1. The role of an integrated referral program for patients with liver disease: A network between hub and spoke centers

Abstract: Access to Liver transplantation (LT) can be affected by several barriers, resulting in delayed referral and increased risk of mortality due to complications of the underlying liver disease. To assess the clinical characteristics and outcomes of patients with acute or chronic liver disease referred using an integrated referral program. An integrated referral program was developed in 1 October 2017 based on email addresses and a 24/7 telephone availability. All consecutive adult patients with liver disease referred for the first time using this referral program were prospectively collected until 1 October 2021. Characteristics and outcomes of inpatients were compared with a historical cohort of patients referred without using the integrated referral program (1 October 2015—1 October 2017). Patients were further divided according to pre‐ and post‐Covid‐19 pandemic. Two hundred eighty‐one referred patients were considered. End stage liver disease was the most common underlying condition (79.3%), 50.5% of patients were referred as inpatients and 74.7% were referred for LT evaluation. When inpatient referrals (n= 142) were compared with the historical cohort (n= 86), a significant increase in acute liver injury due to drugs/herbals and supplements was seen (p= 0.01) as well as an increase in End stage liver disease due to alcohol‐related liver disease and NASH, although not statistically significant. A significant increase in referrals for evaluation for Trans‐jugular intrahepatic portosystemic shunt placement was seen over time (5.6% vs. 1%; p= 0.01) as well as for LT evaluation (84.5% vs. 81%; p= 0.01). Transplant‐free survival was similar between the study and control groups (p= 0.3). The Covid‐19 pandemic did not affect trends of referrals and patient survival. The development of an integrated referral program for patients with liver disease can represent the first step to standardize already existing referral networks between hub and spoke centers. Future studies should focus on the timing of referral according to different etiologies to optimize treatment options and outcomes.

Abstract: Acute kidney injury (AKI) is a common perioperative complication in patients undergoing cardiovascular surgery, increasing mortality, morbidities, and costs. Recently, growing interest has risen in the use of the renal-resistive index (RRI) as a predictor of perioperative AKI. The aim of this study was to evaluate the role of RRI variation to identify postoperative AKI. An observational, prospective, pilot study. Department of Vascular Surgery, University Hospital of Padova. The study authors included 53 consecutive patients undergoing aortic surgery from September 2018 to June 2019. Basal and daily postoperative serum creatinine and urine output were assessed. RRI was measured preoperatively and on the first postoperative day. AKI was defined using Kidney Disease Improving Global Outcome criteria. Twelve patients out of 53 developed AKI. The RRI percentage increase (%RRI) was associated with the development of AKI by univariate regression (p = 0.01). The receiver operating characteristic curve showed an overall diagnostic accuracy of 0.75 (95% confidence interval [CI], 58.2-92.6). The cutoff of 7 percentage points in the %RRI resulted in early identification of AKI onset with 90% specificity (95% CI, 76.9-97.3). The net benefit of postoperative RRI-based management was 11%. RRI variation could be a useful tool to investigate kidney function in patients undergoing aortic surgery. 7. Consideration of the learning effect in the comparison of Fixed Worker and Walking Worker assembly systems

Abstract: Market demand is always more characterized by products variability and life cycle reduction; consequently, manufacturing systems need to be always more flexible and adaptable, in order to well react to this fluctuating demand. The most common assembly systems configuration is the Fixed Worker (FW) assembly line, where the workers always remain in the same workstation, to perform a single and often repetitive set of assembling tasks characterized by manual operations. On the other side, in the Walking Worker (WW) assembly line each operator travels along the workstations of the line performing all assembly tasks. In this context, the introduction of a new product, or a new worker, has an effect on the performance of the assembly system, caused by the learning of the new tasks. This paper aims to investigate the learning effect on the FW and WW systems and to compare them in order to define the best configuration in terms of performance. In particular, it proposes a mathematical model to represent the learning effect, also applying it to a numerical example. Results show that FW configuration is preferable in standard condition, while when the throughput is fluctuating, WW system is better in terms of performance. Future research will extend this study to more case studies in order to better understand the trend of this correlation, also considering particular phenomena like workers’ absenteeism and turnover.

