In Hong Kong, Indonesian migrant domestic workers (IMDWs) are forced to leave behind young children in their home country. Often spending the majority of their time in their employer's households, they must manage their limited personal time to maintain connections with their families back home. Many women IMDWs try to overcome distance limitations by performing transnational mothering using any available media to reach their children and to manage and witness their development. By using communication tools such as Internet-mediated calls for distance nurturing, they are able to keep up with their children's lives and monitor day-to-day developments. This research examines the role of communication technologies with an emphasis on the use of social media by women IMDWs in maintaining connections with their families back home. It examines the role social media plays in caregiving for women IMDWs, for whom the family and children's wellbeing back home are one of their most important concerns. This research incorporates netnography, which aims to understand online social interactions. The research was conducted from 2013 to 2019 physically in Hong Kong and through virtual channels. The findings indicate how social media is used as an emotional outlet by women IMDWs in Hong Kong, and how emotional care can be maintained to empower the migrant mothers. Social media and information and communication technology (ICT) have greatly improved channels of communication for IMDWs and created a new space of intimacy. An introduction is presented in which the editor discusses articles in the issue on topics including labor exploitation of domestic workers, women's roles during the COVID-19 pandemic, and gender-based cyber violence.

• Total Body Irradiation in-vivo dosimetry data over 10 years were analyzed. • Monitor Units' median percentage difference from planned was limited. • Median percentage dose difference at six anatomical locations was inside 10 % range. • The neck region showed the highest dosimetric differences from planned. • In-vivo dosimetry omission impact was evaluated, leading to interesting results. Myeloablative Total Body Irradiation (TBI) used in our Institution, as part of the conditioning scheme for haematopoietic stem cell transplantation, is an extended-distance supine technique that has been implemented using a 15 MV LINAC beam, lead lung compensators, PMMA, and water bolus to improve homogeneity. This study reviews in-vivo dosimetry (IVD) over 10 years of treatments, assessing the technique's robustness, accuracy, and efficiency. A 2-lateral opposite fields plan was calculated from planning CT with validated Oncentra TPS (Elekta AB, Sweden). Monitor units (MUs), lung compensators shape and thickness were calculated to deliver the prescription dose (12 Gy in 6 bi-daily fractions or 9.9 Gy in 3 daily fractions) to the patient's abdomen midline at the umbilical level, maintaining lung dose within ±5 % range of prescription. Data from 103 patients, of which more than 87 % were pediatric, were retrieved and analyzed for a total of 537 treatment fractions. The impact of IVD omission was evaluated, supposing doing it only once or in the first two fractions, if necessary. Median ΔMU from planned was −1.2 %. Median percentage dose deviation from prescription in 6 anatomical regions was below 2 %. IVD omission could have resulted in an increase of 7 patients registering at least one anatomical region outside the ±5 % dose range at the end of treatment. It is possible to confirm the implemented technique's robustness and accuracy in delivering the prescribed dose under IVD monitoring. • Pelvis auto-segmentation tools were compared, from atlas-techniques to deep learning. • Deep learning showed better and faster segmentation results. • Differences found in cervical cancer tapered off in prostate cancer contouring. • Expert radiation oncologists qualitatively preferred deep learning. • Automated tools allow for faster radiotherapy workflow and high-quality treatments. Artificial intelligence can standardize and automatize highly demanding procedures, such as manual segmentation, especially in an anatomical site as common as the pelvis. This study investigated four automated segmentation tools on computed tomography (CT) images in female and male pelvic radiotherapy (RT) starting from simpler and well-known atlas-based methods to the most recent neural networks-based algorithms. The evaluation included quantitative, qualitative and time efficiency assessments. A mono-institutional consecutive series of 40 cervical cancer and 40 prostate cancer structure sets were retrospectively selected. After a preparatory phase, the remaining 20 testing sets per each site were auto-segmented by the atlas-based model STAPLE, a Random Forest-based model, and two Deep Learning-based tools (DL), MVision and LimbusAI. Setting manual segmentation as the Ground Truth, 200 structure sets were compared in terms of Dice Similarity Coefficient (DSC), Hausdorff Distance (HD), and Distance-to-Agreement Portion (DAP). Automated segmentation and manual correction durations were recorded. Expert clinicians performed a qualitative evaluation. In cervical cancer CTs, DL outperformed the other tools with higher quantitative