Patient-specific fabrication of scaffold/implant requires an engineering approach to manufacture the ideal scaffold. Herein, we design and 3D print scaffolds comprised of polyether-ether-ketone (PEEK) and sodium-carboxymethyl cellulose (Na-CMC). The fabricated scaffold was dip coated with Zn and Mn doped bioactive glass nanoparticles (Zn–Mn MBGNs). The synthesized ink exhibit suitable shear-thinning behavior for direct ink write (DIW) 3D printing. The scaffolds were crafted with precision, featuring 85% porosity, 0.3 mm layer height, and 1.5 mm/s printing speed at room temperature. Scanning electron microscopy images reveal a well-defined scaffold with an average pore size of 600 ± 30 μm. The energy dispersive X-ray spectroscopy analysis confirmed a well dispersed/uniform coating of Zn–Mn MBGNs on the PEEK/Na-CMC scaffold. Fourier transform infrared spectroscopy approved the presence of PEEK, CMC, and Zn–Mn MBGNs. The tensile test revealed a Young's modulus of 2.05 GPa. Antibacterial assays demonstrate inhibition zone against Staphylococcus aureus and Escherichia Coli strains. Chick Chorioallantoic Membrane assays also present significant angiogenesis potential, owing to the antigenic nature of Zn–Mn MBGNs. WST-8 cell viability assays depicted cell proliferation, with a 103% viability after 7 days of culture. This study suggests that the PEEK/Na-CMC scaffolds coated with Zn–Mn MBGNs are an excellent candidate for osteoporotic fracture treatment. This paper presents a novel electrochemical sensor based on a pencil graphite electrode (PGE) modified with molecularly imprinted graphene oxide/zinc oxide nanocomposites for a sensitive detection of andrographolide. This is the first report of the novel method of electroanalytical determination of andrographolide through a modified PGE. The modified PGE was successfully fabricated and characterized. Then, quantitative analyses were performed by cyclic voltammetry (CV) and differential pulse voltammetry (DPV). The optimum conditions for this analysis were the supporting electrolyte containing 0.1 M KCl and 0.001 M K 3 Fe(CN) 6 , citrate buffer of pH 4, modulation amplitude of 50 mV, and scan rate of 10 mV/s. Under optimized parameters, a good linear response was obtained for andrographolide detection by DPV with a range of 50–145 µM and a detection limit of 42.6 µM. The relative standard deviation (R.S.D.) of the three measurements is 1.47%, which shows the excellent repeatability of the proposed method, while reproducibility analysis produced a R.S.D. value of 4.46%. The proposed technique with optimum conditions exhibited good selectivity towards the detection of andrographolide in the presence of ascorbic acid, uric acid, and cyclodextrin. This method was successfully applied to determine andrographolide in real water samples, and the results are comparable with the established method. [ABSTRACT FROM AUTHOR]

4. Complementary and alternative medicines use in COVID-19: A global perspective on practice, policy and research.

- Abstract
The COVID-19 pandemic has met international health systems with a low level of preparedness and emergency response. While the emergence of effective vaccines has offered the Governments, scientific communities, and members of the public a possible way out of the pandemic, effective pharmacotherapy, including immunotherapy for COVID-19 prevention and treatment, are yet to be established. Internationally, this has led to a surge in the demand and supply of many complementary and alternative medicines (CAM) and practices. Recent studies have shown increasing CAM information requests made to pharmacists and other healthcare staff from members of public and patients aimed at prevention, symptoms relief or treatment of COVID-19. In this context, it is imperative that healthcare professionals, including pharmacists, are acquainted with current practices, policies, and research in relation to CAM use in COVID-19. This narrative commentary will provide an update on global practices, policies and research in regards to CAM use in the context of COVID-19. Healthcare professionals' understanding of popular CAMs and those tipped for potential benefits in COVID-19, patient and consumer behaviors in relation to CAM use; and healthcare professionals' awareness of cultural, religious, and self-care practices associated with CAM use are imperative to inform effective communication and counselling practices and promote evidence based self-care when patients present for advice. This narrative provides relevant discussions specific to different continents and regions historically linked to diverse CAM practices. [ABSTRACT FROM AUTHOR]

