•Lateral marsupialization is an effective method for restoration of both smell function and nasal breathing in concha bullosa patients.•The presence and quantity of olfactory mucosa in the middle turbinate may not significantly affect the postoperative smell function.•The underlying cause of olfactory deficit in middle turbinate concha bullosa patients seems to be middle meatus obstruction.

Direct rigid laryngoscopy (DRL) is a relatively simple and well-tolerated procedure that can provide both diagnostic and therapeutic benefits. There are no data regarding the risk of acquiring bacteremia after DRL. The purpose of this prospective study was to assess the frequency of bacteremia and nosocomial colonization following DRL and to identify possible risk factors for the development of these complications.

Tuberculosis of the parotid gland is extremely unusual. Tuberculosis comprises 2.5% to 10% of parotid gland lesions. Two clinical forms of parotid gland tuberculosis infection exist. One is a diffuse parenchymatous disease (either primary or secondary to nodal disease), resembling common infection. The second is a chronic, slow-growing, painless, and firm parotid mass mimicking a neoplasm. Most of these patients were diagnosed after parotid gland surgery and histopathologic evaluation. Warthin tumor is a well-known benign neoplasm of the salivary glands. It is the second most common tumor of the parotid gland. Mycobacterium tuberculosiswithin Warthin tumor is also unusual. Five cases with parotid gland tuberculosis within Warthin tumor were reported in the literature. In this report, we present a new patient with parotid gland tuberculosis within the Warthin tumor. This type parotid gland pathology is an extremely rare entity, and to the best of our knowledge, this is the second documented case using polymerase chain reaction. We also discussed the possible mechanisms of development of infection within Warthin tumor.

Basal cell adenoma (BCA) is a rare benign epithelial tumor of the salivary gland. BCA is seen most frequently in the parotid gland and less commonly in the submandibular gland and minor glands of the upper lips, oral cavity, and hard palate. Salivary gland tumors are observed as single tumors in one salivary gland. Double or multiple tumors of the salivary gland tumors are unusual and metachronous or bilateral salivary gland tumors are more observed than synchronous or unilateral tumors. The most commonly seen multiple tumor unilaterally or bilaterally is the Warthin's tumor. A 65-year-old woman with a painful, slowly enlarging mass in front of the left ear, which was present for 6 months, was evaluated. Physical examination revealed two solid and well-delineated masses in the preauricular region, which were 1.5 × 1 cm in diameter and in the tail of the parotid gland, which is 2.5 × 2 cm in diameter. Excision of the superficial lobe of the parotid gland was performed. The macroscopic examination of the specimen showed the two discrete nodular masses. Histologic examination of the two nodular solid lesions was reported as BCA. Multiple synchronous nonmembranous-type BCAs of the unilateral parotid gland is a rare entity. More extensive excision of the parotid gland tumor, careful macroscopic perioperative examination of the surgical specimen, and histologic evaluation of all surgical specimens might be necessary for reducing revision operations and surgical complications.

Myasthenia gravis (MG) is an autoimmune neuromuscular disease. Although most MG cases do not have accompanying malignant disease, patients with thymoma may have an increased risk for secondary malignancy. The reason MG patients are prone to developing cancer is not very well known. Salivary gland tumors account for approximately 3% of head and neck tumors, and approximately 80% of these occur in the parotid gland. Mucoepidermoid carcinoma is the most common malignant tumor of the parotid gland. MG may be associated with squamous cell carcinoma of the head and neck. We present a patient with MG who developed parotid gland mucoepidermoid carcinoma. MG patients with parotid gland cancer have not been previously reported. We also discuss the possible mechanisms of tumor development in an MG patient.

Nasal surgery is occasionally performed to correct traumatic nasal deformity. Septal cartilage is the main tissue to be corrected and is a graft source when needed. A risk in engrafting with cartilage is the possibility of resorption as a result of either necrosis or apoptosis. The authors evaluated the rate of apoptosis in deviated and straight cartilage to investigate the cause of resorption of cartilage tissue.

The possible effect of selenium on the prevention or reduction in occurrence of myringosclerosis was investigated. Fifteen rats were myringotomized bilaterally and separated into two groups. Nine rats were treated with selenium in the study group. Six rats were administered physiological serum and formed the control group. The occurrence of myringosclerotic lesions and anti-apoptotic activity in the tympanic membranes of the two groups were compared otomicroscopically and histopathologically. The sclerosis was occasional in three, moderate in five and severe in three tympanic membranes in the control group. On the other hand sclerosis was observed in only two of 18 specimens in the study group and sclerosis was seen only occasionally in these two sections. Although Bcl-2 staining, which indicates apoptosis, was not statistically different between the groups, it was observed that apoptosis was slightly more apparent in the study group (eight of 18 tympanic membranes versus two of 12 tympanic membranes in the control group). In conclusion, the formation of myringosclerosis following myringotomy in rats can be reduced by intraperitoneal selenium administration.

