AbstractThe game of chess is a valuable extracurricular activity for children, with positive effects on their cognitive skills and academic achievements. We investigated the extent to which the Giant Chess Game (GCG) played on a giant chessboard enhances working memory in “navigational-vista” space and “reaching” space. We also assessed if the GCG enhances mental rotation skills. For 10 weeks, 15 children (GCG group) were involved in a giant chess class, while 15 gender and age-matched children were involved in standard didactics (control group—CG). Children were tested twice, before (T0) and after (T1) the GCG, by tasks aimed at measuring: visuo-spatial working memory (VSWM) in the navigational-vista space (Walking Corsi test); VSWM in the reaching space (Corsi Block-Tapping task); mental rotation (Rotating Flowers test). We found that the GCG group significantly improved its performance more than the CG in VSWM in both navigational-vista space and reaching space, as well as in mental rotation. Our results suggest that the GCG has positive effects on visuo-spatial abilities underlying topographical skills. Therefore, the training using GCG can help enhancing spatial ability and may have a role in contrasting the spreading of navigational deficits such as the Developmental Topographical Disorientation (DTD).

Introduction: Stroke in young people shares traditional modifiable risk factors with older groups, and greatly affects quality of life. However, evidence on the effectiveness of educational interventions in young populations, aiming at spreading stroke knowledge and enhancing prevention, is still scarce. We evaluated baseline knowledge of stroke and possible improvements after an educational intervention among Italian high school students, also considering differences related to sex and type of school.Subjects and methods: Using a mixed educational strategy, a prospective evaluation of stroke knowledge was performed in five humanities and sciences (lyceums) and five vocational high schools of Tuscany (students of the 12th and 13th grade). A baseline assessment with a structured questionnaire (21 questions) was followed by a standardized oral presentation, using audiovisual materials. After 3 months, the same questionnaire was re-administered to evaluate the long-term impact of the educational intervention.Results: Overall, 573 students (50.8% males; age range, 17–19 years) were enrolled; 288 (50.3%) were from lyceums and 285 (49.7%) from vocational schools. Follow-up participation was 97.2%. Baseline performances were comparable between groups for most variables examined. At 3 months, all groups showed a significant improvement from baseline regarding reaction to a stroke event, identification of stroke risk factors, such as smoking (from 62.9% to 83.7%; p< 0.001) and alcohol abuse (from 49.6% to 67.2%; p< 0.001), and symptoms. Knowledge of the existence of stroke units and thrombolysis increased from 25.4% to 60.7% (p< 0.001) and from 35.8% to 84.0% (p< 0.001), respectively.Conclusions: Our educational intervention improved stroke awareness in high school students. The effects persisted after 3 months. Improved knowledge in young populations may reduce stroke burden in adult life, increase timely access to therapies, and spread knowledge across families.

The contemporary era is experiencing an unprecedented dependence on data generated by individuals via an array of interconnected devices constituting the Internet of Things (IoT). The information amassed through IoT devices serves many objectives, including prescriptive analytics and predictive maintenance, preemptive healthcare measures, disaster mitigation, operational efficiency, and increased yield. In contrast, most applications or systems that rely on user-generated data to fulfill their business objectives face challenges in adhering to privacy protocols. Consequently, users are exposed to many privacy risks. Such infringements upon privacy provisions give rise to apprehensions regarding the authenticity of the processed data. Hence, this paper presents the weaknesses and challenges in current practices and proposes “PrivySeC”, a Distributed Ledger Technology (DLT)-based framework for privacy preserving and secure sharing of personally and non-personally identifiable information. The security analysis indicates that the proposed solution ensures data privacy by design and complies with most of the requirements mandated by various privacy regulations. Similarly, PrivySeC promises low transaction latency and provides high throughput. Although we have created a privacy-preserving solution for sharing smart farm data, it can be customized to meet the specific privacy requirements of individual applications.