metrics, qualitative scores, and shorter correction times. On the other hand, in prostate cancer CTs, the performance across all the analyzed tools was comparable in terms of both quantitative and qualitative metrics. Such discrepancy in performance outcome could be explained by the wide range of anatomical variability in cervical cancer with respect to the strict bladder and rectum filling preparation in prostate Stereotactic Body Radiation Therapy (SBRT). Non palpable breast tumors are increasingly diagnosed because of screening programs, and their localization during surgery is essential to ensure an adequate resection. Little is known regarding which techniques are used in "real world". A national web-based survey, with a 21-items questionnaire, was conducted among Breast Centers (BCs) in Italy in 2023. Among 153 BCs, 110 (72 %) participated. Wire-guided localization was reported to be the most used technique, regardless of the volume of malignant tumors treated by each Center (N = 36, 33 %). However, newer techniques such as Radioactive occult lesion localization and magnetic seeds, were reported to be employed in 34 (31 %) BCs, and more often among high-volume BCs (>300 cases/year) (N = 32, 29 % vs. N = 13, 12 %; p = 0.02). Logistic problems of localization were reported to cause delays to the scheduled surgery at least once or in multiple occasions in 26 (24 %) and in 4 (3.5 %) BCs, respectively. Although the majority of BCs declared they were satisfied (N = 48, 44 %) or somewhat satisfied (N = 41, 37 %) with the procedure used in their center, responders stated that they would change their technique, or that they were strongly considering this possibility in 24 (22 %) and in 38 (35 %) cases, respectively. The main barrier to introducing a new technique was associated with procedure costs (90 BCs, 82 %). There are several critical issues regarding localization techniques at a national level. To demonstrate safety of a developed intranasal dexamethasone-infused in situ gelling formulation, quantification of a validated clinical biomarker indicative of cytotoxic potential using a human sinonasal explant model was first confirmed. Systematic cytotoxicity studies using the lactate dehydrogenase (LDH) detection assay revealed no elevation from baseline, in LDH levels, with tissue integrity of explanted human nasal mucosa also maintained; this was further corroborated using tissue histopathological examination. Next, with safety confirmed ex vivo , freshly excised human nasal tissue was utilised to quantify dexamethasone release from the lead sol–gel systems; this being achieved through development and validation of a HPLC-UV analytical method, which reliably quantified controlled therapeutic release and deposition into mucosal tissue. Collectively, these findings indicate promise in the safety of each excipient within the concentrations employed in the functional sol–gel system, complemented by successful and reliable drug release and deposition into human nasal mucosal tissue. These findings pave the way for application of the dexamethasone-based sol–gel system to the extended delivery of corticosteroids to nasal mucosa in the management of localised inflammatory conditions of an acute and chronic nature, such as chronic rhinosinusitis, which can be expected to benefit from controlled and extended drug delivery characteristics imparted by appropriately engineered in situ gelling systems. Terbuthylazine (TBA) has replaced atrazine in many EU countries, becoming one of the most frequently detected pesticides in natural waters. TBA is a compound of emerging concern, due to its persistence, toxicity and proven endocrine disruption activity to wildlife and humans. Techniques applied in water treatment plants remove only partially this herbicide and poor attention is given to the generation and fate of by-products, although some of them have demonstrated an estrogenic activity comparable to atrazine. This paper summarizes the environmental occurrence of TBA and its main metabolite desethylterbuthylazine and reports the performance of an innovative electrochemical cell equipped with a solid polymer electrolyte (SPE) sandwiched between a Ti/RuO 2 cathode and a Boron-Doped Diamond anode, operating at constant current, in the treatment of an aqueous solution of TBA. The herbicide removal in the first 30 min of treatment increases from 42% to 92% as the applied current is increased from 100 to 500 mA. The rate of degradation at 500 mA decreases between 30 and 60 min, with a final abatement of 97%. An 89% removal was reached at 100 mA when the initial TBA concentration was raised from 0.1 to 4 mg L−1 and less than 1% of the herbicide was converted in desethylterbuthylazine and minor metabolites. No chemicals are needed, no sludge is produced. Further research is encouraged, as this technology may be promising for the achievement of a zero-discharge removal of different emerging pollutants as pesticides, pharmaceuticals and personal care products. The management of axilla after Primary Systemic Therapy (PST) for breast cancer is a highly debated field. Despite the proven axillary downstaging occurring after PST, there is still some degree of reluctance in applying sentinel node biopsy (SNB) in the neoadjuvant setting. We