Selective outcome reporting (SOR) can threaten the validity of results found in clinical trials. Some studies in the literature have analyzed SOR in dentistry, but there is no study that has observed SOR in clinical trials in oral and maxillofacial surgery. Impacted third molar surgery is one of the most used models in clinical trials to study mainly analgesic and anti-inflammatory drug interventions. Our study aimed to evaluate the prevalence of SOR in publications employing the third molar extraction clinical trial model, and to verify whether there was an association between the statistical significance of outcomes and other characteristics that could lead to SOR. A systematic search was performed on the ClinicialTrials.gov platform for randomized clinical trial protocols, using the condition of third molar extraction. The corresponding published articles were sourced in PubMed, Scopus, and Embase databases, and compared with the registered protocols regarding the methodological data, in terms of: sample calculation, primary outcome identification, end-point periods, insertion of new outcomes in the publication, and results of outcomes. 358 protocol records were retrieved; 87 presented their corresponding articles. SOR was identified in 28.74% of the publications, and had a significant relationship with changes in the protocol, insertions of new outcomes, and discrepancies in the types of study. General risk of bias was found to be low. There were associations between SOR and the discrepancies in terms of the type of study, the choice of new outcome, and changes in the history of protocol records. The prevalence of SOR in clinical research using the third molar extraction surgery model is moderate. The quality of the scientific reporting of the results and, consequently, the certainty of evidence relating to the intervention tested can be overstated, increasing the chances of misinterpretation by health professionals. Rehabilitation of atrophic maxilla with allografts could be sometimes challenging in the choice. The present pilot clinical trial aimed to compare the change in stability over time of implants placed in particulate demineralized freeze-dried bone (DFDBA) or fresh-frozen bone (FFB) allografts in the posterior maxilla and to examine the microarchitecture of new bone formation. Healthy adult patients of both genders underwent bilateral maxillary sinus lifts. Only patients that presented with maxillary sinus pneumatization and who on tomographic examination were determined to have a residual bone height of less than 7 mm were eligible for inclusion. Each side was randomly grafted with DFDBA or FFB. Six to nine months after grafting, at the time of dental implant installation bone core biopsies, 10 mm in length, were obtained using a 2 mm short trephine drill. The bone biopsies were fixed in buffered formalin and randomly and blindly submitted to microcomputed tomography and histomorphological analysis. Implant stability was measured using resonance frequency equipment at three time points: immediately, 6, and 12 months after dental implant installment. A total of 10 patients were finally included. An increased bone volume was observed tomographically for all allografted regions. Implants placed in FFB show a significant enhanced stability after six months. Specially after one-year implant stability was significantly greater in FFB than in DFDBA grafts. Microtomographic analyses revealed that bone morphometric parameters were similar between allografts. Background: Recombinant human bone morphogenetic protein 2 (rhBMP-2) is an osteoinductor frequently used for bone regeneration in oral and maxillofacial surgery. There is no consensus about the ideal carrier for this growth factor. The aim of this study was to compare the bone augmentation, bone microarchitecture, and biodegradation rate of additional carriers to rhBMP-2/absorbable collagen sponge (ACS) in a vertical guided bone regeneration model.Methods: Four titanium cylinders were fixed onto the calvaria of rabbits (n = 20) that received (n = 10) or not (n = 10) rhBMP-2/ACS in conjunction with one of the carriers: beta-tricalcium phosphate (β-TCP), biphasic calcium phosphate (BCP), bovine bone mineral (BBM) or blood clot. The samples were analyzed by means of microcomputed tomography and histomorphology after 14 weeks.Results: All the materials with rhBMP-2/ACS exhibited improvement on bone augmentation, mainly BCP (P = 0.033) and β-TCP (P = 0.038), in the upper portion of the cylinder. Although trabecular anisotropy was improved in all the materials groups, trabecular connectivity was diminished when the biomaterials received rhBMP-2/ACS. Resorption rate of the remaining biomaterial was improved by rhBMP-2/ACS, mainly in BBM (P <0.01) and β-TCP (P <0.01). BBM exhibited the highest osteoclast density compared with the other materials groups.Conclusions: BCP and β-TCP biomaterials exhibited a synergic effect with rhBMP-2/ACS, acting as suitable and viable carriers for vertical bone augmentation. Evaluate topical use of hydrocortisone as a trans-operative irrigant solution for controlling postoperatory edema and pain in third molar surgery. Twenty-eight healthy patients with necessity of impacted bilateral third molar removal were divided in two groups: group H received 250 ml 500 mg-hydrocortisone solution irrigation during surgery and control group received saline solution at the same volume. Edema was assessed by the percentage of swelling after 48 h postoperative measured by face anatomical points distances. Postoperative pain level parameters considered: measurement in Visual Analog Scale (VAS) at the end of anesthetic perception and at 4, 6, 8, 24 and 48 h after surgery, number of analgesic intakes, time interval between the end of the surgery and the use of the first analgesic tablet. Group H showed significant reduction of edema but similar postoperative pain level comparatively to control group. Trans-operative hydrocortisone solution irrigation is a useful, of low-cost alternative for control swelling in mild and high complex impacted inferior third molar surgeries. [ABSTRACT FROM AUTHOR]