Predicting whether microbial invaders will colonize an environment is critical for managing natural and engineered ecosystems, and controlling infectious disease. Invaders often face competition by resident microbes. But how invasions play out in communities dominated by facilitative interactions is less clear. We previously showed that growth medium toxicity can promote facilitation between four bacterial species, as species that cannot grow alone rely on others to survive. Following the same logic, here we allowed other bacterial species to invade the four-species community and found that invaders could more easily colonize a toxic medium when the community was present. In a more benign environment instead, invasive species that could survive alone colonized more successfully when the residents were absent. Next, we asked whether early colonists could exclude future ones through a priority effect, by inoculating the invaders into the resident community only after its members had co-evolved for 44 weeks. Compared to the ancestral community, the co-evolved resident community was more competitive toward invaders and less affected by them. Our experiments show how communities may assemble by facilitating one another in harsh, sterile environments, but that arriving after community members have co-evolved can limit invasion success.

Background and aim: Benefits of oral anticoagulants (OAC) in atrial fibrillation (AF) patients with moderate-to-high risk of stroke are independent of AF pattern. We evaluated whether AF clinical subtype influenced OAC use in a representative sample of the Italian older population. Methods: A cross-sectional examination of all subjects aged 65 + years from three general practices in northern, central, and southern Italy started in 2016. A double-screening procedure was followed by clinical and ECG confirmation. Patients were categorized as having paroxysmal, persistent, or permanent AF. OAC use was evaluated in confirmed AF patients. Results: The sample included 6016 subjects. Excluding 235 non-eligible, participation was 78.3%, which left 4528 participants (mean age 74.5 ± 6.8 years, 47.2% men). Overall, 319 AF cases were identified: 43.0% had paroxysmal, 21.3% persistent, and 35.7% permanent AF. Frequency of OAC therapy was 91.2% in permanent, 85.3% in persistent, and only 43.0% in paroxysmal AF (P&lt; 0.001). In multivariate analysis, controlled for baseline variables and risk scales, persistent and permanent AF were associated with a significant increase in the likelihood of receiving OAC compared with paroxysmal AF (P &lt; 0.001). This was confirmed for permanent AF also in multivariate analyses considering separately vitamin K antagonists or direct-acting oral anticoagulants (OR, 4.37, 95% CI, 2.43–7.85; and 1.92, 95% CI, 1.07–3.42, respectively) and for persistent AF and direct-acting oral anticoagulants (OR, 4.33, 95% CI, 2.30–8.15). Conclusions: In a population-based survey, AF pattern was an independent predictor of OAC treatment. Paroxysmal AF is still perceived as carrying a lower risk of vascular events.

This article explores the significance of Jineolojî, an emancipatory praxis elaborated by the Kurdish Women’s Movement, for contemporary degrowth and pluriverse politics. Considering Jineolojî as the most original dimension of the Democratic Confederalist model of government in Northern and Eastern Syria (compared to other revolutionary projects), the article contributes to recent debates around the central place of “depatriarchization” in pluriverse debates. In the first part, we highlight a renewed interest in matriarchy, which has emerged at the intersection of ecofeminist with post-development and degrowth thought, noting how this resonates with the rediscovery of Mesopotamia’s matristic culture, which has been key to Democratic Confederalism and its radical critique of capitalist modernity and the nation State. We also highlight the inherent contradictions of the matristic model and formulate the question whether, and under what conditions, it bears potential for emancipatory political ecologies. The second part briefly describes the article’s sources and method, namely militant ethnography carried out with the Kurdish Women’s Movement, both in Rojava and in the European diaspora, cross-referenced with an analysis of some key texts of Jineolojî. The third part investigates the process by which the matristic perspective is being currently performed in Rojava through Jineolojî: a pedagogy for women’s self-defense, the autonomous re-appropriation of communalist and ecological praxis, and men’s liberation from hegemonic masculinity. We conclude that Jineolojî does not configure as a model of society to be recovered from a pre-patriarchal age, but as an original tool for liberating social potential towards gender, decolonial and ecological revolutions.

