Anxiety is the most common co-occurring mental health condition across the lifespan for autistic people. The authors summarize the extant research on prevalence, correlates and predictors, measurement, and treatment of anxiety disorders in autistic people, which has rapidly expanded in the past decade. In addition to addressing gaps within each of these areas, the authors propose that there is a need for future work to center autistic perspectives, employ longitudinal designs, and expand the focus to include autistic adults and those with co-occurring intellectual disability.

- Abstract
Now extinct, the aurochs (Bos primigenius) was a keystone species in prehistoric Eurasian and North African ecosystems, and the progenitor of cattle (Bos taurus), domesticates that have provided people with food and labour for millennia1. Here we analysed 38 ancient genomes and found 4 distinct population ancestries in the aurochs—European, Southwest Asian, North Asian and South Asian—each of which has dynamic trajectories that have responded to changes in climate and human influence. Similarly to Homo heidelbergensis, aurochsen first entered Europe around 650 thousand years ago2, but early populations left only trace ancestry, with both North Asian and European B. primigeniusgenomes coalescing during the most recent glaciation. North Asian and European populations then appear separated until mixing after the climate amelioration of the early Holocene. European aurochsen endured the more severe bottleneck during the Last Glacial Maximum, retreating to southern refugia before recolonizing from Iberia. Domestication involved the capture of a small number of individuals from the Southwest Asian aurochs population, followed by early and pervasive male-mediated admixture involving each ancestral strain of aurochs after domestic stocks dispersed beyond their cradle of origin.

7. Fluorinated N-Heterocyclic Carbene Silver(I) Complexes with High Cancer Cell Selectivity

- Abstract
This work presents the synthesis of five new functionalized (benz)imidazolium N-heterocyclic (NHC) ligands (L) and four new (benz)imidazole silver(I) NHC (Ag(I)-NHC) complexes of mononuclear [Ag(L)2](PF6)or binuclear [Ag2(L)2](PF6)2type. The complexes have been fully characterized, including single crystal X-ray diffraction of three new structures. The complexes and their corresponding free NHC ligands have been screened against breast cancer and noncancerous cell lines, showing the mononuclear benzimidazole complex has the highest activity, while the binuclear benzimidazole complex has the highest cancer cell selectivity. The silver uptake was measured by ICP-MS and highlights a strong link between cytotoxicity and cellular uptake. DNA interaction studies, molecular docking, and evaluation of reactive oxygen species (ROS) have been conducted for the most promising complexes to identify modes of action. Overall, the binuclear benzimidazole complex is the most selective and promising candidate against the MDA-MD-231 (breast cancer) cell line and has potential to be developed for treatment of late-stage breast cancers.

8. Pyroligneous Acid Affects Grapevine Growth, Yield, and Chemical Composition of Leaf, Pomace, and Juice

- Abstract
In the past decade, many studies have investigated the effects of biostimulants on viticulture. However, the impact of pyroligneous acid (PA) on grape (Vitis vinifera) production has not yet been reported. In this study, PA at varying concentrations (0, 4, 8, and 12% PA) and application frequencies (14-, 21-, and 28-day intervals) were applied to enhance the growth, yield, and quality of grapes (cv. KWAD7-1). The results showed that the treated grapes responded differently to PA application. The 4 and 8% PA showed a nonsignificant (p> 0.05) increase in yield of about 0.37- and 0.18-fold, respectively, compared to the 0% PA. The 12% PA, on the other hand, reduced the yield by approximately 0.03-fold compared to the 0% PA. Carotenoid, total phenolics, flavonoid, and sugar were altered by the PA. Interestingly, the 4% PA significantly (p< 0.05) improved total carotenoids (0.34-fold), total phenolics (0.26-fold), and flavonoids (0.26-fold) compared to the 0% PA. The 4% PA applied at 21-day and 28-day intervals remarkably improved vine and leaf growth, respectively. In conclusion, the 21-day interval of PA application significantly (p< 0.05) improved fruit fresh weight, juice weight, juice volume, press weight, °Brix, pH, salinity, total dissolved solids, electrical conductivity, and titratable acidity. Further study is necessary to assess how PA can influence the metabolites present in grape wine.

10. Spermatogonial stem cell technologies: applications from human medicine to wildlife conservation†

- Abstract
Spermatogonial stem cell (SSC) technologies that are currently under clinical development to reverse human infertility hold the potential to be adapted and applied for the conservation of endangered and vulnerable wildlife species. The biobanking of testis tissue containing SSCs from wildlife species, aligned with that occurring in pediatric human patients, could facilitate strategies to improve the genetic diversity and fitness of endangered populations. Approaches to utilize these SSCs could include spermatogonial transplantation or testis tissue grafting into a donor animal of the same or a closely related species, or in vitrospermatogenesis paired with assisted reproduction approaches. The primary roadblock to progress in this field is a lack of fundamental knowledge of SSC biology in non-model species. Herein, we review the current understanding of molecular mechanisms controlling SSC function in laboratory rodents and humans, and given our particular interest in the conservation of Australian marsupials, use a subset of these species as a case-study to demonstrate gaps-in-knowledge that are common to wildlife. Additionally, we review progress in the development and application of SSC technologies in fertility clinics and consider the translation potential of these techniques for species conservation pipelines.Spermatogonial stem cell technologies developed for human infertility offer promise for wildlife conservation; however, bridging species-specific knowledge gaps is crucial for their application in rare and endangered species.Graphical Abstract

