SPARE parts ,PRINCIPAL components analysis ,MATERIALS management ,INDUSTRIAL equipment ,FOOD processing plants - Abstract
This research aims to analyze the importance of inventory control and management, especially regarding the components used in industrial machinery maintenance. The methodology used was based on a quantitative approach, descriptive scope, and non-experimental cross-sectional design. Principal Component Analysis (PCA) and Clustering techniques were employed to evaluate both the quantities consumed and the frequency of usage, to efficiently categorize the components according to their distinctive attributes. This approach is proposed as an objective guidance that will significantly contribute to improving effectiveness and sustainability in the overall management of spare parts inventories. The developed methodology allowed for a detailed description of spare parts behavior; 1,753 references of spare parts used in the maintenance of industrial equipment in a food factory during the period from the second half of 2021 to the first half of 2022. The highest production peaks were recorded during this time, highlighting consumed quantities and usage frequencies, guiding the material management teams in making decisions about spare parts storage. - Abstract
This study will explore the effectiveness of fish skin grafts (FSG) in ulcer healing in diabetic foot disease compared to standard of care (SOC). The systematic review and meta-analysis were performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) standard. The electronic databases of PubMed, EMBASE, and Web of Science (WoS) internet were searched for the outcome rate of complete ulcer healing. The risk of bias assessment was conducted using the tool recommended by the Cochrane Collaboration. Statistical analysis included the individual and combined result of the studies, heterogeneity test, the effect size, sensitivity analysis, and publication bias tests. Five randomised controlled trials (RCTs) with a total of 411 patients were included in this study. This meta-analysis showed a higher rate of complete ulcer healing in groups receiving fish skin grafts (OR = 3.34, 95% CI 2.14–5.20, p < 0.01, I
2 = 0%) compared to control groups. Fish skin grafts have been shown to be more effective for achieving complete ulcer healing compared to current conventional treatments in diabetic foot disease. • Foot complications are one of the leading causes of hospitalisation in patients with diabetes mellitus, demanding prolonged stays. • 15% of the diabetic population will develop diabetic foot disease during their disease, with 25% of them leading to lower limb amputation. • The current standard hospital treatments for patients with diabetic foot disease, do not guarantee success in their closure. • Fish skin grafts is a highly efficient treatment considering its cost and effectiveness in the ability to stimulate cell proliferation and differentiation. - Abstract
Bone morphogenetic proteins (BMPs) are important targets to incorporate in biomaterial scaffolds to orchestrate tissue repair. Glycosaminoglycans (GAGs) such as heparin allow the capture of BMPs and their retention at the surface of biomaterials at safe concentrations. Although heparin has strong affinities for BMP2 and BMP4, two important types of growth factors regulating bone and tissue repair, it remains difficult to embed stably at the surface of a broad range of biomaterials and degrades rapidly in vitro and in vivo. In this report, biomimetic poly(sulfopropyl methacrylate) (PSPMA) brushes are proposed as sulfated GAG mimetic interfaces for the stable capture of BMPs. The growth of PSPMA brushes via a surface-initiated activator regenerated by electron transfer polymerization is investigated via ellipsometry, prior to characterization of swelling and surface chemistry via X-ray photoelectron spectroscopy and Fourier transform infrared. The capacity of PSPMA brushes to bind BMP2 and BMP4 is then characterized via surface plasmon resonance. BMP2 is found to anchor particularly stably and at high density at the surface of PSPMA brushes, and a strong impact of the brush architecture on binding capacity is observed. These results are further confirmed using a quartz crystal microbalance with dissipation monitoring, providing some insights into the mode of adsorption of BMPs at the surface of PSPMA brushes. Primary adsorption of BMP2, with relatively little infiltration, is observed on thick dense brushes, implying that this growth factor should be accessible for further binding of corresponding cell membrane receptors. Finally, to demonstrate the impact of PSPMA brushes for BMP2 capture, dermal fibroblasts were then cultured at the surface of functionalized PSPMA brushes. The presence of BMP2 and the architecture of the brush are found to have a significant impact on matrix deposition at the corresponding interfaces. Therefore, PSPMA brushes emerge as attractive coatings for scaffold engineering and stable capture of BMP2 for regenerative medicine applications.

