6. Biventricular versus His bundle pacing after atrioventricular node ablation in heart failure patients with narrow QRS

- Abstract
AbstractBackground:His bundle pacing (HBP) is a physiological alternative to biventricular (BiV) pacing. We compared short-term results of both pacing approaches in symptomatic atrial fibrillation (AF) patients with moderately reduced left ventricular (LV) ejection fraction (EF ≥35% and <50%) and narrow QRS (≤120 ms) who underwent atrioventricular node ablation (AVNA).Methods:Thirty consecutive AF patients who received BiV pacing or HBP in conjunction with AVNA between May 2015 and January 2020 were retrospectively assessed. Electrocardiographic, echocardiographic, and clinical data at baseline and 6 months after the procedure were assessed.Results:Twenty-four patients (age 68.8 ± 6.5 years, 50% female, EF 39.6 ± 4%, QRS 95 ± 10 ms) met the inclusion criteria, 12 received BiV pacing and 12 HBP. Both groups had similar acute procedure-related success and complication rates. HBP was superior to BiV pacing in terms of post-implant QRS duration, implantation fluoroscopy times, reduction of indexed LV volumes (EDVi 63.8 (49.6–81) mL/m2vs. 79.9 (66–100) mL/m2, p = 0.055; ESVi 32.7 (25.6–42.6) mL/m2vs. 46.4 (42.9–68.1) mL/m2, p = 0.009) and increase in LVEF (46 (41–55) % vs. 38 (35–42) %, p = 0.005). However, the improvement of the NYHA class was similar in both groups.Conclusions:In symptomatic AF patients with moderately reduced EF and narrow QRS undergoing AVNA, HBP could be a conceivable alternative to BiV pacing. Further prospective studies are warranted to address the outcomes between both ‘ablate and pace’ strategies.

9. Cognitive behavioral therapy for managing obesity in patients with chronic kidney disease: Study protocol for a randomized controlled trial

- Abstract
Obesity is one of the main risk factors for progression of chronic kidney disease (CKD). Weight loss interventions have limited efficacy in patients with pre-dialysis CKD. Our objective is to test the efficacy of a cognitive behavioral therapy program for obesity management in patients with CKD. We will conduct a randomized controlled intervention trial to evaluate the effects of cognitive behavioral therapy for obesity on weight loss, change in proteinuria, weight maintenance, quality of life, depression symptoms, and anxiety symptoms in patients with CKD. The duration of the intervention will be 16 weeks. The primary study outcomes will be body mass index (BMI) and proteinuria. CKD patients will be randomized into two groups: an intervention group with cognitive behavioral therapy, sessions with a dietitian and a kinesiologist, and a control group with sessions with a dietitian and a kinesiologist, without cognitive behavioral therapy. Study outcomes will be assessed at baseline, immediately after the 16-week intervention, 3 months after the end of the intervention, and 12 months after the end of the intervention. This study will be the first to evaluate the efficacy of cognitive behavioral therapy for obesity in patients with CKD. We expect that our results will contribute to new ways of non-pharmacological treatment of CKD.

- Abstract
We investigated the effects of hyper- and hypoventilation on gastric (PgCO2) and sublingual (PslCO2) tissuePCO2before, during, and after reversal of hemorrhagic shock. PgCO2was measured with ion-sensitive field-effect transistor sensor and PslCO2with a CO2microelectrode. Under physiological conditions and during hemorrhagic shock, decreases in arterial (PaCO2) and end-tidal (PETCO2)PCO2induced by hyperventilation produced corresponding decreases inPgCO2and PslCO2. Hypoventilation produced corresponding increases inPaCO2,PETCO2,PgCO2, and PslCO2. Accordingly, acute decreases and increases inPaCO2andPETCO2produced statistically similar decreases and increases inPgCO2and PslCO2. No significant changes in the tissuePCO2-PaCO2gradients were observed during hemorrhagic shock in the absence or in the presence of hyper- or hypoventilation. Acute changes inPgCO2and PslCO2should, therefore, be interpreted in relationship with concurrent changes inPaCO2and/orPETCO2.

12. Importance of bedside echocardiography for detection of unsuspected isolated right ventricular infarction as a cause of cardiovascular collapse.