11. Evaluating the Suitability of Linear and Nonlinear Regression Approaches for the Langmuir Adsorption Model as Applied toward Biomass-Based Adsorbents: Testing Residuals and Assessing Model Validity

Regression analysis is a powerful tool in adsorption studies. Researchers often favor linear regression for its simplicity when fitting isotherm models, such as the Langmuir equation. Validating regression assumptions is crucial to ensure that the model accurately represents the data and allows appropriate inferences. This study provides a detailed examination of assumption checking in the context of adsorption studies while simultaneously evaluating the robustness of linear regression methods for fitting the Langmuir equation to isotherm data from 2,4-dichlorophenol (DCP) adsorption onto various biomass-based adsorbents and activated carbon. Different linearized Langmuir equations (Hanes–Woolf, Lineweaver–Burk, Eadie–Hofstee, and Scatchard) were compared to nonlinear regression, and each method was validated by rigorous residual checking. This included visual plots of residuals as well as statistical tests, including the Durbin–Watson test for autocorrelation (independence), the Shapiro–Wilk test for normality, and the White test for homoscedasticity. Key findings indicate that the Hanes–Woolf (type 1) and Lineweaver–Burk (type 2) linearizations were the best for most biomass adsorbents studied and that Eadie–Hofstee (type 3) and Scatchard (type 4) were generally invalid due to the negative parameters or assumption violations. For activated carbon, all linearization methods were unsuitable due to independence violations. In the case of nonlinear regression, there were no major assumption violations for all of the adsorbents. Symbolic regression identified the Langmuir equation only for activated carbon (AC). This study revealed shortcomings in relying solely on linearized Langmuir models. A proposed workflow recommends using nonlinear or weighted nonlinear regression, starting with Hanes–Woolf or Lineweaver–Burk linearization results as initial values for parameter estimation. If assumptions remain violated with nonlinear techniques, novel methods such as symbolic regression should be employed. This advanced regression technique can improve adsorption models’ accuracy and predictive behavior without the stringent need for assumption checking. Symbolic regression can also aid in understanding mechanisms of novel adsorbents.

12. Lactiplantibacillus plantarumInterstrain Variability in the Production of Bioactive Phenolic Metabolites from Flavan-3-ols

- Abstract
Flavan-3-ols intake is associated with numerous health benefits, but these are influenced by their conversion into smaller phenolic metabolites by the gut microbiota. Thus, the identification of bacteria that metabolize flavan-3-ols could lead to targeted interventions to enhance their benefits. To this end, we screened 47 Lactiplantibacillus plantarumstrains for their ability to metabolize (+)-catechin, a flavan-3-ol. Then, we assessed these strains for their capacity to convert various flavan-3-ol structures. Out of the 47 isolates, 12 released 3-(3′,4′-dihydroxyphenyl)-1-(2,4,6-trihydroxyphenyl)-propan-2-ol (a form of diphenylpropan-2-ol) from (+)-catechin. All strains metabolized (+)-catechin, (−)-epicatechin, (−)-epigallocatechin, but only a subset transformed (−)-gallocatechin. Among these simple flavan-3-ol structures, (−)-epicatechin was metabolized the most. A hierarchical cluster analysis identified two groups of flavan-3-ol-metabolizing strains categorized as having “high” and “low” production of diphenylpropan-2-ols. Notably, the strains that produced higher levels of diphenylpropan-2-ol from (+)-gallocatechin and (+)-catechin also performed better with a camu–camu extract, which was studied as a complex source of flavan-3-ols and predominantly contained these two flavan-3-ols. These results demonstrate the interstrain variability in L. plantarummetabolism, which may be useful for developing tailored formulations to enhance the production of flavan-3-ols bioactive metabolites.

13. Development, Validation, and Limits of Freezing of Gait Detection Using a Single Waist-Worn Device

- Abstract
Objective: Freezing of Gait (FOG) is prevalent in people with Parkinson's disease (PD) and severely disrupts mobility. Detecting the exact boundaries of FOG episodes may facilitate new technologies in "breaking" FOG in real-time. This study investigates the performance of automatic device-based FOG detection. Methods: Eight machine-learning classifiers (including Neural Networks, Ensemble methods, and Support Vector Machines) were developed using (i) accelerometer and (ii) combined accelerometer and gyroscope data from a waist-worn device. While wearing the device, 107 people with PD completed mobility tasks designed to elicit FOG. Two clinicians independently annotated exact FOG episodes using synchronized video and a flowchart algorithm based on international guidelines. Device-detected FOG episodes were compared to annotated episodes using 10-fold cross-validation and Interclass Correlation Coefficients (ICC) for agreement. Results: Development used 50,962 windows of data and annotated activities (>10 hours). Strong agreement between clinicians for precise FOG episodes was observed (90% sensitivity, 92% specificity, and ICC1,1 = 0.97 for total FOG duration). Device performance varied by method, complexity, and cost matrix. The Neural Network using 67 accelerometer features achieved high sensitivity to FOG (89% sensitivity, 81% specificity, and ICC1,1 = 0.83) and stability (validation loss 5%). Conclusion: The waist-worn device consistently reported accurate detection of precise FOG episodes and compared well to more complex systems. The strong clinician agreement indicates room for improvement in future device-based FOG detection. Significance: This study may enhance PD care by reducing reliance on visual FOG inspection, demonstrating that high sensitivity in automatic FOG detection is achievable.