- Abstract
BackgroundChronic thromboembolic pulmonary hypertension (CTEPH) is the most severe long-term complication of acute pulmonary embolism (PE). We aimed to evaluate the impact of a symptom screening programme to detect CTEPH in PE survivors.MethodsThis was a multicentre cohort study of patients diagnosed with acute symptomatic PE between January 2017 and December 2018 in 16 centres in Spain. Patients were contacted by phone 2 years after the index PE diagnosis. Those with dyspnoea corresponding to a New York Heart Association (NYHA)/WHO scale≥II, visited the outpatient clinic for echocardiography and further diagnostic tests including right heart catheterisation (RHC). The primary outcome was the new diagnosis of CTEPH confirmed by RHC.ResultsOut of 1077 patients with acute PE, 646 were included in the symptom screening. At 2 years, 21.8% (n=141) reported dyspnoea NYHA/WHO scale≥II. Before symptom screening protocol, five patients were diagnosed with CTEPH following routine care. In patients with NYHA/WHO scale≥II, after symptom screening protocol, the echocardiographic probability of pulmonary hypertension (PH) was low, intermediate and high in 76.6% (n=95), 21.8% (n=27) and 1.6% (n=2), respectively. After performing additional diagnostic test in the latter 2 groups, 12 additional CTEPH cases were confirmed.ConclusionsThe implementation of this simple strategy based on symptom evaluation by phone diagnosed more than doubled the number of CTEPH cases. Dedicated follow-up algorithms for PE survivors help diagnosing CTEPH earlier.Trial registration numberNCT03953560.

- Abstract
ABSTRACTTo reduce demand and discharge, instead of industrial wastewater being not enough treated and disposed of, it can be recycled, reused, or recovered considering well management. Thus, having a substantial decrease in the water need and environmental impacts. The challenge is to select the proper combination of processes based on the wastewater quality and budget limitations. Therefore, we aimed to provide a distinctive and comprehensive perspective on the current state of industrial wastewater treatment technologies by giving a complete overview of their major advantages, disadvantages, and upcoming challenges in the context of the circular economy. This study considered 198 peer-reviewed scientific publications from the Scopus and Google Scholar databases, mostly from the last 5 years. For this, specific search terms and Boolean Operators were applied as inclusion and exclusion criteria. Up to 57 different technologies, from conventional to advanced, were reviewed for reliable and sustainable industrial wastewater treatment and derived sludges. We evidence a vast potential to exploit and make them viable for the circular economy, even in the mature ones. Overall, we identify that regardless of the alternative, in particular those not vastly implemented, future investigations should prioritize: (1) The capacity to remove selected pollutants and decrease impurities; (2) energy recovery and efficiency; (3) environmental safety; (4) economic viability and industrial implementation; (5) hybridization; and (6) sustainability through waste processing.

- Abstract
Post-operative cures with hyaluronic acid (HA) could potentially shorten the period recovery involved in the phenol technique for ingrown toenail. The aim of this study was therefore to compare a standard healing protocol with the experimental one based on hyaluronic acid cream. 70 patients who had undergone phenol technique surgery for ingrown toenail were divided into two groups – control (n = 35) who received post-operative cures following the standard protocol with povidone iodine gel, and experimental (n = 35) who received cures with HA in the first 3 visits. Bleeding, total healing time, and perceived pain were assessed. Patients in the control group recovered from the intervention in a total of 26.17 ± 7.75 days, while those in the HA group recovered in a significantly shorter time – 22.42 ± 2.41 days (p = 0.007, effect size 0.653). However, there were no between-group statistical differences in bleeding or perceived pain over the course of the post-surgery visits. The use of low molecular weight hyaluronic acid is effective in reducing the phenol-technique healing time by 4 days compared with the standard cure. However, no extra effects such as reductions in bleeding or perceived pain can be expected in choosing this healing protocol. • Drugs with HA promote tissue regeneration and wound healing. • Post-operative cures with HA may reduce the recovery period in phenol technique. • Patients cured with HA recovered from the intervention 4 days before than controls. • However, no effects in bleeding or perceived pain can be expected using HA creams. - Abstract
This research designed and built an automatic and portable cleaning mechanism for photovoltaic panels (PVs). The climate variation defined the amount of accumulated dust; this modified the load efficiency (η) by 11.05% on average, reaching a maximum of 39.6% in the hour of greatest solar spectrum. The highest value obtained of fill factor (FF) was 0.29 with a clean PV compared to 0.25 of a dirty PV, in all measurements the dust decreases the FF. In this study the power (W) loss should not be greater than 2.17 W, which can be because of dirty panels with dust; and the FF greater than 71%. The amount of accumulated dust decreases the FF, for this reason the cleaning mechanism must be programmable, must be portable and adjustable to different PV sizes.