10. Influence of task time variation in adopting Walking Worker assembly systems: a design approach

Abstract: Unlike traditional Fixed Worker (FW) assembly systems, in which an operator remains in the same workstation performing repetitive tasks, in the Walking Worker (WW) ones the operators travel along the line stations to produce the finished product. Interestingly, such a configuration enables to achieve a greater level of production volume flexibility, without changing the assembly system layout, parts disposition, and tools assignment. The basic requirement of a WW is to be cross-trained and able to fully assemble the product from beginning to end. The WW travel activity, which increases when the number of stations increases, negatively impacts on the system performance. On the other hand, the possibility to have extra-stations, i.e., a greater number of stations compared with the number of operators, makes the WW system able to dynamically balance the work overload differences between the operators.

15. A parts feeding model for big size products: the active and idle assembly islands strategy

Abstract: In fixed-position assembly systems (or assembly islands) for big-size highly customised products, workers and parts move around the sub-assemblies, which stay fixed in one place. Generally, there are several parallel assembly islands and a single product is assembled in each of them. In some cases, assembly workers can also do the parts feeding activities, which are considered as non-value added activities, with a consequent decrease in the system performance. In this paper, we propose a new mathematical model to reduce parts feeding costs by considering active and idle assembly islands, so that the parts feeding and the assembly activities can be done simultaneously by the respective operators. A sensitivity analysis is carried out to study the effects of the variables and of the parameters on the model results. Moreover, the model is applied to two real applications by evaluating how many idle assembly islands are required to improve the efficiency of the whole system. The results show that the main parameters that influence the final number of idle assembly islands are the percentages of time that assembly operators spend for parts feeding activities and the time required for material preparation and assembly activities.

16. Considering workers’ features in manufacturing systems: a new job-rotation scheduling model

Abstract: The European manufacturing industry is entering a new era in which working populations are ageing. The E.U. had set itself strategy objectives to increase the labour market participation of older workers. However, practical limits arise, and a complete re-thinking of operation management strategies and manufacturing systems design and management is needed. As underlined in several works, older workers have not the same physical capacity as the younger ones. Consequently, they are more subjected to develop work-related musculoskeletal disorders despite younger colleagues. On the other hand, they might present higher skill levels in doing some specific tasks due to their considerable experience. Thus, they can be employed in teaching or training younger or unskilled workers. Starting from these initial considerations, in this paper, we develop a new age-oriented job rotation scheduling model. Both physical capacity and experience level aspects are included in the mathematical model aiming to maximize daily productivity. We quantify physical fatigue by using the energy expenditure rate and the maximum acceptable energy expenditure. Then, the rest allowance concept is evaluated according to the workers’ age and the shift work duration. Variable execution times of each job according to the workers’ experience level and mandatory training activities for unskilled workers are also taken into consideration. Finally, a numerical case derived by a real application is proposed to validate the model and demonstrate benefits we can achieve by applying it.

17. Design of AGV systems in working environments shared with humans: a multi case study

Abstract: To meet the challenges and needs of an ever-changing market and as part of the fourth industrial revolution, factories are transforming into increasingly automated environments. A widely used and well-established solution today is Automated Guided Vehicles (AGVs), which often work closely with humans in crowded environments. Thus, in addition to flexibility, another important criterion associated with automatic handling systems is safety. The purpose of this work is to show how the involvement of three different but equally important roles in the design of an AGV system can benefit the whole project. The advantage of considering three different perspectives is the possibility of obtaining a more complete vision from the earliest stages of implementation, avoiding, as far as possible, the need to make changes in the next stages, which would generate higher costs than necessary. The article is based on two case studies, each one set in a major European manufacturing company: the first one is an Italian automotive manufacturer and the second one is a Swedish manufacturer of mechanical components. Both case companies apply AGVs in their material handling processes and, accordingly, have experience of both implementing and operating AGV systems. The article applies semi-structured interviews to study the three key roles, highlighting the key points for each role and showing the common issues that emerged from the interviews.