15. COVID-19 and mental health treatment in primary care: Impact of a global pandemic on a psychopharmacological collaborative care management program.

Objective: The COVID-19 pandemic has had a profound impact on individuals with mental health (MH) disorders and on the delivery of MH services. Few studies have examined treatment models not requiring substantial changes to the delivery of services during pandemic restrictions, such as collaborative care management (CoCM) programs. Therefore, a longitudinal retrospective cohort analysis was conducted to examine the impact of the COVID-19 pandemic on a psychopharmacological CoCM program. Method: Data were collected on all U.S. Veterans enrolled in a CoCM program at a large VA during the first 10 months of the COVID-19 pandemic and compared to a matched control group one-year prior to that date. Treatment in the program pre-COVID vs. treatment during the pandemic was compared in relation to baseline symptomatology, improvements in MH symptoms, and program adherence. Results: A total of 462 Veterans were referred during the control period, compared to 351 during the same time period during the pandemic. Veterans enrolled during the first four months of each study arm, to allow for a minimum of 6 months of follow up data, had no differences in baseline symptoms of depression or anxiety between groups. Veterans receiving care during the pandemic had higher rates of program completion than pre-pandemic controls. COVID-era Veterans also had higher rates of depression response compared to controls, but no differences were observed between groups on depression remission, anxiety response, or anxiety remission. Conclusions: Psychopharmacological CoCM treatment models can successfully manage depression and anxiety with no observed decrease in the effectiveness of this intervention even during periods of unprecedented disruptions to MH services. 16. A case management strategy to reduce falls in older people with a history of falls: A randomized controlled trial.

• This study worked with a case management program involving a multidimensional assessment and development of individualized care plan according to the identified falls risk. • There were trends indicating lower fall and fracture rates in the intervention group at both the 16-week and 12-month time-points. • There was a difference statistically significant between groups for injurious fall rates at 12 months - IRR=0.18 (95 % CI = 0.04 to 0.74). • A remotely-delivered case-management program was feasible to implement among older people with a history of falls. The present study aimed to determine whether a remotely delivered intervention, based on an individual case management, can reduce falls and their consequences in community-dwelling older people with a history of multiple falls. In this randomized controlled trial, 32 participants were randomized to the intervention group, which comprised a 16-week case management program involving a multidimensional assessment, targeted interventions according to the identified fall risk factors, and development of individualized care plans. The intervention was performed by trained gerontologists, under weekly supervision of professionals with experience in falls. The control group (n = 30) received usual care. Falls were monitored over 12 months with monthly falls calendars and telephone calls. Remotely delivered case management presented an 82 % uptake of recommendations. There was a trend toward a reduced fall incidence in the intervention vs control group, with lower fall, fall injury and fracture rates in the intervention group compared with the control group at both the 16-week and 12-month time-points, with the difference statistically significant for injurious fall rates at 12 months - IRR=0.18 (95 % CI = 0.04 to 0.74). 17. Feasibility Assessment of a Novel Isolation Care Tent in Uganda During the 2022 Sudan ebolavirus Outbreak.