- Abstract
Previous studies have shown that perinatal distress has a negative influence on pregnancy outcome and the physiological development of the baby. The aim of this study was to describe the effects of the COVID-19 pandemic on maternal perinatal mental health in Spain. Seven hundred and twenty-four women (N = 450 pregnancy, N = 274 postpartum) were recruited online during the pandemic. The Edinburgh Postnatal Depression Scale, the Positive and Negative Affect Schedule, and the Satisfaction With Life Scale were administered. Variables related to sociodemographic information, the COVID-19 pandemic, and perinatal care were also assessed. The results showed that 58% of women reported depressive symptoms. Moreover, 51% of women reported anxiety symptoms. On the other hand, a regression analysis for life satisfaction showed that besides the perception about their own health, marital status or being a health practitioner were also significant predictors during pregnancy. However, perception about baby's health and sleep, perception about their own health, and marital status were significant predictors of life satisfaction during the postpartum stage. Women assessed during the COVID-19 pandemic reported high rates of psychological distress. These results highlight the need of clinical support during this period. Knowing the routes to both distress and well-being may help maternity services to effectively cope with the pandemic. [ABSTRACT FROM AUTHOR]

- Abstract
Despite the acceptance of the laparoscopic approach for the treatment of perforated peptic ulcers, its definitive implantation is still a matter of discussion. We performed a comparative study between the open and laparoscopic approach focused on postoperative surgical complications. Retrospective observational study in which patients operated on for perforated peptic ulcus in our center between 2001 and 2017 were analyzed. Only those in whom suture and/or omentoplasty had been performed were selected, either for open or laparoscopic approach. Demographic, clinical, and intraoperative variables, complications, mortality and length of stay were collected. Both groups, open and laparoscopic surgery patients, were compared. The final study sample was 250 patients, 190 (76%) men and 60 (24%) women, mean age 54 years (SD ± 16.7). In 129 cases (52%), the surgical approach was open, and in 121 (48%) it was laparoscopic. Grades III-V complications of the Clavien-Dindo Classification occurred in 23 cases (9%). Operative mortality was 1.2% (3 patients). Laparoscopically operated patients had significantly fewer complications (p = 0.001) and shorter hospital stay (p < 0.001). In multivariate analysis, laparoscopic approach (p = 0.025; OR:0.45–95%CI: 0.22–0.91), age (p = 0.003; OR:1.03–95%CI: 1.01–1.06), and Boey score (p = 0.024 – OR:1.71 – CI95%: 1.07–2.72), were independent prognostic factors for postoperative surgical complications. Laparoscopic surgery should be considered the first-choice approach for patients with perforated peptic ulcer. It is significantly associated with fewer postoperative complications and a shorter hospital stay than the open approach. [ABSTRACT FROM AUTHOR]

- Abstract

- Abstract
Background: Select patients with atrial fibrillation and contraindication to anticoagulation may benefit from percutaneous left atrial appendage closure (pLAAC).Objective: The purpose of this study was to evaluate racial disparities in the nationwide utilization and outcomes of pLAAC.Methods: We identified 16,830 hospitalizations for pLAAC between 2015 and 2017 using the National Inpatient Sample. Baseline characteristics, in-hospital mortality, complications, length of stay, and discharge disposition were assessed between White and Black/African American (AA) populations. Results: Black/AA patients represented 4.1% of nationwide pLAAC recipients and were younger, more likely to be female, and had greater prevalence of hypertension, heart failure, hyperlipidemia, obesity, chronic kidney disease, and prior stroke history (P <.001 for all). Black/AA patients had significantly increased length of stay and nonroutine discharge (P <.001 for both) but comparable in-hospital mortality to White patients. Black/AA patients suffered from greater postoperative stroke (0.7% vs 0.2%), acute kidney injury (4.5% vs 2.1%), bleeding requiring transfusion (4.5% vs 1.4%), and venous thromboembolism (0.7% vs 0.1%; P <.01 for all). After controlling for possible confounding factors, Black/AA race was independently associated with significantly increased odds of bleeding requiring blood transfusion, stroke, venous thromboembolism, and nonroutine discharge. Conclusion: Among pLAAC recipients nationwide, Black/AA populations were underrepresented and had greater complication rates, length of stay, and discharge complexity. This study highlights the importance of addressing ongoing racial disparities in both utilization and outcomes of pLAAC. [ABSTRACT FROM AUTHOR]- Published

