Persistent or chronic intestinal ischemic injury (i3) can lead to severe malnutrition and acute mesenteric ischemia. Although recommended, revascularization of splanchnic arteries is sometimes unrealizable.

In cirrhotic patients, brain natriuretic peptide (BNP) serum level before liver transplant (LT) was an independent negative predictor of post-LT ICU mortality. The use of this biomarker in combination with MELD score could be useful to predict post-LT early outcome.

One of the main concerns after living donor liver transplantation is the risk of morbidity and/or mortality that it imposes on the donors. Respiratory postoperative complications in living liver donors have already been reported but their frequency seems to be underestimated. We designed a prospective study to evaluate the rate and the nature of postoperative pulmonary complications in 112 consecutive donors.

ABSTRACTWe investigated the prevalence, molecular epidemiology, and clinical significance of heterogeneous glycopeptide-intermediate Staphylococcus aureus(hGISA) isolates in 48 liver transplant recipients infected or colonized with methicillin-resistant S. aureusover a 5-year period. Strains were screened for hGISA on Mueller-Hinton agar containing 5 mg of teicoplanin per liter. Heterogeneous glycopeptide resistance was confirmed by the E-test method with a dense inoculum and a simplified method of population analysis. hGISA strains were found in 13 (27%) of the 48 patients studied. Eleven of the 13 strains shared a common multiresistant phenotype with homogeneous methicillin resistance and gentamicin resistance, and they were closely related according to the results of pulsed-field gel electrophoresis. Only 2 of the 13 patients infected or colonized with hGISA strains had previously received glycopeptide therapy. Most patients were successfully treated with vancomycin, but one patient who failed to respond to vancomycin subsequently died. These results suggest that the high prevalence of hGISA among our patients was due to the clonal spread of a multiresistant strain.

L’infection à Clostridium difficile(ICD) est une infection fréquente en réanimation associée à une surmorbidité et une surmortalité. L’immunodépression est susceptible d’en aggraver les conséquences. Peu de données sont disponibles concernant l’ICD en transplantation hépatique (TH). Le but de cette étude est d’identifier l’incidence des ICD en réanimation après TH, d’évaluer les différents facteurs favorisants et d’estimer la morbidité et la mortalité liées à l’ICD.

L’encéphalopathie hépatique (EH) est une pathologie fréquente en cas de maladie hépatique, aussi bien en cas d’insuffisance hépatique aiguë (IHA) qu’en cas de cirrhose. En cas d’IHA, elle est associée à des perturbations de l’hémodynamique intracérébrale et de l’autorégulation cérébrale pouvant aboutir à une hypertension intracrânienne [1]. Dans ces circonstances, les modalités de surveillance neurologique sont très peu étudiées. L’indice de pulsatilité (IP) mesuré par Doppler transcrânien (DTC) pourrait permettre une meilleure surveillance dans cette population spécifique. L’objectif de ce travail était de décrire l’évolution de l’IP avant et au décours de la TH chez des patients souffrant d’EH dans un contexte d’IHA ou de cirrhose.