- Abstract
IMPORTANCE: The Global Burden of Diseases, Injuries, and Risk Factors Study 2019 (GBD 2019) provided systematic estimates of incidence, morbidity, and mortality to inform local and international efforts toward reducing cancer burden. OBJECTIVE: To estimate cancer burden and trends globally for 204 countries and territories and by Sociodemographic Index (SDI) quintiles from 2010 to 2019. EVIDENCE REVIEW: The GBD 2019 estimation methods were used to describe cancer incidence, mortality, years lived with disability, years of life lost, and disability-adjusted life years (DALYs) in 2019 and over the past decade. Estimates are also provided by quintiles of the SDI, a composite measure of educational attainment, income per capita, and total fertility rate for those younger than 25 years. Estimates include 95% uncertainty intervals (UIs). FINDINGS: In 2019, there were an estimated 23.6 million (95% UI, 22.2-24.9 million) new cancer cases (17.2 million when excluding nonmelanoma skin cancer) and 10.0 million (95% UI, 9.36-10.6 million) cancer deaths globally, with an estimated 250 million (235-264 million) DALYs due to cancer. Since 2010, these represented a 26.3% (95% UI, 20.3%-32.3%) increase in new cases, a 20.9% (95% UI, 14.2%-27.6%) increase in deaths, and a 16.0% (95% UI, 9.3%-22.8%) increase in DALYs. Among 22 groups of diseases and injuries in the GBD 2019 study, cancer was second only to cardiovascular diseases for the number of deaths, years of life lost, and DALYs globally in 2019. Cancer burden differed across SDI quintiles. The proportion of years lived with disability that contributed to DALYs increased with SDI, ranging from 1.4% (1.1%-1.8%) in the low SDI quintile to 5.7% (4.2%-7.1%) in the high SDI quintile. While the high SDI quintile had the highest number of new cases in 2019, the middle SDI quintile had the highest number of cancer deaths and DALYs. From 2010 to 2019, the largest percentage increase in the numbers of cases and deaths occurred in the low and low-middle SDI quintiles. CONCLUSIONS AND RELEVANCE: The results of this systematic analysis suggest that the global burden of cancer is substantial and growing, with burden differing by SDI. These results provide comprehensive and comparable estimates that can potentially inform efforts toward equitable cancer control around the world.

- Abstract
Documentation of patterns and long-term trends in mortality in young people, which reflect huge changes in demographic and social determinants of adolescent health, enables identification of global investment priorities for this age group. We aimed to analyse data on the number of deaths, years of life lost, and mortality rates by sex and age group in people aged 10–24 years in 204 countries and territories from 1950 to 2019 by use of estimates from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019.

Countries around the globe have responded to pandemic preparedness and developed strategies to cope with the COVID-19 crisis. In this context, the role of healthcare professionals is of paramount importance. Pharmacists are playing a vital role in dealing, preparedness, prevention, protection, promoting access to medicines and to improve health outcomes during this crisis. In this context, "Drive-thru" pharmacy services improve access to medicines while ensuring the preventive measures suggested by the World Health Organization. This commentary provides an overview of opportunities and challenges related to the implementation of "drive-thru pharmacy services" and their role in improving public health during this crisis. We present updated estimates from the Global Burden of Disease (GBD) study on the prevalence of hearing loss in 2019, as well as the condition's associated disability.

11. Evaluation of an educational intervention on physicians’ knowledge about adverse drug reaction reporting system

- Abstract
Aim: The aim of the study was to develop and evaluate an educational intervention for practicing physicians for reporting of adverse drug reactions. Subjects and Methods: Reporting of adverse drug reactions (ADRs) by healthcare professionals is paramount for improving medicine safety. However, the under-reporting of ADR by physicians is a global problem. A 1-day workshop (comprising a lecture and an information pamphlet) on awareness about ADR reporting and pharmacovigilance system was organized. A pre-tested questionnaire with four questions about ADR reporting and eight on pharmacovigilance system was filled in by the participants before and after the workshop. Descriptive statistics was used for the analysis of demographics as mean and percentage. A paired sample t-test was employed to evaluate the responses before and after the intervention regarding knowledge about ADR reporting and pharmacovigilance system. Results: There was no significant difference in the pre-post evaluation of knowledge about ADR reporting; however, a significant difference was found in the pre-post evaluation of participant’s knowledge regarding pharmacovigilance system. Conclusion: The knowledge about the pharmacovigilance system improved after the intervention. Further studies are required on a larger scale and involving other healthcare professionals to promote a better healthcare system.