Abstract: Isolated right ventricular infarction is a rare and commonly overlooked condition, because of scarce electrocardiographic changes. We present 2 cases of isolated right ventricular infarction that went unrecognized in the ED. Both patients presented with signs of circulatory collapse, third-degree atrioventricular block, and without prominent ischemic changes in the electrocardiogram. Diagnosis was further obfuscated by the absence of chest pain and atypical clinical presentation with left-hand paresis in one patient and hypothermia in the second. Echocardiography was performed as part of the evaluation of unexplained hypotension. In both cases, it revealed right ventricular dysfunction, which suggested possible acute right ventricular infarction. Diagnosis was confirmed by the finding of subtotal ostial thrombotic occlusions of the right coronary artery in both cases. Echocardiography played a decisive role in the diagnostic procedure, enabling prompt therapeutic intervention and subsequent resolution of shock. [Copyright &y& Elsevier]

13. Simultaneous Catheter and Epicardial Ablations Enable a Comprehensive Atrial Fibrillation Procedure

Objective Transmural and contiguous ablations and a comprehensive lesion pattern are difficult to create from the surface of a beating heart but are critical to the successful treatment of persistent, isolated atrial fibrillation. A codisciplinary simultaneous epicardial (surgical) and endocardial (catheter) procedure (Convergent procedure) addresses these issues.Methods Patients with symptomatic atrial fibrillation who failed medical treatment were evaluated. Using only pericardioscopy, the surgeon performed near-complete epicardial isolation of the pulmonary veins and a “box” lesion on the posterior left atrium using unipolar radiofrequency ablation. Simultaneous endocardial catheter radiofrequency ablation completed pulmonary vein isolation, performed a mitral annular and cavotricuspid isthmus line of block, and debulked the coronary sinus. Twelve-month results for the Convergent procedure were compared with 12-month results for concomitant and pericardioscopic (stand-alone transdiaphragmatic/thoracoscopic) atrial fibrillation procedures using unipolar radiofrequency ablation.Results Sixty-five patients underwent the Convergent procedure (mean age, 62 y; mean body surface area, 2.17 m2; mean atrial fibrillation duration, 4.8 y; mean left atrial size, 5.2 cm). Ninety-two percent were in persistent or long-standing persistent atrial fibrillation. At 12 months, evaluation with 24-hour Holter monitors found 82% of patients in sinus rhythm, while only 47% of pericardioscopic and 77% of concomitant patients treated with unipolar radiofrequency ablation were in sinus rhythm.Conclusions Simultaneous epicardial and endocardial ablation improves outcomes for patients with persistent or longstanding persistent atrial fibrillation. This successful collaboration between cardiac surgeon and electrophysiologist is an important treatment option for patients with large left atriums and chronic atrial fibrillation.

Twelve-month results for the Convergent procedure were compared with 12-month results for concomitant and pericardioscopic (stand-alone transdiaphragmatic/thoracoscopic) atrial fibrillation procedures using unipolar radiofrequency ablation.Results Sixty-five patients underwent the Convergent procedure (mean age, 62 y; mean body surface area, 2.17 m2; mean atrial fibrillation duration, 4.8 y; mean left atrial size, 5.2 cm). Ninety-two percent were in persistent or long-standing persistent atrial fibrillation. At 12 months, evaluation with 24-hour Holter monitors found 82% of patients in sinus rhythm, while only 47% of pericardioscopic and 77% of concomitant patients treated with unipolar radiofrequency ablation were in sinus rhythm.Conclusions Simultaneous epicardial and endocardial ablation improves outcomes for patients with persistent or longstanding persistent atrial fibrillation. This successful collaboration between cardiac surgeon and electrophysiologist is an important treatment option for patients with large left atriums and chronic atrial fibrillation.

15. Feasibility of early hospital discharge directly from coronary care unit after primary angioplasty for uncomplicated acute myocardial infarction

Abstract: Current European and American guidelines recommend early discharge for patients with uncomplicated acute myocardial infarction (AMI). However, this concept has not been widely accepted, and experience with direct discharge from the coronary care unit is limited. We aimed to investigate safety and cost effectiveness of early discharge directly from coronary care unit following successful percutaneous coronary angioplasty (PTCA) in patients with uncomplicated AMI. We included consecutive thirty-one patients with uncomplicated AMI and successful PTCA admitted to coronary care unit of a university hospital. Uncomplicated course was defined as absence of reinfarction, ischemia, VF/VT, repeated PTCA, and heart failure within first 72 hours. Incidence of death, reinfarction, VF/VT, need for revascularisation, and hospitalisation due to heart failure at 1, 6, and 12 months was compared with 56 randomly selected AMI patients with successful PTCA but longer hospitalisation. Average hospital stay was 4 days in early and 6.7 days in control group (p<0.05). Control group had more extensive coronary disease (54% two or more vessels vs. 28% in early discharge, p<0,05). During follow up, none of the early discharged patients died, the only observed event was repeat PTCA due to angina pectoris. In the control group, mortality at 12 months was 3.5% (p<0.05). Cumulative 12 month event free survival was 96% in early discharge group and 87% in control group, but difference was not significant (p=0.15, Cox-Mantel test). Cost reduction of early discharge amounted to 1100 Euro per patient. In conclusions, our study confirmed that for a selected population of patients with AMI, successful PTCA, and uncomplicated clinical course during first 72 hours, discharge as early as three days following the admission is safe.

