Documenting and managing their health can be very important for elderly people or people with chronic diseases. The main goal of the AYUDO project is to develop an integrated personal health record as the core of a system that supports these individuals in documenting and analyzing their health and provides them with innovative user interfaces. This paper presents the main results of the AYUDO project, focusing on the following challenges: What kind of health data exist and can be considered, how can the user experience be optimized, and how can interoperability with the user's digital home environment be optimally supported? We therefore present the AYUDO prototype, how it addresses these research questions, and show first evaluation results. [ABSTRACT FROM AUTHOR]