- Abstract
The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 provides a rules-based synthesis of the available evidence on levels and trends in health outcomes, a diverse set of risk factors, and health system responses. GBD 2019 covered 204 countries and territories, as well as first administrative level disaggregations for 22 countries, from 1990 to 2019. Because GBD is highly standardised and comprehensive, spanning both fatal and non-fatal outcomes, and uses a mutually exclusive and collectively exhaustive list of hierarchical disease and injury causes, the study provides a powerful basis for detailed and broad insights on global health trends and emerging challenges. GBD 2019 incorporates data from 281 586 sources and provides more than 3·5 billion estimates of health outcome and health system measures of interest for global, national, and subnational policy dialogue. All GBD estimatesare publicly available and adhere to the Guidelineson Accurate and Transparent Health Estimate Reporting. From this vast amount of information, five key insights that are important for health, social, and economic development strategies have been distilled. These insights are subject to the many limitations outlined in each of the component GBD capstone papers.

- Abstract
Objective: This article aimed to summarize the pharmacological and non-pharmacological management of COPD and limitations and prospects of the currently available pharmacological therapies in the management of COPD. Background: COPD is a complex comorbid disease that irreversibly affects the pulmonary system and is considered the fifth leading cause of death worldwide. Currently, the diagnosis, assessment and management of COPD are difficult because of its complex syndrome involving numerous pulmonary and extra-pulmonary components. It is diagnosed based on the subjective and objective findings of a chronic cough, sputum production, dyspnoea, exercise intolerance and spirometry values. The GOLD report recommends bronchodilators as the first-line treatment agents in all stages of COPD severity, commonly combined with inhaled corticosteroids (ICS), methylxanthines and antibiotics, depending upon the condition and symptoms of patients. Non-pharmacological treatments of COPD include smoking cessation, influenza and pneumococcal vaccination, pulmonary rehabilitation, LTOT and surgical lung volume reduction. The efficacy of the therapy is directly related to the patients’ inhaler use training and education. A new future in COPD medication is essential to ensure the optimal management of COPD. New alternative sources should be considered in the treatment of COPD. Conclusion: New approaches such as e-technology can play an essential role in improving compliance and adherence to therapy. New therapies with better and long-lasting effects and minimized side effects need to be discovered to improve the pharmacological management of COPD.

18. Effect of Vitamin D Supplementation on the Salivary Cytokines of Pregnant Women: A Randomized Placebo-Controlled Trial.

Objective: We aimed to determine the salivary cytokine profiles of IL-2, IL-4, IL-6, IL-10, TNF-α, IFN-γ and IL-17 among pregnant women and determine if the cytokine profiles change upon administration of vitamin D to these women.Methods: It was a community-based, blinded, placebo-controlled, randomized trial. The intervention group comprised pregnant females (n = 36) who received oral vitamin D 4000 IU/day for 6 months (starting from 12 - 16 weeks until the end of pregnancy). Controls comprised pregnant females who received placebo (n = 49) for the same duration. Probing depth (PD), attachment loss (AL) and bleeding on probing (BOP) were recorded. Saliva samples were subjected to multiplex ELISA analysis of cytokines. Serum vitamin D levels were determined. Outcome was assessed within 14 days post-partum. Periodontal examinations were done; blood and saliva samples were subjected to the set of tests described at baseline. SPSS 19.0 and Graph Pad 6.0 were used for analysis. The study was registered at www.clinicaltrials.gov with identifier number NCT01422122.Results: The two study groups were comparable at the baseline. Although IL-6 showed wide variation, none of the pro-inflammatory cytokine levels was affected by six months of oral vitamin D supplementation. Similarly, the mean salivary levels of the anti-inflammatory cytokines IL-4 and IL-10 also remained unaffected in the two groups.Conclusion: There was no association between hypovitaminosis D and periodontal disease among the studied sample of pregnant women. 22. Non-specific immunity of BCG vaccine: A perspective of BCG immunotherapy.