This case study describes a feasibility assessment of a novel isolation care tent used in health facilities in Uganda during the 2022 Sudan ebolavirus outbreak. The Isolation System for Treatment and Agile Response to High-Risk Infections Model 1B (ISTARI 1B) is a single-occupancy, portable, negative-pressure isolation tent designed for the safe delivery of standard care to patients with a communicable disease, including Ebola disease (Sudan). At the request of the Uganda Ministry of Health, the Makerere University Infectious Diseases Institute and University of Nebraska Medical Center partnered to evaluate 7 health facilities across 4 districts in Uganda for infrastructure, case management, and infection prevention and control (IPC) capacity relevant to isolation care and ISTARI 1B use. A 3-day workshop was held with IPC leaders to provide familiarization and hands-on experience with the ISTARI 1B, delineate appropriate use scenarios in Ugandan healthcare settings, contextualize ISTARI 1B use in case management and IPC workflows, develop a framework for site assessment and implementation readiness, and consider ongoing monitoring, assessment, and intervention tools. Workshop participants performed a comprehensive site assessment and mock deployment of the ISTARI 1B. In this case study, we describe lessons learned from health facility assessments and workshop outcomes and offer recommendations to support successful ISTARI 1B implementation. Use scenarios and implementation strategies were identified across facility levels, including tools for site assessment, training, risk communication, and ongoing quality and safety monitoring. - Abstract
Aortic valve replacement in asymptomatic severe aortic stenosis is controversial. The E arly valve replacement in severe ASY mptomatic A ortic S tenosis (EASY-AS) trial aims to determine whether early aortic valve replacement improves clinical outcomes, quality of life and cost-effectiveness compared to a guideline recommended strategy of 'watchful waiting'. In a pragmatic international, open parallel group randomized controlled trial (NCT04204915), 2844 patients with severe aortic stenosis will be randomized 1:1 to either a strategy of early (surgical or transcatheter) aortic valve replacement or aortic valve replacement only if symptoms or impaired left ventricular function develop, or other cardiac surgery becomes nessessary. Exclusion criteria include other severe valvular disease, planned cardiac surgery, ejection fraction <50%, previous aortic valve replacement or life expectancy <2 years. The primary outcome is a composite of cardiovascular mortality or heart failure hospitalization. The primary analysis will be undertaken when 663 primary events have accrued, providing 90% power to detect a reduction in the primary endpoint from 27.7% to 21.6% (hazard ratio 0.75). Secondary endpoints include disability-free survival, days alive and out of hospital, major adverse cardiovascular events and quality of life. Recruitment commenced in March 2020 and is open in the UK, Australia, New Zealand, and Serbia. Feasibility requirements were met in July 2022, and the main phase opened in October 2022, with additional international centers in set-up. The EASY-AS trial will establish whether a strategy of early aortic valve replacement in asymptomatic patients with severe aortic stenosis reduces cardiovascular mortality or heart failure hospitalization and improves other important outcomes. Long-term clinical outcomes of catheter ablation (CA) compared to thoracoscopic surgical ablation (SA) to treat patients with long-standing persistent atrial fibrillation (LSPAF) are not known. The purpose of this study was to compare the long-term (36-month) clinical efficacy, quality of life, and cost-effectiveness of SA and CA in LSPAF. Participants were followed up for 3 years using implantable loop recorders and questionnaires to assess the change in quality of life. Intention-to-treat analyses were used to report the findings. Of the 115 patients with LSPAF treated, 104 (90.4%) completed 36-month follow-up [CA: n = 57 (95%); SA: n = 47 (85%)]. After a single procedure without antiarrhythmic drugs, 7 patients (12%) in the CA arm and 5 (11%) in the SA arm [hazard ratio 1.22; 95% confidence interval (CI) 0.81–1.83; P =.41] were free from atrial fibrillation/tachycardia (AF/AT) ≥30 seconds at 36 months. Thirty-three patients (58%) in the CA arm and 26 (55%) in the SA arm (hazard ratio 1.04; 95% CI 0.57–1.88; P =.91) had their AF/AT burden reduced by ≥75%. The overall impact on health-related quality of life was similar, with mean quality-adjusted life year estimates of 2.45 (95% CI 2.31–2.59) for CA and 2.32 (95% CI 2.13–2.52) for SA. Estimated costs were higher for SA (mean £24,682; 95% CI £21,746–£27,618) than for CA (mean £18,002; 95% CI £15,422–£20,581). In symptomatic LSPAF, CA and SA were equally effective at achieving arrhythmia outcomes (freedom from AF/AT ≥30 seconds and ≥75% burden reduction) after a single procedure without antiarrhythmic drugs. 20. Effect of cognitive training on cognitive function in community‐dwelling older people with mild‐to‐moderate dementia: A single‐blind randomised controlled trial

The purpose of this assessor‐blinded, randomised controlled trial was to determine the effect of computerised cognitive training (CT) on executive function, processing speed and working memory in 61 people with mild‐to‐moderate dementia. The primary outcomes were forward Digit Span and Trail Making Tests (TMT) at the completion of the 6‐month intervention. Secondary outcomes included cognitive and physical performance, rate of falls, participant and caregiver's quality of life and usability and adherence to the CT program. The study was registered with the Australian and New Zealand Clinical Trials Registry (ACTRN12617000364370). Intervention group (n= 31) participants averaged 81 min of CT per week, and system usability scores were acceptable (participants: 68.8 ± 22.1; caregivers: 79.4 ± 23.5). There were no statistically significant differences in cognitive or physical performance outcomes between the intervention and control groups at 6‐ or 12‐months (between‐group differences [95% CI] for primary outcomes at 6‐months: Forward Digit Span −0.3 [−0.8, 0.3]; TMT‐A 2.7 s [−14.1, 19.5]; TMT‐B −17.1 s [−79.3, 45.2]). At the 12‐month follow‐up reassessment, the intervention group reported significantly more depressive symptoms and had lower caregiver‐rated participant quality of life and higher caregiver quality of life compared to control. This study showed no benefit of the CT program on working memory, processing speed and executive function. Future studies are required to better understand how CT can be used to improve cognitive and physical functioning in older people with mild–moderate dementia.

