Background and objective As a new technique developed in recent years, bronchoscopic intervention therapy has the advantages of minimal invasion, high safety and repeatability. The aim of this study is to investigate the clinical characteristics of bronchopleural fistula (BPF) induced by surgeries for lung malignancies or benign diseases and the effect of bronchoscopic intervention therapy for BPF, so as to provide support for prevention and treatment of BPF. Methods Data 64 patients with BPF who were treated by bronchoscopic intervention in Respiratory Disease Center of Dongzhimen Hospital, Beijing University of Chinese Medicine from June 2020 to September 2023 were collected. Patients with fistula diameter ≤5 mm were underwent submucous injection of macrogol, combined with blocking therapy with N-butyl cyanoacrylate, medical bioprotein glue or silicone prosthesis. Patients with fistula diameter >5 mm were implanted with different stents and cardiac occluders. Locations and characteristics of fistulas were summarized, meanwhile, data including Karnofsky performance status (KPS), shortbreath scale (SS), body temperature, pleural drainage volume and white blood cell count before and after operation were observed. Results For all 64 patients, 96 anatomic lung resections including pneumonectomy, lobectomy and segmentectomy were executed and 74 fistulas occurred in 65 fistula locations. The proportion of fistula in the right lung (63.5%) was significantly higher than that in the left (36.5%). Besides, the right inferior lobar bronchial fistula was the most common (40.5%). After operation, KPS was significantly increased, while SS, body temperature, pleural drainage volume and white blood cell count were significantly decreased compared to the preoperative values (P<0.05). By telephone follow-up or readmission during 1 month to 38 months after treament, median survival time was 21 months. 33 patients (51.6%) showed complete response, 7 patients (10.9%) showed complete clinical response, 18 patients (28.1%) showed partial response, and 6 patients (9.4%) showed no response. As a whole, the total effective rate of bronchoscopic intervention for BPF was 90.6%. Conclusion BPF induced by pulmonary surgery can lead to severe symptoms and it is usually life-threating. Pulmonary sarcomatoid carcinoma (PSC) is a rare and highly malignant tumor, which includes the following five pathologic types: pleomorphic carcinoma, spindle cell carcinoma, giant cell carcinoma, carcinosarcoma and pulmonary blastoma. The onset of PSC is occult with non-specific clinical symptoms and signs. The clinical manifestations include irritating cough, bloody sputum, dyspnea, chest pain and so on, which are closely related to the growth and invasion site of the tumor. PSC tends to metastasize early, so most patients are already in local advanced stage or advanced stage with a median survival of 9 months at the time of hospital visit. A patient with primary PSC which led to 90% stenosis in central airway was treated by combined method of vascular and tracheoscopic intervention in our respiratory center. This treatment prolonged the patient's survival time and got a satisfactory effect at 19-month follow-up after surgery. Herein we report the case for clinical reference. [ABSTRACT FROM AUTHOR]

Effects of rolling and coiling temperatures on the microstructure and properties of a low-carbon, high-strength pipeline steel were investigated using a dilatometer, scanning electron microscope, and transmission electron microscope as well as tensile experiments. The results show that during the thermal mechanical control processing, coiling temperature played a more important role in tailoring the microstructure and mechanical properties than the rolling temperature. Finer grain size and higher strength at the cost of a slight decrease in elongation were obtained by decreasing the coiling temperature, while the microstructure and mechanical properties varied little at different rolling temperatures when the coiling temperature was the same. The bainite starting temperature decreased with the increase of rolling temperature, while it was not affected by the coiling temperature. Specimens at a low rolling temperature or a low coiling temperature exhibited a larger peak transformation rate, indicating the promoted transformation kinetics. [ABSTRACT FROM AUTHOR]

Background and objective To observe the clinical effects and safety of endobronchial electrocautery snare combined with CO2 cryosurgery for the treatment of tracheobronchial obstructive lesions. Materials and methods Seventy-seven patients with airway tumor or granuloma were retrospectively reviewed, including 70 malignant airway obstruction and 7 benign airway lesions, for the treatment of endobronchial electrocautery snare plus CO2 cryosurgery and argon plasma coagulation (APC). Results Eighty-five endobronchial snares were performed in 77 cases. 42.9% of the obstructive lesions were located in right bronchial orifice, 38.3% in main trachea 21.4% in left bronchial orifice. 89.7% of the malignant tumor was mixed type of lesions (endobronchial plus bronchial or external bronchial), only 10.3% was endobronchial. Obstructive stenosis was significant relieved after snare (80% before vs 20% after, P<0.01) in all patients. Karnofsky Performance Status (KPS) and short-breath score were obviously improved after snare. There was no severe complications related to the procedures. Background and objective Treating airway fistulas, including esophagorespiratory fistulas (ERFs), bronchopleural fistulas (BPFs), and tracheomediastinal fistulas (TMFs), is difficult. The aim of this study is to evaluate the safety and clinical efficacy of metallic covered Z-type stents (CZTS) for the treatment of airway fistulas through bronchoscopy or fluroscopy. Methods Thirty-eight patients with fistulas between the esophagus, mediastina, and airways (32 ERFs, 5 BPFs, and 1 TMF) were retrospectively reviewed after treatment with covered metallic esophageal and airway stents. The fistulas were caused by esophageal (n=26), bronchogenic (n=11), and thyroid (n=1) carcinomas. Results Forty-six fistulas were found in 38 patients. The fistula size ranged from 0.5 cm to 7.0 cm. Forty airway covered metal stents (24 Y-type, 8 L-type, and 8 I-type) and 24 esophageal metal stents were placed. Complete responses to the sealing effects of fistulas were noted in 4.3% of all the fistulas, 60.9% showed complete clinical responses, 23.9% showed partial responses, and 10.9% showed no response. An effectivity rate of 89.1% was observed, and the median survival duration of all patients was 5 months. Conclusion The use of CZTS appears to be safe and feasible for the palliative treatment of ERFs, BPFs, and TMFs. Airway stent placement is recommended for patients with ERF. In the event that airway stents fail, esophageal stents should be given. Airway bifurcation stents were observed to be especially suitable for the sealing of fistulas near the trachea carina.