- Abstract
BCG is a widely used vaccine worldwide for neonates including Pakistan. BCG has more than 90% coverage through the EPI program which was introduced in 1965 in Pakistan. BCG has limited efficacy against the transmissible form of pulmonary tuberculosis in high TB endemic countries. However, BCG vaccination continues in these countries because BCG confers protection against the disseminated form of TB in children. BCG has also shown some protection against leprosy and certain forms of cancers. One reason for such nonspecific protection may be that BCG activates APCs via PAMPS that interacts with TLRs (2, 4 & 8), which initiate the inflammatory cascade thereby recruiting inflammatory cells to the site of infection and providing maturation signals for neutrophils, macrophages and dendritic cells. Such activation may be crucial for restricting the infection at the initial site. Furthermore, activation of the pro-inflammatory cascade also results in expression of adhesion molecules, co-stimulatory molecules as well as MHC class II molecule. MHC class II molecules engage CD4+ cells via the TCR receptor while the adhesion and costimulatory molecules bind to their respective receptors on CD4+ T cells for additional high affinity binding for T cell activation. Although activation of the innate arm may not provide subsequent memory, activation of T cells may introduce a certain level of memory response and therefore, may form a rational basis for BCG immunotherapy. This review, therefore, focuses on the immune activation related to both the innate and adaptive arm of the immune response that has been reported and further explores the utility of BCG immunotherapy related to non TB conditions. We demonstrate the enhancement of magnetic dipole spontaneous emission from Eu^3+ ions by an engineered plasmonic nanostructure that controls the electromagnetic environment of the emitter. Using an optical microscope setup, an enhancement in the intensity of the Eu^3+ magnetic dipole emission was observed for emitters located in close vicinity to a gold nanohole array designed to support plasmonic resonances overlapping with the emission spectrum of the ions.

Summary: Several cytokines (IFNγ, TNFα, IL10 and IL6) show an association with either disease localization or dissemination in tuberculosis. There are also reports of involvement of extra-pulmonary sites in tuberculosis with differential clinical severity. However, no comparative study of biomarkers across the disease severity spectrum is available. This was the purpose of the current study. Cytokines (IFNγ, TNFα, IL10 and IL6) secreted in response to a panel of stimulants (PHA, LPS or mycobacterial antigens) in whole blood were determined in eighty-two tuberculosis patients. WHO criteria was applied for stratification of patients according to disease severity: disseminated and or severe disease (EPTB1; N=29); disease localized to lung parenchyma (PTB; N=32) and disease localized to peripheral sites without lung involvement (EPTB2; N=21). Mycobacterial antigens induced IFNγ/IL10 ratio showed a direct relationship with disease severity ranking (median ratios: EPTB1=0.21; PTB=0.85; EPTB2=7.7) and the highest correlation (Spearman Rank; rho=0.673, p<0.000001). IFNγ/IL10 ratio also rank ordered clinical severity as it relates to anatomic sites. IFNγ/IL10 ratio may therefore provide a useful objective marker of disease severity in both pulmonary and extra-pulmonary tuberculosis. [Copyright &y& Elsevier]

