De récentes études ont montré un manque de connaissances et des idées reçues sur le cancer du poumon auprès du grand public et des médecins. Nous avons voulu approfondir ce sujet en France.

Un patient, aux antécédents de carcinome épidermoïde pulmonaire du lobe supérieur droit traité par radiothérapie et chimiothérapie concomitante 14ans auparavant, est pris en charge pour dyspnée. L’endoscopie bronchique révèle l’existence d’une obstruction complète de la bronche souche droite dont l’aspect macroscopique est fortement évocateur d’une ré-évolution néoplasique. Néanmoins les prélèvements réalisés ne retrouvent qu’un tissu nécrotique et inflammatoire sans critères de malignité. Malgré la réalisation d’une désobstruction bronchique en bronchoscopie rigide, on assiste à une récidive rapide et complète conduisant à la mise en place d’une prothèse en Y. De nouveau les examens histologiques, bactériologiques et mycologiques reviennent négatifs. Le patient est rapidement ré-hospitalisé pour tableau d’infection pulmonaire qui retrouve une récidive péri- et intraprothétique de l’obstruction. La mise en culture des prélèvements biopsiques permet cette fois la mise en évidence d’un Actinomyces meyeri. Une antibiothérapie adaptée permet alors la régression complète de l’obstruction. Le patient décèdera quelques mois plus tard d’une hémoptysie massive après le retrait de la prothèse, l’autopsie retrouvant un orifice fistuleux entre la bronche souche droite et l’artère pulmonaire, sans argument pour une récidive néoplasique ou la persistance de lésions liées à l’actinomycose.

Peu de données sont disponibles sur l’impact de la mutation KRAS chez les patients avec cancer du poumon non à petites cellules non épidermoïdes, métastatiques (CBNPCm) traités par immunothérapie. Cette analyse précise les caractéristiques des patients avec mutation KRAS G12C, KRAS non-G12C et l’impact de la mutation KRAS sur l’efficacité de l’immunothérapie par Pembrolizumab en première ligne chez les patients CBNPCm, PDL>50 %.

La prise en charge des cancers bronchiques non à petites cellules (CBNPC) a été profondément modifiée par l’arrivée de l’immunothérapie, et notamment des anticorps anti-PD1. Dans ces études, peu de patients étaient porteurs d’une tumeur avec addiction oncogénique. L’objectif de cette étude est d’évaluer l’efficacité de l’immunothérapie anti-PD1/PDL1 dans le CBNPC avec mutation BRAF, HER2, MET, amplification MET ou translocation RET.

An enzyme-linked immunosorbent assay and a Western blot analysis were developed to study the antibody response to Pneumocystis carinii in serum and bronchoalveolar lavage fluid from 27 human immunodeficiency virus 27 (HIV)-infected patients with P. carinii pneumonia (Pcp), 32 patients without Pcp, and 51 HIV-negative controls. Urea was used for the correct dilution of epithelial lining fluid, and albumin was used to evaluate transudation from plasma for the assessment of local production of antibodies to P. carinii. By contrast with those of immunoglobulin G (IgG), IgA responses to P. carinii were increased in serum from HIV-positive patients compared to negative controls. Local production of antibodies to P. carinii, especially IgA, was decreased in patients with Pcp. In a study of 10 patients of each group, IgG and IgA responses to gp116 from P. carinii were lower in patients with Pcp than in other groups. These results suggest that, in addition to alveolar macrophages, local antibodies may play a role in host defense against P. carinii.

Context: Metastatic lung cancer accounts for 40-50% of diagnosed lung cancers, and has a great lethality, with 4% 5-year net survival. Treatment has been recently modified due to the development of immunotherapy. In order to obtain faster results and to overcome the limitations associated with the use of overall survival (OS), the search for surrogate endpoints (SE) has become an area of research. Using the real-life database ESME-AMLC (NCT03848052), we investigated two candidate surrogate endpoints for OS: progression-free survival (PFS) and time to next treatment (TNT) for patients treated with first-line chemotherapy and/or immunotherapy.

L’immunothérapie anticancéreuse est devenue un standard thérapeutique en oncologie. Les inhibiteurs de points de contrôles immunitaires (ICI) sont généralement bien tolérés, mais peuvent aussi générer des effets indésirables d’origine immunologique. Depuis les premiers essais, le risque de pathologie pulmonaire infiltrative diffuse (PPID) est identifié comme un événement rare mais potentiellement mortel.

La Haute Autorité de santé (HAS), conjointement à l’avis rendu lors de l’inscription du gefitinib (IRESSA®) au remboursement le 04 novembre 2009, a demandé la mise en place d’une étude observationnelle destinée à décrire la population de patients (pts) traités par gefitinib en France et l’impact du traitement sur la morbi-mortalité (Fig. 1).

Gemcitabine (GEM), a novel nucleoside analogue, and cisplatin are among the most active single agents in the treatment of NSCLC, producing reproducible response rates of approximately 20%. The two drugs are attractive candidates for combination: dissimilar modes of action with preclinical evidence ror potential synergistic tumour cell killing, and largely non-overlapping side-effect profiles. This phase I/II study was designed to determine the maximum tolerated dose of cisplatin in the combination up to a predetermined maximum of 100mg/m2, and safety and efficacy. GEM 1000mg/m2was given as a 30minute iv infusion weekly ×3 repeated every 4 weeks. Cisplatin was given immediately after the 3rd infusion of GEM in each cycle. In the phase I portion of the study cisplatin was escalated from 60 to 75 to 100mg/m2in successive cohorts of 3 patients, using an adaptive control algorithm. Further patients were then accrued into the phase II portion of the study and treated at 100mg/m2cisplatin. Patients treated at 100mg/m2cisplatin in the phase I portion of the study were also evaluable for response. Characteristics of all 66 patients recruited were: males 55, age range 39–74.