Abstract: Objectives The purpose of this study was to assess the responsiveness of the newly developed Geriatric Assessment in Hematology (GAH) scale to clinical change in older patients diagnosed with hematologic malignancies. Methods A prospective observational study conducted in 164 patients aged ≥ 65 years and diagnosed with myelodysplastic syndrome (MDS)/acute myeloid leukemia (AML), multiple myeloma (MM), or chronic lymphocytic leukemia (CLL). Responsiveness of the GAH scales was studied by means of the Eastern Cooperative Oncology Group (ECOG) score, the Karnofsky performance status (KPS) score, the visual analog scale (VAS), and the physician's subjective assessment, used as clinical anchors to identify whether patients had changed clinically (either improved or worsened) or not since the baseline visit. Responsiveness was evaluated on the basis of effect size (ES). Results 164 patients (men, 63.7%; median age, 77.0 (72.8–81.4) participated. Statistically significant correlations were obtained between the investigator's qualitative assessment and changes in ECOG, KPS, and VAS scores. Likewise, a statistically significant correlation was obtained between the investigator's qualitative assessment and changes in the GAH scale score. Responsiveness of the GAH scale to detect clinical change was satisfactory (ES 0.34). Conclusion Findings confirm that the GAH scale is responsive to clinical changes in patients' health status. 21. Fatigue accumulation in the assignment of manual material handling activities to operators

Abstract: In an industrial context it is not easy to evaluate how to assign activities to operators for reaching the improvement of the performance of the system, since the influence of the activities on the operators is strictly linked to their personal characteristics. Moreover, during the performance of the activity, the operators experience physical fatigue which can accumulate during time if they cannot recover properly. For manual material handling activities, the whole body is involved; subsequently, the physical fatigue can be measured through the energy expenditure rate. In relation to this, the aim of this paper is to improve the scheduling of manual material handling activities of the operators by knowing the energy expenditure rate and the duration of the activity and by knowing how the specific operator accumulates fatigue and recovers. 24. A Model to Optimize the Reference Storage Assignment in a Supermarket to Expedite the Part Feeding Activities

Abstract: In many industrial environments, picking operations are performed with tow trains in order to efficiently feed a production or an assembly system. This operation is often time-consuming, especially for the automotive environment, which employs the supermarket concept very often to manage thousands of different parts. This research aims at improving the picking operations by optimizing the storage assignment of the references in a supermarket. This is a process in which the references are placed in specific racks of the warehouse. A linear programming formulation is provided so that the picking operations can be performed in the minimum amount of time. The model considers some constraints linked to the specific kind of equipment used in a warehouse, and the ergonomic energy expenditure. This approach has been implemented in a numerical case inspired by a real industrial application.

25. Modelling of Rail Guided Vehicles serving an automated parts-to-picker system

Abstract: An automated parts-to-picker picking system usually consists of an automated warehouse, with Automatic Storage and Retrieval Systems (AS/RS) that retrieve the Stock Keeping Units (SKUs) of the various needed products from their stocking locations, and of a picking area, with human operators or robots that pick the needed items in order to create a mixed shipping unit. The automated warehouse and the picking area are connected by an automated transportation system, which moves the SKUs from the warehouse to the picking stations and vice versa. The transportation system can be, for example, a ring rail conveyor on which various Rail Guided Vehicles (RGVs) are able to carry one SKU at a time. The present paper proposes a preliminary simulative analysis and, then, a mathematical formulation for this transportation system, useful to properly estimate the number of RGVs that are required to fulfill a certain picking throughput. In fact, it is shown that the picking throughput does not increase linearly with the number of vehicles employed, due to congestion issues.

26. Ultra Wide Band Indoor Positioning System: analysis and testing of an IPS technology

Abstract: Due to their current operating context, all logistics processes, from the simplest to the most complex ones, are facing always more interesting challenges in terms of management of a huge variety of products and, at the same time, strict lead times. In such a framework, it turns out that logistics inevitably has to aim at avoiding or, at least, reducing, all the possible inefficiencies that could emerge during the execution of the various activities that are needed to deliver a required product to a customer. These inefficiencies could be, among others, delays in the searching of the needed product code within a warehouse, errors in the retrieval or in the picking of an item, waste of time for carts or for operators’ travelling activity, lack of availability of warehouse facilities and devices due to failures and breakdowns. Of course, the overcome of the inefficiencies has to pass through the retrieval of the information that can be useful to increase the awareness of such existing lacks. For example, it would be important to have the data related to the movements of resources and to objects handling. In this paper, an innovative indoor positioning system is presented. Based on a real-time indoor location technology using Ultra Wide Band, it can be used for having an effective overview of a logistic system. After an introduction of the possible technologies for indoor positioning and tracking, the configuration of the system is showed, together with a description of a simple test and of an industrial application. The reported examples highlight some preliminary insights about the system accuracy and its applicability.

