Las patologías del aparato locomotor son la primera causa de discapacidad en adultos en el mundo. Los tratamientos de rehabilitación y de readaptación, de los que el principal es la masokinesiterapia, forman parte de los tratamientos de primera elección en estas patologías, con un beneficio y una eficacia demostradas tanto al nivel individual como colectivo. La prescripción de masokinesiterapia se inscribe en un marco legislativo que se ha modificado en las últimas décadas para ampliar el campo de ejercicio de los masajistas-kinesiterapeutas. Para garantizar el secreto médico, el diagnóstico no debe aparecer en la prescripción, pero puede figurar en un correo del médico prescriptor al masajista-kinesiterapeuta. Éste debe informar al médico prescriptor de la evolución del tratamiento a partir de sus fichas de resumen y puede prescribir, si no existen contraindicaciones médicas, ciertos dispositivos médicos que figuran en una lista limitativa. Por último, el concepto de prescripción cuantitativa y cualitativa, suprimido en el año 2000, permite al masajista-kinesiterapeuta definir, al final de la evaluación diagnóstica de kinesiterapia, el número y la frecuencia de las sesiones, así como las técnicas que se deben utilizar. El kinesiterapeuta tiene a su disposición muchas técnicas que se pueden dividir esquemáticamente en dos categorías: las técnicas pasivas, que engloban los masajes y la fisioterapia, útiles con fines antálgicos, pero cuya eficacia es limitada en el tiempo, y las técnicas activas de ganancia de amplitud articular, de refuerzo muscular y de rehabilitación sensitivomotriz, cuya eficacia a lo largo del tiempo puede aumentarse por la autonomización del paciente y el aprendizaje de un autoprograma. La elección y la asociación de diferentes técnicas durante el tratamiento es frecuente y depende de los objetivos terapéuticos y funcionales, que deben definirse claramente antes del tratamiento, entre el médico prescriptor, el masajista-kinesiterapeuta y el paciente. Estos objetivos deben reevaluarse para adaptar el tratamiento de rehabilitación y decidir las modalidades de interrupción.

•Early rehabilitation is needed for septic arthritis in native knee.•A four-phase rehabilitation strategy has been developed: recovery of joint amplitudes, muscle recovery without resistance, recovery with technical aid (crutches, canes), continuation of the rehabilitation (pursuit of muscle, articular, proprioceptive and endurance recovery).•Prospective validation of this strategy is needed to confirm its feasibility and effectiveness.

ObjectiveTo study pain coping strategies in patients with hip and knee osteoarthritis OA, and to assess the psychometric qualities of the French version of the Pain Coping Inventory PCI.MethodsWe conducted a national, crosssectional survey in a primary care setting in France. A total of 1,811 general practitioners included 5,324 patients with hip and knee OA who completed several questionnaires, including the PCI, which assesses ability to cope with pain.ResultsThe records of 4,719 86.4 patients were analyzed knee 2,781; hip 1,553; hip and knee 385. Supporting the structure of the original questionnaire, we found that the 33 PCI questionnaire items could be grouped into 3 domains defining active coping strategies and 3 defining passive coping strategies. Acceptable convergent validity was found for the PCI Cronbachs alpha coefficient for each domain >0.68. Coping strategy scores were significantly higher in patients with both knee and hip involvement mean ± SD 2.3 ± 0.4 than for patients with OA at 1 site mean ± SD 2.1 ± 0.4, and in women compared with men P< 0.001. The use of passive pain coping strategies increased with OA duration, and was greater in older and overweight patients, in patients with no current physical activity or major impairment, in retired and nonworking patients, and in patients who were not married, and to a lesser extent in patients with higher pain intensity. Compared with previous data, patients with OA demonstrated lower active and higher passive strategies than patients with rheumatoid arthritis and other chronic painful conditions.ConclusionThe PCI has good structural validity and is highly suitable for analyzing active and passive pain coping strategies in OA. In OA, active and passive coping strategies differ significantly as a function of age, body mass index, OA involvement, professional and marital status, sport activities, and OA duration, with pain intensity having a weaker effect.

