5. Validation of the Educational Game for Seniors: “Live Well, Live Healthy!”

- Abstract
A literature review was conducted as part of a development project to flesh out a number of issues: How can game component design be adapted to accommodate seniors’ physical or intellectual limitations? How should a game interface and its components be displayed on the screen to facilitate game navigation? What restrictions should be considered when selecting computer game equipment to be used by seniors? What are the guidelines for efficient game audio, video and text readability? This communication first presents the pedagogical and technological adaptations of the online educational game: “Live Well, Live Healthy!” This adaptation of the Bingo game has allowed us to introduce learning content with the objective of improving the quality of life for seniors. Second, the methodology used for the validation of the ergonomics of this educational game with seniors and third, the results of validation and the recommendations from the seniors to improve the ergonomic conditions.

8. Enhancing Learning Through an Online Secondary School Educational Game.

This article consists of four sections: (1) the problems associated with asthma in the province of Québec and across Canada; (2) the theoretical framework for the learning enhanced by our online educational game entitled Asthme: 1,2,3 .. Respirez! (Asthma: 1,2,3 .. Breath!), created by adapting the popular board game Parcheesi, and intended for students in a senior secondary school health education program (14 to 18 years old); (3) the methods employed, including a description of the educational game, the quasi-experimental research protocol, the sample, the variables being studied, the measurement instruments, the pilot study, the procedures, and the data analysis; and (4) the results of the study and discussion of the results. The results of the paired t-tests showed significant improvements in a variety of cognitive skills after students played the game on laptops in their classrooms for 40-60 minutes. No differences were found between males and females. These results are encouraging for teachers who wish to use educational digital games in their classrooms. [ABSTRACT FROM AUTHOR]

From a holistic perspective, health literacy is a requirement for the well-being of entire populations. It moves beyond the focus on individuals to consider the role of organizations and systems. This perspective offers a context for discussing best practices in health literacy, and implications for research and policy development. This paper offers an overview of the best practices that were presented at the Second Canadian Conference on Literacy and Health. It discusses clear writing in some detail because it was emphasized at the conference. It also considers practices that were addressed less emphatically, such as oral communication between patients and health care professionals, training for health care professionals, non-written means of communication (such as video), and building capacity through action-research. The paper critiques some practices. It also notes the lack of research on the links between health literacy and oral understanding, on the impact of verbal and non-written interventions, and on the effectiveness of these practices on the health outcomes of the population. Background: The results of a study on weight loss products and services were released during a press conference organized by the Association pour la santé publique du Québec (ASPQ). The media widely covered this press conference. The purpose of this article is to answer the question: "How are messages emitted by public health institutions transmitted by the media and received by the population?"Method: The transcripts of the press conference, the press release as well as 43 documents from the print and broadcast media were evaluated according to a press analysis method. The analysis of these documents was based on the concept of the information unit. The compilation of these information units was used to produce charts which include data such as the frequency, partiality, weight tendency ("poids-tendance") and tendency-impact ("tendance-impact"). Furthermore, two focus groups of persons using weight loss products and services were conducted.Results: In this article, we compare the emitted message with the one transmitted. The analysis of the message emitted by the ASPQ and that conveyed by the media revealed that for almost all subjects dealt with during the press conference, the media have been more partial than the ASPQ. The media not only expanded on the emitted message but also gave a negative image of weight loss products and services. Furthermore, the message transmitted by the media was very similar to the initial message. The focus groups on how the message was received by persons using weight loss products and services also revealed that the message was lost in the flow of information on weight loss products and services.Conclusion: This analysis of the emitted, transmitted and received messages has given us some indications on how to readjust the initial message and on what to emphasize in future press conferences. 13. Messages d’activité physique et d’alimentation: que nous offrent les médias québécois?