Abstract: The paper presents the VR-Ergo Log system, an inertial motion capture system integrated with immersive reality and combined with a heart rate monitoring. By using immersive reality, the operator will be able to move and interact within a virtual workplace environment, in order to permit a fast and efficient ergonomic assessment of future workplace solutions and to avoid all cost-consuming activities related to the pre-production design of the workplace or to the prototyping of new products. This integrated system allows to evaluate in advance the time-based and ergo-based indices which can help the practitioners on understanding how to design the workplace and the devices to be used by operators. In addition, the use of the heart rate monitor permits to have a real-time feedback regarding the fatigue the operator is perceiving. The use of such a system will help to make more efficient the early design phases of an industrial workspace, by also considering the impact of human diversity and avoiding non-ergonomic solutions especially when an ageing workforce will be enrolled in the system.

Abstract: The GAH (Geriatric Assessment in Hematology) scale is a psychometrically valid tool aimed at identifying older patients with hematological malignancies at higher risk of treatment-related toxicity. Our objective in this study was to determine the weights for each dimension of the GAH scale and the cut-off point to reliably predict treatment tolerability in this population, estimated by a weighted receiver operating characteristic (ROC) analysis and quantified by the area under the curve (AUC). The RETROGAH was a retrospective cohort study including 126 patients who had previously participated in the GAH study. Patients were ≥ 65 years old with newly diagnosed myelodysplastic syndrome (MDS)/acute myeloid leukemia (AML), multiple myeloma (MM), or chronic lymphoid leukemia (CLL) and treated with standard front-line therapy within three months after having completed the GAH scale. The optimal cut-off value of the GAH total score to discriminate patients at higher risk of treatment toxicity was 42, with 68.5% sensitivity and 55.8% specificity. Using this value, 66.1% of patients evaluated were found to develop some type of toxicity. The AUC was 0.6259 (95% CI: 0.512–0.739; p = 0.035). The GAH scale not only would enable clinicians to individualize therapy based on individual risk of toxicity but also discriminate patients that will benefit most from intensive treatments from those requiring an adapted approach. While futures studies in clinical practice may improve the model and overcome its limitations, the GAH scale should not be used alone when making treatment decisions. [ABSTRACT FROM AUTHOR]

In most cases, this activity is carried on by operators for the flexibility this can assure. To improve the performance of this kind of system it could be useful to consider more carefully human factor. One aspect of this topic is the accumulation of fatigue which can cause bad physical conditions on the long-term period and have impacts on the efficiency. Focalizing on the existing literature, the objective of this paper is to define how to study fatigue and recovery time in order picking systems. The main focus is the understanding of which instrument can be used for this kind of analysis and what are the steps to be taken for the improvement of picking efficiency through the analysis of fatigue accumulation of the operators.

31. Development and psychometric validation of a brief comprehensive health status assessment scale in older patients with hematological malignancies: The GAH Scale.

Abstract: Objectives The purpose of this study was to develop a new brief, comprehensive geriatric assessment scale for older patients diagnosed with different hematological malignancies, the Geriatric Assessment in Hematology (GAH scale), and to determine its psychometric properties. Materials and Methods The 30-item GAH scale was designed through a multi-step process to cover 8 relevant dimensions. This is an observational study conducted in 363 patients aged ≥ 65 years, newly diagnosed with different hematological malignancies (myelodysplasic syndrome/acute myeloblastic leukemia, multiple myeloma, or chronic lymphocytic leukemia), and treatment-naïve. The scale psychometric validation process included the analyses of feasibility, floor and ceiling effect, validity and reliability criteria. Results Mean time taken to complete the GAH scale was 11.9 ± 4.7 min that improved through a learning-curve effect. Almost 90% of patients completed all items, and no floor or ceiling effects were identified. Criterion validity was supported by reasonable correlations between the GAH scale dimensions and three contrast variables (global health visual analogue scale, ECOG and Karnofsky), except for comorbidities. Factor analysis (supported by the scree plot) revealed nine factors that explained almost 60% of the total variance. Moderate internal consistency reliability was found (Cronbach's α: 0.610), and test–retest was excellent (ICC coefficients, 0.695–0.928). Conclusion Our study suggests that the GAH scale is a valid, internally reliable and a consistent tool to assess health status in older patients with different hematological malignancies. The OWAS (Ovako Working posture Analysing System) index and the energy expenditure level are used to evaluate the different systems. The necessary data was collected in a field analysis using a motion capturing system. Mathematical models for the ergonomics analysis are developed and risk factors related to musculoskeletal disorders associated with the specific pallet picking system are discussed.