performed a retrospective analysis on 181 PST patients with axillary positive nodes at presentation treated between 2005 and 2017 at San Raffaele Hospital in Milan. The aim was to observe the application time trend of SNB, to determine the imaging staging power and the axillary downstaging according to molecular subtypes. Median follow-up after surgery was 32.5(IQR: 12–59) months. After PST, 119 (65.7%) patients had no clinically palpable nodes, 72 (39.7%) converted to N0 on final imaging and 34 (18.8%) underwent SNB with an increasing application trend. Axillary-US showed the highest accuracy (69.3%) in re-staging axilla after PST. Staging power of preoperative testing varied with tumour biology: Positive Predictive Value was higher in Luminal A (80% for clinical examination and 100% for axillary-US) and Luminal B (72% and 70.5%) tumours, whilst Negative Predictive Value was higher in HER2 positive (100% and 93.3%), and triple negative (71.4% and 93.3%) tumours. Ninety five (52.5%) patients experienced axillary downstaging after PST, by molecular subtype 15% (3/20) in Luminal A, 46.4% (45/97) in Luminal B, 90.9% (20/22) in HER2+ and 70.3% (26/37) in triple negative breast tumours. SNB application after PST for breast cancer in node positive patients at presentation is increasing. Pre-operative axillary imaging and tumour biology help identify patients who might be candidates for SNB as a single staging procedure. [ABSTRACT FROM AUTHOR]

Background Intensity-modulated radiotherapy (IMRT) improves dose distribution in head and neck (HN) radiation therapy. Volumetric-modulated arc therapy (VMAT), a new form of IMRT, delivers radiation in single or multiple arcs, varying dose rates (VDR-VMAT) and gantry speeds, has gained considerable attention. Constant dose rate VMAT (CDR-VMAT) associated with a fixed gantry speed does not require a dedicated linear accelerator like VDR-VMAT. The present study explored the feasibility, efficiency and delivery accuracy of CDR-VMAT, by comparing it with IMRT and VDR-VMAT in treatment planning for HN cancer. Methods and materials Step and shoot IMRT (SS-IMRT), CDR-VMAT and VDR-VMAT plans were created for 15 HN cancer patients and were generated by Pinnacle 3 TPS (v 9.8) using 6 MV photon energy. Three PTVs were defined to receive respectively prescribed doses of 66 Gy, 60 Gy and 54 Gy, in 30 fractions. Organs at risk (OARs) included the mandible, spinal cord, brain stem, parotids, salivary glands, esophagus, larynx and thyroid. SS-IMRT plans were based on 7 co-planar beams at fixed gantry angles. CDR-VMAT and VDR-VMAT plans, generated by the SmartArc module, used a 2-arc technique: one clockwise from 182° to 178° and the other one anti-clockwise from 178° to 182°. Comparison parameters included dose distribution to PTVs ( D mean , D 2% , D 50% , D 95% , D 98% and Homogeneity Index), maximum or mean doses to OARs, specific dose-volume data, the monitor units and treatment delivery times. Results Compared with SS-IMRT, CDR-VMAT significantly reduced the maximum doses to PTV1 and PTV2 and significantly improved all PTV3 parameters, except D 98% and D 95% . It significantly spared parotid and submandibular glands and was associated with a lower D mean to the larynx. Compared with VDR-VMAT, CDR-VMAT was linked to a significantly better D mean , to the PTV3 but results were worse for the parotids, left submandibular gland, esophagus and mandible. Furthermore, the D mean to the larynx was also worse. Compared with SS-IMRT and VDR-VMAT, CDR-VMAT was associated with higher average monitor unit values and significantly shorter average delivery times. Conclusions CDR-VMAT appeared to be a valid option in Radiation Therapy Centers that lack a dedicated linear accelerator for volumetric arc therapy with variable dose-rates and gantry velocities, and are unwilling or unable to sanction major expenditure at present but want to adopt volumetric techniques. Waste heat revalorization creates interesting opportunities to energy intensive industries. In the present project, a large-scale ORC pilot plant along with a waste heat recovery unit (WHRU) in a steel mill has been designed, commissioned and operated. The plant is part of the European Commission funded PITAGORAS project and it has been installed at ORI MARTIN in Brescia (Italy). Waste heat is recovered from the fumes of the Electric Arc Furnace (EAF) to produce saturated steam which is then delivered to a district heating (DH) network during heating season and to the ORC for electricity generation during the rest of the year. The main challenge was the integration of these systems in a single plant since the heat source is highly unstable and steady heat load is preferable for the DH and ORC for their safe operation. A steam accumulator of 150m 3 volume was implemented between the WHRU and the ORC/DH systems to maintain a steady discharge pressure, to reduce the fast transients and to extend the supply over longer periods. The ORC has a nominal power output of 1,8MW and the preliminary results of the first weeks of operation of the ORC unit resulted in a net efficiency of 21.7%. Currently the plant is undergoing monitoring campaign which will provide additional data to further evaluate and optimize the system. [ABSTRACT FROM AUTHOR]