Abstract: Treatment options for atrial fibrillation (AF) have evolved from simple, fluoroscopy-guided pulmonary vein isolation for those patients with paroxysmal AF to complex, multi-modality procedures targeting not only anatomic structures but also electrophysiologic phenomena including complex fractionated electrograms, sites of dominant frequency and local non-venous drivers in patients with persistent and permanent AF. The stepwise ablation approach is a novel technique whereby structures contributing to initiation and maintenance of AF are sequentially targeted by radiofrequency ablation. Broadly divided into pulmonary veins, left atrial (LA) roof, left atrium (incorporating all anatomic regions of the chamber), mitral isthmus and non-LA structures, each region is targeted in sequence and the impact of ablation upon the global fibrillatory process assessed by measurement of AF cycle length (AFCL) at a site remote from the ablation target. In addition to pulmonary vein electrical disconnection and demonstrable complete conduction block across the roof and mitral isthmus lines (when performed), ablation is performed at those sites displaying continuous electrical and complex fractionated activity, with the endpoint of local organization, as well as at sites displaying electrograms consistent with focal sources driving AF. Ablation is accompanied by a cumulative increase in the AFCL prior to termination of AF by conversion either directly to sinus rhythm or to an atrial tachycardia which is then mapped conventionally and ablated. There is a ceiling of ablation within the LA beyond which further ablation is unlikely to result in a clinical benefit and should prompt evaluation of the contribution of the right atrium to maintenance of AF. The stepwise approach benefits from the integration of anatomic and electrophysiologic information to achieve a high level of success in termination of chronic AF by catheter ablation.

17. From Planning to Improvement: Monash University Library's Quality Review

ABSTRACTDuring 2003, Monash University Library conducted a comprehensive quality review as part of the university's program of reviews. A Quality Management Group was established in September 2002 to drive the process. All staff were given the opportunity to contribute input to the initial self-review. Methods of collecting data, collating responses and an insight into the self-review report are provided as well as an overview of the subsequent external panel visit and report. Issues that arose, solutions to problems, consideration of the self-review process and benefits of the quality review are included. The library's approach to quality review has been recognised as best practice within the university, thus enhancing the library's profile.

A number of recently introduced initiatives demonstrate the library's commitment to meeting the personal study needs of students whether they are enrolled on or off campus. Some directions for further development of such services are identified as well as issues which may influence the planning of new library information services in an academic environment which provides students with a choice of study modes to suit individual needs.

- Abstract
Disopyramide as an antiarrhythmic can be prescribed to patients with atrial fibrillation and, owing to its negative inotropic effect, to patients with hypertrophic obstructive cardiomyopathy. It is known that in patients with cardiac conduction disturbances and heart failure, disopyramide can adversely affect heart rhythm and conduction and induce cardiovascular collapse. A patient with hypertrophic obstructive cardiomyopathy and paroxysms of atrial fibrillation is described who was treated with disopyramide and also, during the 5 days before admission, with metoprolol. In spite of normal cardiac conduction and function before disopyramide, this treatment was followed by hypotension, bradycardia, and cardiac conduction disturbances. Our case shows the potential for disopyramide, especially when combined with metoprolol, to induce grave adverse effects even in patients with normal cardiac conduction and ventricular function.

- Abstract
Background: Radial access for PCI is increasing but its use in emergency still remains confidential. We report our single center experience where all interventional cardiologists use the radial access as default strategy for primary PCI. Methods and Results: STEMI patients (n = 671) were evaluated for bleeding complications using a web-based registry (e-PARIS). In-hospital bleeding was adjudicated using various definitions (TIMI, GUSTO, STEEPLE). MACE was the composite of death, MI and stroke. In this non-selected, high risk population, 6.1% had cardiogenic shock on admission, 3.9% out-of-hospital cardiac arrest and 51.2% multivessel disease. Radial access (88%) was the default strategy as was abciximab (78%). Clopidogrel loading dose ranged from 300 to 900 mg. Pre-hospital fibrinolysis was used in 7.1%. Hemodynamic support devices (IABP, ECMO, Tandem Heart) were needed in 7.0%. In-hospital bleeding rates varied widely according to the definitions used: 2.5%, 1.5%, 5.7%, 9.2% and 10.9% with TIMI Major, GUSTO Severe, TIMI Major/minor, GUSTO Severe/moderate or STEEPLE Major, respectively. In-hospital death rate was 5.5%. One-year mortality (8.2%) was seriously impacted by in-hospital bleeding (31.6% vs 6.8%, p < 0.001). The most frequent bleeding site was gastro-intestinal (figure). GUSTO Severe/moderate bleeding was independently correlated with MACE (OR 2.60; 95%CI 1.21–5.59; p = 0.01) and radial access was a strong predictor of survival (OR 0.33; 95%CI 0.17–0.56; p = 0.002). Conclusions: The gastro-intestinal tract is the most frequent site of bleeding when the radial artery is the predominant vascular access site for primary PCI. GUSTO Severe/moderate bleeding is an independent predictor of MACE while radial access predicts survival.