21. Environmental Sensing Using Commodity Optical Transceivers: Laboratory and Field Studies

- Abstract
The widespread deployment of communication network optical fiber and communication systems offers compelling opportunities for sensing that can augment traditional geophone arrays and other fiber-based sensing methods. The immediate challenges to realizing these opportunities are determining (i) how systems that already measure characteristics of optical signals can be adapted for sensing applications and (ii) the sensitivity of signals available from these systems to a range of environmental effects. In this paper, we use an optical transceiver widely deployed in network communications to assess how optical fiber strains are revealed in polarization transience measurements. We begin by evaluating the level of sensitivity to optical fiber strain in controlled laboratory tests. We then deploy our sensing system in the field to assess its efficacy for environmental sensing. We characterize the specific geometry and coupling of the fiber runs and report on the range and scope of capabilities. Our results highlight the potential for environmental sensing with optical fiber and communications network hardware and emphasize the necessary metrics reporting and cable installation specifications that are beneficial for monitoring applications.

22. Approach to the Neonate With Hypoglycemia

- Abstract
After birth, healthy neonates undergo a period of altered glucose metabolism, known as “transitional hypoglycemia.” During the first 0 to 4 hours of life, the mean plasma glucose concentration decreases to 57 mg/dL, then by 72 to 96 hours of life increases to 82 mg/dL, well within the normal adult range. Recent data suggest that transitional hypoglycemia is due to persistence of the fetal beta cell's lower threshold for insulin release, resulting in a transient hyperinsulinemic state. While hypoglycemia is an expected part of the transition to postnatal life, it makes the identification of infants with persistent hypoglycemia disorders challenging. Given the risk of neurologic injury from hypoglycemia, identifying these infants is critical. Hyperinsulinism is the most common cause of persistent hypoglycemia in neonates and infants and carries a high risk of neurocognitive dysfunction given the severity of the hypoglycemia and the inability to generate ketones, a critical alternative cerebral fuel. Screening neonates at risk for persistent hypoglycemia disorders and completing evaluations prior to hospital discharge is essential to prevent delayed diagnoses and neurologic damage.

COVID-19 is known to be associated with increased risks of cognitive and psychiatric outcomes after the acute phase of disease. We aimed to assess whether these symptoms can emerge or persist more than 1 year after hospitalisation for COVID-19, to identify which early aspects of COVID-19 illness predict longer-term symptoms, and to establish how these symptoms relate to occupational functioning.

- Abstract
IMPORTANCE: Anti–vascular endothelial growth factor (VEGF) intravitreal injections, a mainstay of treatment for many retinal diseases to optimize visual outcomes, have been included in prior authorization (PA) initiatives. However, if clinicians are extremely accurate in their use of anti-VEGF medications, such administrative burdens may need reconsideration. OBJECTIVE: To quantify PA for anti-VEGF medications (aflibercept, ranibizumab, and bevacizumab) that were approved and determine associated administrative burdens experienced by retina practices. DESIGN, SETTING, AND PARTICIPANTS: Prospective multicenter quality improvement study conducted from January 2022 through June 2022, and participants were 9 private retina practices across the US. MAIN OUTCOMES AND MEASURES: Overall rate of approval of PA requests, reasons for requesting PA, and overall rate of delay of care resulting from PA procedures. RESULTS: In total, 2365 PA requests were recorded, 2225 of which met inclusion criteria. Overall, 2140 (96.2%) requests were approved. The most common reason for requesting PA, at 64% (1423 of 2225 requests), was reauthorization for a previously utilized medication. Of the 2140 approvals, 59.6% (1277) resulted in a delay in care greater than 24 hours, and 40% (863) were given on the date of service. In a granular analysis of a subset of delayed approvals, 23.9% (173 of 725) were approved within 1 day, 15.9% (115 of 725) were approved within 2 to 3 days, 21.5% (156 of 725) were approved within 4 to 7 days, 26.3% (191 of 725) were approved within 8 to 31 days, and 12.4% (90 of 725) were approved within more than 31 days. Overall, PA denial for step therapy was 2.9% (65 of 2225) of requests and uncovered diagnoses was 0.9% (20 of 2225) of requests. The median staff time spent to obtain a single PA was 100 (range, 0-200) minutes. CONCLUSIONS AND RELEVANCE: In this study, PA requests were almost always approved but led to a delay in patient care in most patients. The current study suggests that the PA process may not be effective for retina specialists if these results can be generalized to other practices in the US and if less burdensome and less costly approaches could result in similar approval rates. Potential short-term solutions may include eliminating the PA process for bevacizumab and reauthorizations for established patients.

29. Geopolitical risk, economic policy uncertainty, financial stress and stock returns nexus: evidence from African stock markets