The odontogenic keratocyst (OKC) is a recurrent cyst that has been recently reclassified from an odontogenic tumor to an odontogenic cyst. The aim of the present study was to investigate its treatment and address issues related to its association with nevoid basal cell carcinoma syndrome (NBCCS). Lesions from the cohort of patients included in the present study consisted of 40 OKCs, of which 27 lesions were treated by enucleation (GE) and 13 underwent decompression (GD). Complementary treatment occurred in 38 (95%) lesions, of which 10 underwent isolated peripheral ostectomy (GO) and 28 underwent peripheral ostectomy combined with Carnoy's solution (GC). Thirteen lesions were associated with NBCCS (GS), while the others (n=27) were non-syndromic lesions (GnS). The recurrence-free periods (RFP) in the sample groups were compared using the Kaplan-Meier function and log-rank test at a significance level of 5% (p < 0.05) and were used to calculate the cumulative risk of recurrence (CRR) in each postoperative year. During the follow-up period, which had a mean of 43.5 months (range: 12-102 months), six (15%) recurrences were diagnosed. There was no significant difference among the RFP for the compared groups (p > 0.05) or increased CRR for the decompression (15.4%) over five years. Application of Carnoy's solution did not increase the efficacy of the peripheral ostectomy, but was related to a CRR of 0% for the syndromic lesions over five years. Therefore, 1) decompression did not increase the recurrence risk; 2) peripheral ostectomy demonstrated a similar efficacy as the combination with Carnoy's solution; 3) the association of NBCCS did not seem to significantly influence OKC recurrence; and 4) syndromic lesions seem to behave in the same manner as non-syndromic lesions when submitted to complementary treatments. Objective: The objective of this study was to assess the outcomes of secondary alveolar bone grafting (SABG) in patients with complete, unilateral cleft lip and palate (UCLP) operated on before eruption of the permanent canine. Study design: Sixty-five periapical radiographs from 41 patients with left UCLP and 24 with right UCLP (9 to 12 years old at SABG), were analyzed retrospectively for the amount of bone in the cleft site according to the Bergland and Chelsea scales, and for the occurrence of canine eruption (CE) through the neoformed bone. Results: Of the cases, 71% were classified as Bergland type I and Chelsea type A; 15% as types II/C, and 14% could not be classified. CE was observed in 95% of the cases operated on 4 years before the study. Conclusions: SABG performed before CE is a procedure with a high rate of success. The origin of paradental cysts is related to inflammatory processes, especially pericoronaritis involving impacted or semi-impacted teeth. The authors present a case of paradental cyst related to lower second molar that did not show clinical evidence of inflammatory process. Traumas to deciduous teeth may have severe consequences. This article addresses a dentigerous cyst case report associated with an upper permanent incisor, unusual site of occurrence, which was impacted and dislodged from its natural site of eruption after trauma at the deciduous predecessor. The main aspects of etiopathology and its clinical characteristics are also discussed, with especial focus on the radiographic features of the diagnosis in order to allow for an accurate surgical planning. Objective: The aim of this study was to assess analgesic and adjuvant anesthetic effects of submucosal tramadol after third molar extraction. Study Design: In this double-blind, split-mouth, placebo-controlled, single-dose, crossover investigation, 52 patients underwent mandibular third molar extraction under local anesthesia. Surgical side was randomly assigned to submucosal 2 mL 100 mg tramadol injection (group T) or normal saline solution (group P) immediately after surgery. Anesthetic blockade duration, time of intake and amount of analgesic rescue drug, and postoperative pain intensity were recorded immediately after anesthesia cessation and 4, 8, 24, 48, and 72 hours after surgery. Data were submitted to analysis of variance and Wilcoxon tests. Results: Anesthetic blockade duration between groups was similar. Group T took significantly less rescue drug after 72 hours (P = .008). Time elapsed before first intake of rescue drug was longer (P = .006), and pain intensity was significantly lower (P = .001) in group T. Conclusions: Submucosal tramadol injection after oral surgery improved postoperative analgesia, but did not extend anesthetic action duration. [Copyright &y& Elsevier]