Éléments socioculturels de l’environnement, les médias peuvent agir comme des déterminants sur les croyances, les normes et les habitudes de vie des individus et des populations. Leurs messages étant susceptibles d’influencer la santé populationnelle, nous avons cherché à connaître l’offre médiatique québécoise au regard des messages d’alimentation et d’activité physique, sous l’angle de la santé publique. Dans un premier temps, nous avons analysé le contenu de téléromans (n=13) diffusés sur les quatre chaînes généralistes, ainsi que le contenu des publicités durant ces émissions (n=68). Nous avons par la suite étudié le contenu du quotidien La Presseentre 1986 et 2005, de même que celui des émissions de la Société Radio-Canadaau cours de la même période, relativement aux messages d’alimentation et d’activité physique. Nos données révèlent un écart entre la représentation des hommes et des femmes en vigueur à la télévision québécoise, la corpulence des hommes s’avérant plus forte que celle des femmes. De plus, au cours des vingt années analysées, les messages liés à l’alimentation furent plus nombreux que ceux abordant l’activité physique. Pour atteindre leurs objectifs d’amélioration de la santé des populations, les concepteurs de messages gagneraient à associer ces deux sujets. As social elements of our environment, mass media are regarded as determinants of individual and population beliefs, social norms and habits. Since it is recognized that they influence population health, this study aims to obtain a better portrait of Quebec media content regarding physical activity and nutrition messages on a public health level. First, we analyzed the content of fictional television shows (n=13) and advertisements broadcast during those shows (n=68). Second, we reviewed the content of La Pressenewspaper and of French television Société Radio-Canadafrom 1986 to 2005 with regard to physical activity and nutrition messages. Our results indicate a difference between how men and women are portrayed on French television, with women more often being shown as underweight and men as at or above healthy body weight. The results also show that during the 20-year period of the reviewed content, there were fewer messages about physical activity than about nutrition. To be successful in their goal of improving population health, mass media should address both subjects together in their messages.

14. Implementation and Outcomes of a Comprehensive Worksite Health Promotion Program

- Abstract
This paper reports on the implementation and results of a three-year comprehensive worksite health promotion program called Take care of your health!, delivered at a single branch of a large financial organization with 656 employees at the beginning of the implementation period and 905 at the end. The program included six educational modules delivered over a three-year period. A global health profile was part of the first and last modules. The decision to implement the program coincided with an overall program of organizational renewal. The data for this evaluation come from four sources: analysis of changes in employee health profiles between the first and last program sessions (n=270); questionnaires completed by participating employees at the end of the program (n=169); organizational data on employee absenteeism and turnover; and qualitative interviews with company managers (n=9). Employee participation rates in the six modules varied between 39% and 76%. The assessment of health profile changes showed a significant increase in the Global Health Score. Participants were significantly more likely to report more frequent physical activity and better nutritional practices. The proportion of smokers among participants was significantly reduced (p=0.0147). Also reduced significantly between the two measurements were self-assessment of high stress inside and outside the workplace, stress signs, and feelings of depression. Employees were highly satisfied with the program and felt that it had impacts on their knowledge and capacities to manage their health behaviour. During the same period, absenteeism in the organization declined by 28% and turnover by 54%. From the organization’s perspective, program implementation was very successful. This study’s results are in line with previous findings of significant benefits to organizations and employees from worksite health promotion. The close relationship between the program outcomes and the overall process of organizational renewal that it accompanied supports previous arguments that worksite health promotion will be most effective when it promotes overall organizational health. Cet article présente un compte rendu de l’implantation et des résultats d’un programme de promotion de la santé intitulé Ma santé, je m’en occupe!au sein du premier groupe à s’en prévaloir. Le programme a initialement été mis en œuvre dans l’un des services d’une grande entreprise financière. Ce service comptait 656 employés au début de la période d’implantation et 905 à la fin. Le programme comportait six modules éducatifs offerts sur une période de trois ans. Le premier et le dernier modules comprenaient un bilan de santé globale. La décision d’implanter ce programme a coïncidé avec un projet global de renouveau organisationnel. Les données de notre évaluation proviennent de quatre sources: l’analyse des profils de santé globale des employés qui ont participé au premier et au dernier modules du programme (n=270); les questionnaires remplis par ces mêmes employés à la fin du programme (n=169); les données organisationnelles sur l’absentéisme et le roulement du personnel, ainsi que des entrevues qualitatives auprès de gestionnaires de l’entreprise (n=9). Les taux de participation des employés aux six modules ont varié entre 39 % et 76 %. L’analyse des profils de santé a montré une augmentation significative du score de santé globale. Les participants étaient, de façon significative, plus nombreux à déclarer une fréquence d’activité physique accrue ainsi que de meilleures habitudes alimentaires. Le pourcentage de fumeurs parmi les participants a diminué de façon significative (p=0,0147). Les niveaux de stress élevés au travail ou à l’extérieur, les symptômes de stress et les sentiments de déprime autodéclarés ont aussi diminué significativement entre les deux mesures. Les employés se sont montrés très satisfaits du programme et ont senti qu’il avait eu des impacts sur leurs connaissances et leurs capacités de prendre leur santé en main. Au cours de la même période, l’absentéisme a diminué de 28 % et le roulement du personnel de 54 % dans l’organisation. Du point de vue de gestionnaires interviewés, l’implantation du programme a été une réussite. Les résultats de cette étude sont conformes aux résultats antérieurs ayant démontré les avantages considérables des programmes de promotion de la santé en milieu de travail pour les organisations et les employés. Le lien étroit entre les résultats du programme et le projet global de renouveau organisationnel qu’il a accompagné confirme les observations antérieures selon lesquelles les programmes de promotion de la santé en milieu de travail sont plus efficaces lorsqu’ils encouragent la santé organisationnelle de façon globale.