We investigated live Mycobacterium- and recombinant antigen-induced cytokine and chemokine responses in patients with active tuberculosis (TB) compared with those of healthy controls from an area where TB is endemic (ECs). M. tuberculosis-, M. bovisBCG-, ESAT6-, and culture filtrate protein 10 (CFP10)-induced responses were determined in peripheral blood mononuclear cells from patients with pulmonary TB (n= 38) and ECs (n= 39). The levels of the cytokines gamma interferon (IFN-?) and interleukin-10 (IL-10) and the chemokines CCL2, CCL3, and CXCL9 were measured. The levels of M. tuberculosis- and BCG-induced IFN-? secretion were significantly reduced (P= 0.002 and P< 0.01, respectively), while the amount of IL-10 induced by both virulent (P< 0.01) and avirulent (P= 0.002) mycobacteria was increased in patients with TB. The ESAT6-induced IFN-? responses were increased in the patients with TB (P= 0.013) compared with those in the EC group. When tuberculin skin test (TST)-negative (TST-; induration, <10 mm) and TST-positive (TST+) donors were studied separately, both TST-and TST+individuals showed increased IFN-? responses to M. tuberculosiscompared with the responses of the patients with TB (P= 0.037 and P= 0.006, respectively). However, only TST+ECs showed reduced IFN-? responses to ESAT6 (P= 0.008) compared with the responses of the patients with TB. The levels of M. tuberculosis-induced CCL2 (P= 0.006) and CXCL9 (P= 0.017) were greater in the patients with TB. The levels of CCL3 secretion in response to Mycobacteriumand antigen stimulation were comparable between the two groups. While the levels of ESAT6-induced chemokines did not differ between the patients with TB and the ECs, the levels of CFP10-induced CCL2 (P= 0.01) and CXCL9 (P= 0.001) were increased in the patients. These data indicate differential host IFN-?, CXCL9, and CCL2 responses to live mycobacteria and mycobacterial antigens and have implications for the identification of potential biomarkers of infection which could be used for the diagnosis of TB.

ABSTRACTThe detection of hundreds of thousands of new cases of leprosy every year suggests that transmission of Mycobacterium lepraeinfection still continues. Unfortunately, tools for identification of asymptomatic disease and/or early-stage M. lepraeinfection (likely sources of transmission) are lacking. The recent identification of M. leprae-unique genes has allowed the analysis of human T-cell responses to novel M. lepraeantigens. Antigens with the most-promising diagnostic potential were tested for their ability to induce cytokine secretion by using peripheral blood mononuclear cells from leprosy patients and controls in five different areas where leprosy is endemic; 246 individuals from Brazil, Nepal, Bangladesh, Pakistan, and Ethiopia were analyzed for gamma interferon responses to five recombinant proteins (ML1989, ML1990, ML2283, ML2346, and ML2567) and 22 synthetic peptides. Of these, the M. leprae-unique protein ML1989 was the most frequently recognized and ML2283 the most specific for M. lepraeinfection/exposure, as only a limited number of tuberculosis patients responded to this antigen. However, all proteins were recognized by a significant number of controls in areas of endemicity. T-cell responses correlated with in vitro response to M. leprae, suggesting that healthy controls in areas where leprosy is endemic are exposed to M. leprae. Importantly, 50% of the healthy household contacts and 59% of the controls in areas of endemicity had no detectable immunoglobulin M antibodies to M. leprae-specific PGL-I but responded in T-cell assays to =1 M. lepraeprotein. T-cell responses specific for leprosy patients and healthy household contacts were observed for ML2283- and ML0126-derived peptides, indicating that M. lepraepeptides hold potential as diagnostic tools. Future work should concentrate on the development of a sensitive and field-friendly assay and identification of additional peptides and proteins that can induce M. leprae-specific T-cell responses.

27. Longitudinal Tracking of Cytokines after Acute Exposure to Tuberculosis: Association of Distinct Cytokine Patterns with Protection and Disease Development

ABSTRACTHousehold contacts (HCs) of patients with tuberculosis (TB) are at higher risk of infection as well as the development of active disease. Longitudinal tracking of antigen-specific cytokines after acute exposure may significantly advance our understanding of the dynamic changes in cytokine patterns associated with disease establishment. To achieve this objective, we carried out a prospective cohort study with healthy HCs after exposure to TB. The patterns of cytokines (gamma interferon [IFN-?] and interleukin 10 [IL-10]) in response to mycobacterial antigens (culture filtrate [CF] proteins) and nonspecific mitogens (phytohemagglutinin [PHA] and lipopolysaccharide [LPS]) were assessed at 0, 6, 12, and 24 months after exposure. Seven of 109 (6.4%) HCs developed active disease. Six of the seven individuals were females, and active disease developed between 12 and 15 months after exposure in 5/20 families. The most significant findings were the exponential increases (~1,000-fold) in both the CF protein- and the PHA- or LPS-induced IFN-?/IL-10 ratio in healthy HCs (n= 26), which peaked at 12 months, compared to the levels in HCs who developed disease (n= 7), in whom relatively flat responses were observed during the 24-month period. Linear trends for 0 to 12 and 0 to 24 months for the CF protein-induced IFN-?/IL-10 ratio showed significant differences between the two groups, as determined by the use of the Mantel extension test for ?2analysis (odds ratio = 0.45; 95% confidence interval = 0.295 to 0.685; P= 0.0002). Our results strongly suggest that the magnitude of the IFN-?/IL-10 ratio at 12 months after exposure may be a critical determinant in the resolution of infection. These studies provide new insights into the cytokine responses associated with disease establishment or the resolution of infection after natural exposure to TB and have implications for TB control programs as well vaccine efficacy studies.

