2. Echocardiography, a non-invasive method for the assessment of cardiac function and morphology in preclinical drug toxicology and safety pharmacology

Abstract

Background: Echocardiography (EC) is a method used for the investigation of cardiac morphology and function. Two-dimensional EC gives a visualisation of the morphology of the heart. M-mode EC allows heart function to be monitored. Pulsed Doppler EC is the method of choice to measure blood flows. Objective: To describe the information EC can provide for cardiovascular investigation in laboratory animals, with a special focus on the potential helpfulness of EC in preclinical toxicology and safety pharmacology. Methods: This review includes publications describing the methodology of EC and its application to several animal species used in biological experimentation. Results/conclusion: EC has been established in dogs, monkeys, rodents, rabbits and pigs. As demonstrated by experiments in different species, EC can be particularly helpful in toxicology and safety pharmacology, based on the amount of information it can give on the causes and consequences of drug adverse effects on the cardiovascular system. Furthermore, EC does not require any surgery and is therefore a key refinement compared to invasive methods generally used for investigating the cardiovascular function in laboratory animals. Despite some limitations of the method (the need for trained people, time required for an accurate EC recording, lack of current validation), EC should be further developed in preclinical toxicology and safety pharmacology.

We have elaborated a new software called ACS designed for left ventricle 3-dimensional reconstruction and quantitative analysis of regional wall motion using gated blood-pool tomography (GBPS). This paper presents the principle and the validation of this software.

5. Technetium‐99m ECD Single Photon Emission Computed Tomography in Brain Trauma: Comparison of Early Scintigraphic Findings With Long‐Term Neuropsychological Outcome

Abstract

Objective. The aim of this study was to assess the prognostic value of early brain scintigraphy in head injury in relation to long‐term neuropsychological behavior. Twenty‐four patients underwent technetium‐99m (Tc‐99m) ECD single photon emission computed tomography (SPECT) within 1 month of the trauma. Scintigraphic abnormalities were evaluated both visually and semiquantitatively using the brain‐to‐cerebellum ratio method. The clinical neuropsychological investigation was conducted to evaluate abnormalities related to motor deficit, frontal behavior, and memory and language disorders. All patients had abnormalities on SPECT scan. One year after trauma, 14 patients (58%) had neuropsychological sequelae. The brain‐to‐cerebellum ratios in the left basal ganglia and brain stem were significantly decreased in patients with memory disorders (P= .03 and P= .02, respectively). Moreover, SPECT visual analysis indicated that low uptake in the basal ganglia, thalamus, and brain stem was associated with subsequent motor deficit, frontal behavior, and language and memory disorders. The authors conclude that brain SPECT can be valuable in predicting the neuropsychological behavior of survivors of severe head injury.

6. Fourier phase analysis of SPECT equilibrium radionuclide angiography in symptomatic patients with mitral valve prolapse without significant mitral regurgitation: Assessment of biventricular functional abnormalities suggesting a cardiomyopathy

Abstract

Background: Ventricular premature beats are common in patients with mitral valve prolapse (MVP). The purpose of this study was to determine whether symptomatic patients with MVP had certain functional characteristics and if ventricular arrhythmia (VA) could be explained by functional extravalvular abnormalities. Single photon emission computed tomography equilibrium radionuclide angiography with Fourier phase analysis was preferred to the planar radionuclide method. Only patients without significant mitral regurgitation were studied. Methods and Results: A total of 23 symptomatic patients with MVP (13 men, 10 women, mean age, 47 ± 14 years) without mitral regurgitation underwent single photon emission computed tomography equilibrium radionuclide angiography. Symptoms were present in 20 patients, and VA was present in 14 patients. Ejection fraction, regional wall motion, and Fourier phase analysis were examined in both ventricles and compared with results for normal subjects. Ventricular abnormalities were observed in 20 (87%) patients: decreased left ventricular and right ventricular ejection fractions, increased standard deviations of the mean phase and focal wall motion, and/or delayed phase abnormalities. Abnormalities were less frequent but more marked in the right ventricular free wall, the infundibulum, or the septum compared with left ventricular delayed abnormalities, which were more frequent but limited. In 12 of 14 patients with VA, phase-delayed areas were observed in the ventricle where the origin of ventricular premature beats was suspected on the basis of their electrocardiographic morphologic features. A relation was found between late potentials and delayed-phase areas (right ventricle or septum) and left bundle branch block morphologic features of VA. Conclusions: Symptomatic patients with MVP frequently have ventricular dysfunction in 1 or both ventricles, sometimes limited but more marked in the presence of severe VA even without significant mitral regurgitation, suggesting structural modification. The use of a sensitive, accurate, and 3-dimensional method such as single photon emission computed tomography equilibrium radionuclide angiography may be of interest for a noninvasive investigation, especially in young symptomatic patients with MVP and VA.

Abstract

Phase imaging assessed with a 3-dimensional analysis is particularly interesting for its capacity to detect an area with early contraction corresponding to the site of VT with accuracy and to demonstrate 1 or several areas with delayed contraction because of the loss of contractile tissue by fatty or fibrofatty replacement, as has been suggested with magnetic resonance images. We believe that this 3-dimensional Fourier analysis of SPECT ERNA should be widely used in sinus rhythm and when possible, during episodes of VT before treatment by ablation techniques or surgery.

8. 0334: Coronary artery calcium score as a predictor of myocardial ischemia in asymptomatic diabetic patients.

Abstract

Background and aims High coronary artery calcium (CAC) scores were shown to predict a higher likelihood of inducible myocardial ischemia and to be associated with a poor cardio-vascular prognosis. However the predictive value for coronary stenoses (CS) has not been tested in asymptomatic diabetic patients. This study aimed to evaluate the predictive value of a high CAC score for silent myocardial ischemia (SMI) and CS in high risk asymptomatic diabetic patients. Materials and methods CAC score was measured by computed tomography in 150 diabetic patients without cardiac history or symptom, with a normal resting ECG and ≥1 additional risk factors. SMI was assessed using stress myocardial scintigraphy and/or stress echocardiography, and CS using coronary angiography in those with an abnormal SMI test. Results CAC score was ≥100 Agatston units in 35.3% of the patients. SMI was detected in 27 patients (18.0%). A coronary angiography was performed in 17 of SMI patients and detected significant CS in six of them. CAC score was associated with coronary status (no SMI: median value 14 (range 0-2900); SMI without CS: 101 (23-3230) and SMI with CS: 800 (76-2978); (p<0.01)), and similarly for a CAC score ≥100 (in 37/123 patients without SMI (30.2%), 6/11 with SMI and no CS (54.5%) and 5/6 patients with SMI and CS (83.3%) (p<0.05). A CAC score ≥100 predicted the presence of SMI: 30.2% of patients had SMI vs 11.3% of those with CAC <100 (odds ratio 3.4 [1.4-8.0], p<0.01), with a sensitivity and specificity of 59% and 70%, and positive and negative predictive values of 30 and 87%, respectively. Conclusion These data suggest that in asymptomatic high risk diabetic patients CAC score is associated with cardiac ischemic status, with a 3.4-fold increased risk of SMI when the score is ≥100. The negative predictive value of CAC score for SMI is an interesting finding. The predictive value for CS remains to be determined in a larger sample of patients with CS. [ABSTRACT FROM AUTHOR]