Media-based interventions are common in health promotion, yet their conceptual underpinnings tend to be based on a simple linear model of direct influence on individuals’ health behaviour. Recent studies have suggested that the processes through which media influence health behaviours are actually far more complex. This paper presents a conceptual model of how the medias influence the emergence and maintenance of the social norms that can contribute to shaping health behaviours. Through positive (amplifying) and negative (dampening) loops, a total of six potential influence pathways are proposed, and the role of opinion leaders and specialists is specified. Future directions for empirical tests of the model are identified. Les interventions médiatiques sont fréquentes en promotion de la santé. Leurs fondements conceptuels tendent à être basés sur un modèle linéaire d’influence ayant un effet direct sur les comportements sociosanitaires des individus. Or, de récentes recherches suggèrent que les processus médiatiques influençant les comportements sont davantage complexes. Cet article présente un modèle conceptuel au sujet des mécanismes médiatiques façonnant les normes et les comportements. À travers un positionnement positif et négatif, le modèle propose six voies d’action et définit le rôle des spécialistes et des leaders d’opinion. Des pistes de recherche sont proposées.

L’Association pour la santé publique du Québec (ASPQ) a organisé une conférence de presse pour dévoiler les résultats d’une étude sur les produits, services et moyens amaigrissants (PSMA). Les médias ont amplement couvert l’événement. Nous désirons analyser «comment les messages émis par les institutions de santé sont transmis par les médias et décodés par la population.» La conférence de presse, le communiqué de presse ainsi que les 43 documents provenant de divers médias écrits et électroniques, ont fait l’objet d’une analyse comparant le discours émis de l’ASPQ à celui transmis par la presse. Tous ces documents ont fait l’objet d’une analyse basée sur le concept de l’unité d’information. La compilation des unités d’information permet de produire des tableaux indiciels mesurant la fréquence, la partialité, le poids-tendance et la tendance-impact du contenu. De plus, deux entrevues de groupe ont été réalisées auprès de personnes utilisant des PSMA afin d’examiner la notoriété et la perception des messages. Nous comparons le message émis et le message transmis. L’analyse comparative du message émis par l’ASPQ et du message transmis par les médias révèle que pour tous les sujets étudiés, sauf celui des PSMA en général, les médias ont été plus partiaux que l’ASPQ. Les médias ont non seulement amplifié le message émis, mais ont aussi transmis un message très défavorable au regard des PSMA. Par ailleurs, les messages véhiculés par les médias reprennent de manière assez fidèle le message émis lors de la conférence de presse. Quant à la réception du message, les entrevues de groupes («focus group») effectués auprès des personnes concernées par l’utilisation des PSMA nous révèlent que les messages sont passés inaperçus. Cette analyse entre les messages émis, transmis et reçus nous suggère des pistes pour ajuster notre message et identifier les composantes devant être privilégiées lors d’une prochaine conférence de presse. 18. Le fardeau perçu par les proches de personnes ayant des incapacités physiques