ABSTRACTTo identify Mycobacterium leprae-specific human T-cell epitopes, which could be used to distinguish exposure to M. lepraefrom exposure to Mycobacterium tuberculosisor to environmental mycobacteria or from immune responses followingMycobacterium bovisBCG vaccination, 15-mer synthetic peptides were synthesized based on data from the M. lepraegenome, each peptide containing three or more predicted HLA-DR binding motifs. Eighty-one peptides from 33 genes were tested for their ability to induce T-cell responses, using peripheral blood mononuclear cells (PBMC) from tuberculoid leprosy patients (n= 59) and healthy leprosy contacts (n= 53) from Brazil, Ethiopia, Nepal, and Pakistan and 20 United Kingdom blood bank donors. Gamma interferon (IFN-γ) secretion proved more sensitive for detection of PBMC responses to peptides than did lymphocyte proliferation. Many of the peptides giving the strongest responses in leprosy donors compared to subjects from the United Kingdom, where leprosy is not endemic, have identical, or almost identical, sequences in M. lepraeand M. tuberculosisand would not be suitable as diagnostic tools. Most of the peptides recognized by United Kingdom donors showed promiscuous recognition by subjects expressing differing HLA-DR types. The majority of the novel T-cell epitopes identified came from proteins not previously recognized as immune targets, many of which are cytosolic enzymes. Fifteen of the tested peptides had ≥5 of 15 amino acid mismatches between the equivalent M. lepraeand M. tuberculosissequences; of these, eight gave specificities of ≥90% (percentage of United Kingdom donors who were nonresponders for IFN-γ secretion), with sensitivities (percentage of responders) ranging from 19 to 47% for tuberculoid leprosy patients and 21 to 64% for healthy leprosy contacts. A pool of such peptides, formulated as a skin test reagent, could be used to monitor exposure to leprosy or as an aid to early diagnosis.

Humoral immune parameters like total immunoglobulins and specific antibody levels in serum were studied in filarial chyluria patients. Mean serum IgG was significantly reduced in this group compared to normal controls, while IgA and IgM levels remained comparable to controls. Anti-filarial antibody titre as measured by enzyme-linked immunosorbent assay also was significantly reduced. However, the total and specific IgE antibody titre was similar to that of controls. Specific IgE contents of the patients’ sera could be related to their microfilaraemic status.

- Abstract
• Erythema annulare centrifugum (EAC) is a figurate erythema that has been associated with many diverse entities. A case of EAC was related to infestation by the nematode, Ascaris lumbricoides. A careful examination of the patient did not define an immunologic basis for EAC. Results of lymphocyte stimulation tests of the patient showed an immune response to A lumbricoides extracts but not to A suum extracts. This finding is of practical importance, since most laboratories use A suum extracts in routine skin and serologic tests for human ascariasis.(Arch Dermatol 1981;117:582-585)

- Abstract
Measuring routine childhood vaccination is crucial to inform global vaccine policies and programme implementation, and to track progress towards targets set by the Global Vaccine Action Plan (GVAP) and Immunization Agenda 2030. Robust estimates of routine vaccine coverage are needed to identify past successes and persistent vulnerabilities. Drawing from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2020, Release 1, we did a systematic analysis of global, regional, and national vaccine coverage trends using a statistical framework, by vaccine and over time.