Here, we present an integrated multivariate, univariate, network reconstruction and differential analysis of metabolite–metabolite and metabolite–lipid association networks built from an array of 18 serum metabolites and 110 lipids identified and quantified through nuclear magnetic resonance spectroscopy in a cohort of 248 patients, of which 22 died and 82 developed a poor functional outcome within 3 months from acute ischemic stroke (AIS) treated with intravenous recombinant tissue plasminogen activator. We explored differences in metabolite and lipid connectivity of patients who did not develop a poor outcome and who survived the ischemic stroke from the related opposite conditions. We report statistically significant differences in the connectivity patterns of both low- and high-molecular-weight metabolites, implying underlying variations in the metabolic pathway involving leucine, glycine, glutamine, tyrosine, phenylalanine, citric, lactic, and acetic acids, ketone bodies, and different lipids, thus characterizing patients’ outcomes. Our results evidence the promising and powerful role of the metabolite–metabolite and metabolite–lipid association networks in investigating molecular mechanisms underlying AIS patient’s outcome.

Cerebral edema (CE) and hemorrhagic transformation (HT) are frequent and unpredictable events in patients with acute ischemic stroke (AIS), even when an effective vessel recanalization has been achieved. These complications, related to blood-brain barrier (BBB) disruption, remain difficult to prevent or treat and may offset the beneficial effect of recanalization, and lead to poor outcomes. The aim of this translational study is to evaluate the association of circulating and imaging biomarkers with subsequent CE and HT in stroke patients with the dual purpose of investigating possible predictors as well as molecular dynamics underpinning those events and functional outcomes. Concurrently, the preclinical study will develop a new mouse model of middle cerebral artery (MCA) occlusion and recanalization to explore BBB alterations and their potentially harmful effects on tissue. The clinical section of the study is based on a single-center observational design enrolling consecutive patients with AIS in the anterior circulation territory, treated with recanalization therapies from October 1, 2015 to May 31, 2020. The study will employ an innovative evaluation of routine CT scans: in fact, we will assess and quantify the presence of CE and HT after stroke in CT scans at 24 h, through the quantification of anatomical distortion (AD), a measure of CE and HT. We will investigate the relationship of AD and several blood biomarkers of inflammation and extracellular matrix, with functional outcomes at 3 months. In parallel, we will employ a newly developed mouse model of stroke and recanalization, to investigate the emergence of BBB changes 24 h after the stroke onset. The close interaction between clinical and preclinical research can enhance our understanding of findings from each branch of research, enabling a deeper interpretation of the underlying mechanisms of reperfusion injury following recanalization treatment for AIS.

The poor transport of molecular and nanoscale agents through the blood–brain barrier together with tumour heterogeneity contribute to the dismal prognosis in patients with glioblastoma multiforme. Here, a biodegradable implant (μMESH) is engineered in the form of a micrometre-sized poly(lactic-co-glycolic acid) mesh laid over a water-soluble poly(vinyl alcohol) layer. Upon poly(vinyl alcohol) dissolution, the flexible poly(lactic-co-glycolic acid) mesh conforms to the resected tumour cavity as docetaxel-loaded nanomedicines and diclofenac molecules are continuously and directly released into the adjacent tumour bed. In orthotopic brain cancer models, generated with a conventional, reference cell line and patient-derived cells, a single μMESH application, carrying 0.75 mg kg−1of docetaxel and diclofenac, abrogates disease recurrence up to eight months after tumour resection, with no appreciable adverse effects. Without tumour resection, the μMESH increases the median overall survival (∼30 d) as compared with the one-time intracranial deposition of docetaxel-loaded nanomedicines (15 d) or 10 cycles of systemically administered temozolomide (12 d). The μMESH modular structure, for the independent coloading of different molecules and nanomedicines, together with its mechanical flexibility, can be exploited to treat a variety of cancers, realizing patient-specific dosing and interventions.