Cette étude visait à mesurer la perception de fardeau chez les proches de personnes ayant des déficiences physiques, vivant à domicile et fréquentant un Centre de jour. Cinquante-cinq aidants ont répondu à des questionnaires portant sur trois dimensions du fardeau ainsi que sur les améliorations perçues depuis que l'aidé fréquente le Centre de jour. Des analyses en fonction de variables socio-démographiques ont été faites ainsi que des comparaisons avec une clientèle psychiatrique, dont les données provenaient d'une étude précédente. Les résultats montrent un fardeau élevé pour le soutien concret offert et pour la préoccupation pour le bien-être de l'aidé. Les problèmes de déplacements extérieurs sont identifiés comme source importante de fardeau. En général, le fardeau s'est avéré plus lourd en déficience physique que dans le secteur psychiatrique. Les conclusions portent sur l'importance de considérer les facteurs environnementaux et d'intégrer des mesures du fardeau dans l'évaluation des programmes en déficience physique.

En faisant le consensus sur un nouveau paradigme d'intervention qui intègre toutes les dimensions de l'environnement physique, politique et économique pouvant avoir un impact sur la santé, la Charte d'Ottawa, le Plan d'ensemble pour la promotion de la santé, la Politique de la Santé et du Bien-être proposent aux professionnels de la santé publique un élargissement majeur de leurs rôles et de leurs mandats. D'éducateurs sanitaires qu'ils étaient, on leur demande maintenant d'orienter leur action de façon à soutenir la création de milieux favorables, le renforcement de l'action communautaire, la réorientation des services de santé et l'élaboration d'une politique publique saine, tout en continuant à favoriser l'acquisition d'aptitudes individuelles. Le présent article discute de cette transition des rôles et mandats des professionnels de la santé.

- Abstract
Background.Few controlled studies have evaluated the impact of community-based screening to detect risk factors for cardiovascular disease (CVD) on behavior change. This study examined the short-term impact of school-based screening on smoking, leisure time exercise, and fat consumption of personnel working in schools offered CVD risk factor screening. Also, the impact of screening on the perceptions that teachers have of their role in promoting heart health at school and the frequency of discussing heart health issues with students was investigated.

23. Recherche évaluative sur la distribution de vidéocassettes santé destinées à une population défavorisée de Montréal

Le présent article décrit la mise en oeuvre et l’évaluation d’une intervention mise de l’avant dans le cadre d’un programme communautaire et multifactoriel en santé du coeur. L’intervention consiste à offrir à une clientèle défavorisée et peu scolarisée six vidéocassettes sur la santé cardiovasculaire par l’intermédiaire de cinq vidéoclubs. Tous les vidéoclubs (n = 5) du quartier Saint-Henri/Petite Bourgogne ont accepté de distribuer gratuitement les vidéocassettes. Deux composantes sont examinées: la recevabilité de l’intervention auprès des propriétaires de vidéoclub et la réceptivité de la clientèle. Les résultats démontrent que tous les vidéoclubs ont offert un emplacement et un service de prêt gratuit des vidéocassettes. En quatre mois, 212 personnes ont emprunté l’une ou l’autre des vidéocassettes. L’article présente également une discussion sur la faisabilité et les possibilités d’élargissement de ce type d’action. The present article describes the implementation and the evaluation of an intervention done in the context of a heart health community and multifactorial program. The intervention aimed to offer six videocassettes on cardiovascular health to a low-income and low-educated clientele through five videoclubs. All videoclubs (n = 5) from St. Henri/Petite Bourgogne agreed to distribute the videocassettes free of charge. Two components were examined: admissibility by videoclubs’ owner and clientele’s receptivity towards the intervention. Results showed that all videoclubs offered free display and service for the videocassettes. The article also presents a discussion of the feasability and the possibilities to broaden this type of action.

