irrespective of the method used and the observer who performs the analysis is only partly known. Intra-observer and inter-observer variability were determined for echo and magnetic resonance imaging. Left ventricular ejection fraction measured by these techniques was related to radionuclide angiograms performed in the same period. Intra-observer variability for both echo and MRI was low and in most instances below 5%. Inter-observer variability for the echo and MRI measurements were substantially higher than intra-observer variability. Comparison of the three imaging modalities revealed systematic differences. Therefore, in clinical studies, left ventricular volume and function parameters have to be measured with the same technique and by the same observer in qualified core laboratories.

Uptake of thallium 201 (201Tl) in the lungs has been proposed as a measure of left ventricular dysfunction with exercise. To study this hypothesis, we compared the lung/-heart (LH) ratio assessed from anterior planar images (ANT-P), from anterior images obtained during single photon emission tomography (SPET) acquisition (ANT-T) and from short-axis tomographic cross-sections (CS) in early post-exercise thallium 201 scintigrams. The study population consisted of 54 prepercutaneous transluminal coronary angioplasty (PTCA) studies (82% with single-vessel disease), 50 post-PTCA studies, 33 pre-coronary artery bypass surgery (CABG) studies (71 % with three-vessel disease), 30 post-CABG studies and 30 patients with a left ventricular dysfunction (LVD) due to an acute myocardial infarction; 18 individuals with a low likelihood of coronary artery disease (CAD) served as a control group. The results demonstrated that, on average, the LH ratios obtained from ANT-P and ANT-T were not significantly different for all study groups; these ratios increased significantly with ischaemia and with LVD relative to non-ischaemic situations. However, the LH ratios in CS did not show a relation with ischaemia nor with LVD and differed significantly from the LH-ratios assessed from the anterior approaches. Each of the three approaches (ANT-P, ANT-T, CS) was characterized by large overlaps of LH ratios for the different study groups. In conclusion, the LH ratio should be determined from ANT-P or ANT-T projection images, not CS. However, due to large overlaps in the measurement data, it is not a clinically useful parameter for the detection of CAD or LVD in the individual patient.

Summary The assessment of coronary flow reserve from the instantaneous distribution of the contrast agent within the coronary vessels and myocardial muscle at the control state and at maximal flow has been limited by the superimposition of myocardial regions of interest in the two-dimensional images. To overcome these limitations, we are in the process of developing a three-dimensional (3D) reconstruction technique to compute the contrast distribution in cross sections of the myocardial muscle from two orthogonal cineangiograms. To limit the number of feasible solutions in the 3D-reconstruction space, the 3D-geometry of the endo- and epicardial boundaries of the myocardium must be determined. For the geometric reconstruction of the epicardium, the centerlines of the left coronary arterial tree are manually or automatically traced in the biplane views. Next, the bifurcations are detected automatically and matched in these two views, allowing a 3D-representation of the coronary tree. Finally, the circumference of the left ventricular myocardium in a selected cross section can be computed from the intersection points of this cross section with the 3D coronary tree using B-splines. For the geometric reconstruction of the left ventricular cavity, we envision to apply the elliptical approximation technique using the LV boundaries defined in the two orthogonal views, or by applying more complex 3D-reconstruction techniques including densitometry. The actual 3D-reconstruction of the contrast distribution in the myocardium is based on a linear programming technique (Transportation model) using cost coefficient matrices. Such a cost coefficient matrix must contain a maximum amount of a priori information, provided by a computer generated model and updated with actual data from the angiographic views. We have only begun to solve this complex problem. However, based on our first experimental results we expect that the linear programming approach with advanced cost coefficient matrices and computed model will lead to acceptable solutions in the 3D-reconstruction of the myocardial contrast distribution from biplane cineangiograms.

For multi atlas-based segmentation approaches, a segmentation fusion scheme which considers local performance measures may be more accurate than a method which uses a global performance measure. We improve upon an existing segmentation fusion method called SIMPLE and extend it to be localized and suitable for multi-labeled segmentations. We demonstrate the algorithm performance on 23 CT scans of COPD patients using a leave-one- out experiment. Our algorithm performs significantly better (p < 0.01) than majority voting, STAPLE, and SIMPLE, with a median overlap of the fissure of 0.45, 0.48, 0.55 and 0.6 for majority voting, STAPLE, SIMPLE, and the proposed algorithm, respectively.

Cardiac magnetic resonance perfusion imaging (CMR) and computed tomography angiography (CTA) are widely used to assess heart disease. CMR is used to measure the global and regional myocardial function and to evaluate the presence of ischemia; CTA is used for diagnosing coronary artery disease, such as coronary stenoses. Nowadays, the hemodynamic significance of coronary artery stenoses is determined subjectively by combining information on myocardial function with assumptions on coronary artery territories. As the anatomy of coronary arteries varies greatly between individuals, we developed a patient-specific tool for relating CTA and perfusion CMR data. The anatomical and functional information extracted from CTA and CMR data are combined into a single frame of reference. Our graphical user interface provides various options for visualization. In addition to the standard perfusion Bull's Eye Plot (BEP), it is possible to overlay a 2D projection of the coronary tree on the BEP, to add a 3D coronary tree model and to add a 3D heart model. The perfusion BEP, the 3D-models and the CTA data are also interactively linked. Using the CMR and CTA data of 14 patients, our tool directly established a spatial correspondence between diseased coronary artery segments and myocardial regions with abnormal perfusion. The location of coronary stenoses and perfusion abnormalities were visualized jointly in 3D, thereby facilitating the study of the relationship between the anatomic causes of a blocked artery and the physiological effects on the myocardial perfusion. This tool is expected to improve diagnosis and therapy planning of early-stage coronary artery disease.