Surface PEGylation, biological camouflage, shape and stiffness modulation of nanoparticles as well as liver blockade and macrophage depletion have all improved the blood longevity of nanomedicines. Yet, the mononuclear phagocytic system still recognizes, sequesters, and processes the majority of blood borne particles. Here, the natural fatty acid methyl palmitate is combined with endogenous blood components – albumin – realizing ∼200 nm stable, spherical nanoparticles (MPN) capable of inducing a transient and reversible state of dormancy into macrophages. In primary bone marrow derived monocytes (BMDM), the rate of internalization of 5 different particles, ranging in size from 200 up to 2000 nm, with spherical and discoidal shapes, and made out of lipids and polymers, was almost totally inhibited after an overnight pre-treatment with 0.5 mM MPN. Microscopy analyses revealed that MPN reversibly reduced the extension and branching complexity of the microtubule network in BMDM, thus altering membrane bulging and motility. In immunocompetent mice, a 4 h pre-treatment with MPN was sufficient to redirect 2000 nm rigid particles from the liver to the lungs realizing a lung-to-liver accumulation ratio larger than 2. Also, in mice bearing U87-MG tumor masses, a 4 h pre-treatment with MPN enhanced the therapeutic efficacy of docetaxel-loaded nanoparticles significantly inhibiting tumor growth. The natural liver sequestering function was fully recovered overnight. This data would suggest that MPN pre-treatment could transiently and reversibly inhibit non-specific particle sequestration, thus redirecting nanomedicines towards their specific target tissue while boosting their anti-cancer efficacy and imaging capacity.

The ictal‐interictal continuum represents a diagnostic challenge even for expert neurrophysiologists, often requiring an additional multimodal diagnostic workup to understand its clinical significance. Lateralised rhythmic delta activity (LRDA) is an ictal‐interictal continuum pattern that has only recently been investigated and recognised as potentially ictogenic or sometimes even ictal. We describe a patient who presented with acute‐onset aphasia, initially suspected of having a stroke; advanced brain imaging with CT‐perfusion showed features suggesting regional left temporo‐parietal hyperperfusion and an EEG revealed LRDA with fluctuations and intermixed sharp waves in the same areas. Treatment with lacosamide caused both clinical and EEG improvement after a few hours, supporting the hypothesis that the EEG pattern represented an ictal/interictal phenomenon. In the literature, a correlation between metabolic/perfusion imaging and ictal‐interictal continuum patterns is described regarding lateralised periodic discharges but less studied for LRDA. In this case, we adopted a multimodal approach, integrating advanced imaging, EEG, clinical features, and response to therapy, to consider the overall clinical presentation as focal NCSE.

AbstractMemory for object location requires at least three processes: object recognition, object location, and object-location binding. Gender-related differences during childhood are still a matter of debate, especially concerning memory for object location, where females are expected to outperform males. Memory for object position is pivotal for spatial navigation and its investigation during childhood is crucial in order to understand the roots of gender-related differences in spatial orientation. Actually, environmental objects, namely landmarks, can be located using egocentric and/or allocentric frames of references, as well as using the spatial translation between them. Here, we investigated gender-related differences during childhood in object recognition and location whenever a shift between egocentric and allocentric frame of reference is required. Sixty-three boys and 44 girls (aged between four and ten years old) were asked to egocentrically learn a path on the Walking Corsi Test enriched with three landmarks. Then, children were asked to recognize (object recognition) the landmarks encountered along the path and locate them (object location) on an allocentric configuration of the spatial array. Girls outperformed boys in locating landmarks, whereas no difference occurred in landmark recognition. These results provide insights into the gender-related differences in location memory of landmarks, suggesting that females are better than males in the object location component well before the age of 13.