Abstract: Determining safety stock is one of the most critical issues in inventory and logistics management. This paper presents a cost-based analytical model to determine the best safety stock and customer service level for uncertain demand of products during generic replenishment lead times and a continuous-review inventory system. The proposed model uses the Weibull distribution to model the generic stochastic demand. α and β are the scale parameter and the shape parameter, respectively. An experimental analysis based on the assumption of different distributions of demand of product demonstrates the effectiveness of the proposed model compared to the so-called normal-based traditional model. Significantly, the proposed Weibull-based model is a comprehensible and user-friendly tool that can be useful for production system managers and practitioners.

34. Sustainable humanitarian operations: closed-loop supply chain

Abstract: This paper explores the closed loop supply chain (CLSC) topic by providing a model and evaluating regular humanitarian logistics (R-HL) where the main objective of the operation moves from the reduction of population needs to a minimisation of the overall impact of the disaster and operations. The research focuses on the logistic facility management coupled with material distribution modalities in a situation where items are not just delivered using 'forward channels'. It considers others flows that usually have not been faced by the humanitarian literature, as 'reverse channels' on the management of operations waste. Constraints related to transportation resources in a complex environment, transportation vehicle capacities and delivery time restrictions are considered and different scenarios are analysed. The proposed model optimises resources allocation and prepositioning decisions on a number of test problems. The impact of each proposed practice is evaluated in order to understand its applicability in this particular context.

37. Multi-criteria decision-making in the management of humanitarian operations

Abstract: Although multi-criteria decision making topic is well suggested by some papers there are no researches that have been focused on its application into humanitarian perations with the presentation of a hierarchy process to support the operators in making choices. The purpose of this paper is to present a general multi-criteria decision making process to assist the evaluation of suitable alternative solutions to humanitarian operations management issues. Humanitarian operations are described in terms of a multi-objective field that reflects service level, timing and costing as final goals, and is composed of different life cycle phases, each with its particular features. Through a deep literature review, a framework is built and analysed to support decision makers in their evaluation of operations issues in which the objective hierarchy is defined. The structured procedure is developed and validated with some examples of possible applications. Thereafter, using a sensitivity analysis, a real case is discussed.

38. Routing strategy in a distribution network when the driver learning effect is considered

Abstract: This paper faces one critical short term decision: the construction of routes for daily goods' deliveries in a distribution network. It investigates the possibility of applying a fixed routing strategy instead of a daily routing optimisation strategy, analysing the benefits derived from the driver's familiarity with his/her surroundings, customers' habits and set-ups. The final aim is to provide an effective and flexible decision-making tool to identify the best routing strategy, considering the level of learning invested by each driver. It is assumed that learning effect impacts on the service time, i.e., on the time spent in service operations once arrived at the customer's site. Results demonstrate that fixed routes strategy can often be better than the daily optimised strategy and strongly depends on the parameters investigated in this work. Two case applications are provided to help the reader's comprehension and demonstrate the methodology's potential and its practical implications.

Abstract: In today's industrial reality distribution network optimisation is one of the most crucial topics and in particular, supply chain management (SCM), is widely studied and discussed. SCM plays an even more critical role within the food industry network, where, both on the manufacturer's/wholesaler's side and the retailer's side, the margins are continuously decreasing and the competition is fierce. How convenient is it to manage the deliveries, inside a food distribution network, with according to customer demand, regarding frequency/quantity? Is it possible and efficient to satisfy the demand with direct deliveries with multi products pallets? These and other relevant questions have inspired this paper together with the desire to integrate the lean principles (pull philosophy, value maximisation, waste elimination, etc.) into an innovative SCM approach based on a linear programming model that focuses on one products category at the time, on its logistic characteristics and on the specific food supply chain (SC) features. To test the proposed model, a real application in a food SC is presented and a sensitive analysis of the lean–keys factors is developed.