- Abstract
Cette étude exploratoire sur la participation des parents dans des activités scolaires ou parascolaires se situe dans le cadre global du projet Coeur en santé de Saint-Louis du Parc, quartier défavorisé et multiethnique de Montréal. Une compréhension générale de la participation est indispensable pour adapter la composante parentale des programmes de santé développés en lien avec l’école. Un cadre conceptuel visant à comprendre le processus de la participation des parents d’élèves d’écoles primaires dans des activités développées à l’école ou à partir de celle-ci a été élaboré. Quinze entrevues semi-dirigées menées avec des intervenants (écoles, organismes) ont permis de mettre en évidence à travers l’analyse qualitative des obstacles et des facteurs qui facilitent l’implication des parents. La distance socioculturelle entre les parents et le milieu scolaire est notable. La perception d’obstacles est un facteur essentiel. Les intervenants du corps professoral mettent de l’avant les contraintes organisationnelles et les difficultés de communication liées à la langue. Les autres intervenants relèvent plutôt l’absence de connaissance du milieu, la faible qualité de la communication et l’absence d’habiletés pour que les parents s’impliquent activement. l’ouverture institutionnelle, l’attitude proactive et positive des intervenants envers les parents sont des conditions indispensables mais non suffisantes. l’adaptation des activités à différents niveaux est aussi un élément nécessaire. Des entrevues ultérieures avec les parents devraient permettre d’approfondir cette étude. This preliminary study of parental participation in curricular and extra-curricular activities was conducted in the general framework of the healthy heart project in St. Louis du Parc, an underprivileged and multiethnic neighbourhood in Montreal. A general understanding of this type of participation is essential in order to adapt the role parents play in health programs developed in conjunction with the school. A conceptual framework was developed to help understand how and why parents of primary school children participate in school activities. Qualitative analysis of 15 semi-directed interviews conducted with various schools and other organizations revealed those obstacles to parental involvement, and the factors that encourage parents to participate. It was clear that there were wide social and cultural gaps between parents and schools. An important factor is how people perceive those barriers. Teachers stressed organizational and language communications difficulties. Others pointed to the lack of knowledge of the community, to poor communications skills and the parents’ lack of those skills that would allow them to participate actively. A more open-minded attitude by school authorities and a more proactive and positive attitude of teachers toward parents are essential requirements, but not the only ones. Further interviews with parents should help provide more details and information.

26. L’institutionnalisation des programmes communautaires : revue des modèles théoriques et proposition d’un modèle

Les programmes communautaires en promotion de la santé doivent durer longtemps pour obtenir un impact significatif sur les statistiques sociosanitaires liées à ces habitudes et conditions de vie. Leur efficacité réside notamment dans l’institutionnalisation de ces programmes. Ces programmes souvent nouveaux pour l’organisation hôte devraient survivre au-delà des ressources formelles de santé publique investies. Cependant, il s’avère que certains programmes disparaissent avec le retrait du financement extérieur, ce qui coûte cher à l’organisation puisque le programme aura mobilisé et créé des attentes auprès des acteurs locaux générant parfois de la résistance face à l’implantation d’autres programmes de promotion de la santé. C’est pourquoi il devient pertinent de comprendre les principes et les processus impliqués dans le maintien ou l’institutionnalisation des programmes. À partir d’une revue de littérature de 28 articles, le présent article examine différents modèles théoriques (environnemental, organisationnel, communautaire ou de marketing) qui mettent en relief diverses variables explicatives ou prédictrices du maintien d’un programme de promotion de la santé dans une organisation. Puis, l’article propose un modèle qui tente d’amalgamer plusieurs de ces variables sous diverses catégories: caractéristiques liées au programme, caractéristiques liées à la ciblehôte, caractéristiques liées à l’adoption, à l’implantation, à l’appropriation et à l’incorporation, enfin caractéristiques liées à l’ajustement mutuel. Community-based health promotion programs to change lifestyle habits must remain in their host organizations for extended periods of time in order to have impact. Their effectiveness can be closely linked to their long term viability or institutionalization. To remain viable, these programs must survive beyond initial investment and support by external organizations. However, some programs disappear when external investment is withdrawn. This can be costly and in addition can generate resistance to the implementation of other health promotion programs in the future. Recently, interest in the processes involved in the institutionalization of these programs has increased. Based on 28 publications, this article reviews selected conceptual models that highlight environmental, organizational, community and marketing-related variables possibly related to the institutionalization process. A new model is proposed to link these diverse models according to: characteristics of the program, characteristics of the host organization, characteristics related to the adoption, implementation and incorporation of the program, and finally characteristics related to the fit (mutual adjustment) between the host and the program.