Purpose: Despite the growing recognition of the complex interplay between macroeconomic shock indexes and stock market dynamics, there is a significant research gap concerning their interconnectedness and return spillovers in the context of the African stock market. This leaves much to be desired, given that the financial market in Africa is arguably one of the most preferred destinations for hedge and portfolio diversification (Alagidede, 2008; Anyikwa and Le Roux, 2020). Further, like other financial markets across the globe, the increased capital flow, coupled with declining information asymmetry in Africa, has deepened intra and inter-sectoral integration within and across national borders. This has, thus, increased the susceptibility of financial markets in Africa to spillover of shocks from other sectors and jurisdictions. Additionally, while previous studies have investigated these factors individually (Asafo-Adjei et al., 2020), with much emphasis on developed markets, an all-encompassing examination of spillovers and the connectedness between the aforementioned macroeconomic shock indexes and stock market returns remains largely unexplored. This study happens to be the first to consider the impact of each of the indexes on stock returns in Africa, with evidence spanning from May 2007 to April 2023, covering notable global crisis episodes such as the Global Financial Crisis (GFC), the COVID-19 pandemic and the Russia–Ukraine war. Design/methodology/approach: This study employs the novel quantile vector autoregression (QVAR) model, making it the first of its kind in literature. By applying the QVAR, the study captures the potential nonlinear and asymmetric relationship between stock returns and the factors of interest across different quantiles, i.e. bearish, normal and bullish market conditions. Thus, the approach allows for a more accurate and nuanced examination of the tail dependence and extreme events, providing insights into the behaviour of the variables under extreme events. Findings: The study revealed that connectedness and spillovers intensified under bearish and bullish market conditions. It was also observed that, among the macroeconomic shock indicators, FSI exerted the highest influence on stock returns in Africa in both bullish and normal market conditions. Across the various market regimes, the Egyptian Exchange (EGX) and the Nairobi Stock Exchange (NSE) were net receiver of shocks. Originality/value: This study happens to be the first to consider the impact of each of the indexes on stock returns in Africa, with evidence spanning from May 2007 to April 2023, covering notable global crisis episodes such as the GFC, the COVID-19 pandemic and the Russia–Ukraine war. - Abstract
Background and hypothesis: Kidney grafts from donors who died of stroke and related traits have worse outcomes relative to grafts from both living donors and those who died of other causes. We hypothesise that deceased donors, particularly those who died of stroke, have elevated polygenic burden for cerebrovascular traits. We further hypothesise that this donor polygenic burden is associated with inferior graft outcomes in the recipient. Methods: Using a dataset of 6666 deceased and living kidney donors from seven different European ancestry transplant cohorts, we investigated the role of polygenic burden for cerebrovascular traits (hypertension, stroke, and intracranial aneurysm (IA)) on donor age of death and recipient graft outcomes. Results: We found that kidney donors who died of stroke had elevated intracranial aneurysm and hypertension polygenic risk scores, compared to healthy controls and living donors. This burden was associated with age of death among donors who died of stroke. Increased donor polygenic risk for hypertension was associated with reduced long term graft survival (HR: 1.44, 95% CI [1.07, 1.93]) and increased burden for hypertension, and intracranial aneurysm was associated with reduced recipient estimated glomerular filtration rate (eGFR) at 1 year. Conclusions: Collectively, the results presented here demonstrate the impact of inherited factors associated with donors' death on long-term graft function. Graphical Abstract:

This systematic review and meta-analysis aimed to investigate the impact of COVID19 lockdown on the anthropometric and glycaemic outcomes of adults with type 2 diabetes mellitus (T2DM) and assess whether socioeconomic status (SES) was relevant to these changes. A search of three databases was conducted. Meta-analyses using random effects models were undertaken to combine anthropometric and glycaemic measures pre- and post-confinement. Subgroup analyses according to SES were also conducted. This systematic review of 19 articles demonstrated that prolonged pandemic-related confinement is associated with a deterioration in both anthropometric and glycaemic outcomes among adults with T2DM. Furthermore, SES was found to be relevant to these changes. Specifically, BMI (kg/m
2 ) showed an increase in mean difference of 0.72 (95% CI; 0.13, 1.31; p<0.05) between pre and post lockdown cohorts. High income countries displayed a greater increase in BMI compared to their lower middle-income counterparts. Regarding, fasting blood glucose (FBG), a statistically significant difference was observed in the upper middle-income group (mean difference: 5.10; 95% CI: 2.92, 7.27), and high-income group (mean difference: 6.03; 95% CI: 0.04, 12.02). There were no significant changes to weight, waist circumference, or HbA1C over the lockdown period. Our findings suggest adults with T2DM may have received less effective care over the lockdown period, particularly in high income countries. Clinics and care providers may need to adopt more intensive contact and treatment plans in the post lockdown period to prevent lasting impacts on disease progression and metabolic sequelae. • Prolonged COVID-19 lockdown is associated with deterioration in anthropometric and glycaemic outcomes in adults with T2DM. • High SES is associated with poorer anthropometric and glycaemic outcomes following prolonged confinement. • Adults with T2DM may have received less effective care over the lockdown period, particularly in high income countries. Since its endorsement as a global network Programme in June 2021, the Early Career Ocean Professional (ECOP) Programme has been instrumental in supporting ocean professionals worldwide through funding, training, networking opportunities, and career development initiatives. Rooted in the vision of elevating diverse perspectives, the ECOP Programme fosters knowledge exchange between experienced professionals and ECOPs to promote ocean sustainability. As the UN Decade of Ocean Science for Sustainable Development (2021-2030) enters its fourth year of implementation, the ECOP Programme has grown and diversified to a formidable extent, both geographically and thematically. With over 5,200 ECOPs from 151 countries and overseas territories, the global ECOP community is currently dispersed across five regional nodes and more than 40 national hubs. This commentary brings some of the most active ECOP nodes into the limelight, amplifying their collective efforts to better understand, engage, and empower the next generation of ocean professionals across different cultural contexts. This is also an opportunity to showcase the successes, milestones, and challenges that ECOP nodes have encountered over the past 2 years, including on thematic priorities such as communications and outreach, capacity development, and ocean literacy. Moving forward, the ECOP Programme aims to further expand geographically, enhance funding capacities, deepen capacity development efforts, and promote greater awareness of the UN Ocean Decade ecosystem and its vast portfolio of endorsed Actions. As the next generation of ocean professionals will inherit our interconnected ocean beyond 2030, building trust, transferring knowledge, and taking true ownership of the global ocean agenda are paramount. 38. Janus Kinase Inhibitors Differentially Inhibit Specific Cytokine Signals in the Mesenteric Lymph Node Cells of Inflammatory Bowel Disease Patients.