Introduction Although pathogenesis of small vessel disease is poorly understood, increasing evidence suggests that endothelial dysfunction may have a relevant role in development and progression of small vessel disease. In this cross-sectional study, we investigated the associations between imaging signs of small vessel disease and blood biomarkers of endothelial dysfunction at two different time points in a population of ischaemic stroke patients.Patients and methods In stroke patients treated with intravenous thrombolysis, we analysed blood levels of von Willebrand factor, intercellular adhesion molecule-1, vascular cell adhesion molecule-1 and vascular endothelial growth factor. Three reviewers independently assessed small vessel disease features using computed tomography. At baseline and 90 days after the index stroke, we tested the associations between single and combined small vessel disease features and levels of blood biomarkers using linear regression analysis adjusting for age, sex, hypertension, diabetes, smoke.Results A total of 263 patients were available for the analysis. Mean age (±SD) was 69 (±13) years, 154 (59%) patients were male. We did not find any relation between small vessel disease and endothelial dysfunction at baseline. At 90 days, leukoaraiosis was independently associated with intercellular adhesion molecule-1 (β = 0.21; p = 0.016) and vascular cell adhesion molecule-1 (β = 0.22; p = 0.009), and lacunes were associated with vascular endothelial growth factor levels (β = 0.21; p = 0.009) whereas global small vessel disease burden was associated with vascular endothelial growth factor (β = 0.26; p = 0.006).Discussion Leukoaraiosis and lacunes were associated with endothelial dysfunction, which could play a key role in pathogenesis of small vessel disease.Conclusions Small vessel disease features and total burden were associated with endothelial dysfunction 90 days after the stroke, whereas there was no relation during the acute phase. Our results suggest that endothelial dysfunction, particularly vascular endothelial growth factor, is involved in pathological process of small vessel disease.

ABSTRACTWe report Developmental Landmark Agnosia (DLA) in a 6-year-old boy (L.G.) who was referred to us for congenital prosopagnosia (see Pizzamiglio et al., 2017, in which both testing and rehabilitation of Congenital Prosopagnosia are reported). We investigated his performance using a neuropsychological battery and eye movement recordings. The assessment showed the presence of deficits in recognizing familiar places (along with Congenital Prosopagnosia), but not common objects. Eye movement recordings confirmed his problems in recognizing familiar landmarks and misrecognition of unfamiliar places. L.G. is the first evidence of a DLA, suggesting identification of taxonomy of navigational disorders in Developmental Topographical Disorientation is possible, as in the Acquired Topographical Disorientation.

Background: The genomes of present-day non-Africans are composed of 1–3% of Neandertal-derived DNA as a consequence of admixture events between Neandertals and anatomically modern humans about 50–60 thousand years ago. Neandertal-introgressed single nucleotide polymorphisms (aSNPs) have been associated with modern human disease-related traits, which are risk factors for pancreatic ductal adenocarcinoma (PDAC), such as obesity, type 2 diabetes, and inflammation. In this study, we aimed at investigating the role of aSNPs in PDAC in three Eurasian populations. Results: The high-coverage Vindija Neandertal genome was used to select aSNPs in non-African populations from 1000 Genomes project phase 3 data. Then, the association between aSNPs and PDAC risk was tested independently in Europeans and East Asians, using existing GWAS data on more than 200 000 individuals. We did not find any significant associations between aSNPs and PDAC in samples of European descent, whereas, in East Asians, we observed that the Chr10p12.1-rs117585753-T allele (MAF = 10%) increased the risk to develop PDAC (OR = 1.35, 95%CI 1.19–1.54, P = 3.59 × 10–6), with a P-value close to a threshold that takes into account multiple testing. Conclusions: Our results show only a minimal contribution of Neandertal SNPs to PDAC risk.