27. Formative Evaluation of a Nutritional Marketing Project in City-Center Restaurants

- Abstract
Eating habits are influenced by both individual and collective factors. From this perspective, a community health project designed to provide and promote low-fat foods in restaurants, was elaborated with the collaboration of seven downtown Montreal restaurants. This article reviews two major questions pertaining to the feasibility of this nutritional marketing project. The first question focuses on restaurateurs' acceptance of this type of initiative, and the second, on consumers' receptiveness. The formative evaluation shows that restaurateurs are willing to collaborate in the implementation of this type of project, but not in training sessions for waiters. On the other hand, 18% of the market currently favors low-fat menu choices and the project did not significantly contribute to an increase in market demand The evaluation also points to a number of ways in which the project should be changed if it is expanded

Le système unique de santé du Brésil comporte 27 000 équipes de santé familiale avec un réseau de quelques 300 000 agents communautaires de santé dont le mandat est de faire le pont entre les familles qu’ils visitent à chaque mois et les professionnels de l’unité de santé familiale. Depuis 2003, le Québec déploie une réforme dans le domaine de la santé et des services sociaux, avec l’accent mis sur l’intégration des soins et services (publics, associatifs et privés) et leur inscription dans des réseaux locaux dans une optique de responsabilité populationnelle. Les commentaires tentent de souligner quelques éléments de ressemblance ou différence entre le programme des ‘unités de santé familiale’ (USF) du Brésil et celui des Cliniques de réseau intégré (CRI) ou des Groupes de médecine familiale (GMF) du Québec. Dans les deux cas il y a ce souci de services de plus grande proximité de la population d’un territoire donné, d’une plus grande accessibilité et adéquation des services. Par ailleurs, le Brésil, à travers ces agents communautaires de santé, permet un filet social qui assure un accompagnement réel de tous les membres de la population vers les services de santé. The Brazilian unified health system consists of 27,000 family health teams with a network of approximately 300,000 community health agents whose mandate is to link the families they visit every month with health professionals at the family health unit. Since 2003, Quebec has been implementing its health and social services reforms by emphasizing the integration of care and services (public, associative and private) in local networks which have a formal population health responsibility. This commentary aims to underline some similarities and differences between the program of the Brazilian 'units of family health' and that of integrated network clinics (CRI) or Groups of family medicine (GMF) in Quebec. In both cases, a major concern is to organize primary care services in as close proximity as possible to the population of a given territory, and to improve accessibility and adequacy of services. In addition, through these community health agents, Brazil enables the creation of a social security network that assures an effective follow-up of all members of the population with regard to health services.

L’étude de cas multiples est appliquée pour expliquer l’implantation d’un curriculum de promotion de la santé cardiovasculaire intitulé «Coeur en santé», dans les classes de 4eannée de huit écoles d’un quartier multiethnique et défavorisé de Montréal. Une entrevue structurée et un journal de bord ont été utilisés pour mesurer les variables suivantes: 1) les caractéristiques personnelles des professeurs; 2) les caractéristiques organisationnelles de l’école; 3) les caractéristiques du programme; 4) la collaboration entre les secteurs de l’éducation et de la santé; et 5) la dose et la fidélité d’administration du curriculum. Les résultats démontrent que les caractéristiques personnelles des professeurs et certaines caractéristiques du programme semblent expliquer les niveaux d’implantation du curriculum «Coeur en santé» dans les classes de 4eannée. A multiple case study design is used to explain the level of implementation of a “Heart Health” curriculum by grade four teachers of eight schools in a Montreal multiethnic and underprivileged district. An interview and logbook examine the following variables: 1) personal characteristics of the teachers; 2) organizational characteristics of the schools; 3) characteristics of the program; 4) collaboration between the health and educational sectors; and 5) curriculum level of use and fidelity of implementation. The results show in particular that the personal characteristics of the teachers and the characteristics of the program explain the level of implementation of the Heart Health curriculum.