Background Janus kinase [JAK] inhibitors [JAKinibs] are effective small molecule therapies for treating Crohn's disease [CD] and ulcerative colitis [UC], collectively known as inflammatory bowel disease [IBD]. By preventing JAKs from phosphorylating signal transducer and activator of transcription proteins, JAKinibs disrupt cytokine signalling pathways that promote inflammation. Despite considerable overlap in the JAKs they target, first- and second-generation JAKinibs display different clinical efficacies in CD and UC. Methods We conducted a comparative phosflow study of four JAKinibs [filgotinib, upadacitinib, tofacitinib, and deucravacitinib] to observe subtle mechanistic differences that may dictate their clinical behaviour. Resected mesenteric lymph node [MLN] cells from 19 patients [9 CD, 10 UC] were analysed by flow cytometry in the presence or absence of different cytokine stimuli and titrated JAKinibs. Results We found a higher potency of the JAK 1/3-preferential inhibitor, tofacitinib, for JAK 3-dependent cytokine signalling pathways in comparison to filgotinib, but a higher potency of the JAK 1-preferential inhibitors, filgotinib and upadacitinib, for JAK 3-independent cytokine signalling pathways. Deucravacitinib, a TYK2-preferential inhibitor, demonstrated a much narrower selectivity by inhibiting only IL-10 and IFN-β pathways, albeit more potently than the other JAKinibs. Additionally, we found some differences in the sensitivity of immune cells from CD versus UC, and patients with versus without a CD-associated NOD2 polymorphism, to phosphorylate signal transducer and activator of transcriptions in response to specific cytokine stimulation. Conclusions Despite their similarities, differences exist in the relative potencies of different JAKinibs against distinct cytokine families, to explain their clinical efficacy. Paediatric multisystem inflammatory syndrome temporally associated with SARS-CoV-2 (PIMS-TS), also known as multisystem inflammatory syndrome in children (MIS-C) emerged in April, 2020. The paediatric comparisons within the RECOVERY trial aimed to assess the effect of intravenous immunoglobulin or corticosteroids compared with usual care on duration of hospital stay for children with PIMS-TS and to compare tocilizumab (anti-IL-6 receptor monoclonal antibody) or anakinra (anti-IL-1 receptor antagonist) with usual care for those with inflammation refractory to initial treatment.

- Abstract
Cationic polysaccharides have been extensively studied for drug delivery via the bloodstream, yet few have progressed to clinical use. Endothelial cells lining the blood vessel wall are coated in an anionic extracellular matrix called the glycocalyx. However, we do not fully comprehend the charged polysaccharide interactions with the glycocalyx. We reveal that the cationic polysaccharide poly(acetyl, arginyl) glucosamine (PAAG) exhibits the highest association with the endothelial glycocalyx, followed by dextran (neutral) and hyaluronan (anionic). Furthermore, we demonstrate that PAAG binds heparan sulfate (HS) within the glycocalyx, leading to intracellular accumulation. Using an in vitroglycocalyx model, we demonstrate a charge-based extent of association of polysaccharides with HS. Mechanistically, we observe that PAAG binding to HS occurs via a condensation reaction and functionally protects HS from degradation. Together, this study reveals the interplay between polysaccharide charge properties and interactions with the endothelial cell glycocalyx toward improved delivery system design and application.

45. A Comparative Investigation of Equine-Related and Bovine-Related Human Fatalities in Oklahoma

- Abstract
This study examined 71 cases, where 45 cases were equine-related and 26 were bovine-related. Data for this study were collected by examining cases between 2000 and 2022 from the Oklahoma Office of the Chief Medical Examiner database.A majority of the equine-related fatality cases involved males aged 0 to 18 and 60 to 69 years, with sustained injuries of the head, neck, and thoracic regions while being mounted. These injuries were most often inflicted by being kicked or resulted from blunt force of impact. A majority of the bovine-related fatality cases involved males aged 60 to 79 years, with sustained injuries of the head, neck, and thoracic regions while being unmounted. These injuries were most often inflicted by being butted, trampled, or resulted from blunt force of impact. Of the total cases, approximately 42% of the causes of death were blunt force trauma of the head/neck and nearly 34% were multiple blunt force injuries. Only 3 mechanisms of death were discussed.There are distinct similarities in the most prominent gender, cause of sustained injury, and location of injury between equine- and bovine-related fatalities in Oklahoma. This study lends significant support to the need for increased awareness of safe handling practices and safety precaution education for both equine and bovine activities.

