This study was designed to identify the most frequent shoulder patient-reported outcome measures (PROMs) reported in high-quality literature.A systematic review was performed to identify shoulder PROMs, and their diffusion within the scientific literature was tested with a subsequent dedicated search in MEDLINE.506 studies were included in the final data analysis, for a total number of 36,553 patients.The Disabilities of the Arm, Shoulder and Hand questionnaire (DASH), the American Shoulder, Elbow Surgeons Score (ASES) and the Shoulder Pain and Disability Index (SPADI) were the most frequently reported PROMs in the analysed publications, with disease-specific PROMs being used with increasing frequency.A core set of outcome measures for future studies on patients with shoulder pathologies, based on the international acceptance and diffusion of each PROM, is needed.A combination of the DASH score for shoulder outcome assessment with more specific PROMs, such as the ASES for rotator cuff pathology and osteoarthritis and the SPADI for shoulder stiffness and shoulder pain of unspecified origin, is proposed as a recommended set of PROMs.Cite this article: EFORT Open Rev2021;6:779-787. The adoption of Industry 4.0 (I4.0) being a global technology advancement have challenged the adoption decision of stakeholders due to inadequate management strategies provided in the current literature. As a result, there exists a disparity of perception among organisations of whether or not the adoption of I4.0 should be pursued. As a part of this management strategy, this paper attempts to review drivers and barriers and characterise each as being intrinsic or extrinsic. Furthermore, these drivers and barriers are categorised according to technological, economic and regulatory, and social factors. As such, the nature of management strategy can be well-established to aid stakeholders in developing appropriate I4.0 programs and initiatives.

A retrospective study was performed, using the Short Form-36 Health Survey and the Roland and Morris Disability Questionnaire, to investigate patient outcomes after fusion for adolescent idiopathic scoliosis using Harrington rod instrumentation.

Abstract: Psoriasis is a chronic, recurrent papulosquamous dermatosis that involves the skin and sometimes the joints. Fow orthopedic surgical complications have been reported in psoriatic patients. We report two cases of surgery failure for lumbar disc herniation in patients affected by psoriasis. Both cases underwent surgical treatment because neurological deficits appeared after a period of conservative treatment. The symptoms were completely resolved after surgery. Both patients complained of recurrent pain (no neurological impairment) a few (15 and 20) days after surgery. The imaging evaluation pointed out an over-production of fibrous tissue compressing the nerve root. In our opinion, this condition may be related to the dermatosis. Four hypotheses are proposed to explain the exuberant spinal fibrosis, developed a few days after surgery. This report may represent the starting point for a deeper study of spinal surgery in psoriatic patients.

Objective Regenerative medicine is experiencing a strong expansion worldwide, including the treatment of some common orthopedic pathologies, with an increase in physicians adopting these technologies. This growing interest has been associated with an equally significant spread of websites dedicated to public information on this topic, often lacking scientific bases. This study aims to evaluate the quality of information on the World Wide Web about stem cells for cartilage disorders in orthopedic practice.Design On February 19, 2021 we performed a search on Google using as keywords “cartilage stem cells,” considering only the freely accessible sites in Italian, English, French, and Spanish. We selected the first 50 valid results for each language and evaluated them using the DISCERN scale.Results A total of 249 sites were observed, of which 49 were discarded as duplicates (8) or not relevant to the topic (41). Of the 200 sites surveyed, 47 were affiliated to a singular surgeon (23.5%), 31 to private stakeholders (15.5%), 73 to information sites (36.5%), 8 to public hospitals (4%), 13 to universities (6.5%), and 28 referred to international scientific journals (14%). Only 76 sites (38%) were rated as fair or better, while the remaining 124 (62%) were classified as poor or very poor.Conclusions The quality of the information promoted for stem cells in orthopedics is generally low, although a significant minority of the sites offers good quality information. A greater commitment on the part of surgeons and other stakeholders is desirable to promote information on regenerative medicine through scientific criteria supported by adequate literature.

Abstract: Surgical decompression of the median nerve is a standardized treatment for carpal tunnel syndrome. After surgery a clinical and neurophysiological improvement is observed also on severe cases. We report 4 cases of carpal tunnel syndrome (CTS) in the hands of 3 patients with prolonged median wrist-thenar distal motor latency (longer than 10 ms, normal values < 4.0 ms) and absence of digit-wrist sensory responses. In these patients surgery caused complete loss of median nerve function. Such a marked increase of median distal motor latency (DML) is rare (0.6% in 500 CTS hands consecutively examined in our laboratory) and suggests the presence of a chronic condition with severe disruption of the myelin. On the basis of these anecdotal observations, we suggest that patients with median DML of 10 ms or more be considered at high risk for CTS surgery failure.

We report 6 cases of a suprascapular nerve lesion: 3 of them presented a nerve entrapment at the suprascapular notch and 3 at the spinoglenoid notch. Electrophysiological investigation localized the site of entrapment in all cases.