Monte Carlo simulation is an essential tool in emission tomography that can assist in the design of new medical imaging devices, the optimization of acquisition protocols and the development or assessment of image reconstruction algorithms and correction techniques. GATE, the Geant4 Application for Tomographic Emission, encapsulates the Geant4 libraries to achieve a modular, versatile, scripted simulation toolkit adapted to the field of nuclear medicine. In particular, GATE allows the description of time-dependent phenomena such as source or detector movement, and source decay kinetics. This feature makes it possible to simulate time curves under realistic acquisition conditions and to test dynamic reconstruction algorithms. This paper gives a detailed description of the design and development of GATE by the OpenGATE collaboration, whose continuing objective is to improve, document and validate GATE by simulating commercially available imaging systems for PET and SPECT. Large effort is also invested in the ability and the flexibility to model novel detection systems or systems still under design. A public release of GATE licensed under the GNU Lesser General Public License can be downloaded at http://www-lphe.epfl.ch/GATE/. Two benchmarks developed for PET and SPECT to test the installation of GATE and to serve as a tutorial for the users are presented. Extensive validation of the GATE simulation platform has been started, comparing simulations and measurements on commercially available acquisition systems. References to those results are listed. The future prospects towards the gridification of GATE and its extension to other domains such as dosimetry are also discussed.

ObjectiveTo assess and compare the test–retest reliability and the construct validity of the Lequesne and the French-Canadian version of the WOMAC indexes.

We investigate the possibility that alterations in the calcium movements of the hypertrophied rat heart might involve sarcoplasmic reticulum (SR) ryanodine receptors. A decreased receptor density was observed with severe hypertrophy (0.26±0.05 and 0.35±0.06 pmol/mg protein and 170 and 366 receptors/μm 2of SR in 50–80% hypertrophy and control, respectively); however, the total number of receptors per left ventricle was unchanged. The dissociation constant (0.7 nM) was similar in both hypertrophied and control left ventricles. Thus the decreased density of the ryanodine receptors may participate in altered calcium movements in hypertrophied rat heart.

Abnormal intracellular calcium handling in cardiomyopathic human hearts has been associated with an impaired function of the sarcoplasmic reticulum, but previous reports on the gene expression of the ryanodine receptors (Ry2) are contradictory. We measured the mRNA levels, the protein levels and the number of high affinity [3H]ryanodine binding sites in the left ventricle of non-failing (n=9) and failing human hearts [idiopathic dilated (IDCMn=16), ischemic (ICMn=7) or mixed (MCMn=8) cardiomyopathies]. Ry2 mRNA levels were significantly reduced in IDCM (−30%) and unchanged in MCM and ICM and Ry2 protein levels were similar. In contrast, we observed a two-fold increase in the number of high affinity Ry2 (Bmax=0.43±0.11v0.22±0.13 pmol/mg protein, respectively;P<0.01) and an unchanged Kd. Furthermore, levels of myosin heavy chain mRNA and protein per g of tissue were similar in failing and non-failing hearts, suggesting that the observed differences in Ry2 are not caused by the increase in fibrosis in failing heart. Therefore, the dissociation between the two-fold increase in the number of high affinity ryanodine receptors observed in all failing hearts and the slightly decreased mRNA level or unchanged protein level suggests that the ryanodine binding properties are affected in failing myocardium and that such modifications rather than a change in gene expression alter the channel activity and could contribute to abnormalities in intracellular Ca2+handling.

This paper investigates the use of coupon specimens to study the severity of defects in filament would tubes. Composites based on two types of resin are studied, an epoxy and a vinylester. First, the properties of the unreinforced resins are measured. Next the properties of flat filament wound unidirectional glass fibre reinforced composite plates using these resins are determined, and in particular their delamination resistance under mode I loading. These results are then compared with those determined on specimens taken from filament wound tubes with different winding angles (±30°, ±45° and ±60°) and diameters.

The French social security has had a balanced budget as a recurring objective for several decades, with a specific target on the National Health Insurance (NHI) branch. Over the past several years, efforts have intensified, with a continuing policy on decreasing drug prices, developing ambulatory care to limit hospitalizations or reducing the reimbursement of some specialists. Decreasing the reimbursement rate of drugs reduces, as a direct effect, the prescription and delivery of the targeted drugs. However, the substitution effect in favor of still reimbursed drugs should not be forgotten, because it could minimize the intended effect in economic terms as well as in terms of public health. The recent French decision to terminate the reimbursement of Symptomatic Slow-Acting Drugs for Osteoarthritis (SYSADOAs) is an interesting case study for the impact of terminating the reimbursement a drug from a collective perspective. The aim of the present study was to investigate prescription substitutions in favor of analgesics and non-steroidal anti-inflammatory drugs (NSAIDs), both in terms of volumes and costs, as well as the occurrence of their potentially associated adverse effects (AEs) using a methodology based on access to national administrative databases.