The use of magnetic nanoparticles in oncothermia has been investigated for decades, but an effective combination of magnetic nanoparticles and localized chemotherapy under clinical magnetic hyperthermia (MH) conditions calls for novel platforms. In this study, we have engineered magnetic thermoresponsive iron oxide nanocubes (TR-cubes) to merge MH treatment with heat-mediated drug delivery, having in mind the clinical translation of the nanoplatform. We have chosen iron oxide based nanoparticles with a cubic shape because of their outstanding heat performance under MH clinical conditions, which makes them benchmark agents for MH. Accomplishing a surface-initiated polymerization of strongly interactive nanoparticles such as our iron oxide nanocubes, however, remains the main challenge to overcome. Here, we demonstrate that it is possible to accelerate the growth of a polymer shell on each nanocube by simple irradiation of a copper-mediated polymerization with a ultraviolet light (UV) light, which both speeds up the polymerization and prevents nanocube aggregation. Moreover, we demonstrate herein that these TR-cubes can carry chemotherapeutic doxorubicin (DOXO-loaded-TR-cubes) without compromising their thermoresponsiveness both in vitro and in vivo. In vivo efficacy studies showed complete tumor suppression and the highest survival rate for animals that had been treated with DOXO-loaded-TR-cubes, only when they were exposed to MH. The biodistribution of intravenously injected TR-cubes showed signs of renal clearance within 1 week and complete clearance after 5 months. This biomedical platform works under clinical MH conditions and at a low iron dosage, which will enable the translation of dual MH/heat-mediated chemotherapy, thus overcoming the clinical limitation of MH: i.e., being able to monitor tumor progression post-MH-treatment by magnetic resonance imaging (MRI).

Recent years have witnessed an increasing need for the automated classification of sequential data, such as activities of daily living, social media interactions, financial series, and others. With the continuous flow of new data, it is critical to classify the observations on-the-fly and without being limited by a predetermined number of classes. In addition, a model should be able to update its parameters in response to a possible evolution in the distributions of the classes. This compelling problem, however, does not seem to have been adequately addressed in the literature, since most studies focus on offline classification over predefined class sets. In this paper, we present a principled solution for this problem based on an adaptive online system leveraging Markov switching models and hierarchical Dirichlet process priors. This adaptive online approach is capable of classifying the sequential data over an unlimited number of classes while meeting the memory and delay constraints typical of streaming contexts. In this paper, we introduce an adaptive “learning rate” that is responsible for balancing the extent to which the model retains its previous parameters or adapts to new observations. Experimental results on stationary and evolving synthetic data and two video data sets, TUM Assistive Kitchen and collated Weizmann, show a remarkable performance in terms of segmentation and classification, particularly for sequences from evolutionary distributions and/or those containing previously unseen classes.

Sequential labeling addresses the classification of sequential data, which are widespread in fields as diverse as computer vision, finance, and genomics. The model traditionally used for sequential labeling is the hidden Markov model (HMM), where the sequence of class labels to be predicted is encoded as a Markov chain. In recent years, HMMs have benefited from minimum-loss training approaches, such as the structural support vector machine (SSVM), which, in many cases, has reported higher classification accuracy. However, the loss functions available for training are restricted to decomposable cases, such as the 0–1 loss and the Hamming loss. In many practical cases, other loss functions, such as those based on the $F_{1}$ measure, the precision/recall break-even point, and the average precision (AP), can describe desirable performance more effectively. For this reason, in this paper, we propose a training algorithm for SSVM that can minimize any loss based on the classification contingency table, and we present a training algorithm that minimizes an AP loss. Experimental results over a set of diverse and challenging data sets (TUM Kitchen, CMU Multimodal Activity, and Ozone Level Detection) show that the proposed training algorithms achieve significant improvements of the $F_{1}$ measure and AP compared with the conventional SSVM, and their performance is in line with or above that of other state-of-the-art sequential labeling approaches.

α-Alkynyl-α′-trimethylsilylhydrazones are readily oxidized into diazo compounds under simple experimental conditions. These stable diazo species can in turn react with a range of carboxylic acids via a protonation–nucleophilic substitution sequence, leading to valuable α-acyloxy-α-alkynyltrimethylsilanes. This procedure avoids the delicate preparation and manipulation of α-hydroxypropargyltrimethylsilanes.

