To identify antemortem CSF diagnostic biomarkers that can potentially distinguish between the 2 main causes of frontotemporal lobar degeneration (FTLD), i.e., FTLD with TDP-43 pathology (FTLD-TDP) and FTLD with tau pathology (FTLD-tau).

To quantify the isoprostane 8,12-iso-iPF2-VI in brain tissues obtained from patients with AD, patients with frontotemporal dementia (FTD), and controls.

Objective:To compare rates of cognitive decline between probable Alzheimer’s disease (AD) patients treated with long-duration cholinesterase inhibitors (ChE-Is) and those who remained untreated. Background:ChE-Is, including donepezil and tracrine, have shown beneficial effects on cognition and global functioning in patients with AD. The duration of these benefits is unknown because the longest double-blind placebo-controlled studies reported were only approximately 6 months long. Ethical concerns regarding randomization of patients to placebo for long periods make it difficult to undertake trials of longer duration. Methods:We identified patients in 4 AD centers who were or were not consistently treated with ChE-Is and who had demographic, psychometric and follow-up data. We compared 205 ChE-I-treated and 218 untreated AD patients on baseline variables hypothesized to differ between these groups, on baseline Mini Mental Status Examination (MMSE) scores and on rates of MMSE change at 1 year. The analysis was performed initially with all ChE-I-treated patients as a single group versus untreated subjects, and then with donepezil versus untreated subjects and tacrine versus untreated subjects. Results:As expected, treated and untreated patients differed with respect to age, education, ethnicity, percentage of community dwelling and exact days of follow-up (ANOVA and χ2) in several comparisons, but did not differ on baseline MMSE score. These baseline variables were highly intercorrelated. MMSE scores declined significantly more slowly after 1 year of ChE-I treatment compared to untreated patients (p = 0.05) after controlling for baseline differences in age, education, ethnicity and percentage of community dwelling. Slowing of decline was significant in the donepezil-treated patients (p = 0.007) but not in the tacrine-treated group (p = 0.33). Conclusions:This study, utilizing concurrent, nonrandomized controls, suggests that donepezil continues to have efficacy over at least the first year of therapy. Other studies are needed to determine whether the benefits are maintained beyond 1 year.

A case of a schizophrenic patient who suffered a frontal lobotomy is presented as the impetus for a discussion of an alternative neurobiologic model of schizophrenia to integrate the findings of prefrontal lobotomy and “hypofrontality”. The proposal that primary temporolimbic pathology may result in secondary hypofunction in the prefrontal lobes is examined in light of current structural neuropathology evidence. Directions for future research suggested by such a model are explored.

The purpose of this study was to determine if the discrepancy in measured intelligence between identical twins concordant for handedness differed measureably from the discrepancy between identical twins discordant for handedness. If handed concordant and discordant twins did differ in terms of the between twin discrepancy in measured intelligence, then one could infer that congenital factors had caused dissociation of handedness and measured intelligence within a twin set. This finding would support Nagylaki and Levy's contention that the found discordance rates for handedness in identical twins is a function of “pathogenic” congenital factors disrupting the genetically determined handedness pattern. More generally, this finding would also support Satz's model with respect to pathological shifts in handedness due to brain insult either in uteroor perinatally. However, in the current study, eight sets of identical twins were examined and no evidence was found to support these hypotheses. Instead, in one set where there was a marked discrepancy in measured intelligence, handedness could not be determined precisely for the twin with the lower intelligence quotient.

Dr Mark Horton, junior research fellow at St. Hugh's College, Oxford, and his wife, Kate Clark, a research fellow at the Institute of Industrial Archaeology at Ironbridge, have prepared a detailed report on their archaeological survey of Zanzibar and Pemba islands in 1984. They intended to follow up this year. Meanwhile, this note draws attention to some of the more important findings so far.