47. The sustainable routing problem

Abstract: Nowadays environmental, social, and political pressures to limit the impacts linked to CO 2 emissions are rapidly increasing. This paper aim at the solution of a new class of A routing problem: the sustainable routing problem, where the objective, unlike the classical approaches, is the CO 2 emission minimisation. The amount of CO 2 emitted by a vehicle depends on different factors. If the problem is analysed from the driver’s point of view, these factors can be divided into ‘internal’, i.e., directly related to him/her (i.e., driving style, acceleration, average speed, route knowledge, etc.), and into ‘external’, i.e., depending on the environment in which the trip is made (i.e., traffic congestion, speed limits, vehicle type, etc.). Firstly the paper, starting from Fonseca et al. (2011) study, proposes a CO 2 emission estimation as function of the different internal/external factors. Secondly, the authors formalise a sustainable routing model, validating it through a case study as well as a parametric analysis. The results suggest that, contrary to the classical vehicle routing problem, the proposed model allows a larger reduction of CO 2 emission versus the increasing time and distance, suggesting interesting potentials in the environmental preservation.

48. Determination of N-benzylpiperazine in human plasma using derivatization-free liquid–liquid extraction and gas chromatography mass spectrometry

Abstract: A simple, fast and low-cost liquid–liquid extraction (LLE) method without a derivatization step for quantification of N-benzylpiperazine (BZP) in human plasma using a gas chromatography mass spectrometry (GC-MS) technique was developed. Among the tested extraction agents the best results were obtained with the use of ethyl acetate at pH 12. Optimization of the gas chromatography conditions was conducted including split ratio, temperature gradient of the analytical column and injection volume. The main validation parameters of the proposed method were determined. The extraction recovery was 68% with intra-day and inter-day precision (RSD%) below 5% (n= 3). The limit of detection (LOD) and quantification (LOQ) of BZP was 24.8 and 82.7 ng mL−1, respectively. A linear calibration curve in the range from LOQ to 1000 ng mL−1was obtained with the use of the selected ion monitoring (SIM) mode with determination coefficient (r2) higher than 0.999. The optimized and validated LLE-GC-MS method was effectively applied for analysis of clinical plasma samples. The proposed sample preparation protocol was found to be a good alternative to the derivatization step and may be applicable in tests for proof of BZP intake in toxicological routine analysis.

49. Buffer design for availability: a new simulative study in case of infant and random failures

Abstract: In automatic production systems, the size of intermediate buffers, significantly affects the success of the production system, especially when their functions is to protect the system from the inevitable processing time fluctuations caused by machine disruptions. In Battini et al. (2009), the authors provided a new experimental formula to define the optimal buffer size to maximise throughput without the need for oversized machines. The authors observed only ‘wear-out failures’, in which the failure rate increases with time. In order to better understand the potential benefits of previous results and extend their applications in industrial contexts, a new simulation study is now presented, with reference to all those interesting industrial cases in which production flow lines are largely affected by infant failures or random machine failures. The final results show a completely new experimental matrix that cover all the possible kinds of failures, able to work in different industrial settings and under different reliability performances.

50. Multi-stage supply network design in case of reverse flows: a closed-loop approach

Abstract: Design of distribution networks is one of the most critical issues in the management of supply networks. When the supply chain takes into account the whole life of the product (warranty, remanufacturing, recycle, disposal, etc.), the adverse effects on the logistic flow is quite considerable on the structure of the network. For this reason, these aspects should be considered in network design. This paper addresses the possibility to apply different supply chain (SC) design approaches in presence of reverse flows, analysing the network structure where the considered flows are forward flow exclusively, or forward and reverse flows, or integral closed-loop flows. The study also presents an integrated methodology in closed-loop network design, based on mixed-integer programming, considering as inputs the most important driver as fixed and variable costs (installation, transportation, handling, inventory and production), facilities attributes (type, location, capacity and costs), stochastic demand, multi-echelon, multi-product, multi-production, multi-distribution and multi-transportation system. A real industrial application to validate the proposed closed-loop SC design methodology and a comparison between different SC design approaches are presented as a result of this paper.
