The tympanic membrane (TM) integrity is necessary for our normal hearing function. TM perforations pose significant morbidities such as middle ear infection, recurrent otorrhea, hearing loss, and acquired cholesteatoma. Myringoplasty is commonly performed to repair chronic perforations; although various grafts and materials have been utilized to promote TM regeneration, all have associated restrictions. This study aimed to prepare and evaluate the efficacy and feasibility of a tissue-engineered patch based on silk fibroin and gelatin (SF-GT) to improve regeneration. A casting technique was used to make the SF-GT films, using 0–30 v/v% GT, as it is shown that films are remarkably efficient for TM regeneration, easy to mass produce and cheap to make. The surface morphology (SEM), chemical structure (FTIR), crystallinity (XRD), mechanical strength (tensile test), water uptake and in vitrodegradation, and cellular biocompatibility (MTT and scratch) were investigated for SF-GT films, and the optimal patch was chosen based on the results (SF-30GT). SF-30GT was used for in vivostudies in a rat model with TM perforations to induce TM regeneration. The morphology of the TM was assessed at days 0, 3, 5, 7, 9 and 11 post-implantation using otoscopy and functional outcomes by auditory brainstem responses (ABRs). This study indicates that compared with the control group (spontaneous healing), the SF-30GT scaffold was more effective in promoting early TM regeneration (complete healing at day 7) and normal hearing (back to 20 dB after healing), suggesting that this scaffold can be a potential substitute for clinical use.

•ACP mucocele is a rare and ominous cause of unilateral visual loss.•Spiral Brain CT-scan as a primary evaluation is demanding for all patients with visual impairment.•Duration and severity of symptoms are the most prognostic factor for ACP mucocele.•Surgical decompression is the main choice of treatment for patients with ACP mucocele.

IMPORTANCE: Because large dorsal reduction may weaken the keystone area, later notching of this area should be prevented while reducing large humps during septorhinoplasty. OBJECTIVE: To determine whether a triangular-shaped flap of upper lateral cartilages (ULCs) that we designed can prevent secondary deformity of the nasal dorsum in the keystone area following large hump reduction. DESIGN, SETTING, AND PARTICIPANTS: In this retrospective study, medical records as well as preoperative and postoperative photographs of septorhinoplasty cases in which triangular flaps had been used between April 1, 2012, and March 3, 2013, were reviewed. Data analysis was conducted from March 3 to May 10, 2014. Demographic data, amount of hump reduction, and any irregularity of the dorsum in the keystone area mentioned in the medical record or identified in postoperative profile view photographs were assessed. The study was conducted in a research center, and the operations were performed in a private setting. Patients had been scheduled for septorhinoplasty with flap reconstruction if more than 3 mm of dorsal hump reduction was planned and their skin was not thin. Of 41 identified patients, 3 could not be monitored for 1 year; 38 patients were included in the analysis. EXPOSURES: Open septorhinoplasty had been performed, and more than 3 mm of dorsum had been removed in all patients whose data were analyzed. During stepwise resection of the nasal dorsum, a triangular-shaped remnant of the most cephalomedial part of the ULC was maintained intact on each side over the keystone area. MAIN OUTCOMES AND MEASURES: Irregularity of the nasal dorsum over the keystone area in postoperative profile view photographs. RESULTS: More than 3 mm of hump reduction had been made in all 38 patients. No irregularities were observed in the postoperative photographs or had been found on digital examination of the keystone area after at least 12 months of follow-up. CONCLUSION AND RELEVANCE: Maintaining a remnant of the ULC over the keystone area in the form of a triangular-shaped flap on each side is a simple, reliable, and durable way to prevent notching of the keystone area during lowering of the nasal dorsum in large noses with sufficient skin thickness. We present an extremely rare case of transsellar transsphenoidal meningoencephalocele in a 36-year-old woman with pituitary dwarfism complaining of nasal obstruction. Imaging studies showed a bony defect in the sellar floor and sphenoid sinus with huge nasopharyngeal mass and 3rd ventricle involvement. Using endoscopic endonasal approach the sac was partially removed and the defect was reconstructed with fat and fascial graft, and buttressed with titanium mesh and septal flap. Visual field improvement was noticed post-operatively and no complication was encountered during follow-up. So, endoscopic endonasal approach with partial resection of the sac is a safe and effective treatment for this disease

Functional improvement after spinal cord injury remains an unsolved difficulty. Glial scars, a major component of SCI lesions, are very effective in improving the rate of this recovery. Such scars are a result of complex interaction mechanisms involving three major cells, namely, astrocytes, oligodendrocytes, and microglia. In recent years, scientists have identified two subtypes of reactive astrocytes, namely, A1 astrocytes that induce the rapid death of neurons and oligodendrocytes, and A2 astrocytes that promote neuronal survival. Moreover, recent studies have suggested that the macrophage polarization state is more of a continuum between M1 and M2 macrophages. M1 macrophages that encourage the inflammation process kill their surrounding cells and inhibit cellular proliferation. In contrast, M2 macrophages promote cell proliferation, tissue growth, and regeneration. Furthermore, the ability of oligodendrocyte precursor cells to differentiate into adult oligodendrocytes or even neurons has been reviewed. Here, we first scrutinize recent findings on glial cell subtypes and their beneficial or detrimental effects after spinal cord injury. Second, we discuss how we may be able to help the functional recovery process after injury.

Objective. To evaluate the changes in nasal dimensions of healthy Iranian volunteered for cosmetic rhinoplasty after surgery using acoustic rhinometry. Methods. Pre- and postoperative nasal dimension of 36 cases undergoing cosmetic rhinoplasty were compared using acoustic rhinometry (AR), and the measured variables were distance to first and second constriction (d1, d2), first and second minimal cross-sectional area (MCA1, 2), and volume. Results. Mean age (SD) of cases were 24.63 (4.4) years. Septoplasty was performed in 12 cases (33.3%). After surgery, bilateral d1 and both MCA2 decreased significantly, while significant increase was observed in MCA1 postoperatively using decongestant. Cases with septoplasty experienced more increase in MCA1 and less constriction in MCA2 postoperatively. In cases with rhinoplasty alone, they received benefit from double osteotomy in MCA1. In either group of rhinoplasty with and without septoplasty, placing a strut was beneficial for patients. Discussion. The cross-sectional area of the nose is a major factor in the determination of airflow. Cosmetic rhinoplasty may generate a mix effect on nose function. Performing osteotomy may better help patients to save nasal patency, septoplasty is beneficial even in mildly deviated septums, and placing a strut may be beneficial in most of the cases.