Sinolith is a calculus in the paranasal sinuses. It has been also known as antrolith, rhinolith, antral calculi, antral stone, or antral rhinolith. The pathogenesis of calculi formation within a paranasal sinus is still not known. Chronic infection, foreign material, poorly draining sinus, and fungal infection are the main predisposing factors. Isolated sphenoid sinus lesions are rare, and most of them are inflammatory diseases. The main symptom of sphenoid sinus lesions is headache. Headache may be the only symptom of sphenoid sinus lesions. Sinolith is mostly encountered in the maxillary sinus followed by the frontal sinus and the ethmoid sinus. There was only 1 publication about sinolith localized in the sphenoid sinus in the English language literature. We report a case of an isolated sinolith localized in the sphenoid sinus. The treatment of choice should be surgical removal of the sinolith. Endoscopic surgery especially through the transnasal route should be the first-choice surgical treatment of isolated sphenoid sinus lesions.

Lymphadenitis is the most common form of tuberculosis in the head and neck region, but it can be seen in the other areas of the head and neck. Nasopharyngeal tuberculosis is a rare condition without pulmonary and systemic involvement. The majority of patients present with neck mass. A 17-year-old female patient admitted to our outpatient clinic with the complaints of swelling on both sides of the neck and hearing loss. The endoscopic examination revealed a nasopharyngeal mass, and biopsies were taken from the mass. The result of pathologic examination was reported as caseating granulomatous inflammation compatible with tuberculosis. In this report, a nasopharyngeal tuberculosis case associated with massive cervical lymphadenopathy was reported, and etiopathogenesis and treatment were also discussed.

This study investigated the common flora of human cerumen in patients with recurrent otitis externa, and subjects who had been operated on and had an open mastoidectomy cavity from chronic otitis media.Cerumen samples were collected from three groups; group A (n = 20) consisted of patients with recurrent otitis externa, group B (n = 20) consisted of patients with an open cavity and group C (n = 30) consisted of healthy subjects.The mean of the microbial count was 3.4 × 104 in group A, 3.08 × 104 in group B and 2.48 × 104 in group C. The most commonly isolated microorganism from the three groups was Staphylococcus epidermidis. No growth was observed in five cases (25 per cent) in group A and in three cases (10 per cent) in group C. In groupB antimicrobial growth was observed in all samples. In 46 (65 per cent) of the cerumen samples,the isolates were monomicrobial and 24 (35 per cent) of the cerumen samples were polymicrobial. The isolates were polymicrobial in 65 per cent of group A, 20 per cent in group B and 23.3 per cent in group C.In the process of investigating the microbial flora of cerumen in all the three groups, microbial growth was observed from all the samples from patients with an open cavity, unlike the other groups, and it was determined that the group with recurrent external otitis had the most abundant microbial flora.

Several problems have been observed when performing the classic endoscopic technique for voice restoration following laryngectomy. An easy and safe method is described for the insertion of a Provox® voice prosthesis using a Yankauer tonsil suction tube and an antrum trocar with an irrigation cannula.

AbstractWe investigated serum interleukin (IL)-1β, IL-4, IL-6, IL-8 and tumor necrosis factor (TNF)-α levels in 17 children aged 5–12 years (mean 7) with chronic tonsillitis before and after tonsillectomy. Cytokine concentrations were measured by enzyme-linked immunosorbent assay. IL-1β and IL-6 levels were significantly higher than the control levels (p < 0.05) in preoperative serum samples. Other cytokine levels were within normal limits. After tonsillectomy, IL-1β and IL-6 levels were significantly reduced (p < 0.05). It is suggested that IL-1β and IL-6 may be mediators which have a role in chronic tonsillitis disease and that tonsillectomy has a significant effect on preoperative IL-1β and IL-6 serum levels.Copyright © 2002 S. A choanal polyp(CP) can be defined as a solitary soft tissue lesion that passes through the sinus ostia and extends toward the choana. Although a CP usually originates from the maxillary sinus, unusual origins have been reported.

Irrigation with isotonic saline is one of the most frequently used solutions after nasal surgery. However, the effect of saline solutions on mucociliary clearance is not well known. In a previous study, it was found that isotonic saline solution had a negative effect on ciliary beat frequency but Ringer-Locke solution had no effect in vitro. In this study we compared the effects of Ringer-Lactate solution and isotonic saline solution on mucociliary transport time before, and after, nasal septal surgery in patients with nasal septal deviation. We found that patients who used Ringer-Lactate solution as irrigation after surgery had a significantly better mucociliary transport time than the patients using isotonic saline solution (p < 0.05). In conclusion, it is better to use Ringer-Lactate solution instead of 0.9 per cent saline solution for nasal irrigation.