- Abstract
Little is known about the prognosis of patients with massive pulmonary embolism (PE) and its risk of recurrent venous thromboembolism (VTE) compared with non‐massive PE, which may inform clinical decisions. Our aim was to compare the risk of recurrent VTE, bleeding, and mortality after massive and non‐massive PE during anticoagulation and after its discontinuation. We included all participants in the RIETE registry who suffered a symptomatic, objectively confirmed segmental or more central PE. Massive PE was defined by a systolic hypotension at clinical presentation (<90 mm Hg). We compared the risks of recurrent VTE, major bleeding, and mortality using time‐to‐event multivariable competing risk modeling. There were 3.5% of massive PE among 38 996 patients with PE. During the anticoagulation period, massive PE was associated with a greater risk of major bleeding (subhazard ratio [sHR] 1.72, 95% confidence interval [CI] 1.28–2.32), but not of recurrent VTE (sHR 1.15, 95% CI 0.75–1.74) than non‐massive PE. An increased risk of mortality was only observed in the first month after PE. After discontinuation of anticoagulation, among 11 579 patients, massive PE and non‐massive PE had similar risks of mortality, bleeding, and recurrent VTE (sHR 0.85, 95% CI 0.51–1.40), but with different case fatality of recurrent PE (11.1% versus 2.4%, P= .03) and possibly different risk of recurrent fatal PE (sHR 3.65, 95% CI 0.82–16.24). In this large prospective registry, the baseline hemodynamic status of the incident PE did not influence the risk of recurrent VTE, during and after the anticoagulation periods, but was possibly associated with recurrent PE of greater severity.

- Abstract
In elderly (75 years or older) patients living in Latin America with severe symptomatic aortic stenosis candidates for transfemoral approach, the panel suggests the use of transcatheter aortic valve implant (TAVI) over surgical aortic valve replacement (SAVR). This is a conditional recommendation, based on moderate certainty in the evidence (⨁⨁⨁Ο).This recommendation does not apply to patients in which there is a standard of care, like TAVI for patients at very high risk for cardiac surgery or inoperable patients, or SAVR for non-elderly patients (eg, under 65 years old) at low risk for cardiac surgery. The suggested age threshold of 75 years old is based on judgement of limited available literature and should be used as a guide rather than a determinant threshold. The conditional nature of this recommendation means that the majority of patients in this situation would want a transfemoral TAVI over SAVR, but some may prefer SAVR. For clinicians, this means that they must be familiar with the evidence supporting this recommendation and help each patient to arrive at a management decision integrating a multidisciplinary team discussion (Heart Team), patient’s values and preferences through shared decision-making, and available resources. Policymakers will require substantial debate and the involvement of various stakeholders to implement this recommendation.

Addressing access to oral health care in many low- to middle-income countries is a complicated issue. Oral and maxillofacial surgeons may help engage with vulnerable populations through carefully planned dentoalveolar mission trips. The process of planning a mission includes selecting a population and identifying their unique needs, designing clinic layouts and workflows, team preparation, collection of supplies, fundraising, and advertising. During the mission, methods for protecting privacy, delivering treatment that is standard of care, and sanitation/sterilization options are reviewed. Ethical considerations include avoiding exploitation of vulnerable populations, offending local hosts, need for data collection, and long-term mission sustainability.

- Abstract
Parenteral anticoagulants and vitamin K antagonists (VKAs) have constituted the cornerstone of venous thromboembolism (VTE) treatment. Meanwhile, direct oral anticoagulants (DOACs) provide physicians with an alternative. The Global Anticoagulant Registry in the FIELD (GARFIELD)‐VTE observes real‐world treatment practices.

- Abstract

- Abstract
To compare the mortality rates from COVID-19 among indigenous populations of the Amazon and Andean regions of Peru during the years 2020, 2021 and 2022. Secondary analysis of 33,567 data from the COVID-19 Notification System of the National Epidemiology Center, Prevention and Control of Diseases (CDC-Peru), from the years 2020–2022. The variables were age, sex, belonging to the Andean or Amazonian ethnic group, number and type of symptoms and risk conditions, abnormal findings in chest X-rays, year of data collection for hospitalization and death from COVID-19. Poisson family generalized linear regression models with logarithmic linkage and robust variance were used to establish differences in mortality between ethnic groups. Crude and adjusted risk ratio (RR) with 95 % confidence intervals (CI) were calculated. 33,567 participants with an average age of 33.6 years were included, 44.4 % were men and 70.2 % belonged to the Amazonian ethnic group. Most of those affected by COVID-19 presented 2 symptoms (38.8 %), 4.8 % presented some risk condition, 1451 (4.3 %) were hospitalized, and 433 (1.3 %) died. The adjusted analysis showed that the Andean group, compared to the Amazonian, tended to have a higher probability of death, and this association was statistically significant, RR =7.6, 95 % CI (5.5–10.5). Patients from Andean indigenous communities had an almost 8 times higher risk of death from COVID-19. [ABSTRACT FROM AUTHOR]