46. Cognitive-Behavioral Therapy Enhancement Strategies

- Abstract
We review the literature on various strategies to augment cognitive-behavioral therapy (CBT). Although traditional pharmacotherapy has only a small additive effect, research demonstrates that it is possible to select interventions that potentiate known mechanisms of CBT. D-cycloserine appears to potentiate activity at the N-methyl D-ethyl aspartate receptor and thereby facilitates fear extinction. Exercise may increase neural plasticity and thereby increase the efficacy of CBT for depression and anxiety. Noninvasive brain stimulation is thought to target the specific cortical regions needed for CBT response, but results have been mixed. Several other compounds appear promising but await controlled research before their efficacy as an augmentation strategy can be determined.

47. Community rehabilitation for people with severe and enduring mental health difficulties: a description and evaluation of a new community enhanced rehabilitation team

Background/AimsThe community enhanced rehabilitation team was implemented in 2021 as part of the NHS Long Term Plan. This evaluation assesses whether the team is meeting its aims and objectives..MethodsQualitative and quantitative methods were used to assess the team's progress. This included semi-structured interviews with service users, staff questionnaires and a review of data collected as part of routine clinical care..ResultsFindings showed 117 interventions had been provided via the community enhanced rehabilitation team since May 2021. Service user anxiety had reduced following intervention, and service users were less likely to be re-hospitalised if they were under the community enhanced rehabilitation team compared to other teams.ConclusionsThe community enhanced rehabilitation team is meeting its aims and objectives, suggesting this is an effective model for community rehabilitation teams.Implications for practiceCommunity enhanced rehabilitation teams can be created using this model as an example.

ABSTRACTThe importance of play is well established in early childhood development; however the importance of play appears to diminish in more advanced levels of education. Despite this, the demand for experiential, engaging learning experiences that seek to differentiate graduate-level programs in a fiercely competitive market continues to increase. This research sought to explore the phenomenon of bringing play and playfulness to the graduate-level classroom as a means through which to enhance creativity, student engagement, and teamwork. The LEGO® Serious Play (LSP) activity was originally created to be used as a facilitation strategy for business executives seeking to enhance innovation and business performance. This research sought to develop a protocol to adapt the LSP activity for masters’ students completing a mandatory marketing course. The primary aim of the research considered whether LSP would provide a valuable learning activity for future graduate-level marketing classes. As such, feedback was collected on the activity from students following engagement with LSP. The results of the study provide guidelines for marketing educators to seamlessly incorporate novel activities, such as LSP, into their teaching practice.

Globally, more than 1 billion people with disabilities are disproportionately and differentially at risk from the climate crisis. Yet there is a notable absence of climate policy, programming, and research at the intersection of disability and climate change. Advancing climate justice urgently requires accelerated disability-inclusive climate action. We present pivotal research recommendations and guidance to advance disability-inclusive climate research and responses identified by a global interdisciplinary group of experts in disability, climate change, sustainable development, public health, environmental justice, humanitarianism, gender, Indigeneity, mental health, law, and planetary health. Climate-resilient development is a framework for enabling universal sustainable development. Advancing inclusive climate-resilient development requires a disability human rights approach that deepens understanding of how societal choices and actions—characterised by meaningful participation, inclusion, knowledge diversity in decision making, and co-design by and with people with disabilities and their representative organisations—build collective climate resilience benefiting disability communities and society at large while advancing planetary health.

- Abstract
Objectives Novel or rare damaging mutations have been implicated in the developmental pathogenesis of nonsyndromic cleft lip with or without cleft palate (nsCL ± P). Thus, we investigated the human genome for high-impact mutations that could explain the risk of nsCL ± P in our cohorts.Materials and Methods: We conducted next-generation sequencing (NGS) analysis of 130 nsCL ± P case-parent African trios to identify pathogenic variants that contribute to the risk of clefting. We replicated this analysis using whole-exome sequence data from a Brazilian nsCL ± P cohort. Computational analyses were then used to predict the mechanism by which these variants could result in increased risks for nsCL ± P.Results We discovered damaging mutations within the AFDNgene, a cell adhesion molecule (CAMs) that was previously shown to contribute to cleft palate in mice. These mutations include p.Met1164Ile, p.Thr453Asn, p.Pro1638Ala, p.Arg669Gln, p.Ala1717Val, and p.Arg1596His. We also discovered a novel splicing p.Leu1588Leu mutation in this protein. Computational analysis suggests that these amino acid changes affect the interactions with other cleft-associated genes including nectins (PVRL1, PVRL2, PVRL3, and PVRL4) CDH1, CTNNA1, and CTNND1.Conclusion This is the first report on the contribution of AFDNto the risk for nsCL ± P in humans. AFDNencodes AFADIN, an important CAM that forms calcium-independent complexes with nectins 1 and 4 (encoded by the genes PVRL1and PVRL4). This discovery shows the power of NGS analysis of multiethnic cleft samples in combination with a computational approach in the understanding of the pathogenesis of nsCL ± P.