We demonstrate thermo-optic control on the propagation of optical spatial solitons in nematic liquid crystals. By varying the sample temperature, both linear and nonlinear optical properties of the reorientational material are modulated by acting on the refractive indices, the birefringence, and the elastic response. As a result, both the trajectory and transverse confinement of spatial solitons can be adjusted, demonstrating an effective means to tune and readdress self-induced optical waveguides.

Modern production is increasingly fragmented across countries. To disentangle the world production system at sector level, we use the World Input–Output Database to construct the World Input–Output Network (WION) where the nodes are the individual sectors in different countries and the edges are the transactions between them. In order to explore the features and dynamics of the WION, in this article we detect the communities in the WION and evaluate their significance using a random walk Markov chain approach. Our results contribute to the recent stream of literature analysing the role of global value chains in economic integration across countries, by showing global value chains as endogenously emerging communities in the world production system, and discussing how different perspectives produce different results in terms of the pattern of integration.

On 24 August 2016 a seismic event (Mw6.0) was the first of the long Central Italy sequence (ongoing at the end of 2017) of medium‐to‐high magnitude earthquakes, with nine Mw≥5 up to October 2017, and with about 74.000 seismic events registered after 1 year. The largest was the Mw6.5 30 October 2016 event near Norcia. After the major seismic events, 17 mud volcanoes erupted around Monteleone di Fermo village (Marche region). Mud volcano eruptions generally occurred a few hours to a few days after the main earthquakes, suggesting a seismic triggering. We analyzed the peak ground velocities and dynamic stresses during the three largest earthquakes. We also evaluated the static stress changes in order to assess the potential influence of normal stress changes on the feeder system of the activated mud volcanoes. We find a correlation with dynamic stresses, whereas static stress changes are negligible or negative (with values reaching −0.44 bar, clamping feeder dykes). We conclude that seismic shaking (up to ~3.9 bar during Norcia earthquake) is the dominant driver for these eruptions. Finally, we evaluated the response ratio as a function of the dynamic stress. It increases exponentially with peak dynamic stress varying from <10% for peak dynamic stress >0.3 bar to >50% for peak dynamic stress >2 bar, indicating a link between earthquake shaking and mud volcano activity. ABSTRACTIncontinentia Pigmenti (IP, OMIM#308300) is a rare X-linked genomic disorder (about 1,400 cases) that affects the neuroectodermal tissue and Central Nervous System (CNS). The objective of this study was to describe the cognitive-behavioural profile in children in order to plan a clinical intervention to improve their quality of life. A total of 14 girls (age range: from 1 year and 2 months to 12 years and 10 months) with IP and the IKBKG/NEMOgene deletion were submitted to a cognitive assessment including intelligence scales, language and visuo-spatial competence tests, learning ability tests, and a behavioural assessment. Five girls had severe to mild intellectual deficiencies and the remaining nine had a normal neurodevelopment. Four girls were of school age and two of these showed no intellectual disability, but had specific disabilities in calculation and arithmetic reasoning. This is the first description of the cognitive-behavioural profile in relation to developmental age. We stress the importance of an early assessment of learning abilities in individuals with IP without intellectual deficiencies to prevent the onset of any such deficit.

AbstractContext:Various processes including inflammation and endothelial dysfunction have been implicated in the pathogenesis of cardioembolic (CE) strokes.Objective:To review the evidence and investigate the association between immune-inflammatory biomarkers and CE strokes versus other stroke subtypes.Methods:We systematically reviewed the literature (sources: MEDLINE, web-based register http://stroke-biomarkers.com, reference lists) with quality assessment and meta-analysis of selected articles.Results:The most consistent association was found between C-reactive protein (CRP) and CE strokes when compared to other stroke subtypes (standardized mean difference 0.223 (0.116, 0.343); p < 0.001)Conclusions:Our findings confirm a possible association between selected inflammatory biomarkers and CE stroke.