L’objectif de retour à l’équilibre du Régime général de la Sécurité sociale repose notamment sur la réduction du déficit de la branche maladie. Les efforts en ce sens incluent une baisse du prix des médicaments, un développement des soins ambulatoires ou encore la réduction du remboursement de certaines spécialités. La baisse des taux de remboursement a pour effet direct de diminuer la prescription et la délivrance des traitements. Il faut cependant prendre en compte les éventuels reports de prescription vers des classes thérapeutiques toujours remboursées qui peuvent avoir des conséquences en termes d’économies et de santé publique. L’exemple du déremboursement de la classe des anti-arthrosiques symptomatiques d’action lente (AASAL) en France constitue un cas d’étude de l’impact d’un déremboursement du point de vue de la collectivité. L’objectif de l’étude était d’investiguer les reports de prescriptions vers les antalgiques et les anti-inflammatoires non stéroïdiens (AINS) et d’autres classes thérapeutiques associées, en termes de volumes et de coûts, ainsi que les éventuels événements indésirables.

La définition actuelle de la BPCO est basée sur la présence de symptômes associée à un critère spirométrique (obstruction bronchique persistante après inhalation de bronchodilatateur [BD]). Cependant, la plupart des études épidémiologiques ne tient compte que du critère spirométrique pour définir la BPCO. Lorsqu’une obstruction bronchique persistante est identifiée lors d’un dépistage, les éléments qui différencient les sujets symptomatiques (BPCO) des sujets asymptomatiques (OBS) sont peu connus.

Certaines activités agricoles sont associées à un risque accru de BPCO. La mutualité sociale agricole effectue des campagnes de dépistage de la BPCO. Afin de limiter le coût d’un dépistage systématique, l’objectif a été d’établir et valider un score afin d’identifier les sujets redevables d’une spirométrie. L’analyse a porté sur 5095 sujets (40–75ans) ayant complété un questionnaire portant sur les antécédents respiratoires et cardiaques, les symptômes (dyspnée, toux, expectoration), le tabagisme et le cursus professionnel. La BPCO a été définie selon le critère du GOLD. La population a été séparée en deux par tirage au sort: une sous-population « training » (n=3397) pour la détermination du modèle et une sous-population « test » (n=1698) pour sa validation en utilisant la méthode du bootstrap. Les variables obtenant une p-valeur inférieure à 0,2 en analyse bivariée ont été insérées dans un modèle logistique multivarié. La valeur-seuil optimale du score a été sélectionnée par analyse en courbe ROC. Les variables qualitatives retenues pour le calcul du score ont été: antécédent d’asthme; antécédent de maladie respiratoire; tabagisme (3 classes); sexe; âge (3 classes); dyspnée (O/N); métier (7 classes). Le c-index [IC à 95 %] dans les sous-populations « training » et « test » étaient 0,86 [0,83–0,892] et 0,79 [0,74–0,84], respectivement. Pour la valeur-seuil optimale, la sensibilité était de 85,2 %, la spécificité de 71,3 %, la VPP de 10,5 % et la VPN de 94,7 %. Ce score permettrait, avec le recueil de 7 items, de diviser par 1,6 le nombre de sujets chez qui une spirométrie est réalisée pour détecter une BPCO lors d’examens de santé en milieu agricole.

La prévalence de la BPCO en milieu agricole est plus élevée que dans les emplois du secteur tertiaire, mais elle varie considérablement selon les pays, les professions agricoles et les critères spirométriques utilisés. La prévalence de la BPCO a été étudiée dans deux régions agricoles (Bretagne et Franche-Comté) chez 4771 sujets répartis en tertiaires (n=917), cultivateurs (n=502), éleveurs de bovins (n=1822), de porcs (n=266), de volailles (n=226), de plusieurs espèces animales (n=354) et autres professions agricoles (n=617). La population tertiaire a été prise comme référence. Les Odds Ratios (OR) ont été ajustés sur l’âge, le tabagisme et le sexe.
