1. Ileal Paneth Cell Phenotype is a Cellular Biomarker for Pouch Complications in Ulcerative Colitis.

Abstract: Background and Aims Biomarkers that integrate genetic and environmental factors and predict outcome in complex immune diseases such as inflammatory bowel disease (IBD; including Crohn's disease [CD] and ulcerative colitis [UC]) are needed. We showed that morphological patterns of ileal Paneth cells (Paneth cell phenotype [PCP]; a surrogate for PC function) is one such cellular biomarker for CD. Given the shared features between CD and UC, we hypothesised that PCP is also associated with molecular/genetic features and outcome in UC. Because PC density is highest in the ileum, we further hypothesised that PCP predicts outcome in UC subjects undergoing total colectomy and ileal pouch-anal anastomosis [IPAA]. Methods Uninflamed ileal resection margins from UC subjects with colectomy and IPAA were used for PCP and transcriptomic analyses. PCP was defined using defensin 5 immunofluorescence. Genotyping was performed using Immunochip. UC transcriptomic and genotype associations of PCP were incorporated with data from CD subjects to identify common IBD-related pathways and genes that regulate PCP. Results The prevalence of abnormal ileal PCP was 27%, comparable to that seen in CD. Combined analysis of UC and CD subjects showed that abnormal PCP was associated with transcriptomic pathways of secretory granule maturation and polymorphisms in innate immunity genes. Abnormal ileal PCP at the time of colectomy was also associated with pouch complications including de novo CD in the pouch and time to first episode of pouchitis. 2. Mucosal Single-Cell Profiling of Crohn's-Like Disease of the Pouch Reveals Unique Pathogenesis and Therapeutic Targets.

Abstract: The pathophysiology of Crohn's-like disease of the pouch (CDP) in patients with a history of ulcerative colitis (UC) is unknown. We examined mucosal cells from patients with and without CDP using single-cell analyses. Endoscopic samples were collected from pouch body and prepouch ileum (pouch/ileum) of 50 patients with an ileal pouch-anal anastomosis. Single-cell RNA sequencing was performed on pouch/ileal tissues of patients with normal pouch/ileum and CDP. Mass cytometry was performed on mucosal immune cells from patients with UC with normal pouch/ileum, CDP, pouchitis, and those with familial adenomatous polyposis after pouch formation. Findings were independently validated using immunohistochemistry. The cell populations/states in the pouch body differed from those in the prepouch ileum, likely secondary to increased microbial burden. Compared with the familial adenomatous polyposis pouch, the UC pouch was enriched in colitogenic immune cells even without inflammation. CDP was characterized by increases in T helper 17 cells, inflammatory fibroblasts, inflammatory monocytes, TREM1+ monocytes, clonal expansion of effector T cells, and overexpression of T helper 17 cells–inducing cytokine genes such as IL23 , IL1B , and IL6 by mononuclear phagocytes. Ligand-receptor analysis further revealed a stromal-mononuclear phagocytes-lymphocyte circuit in CDP. Integrated analysis showed that up-regulated immune mediators in CDP were similar to those in CD and pouchitis, but not UC. Additionally, CDP pouch/ileum exhibited heightened endoplasmic reticulum stress across all major cell compartments. CDP likely represents a distinct entity of inflammatory bowel disease with heightened endoplasmic reticulum stress in both immune and nonimmune cells, which may become a novel diagnostic biomarker and therapeutic target for CDP. 3. Polytrauma in Children: Epidemiology, Acute Diagnostic Evaluation, and Treatment.

Abstract: Background: Inadequate clinical experience still causes uncertainty in the acute diagnostic evaluation and treatment of polytrauma in children (with or without coagulopathy). This review deals with the main aspects of the acute care of severely injured children in the light of current guidelines and other relevant literature, in particular airway control, volume and coagulation management, acute diagnostic imaging, and blood coagulation studies in the shock room. Methods: This review is based on literature retrieved by a selective search in PubMed, Medline (OVIDSP), the Cochrane Central Register of Controlled Trials, and Epistemonikos covering the period January 2001 to August 2023. Review articles and the updated S2k clinical practice guideline on polytrauma management in childhood were considered. Results: Most accidents in childhood occur at home and in the child's free time, with varying mechanisms and patterns of injury depending on age. The outcome of treatment depends largely on the presence or absence or traumatic brain injury, which affects 66% of children with polytrauma and is thus the most common type of injury in this group, and of hemorrhagic shock with or without coagulopathy. Acute care follows the ABCDE algorithms with attention to special features in children, including age-specific reference values. According to a registry study, coagulopathy and hypovolemic shock are associated with 22% and 17% mortality, respectively. Treatment in a pediatric trauma reference center of the trauma network is recommended. Computed tomography (CT) should be carried out in children in accordance with defined criteria (PECARN), as a team decision and with the use of age-specific low-dose CT protocols. In children as in adults, viscoelasticity-based point-of-care tests enable the prompt diagnosis of relevant coagulopathies and their treatment in consideration of age-specific target values. The administration of tranexamic acid remains controversial. Conclusion: 4% of polytrauma patients are children. Because children differ from adults both anatomically and physiologically, the diagnostic evaluation and management of polytrauma in children presents a special challenge. The evidence base for pediatric polytrauma management is still inadequate; current recommendations are based on consensus, in consideration of the special features of children compared to adults. 4. WATER QUALITY OF SOME FISH PONDS FROM THE REPUBLIC OF MOLDOVA AND ROMANIA.

Abstract: The study aims to determine the chemical composition of water in six ponds from the Republic of Moldova (Falesti and Glodeni districts) and two ponds from eastern part of Romania (Iasi and Botosani counties) and to evaluate their water quality as one of the vital resources for an efficiency-oriented fish production and from an environmental perspective. Instantaneous samples were collected in April and July 2021 in Moldovan ponds and in May 2021 - in Romanian ones. It was revealed that the thermal regime of water, regime of oxygen, content of nutrients, main ions and mineralization were favourable for the growth of European carp and Asian cyprinids. In 38% of analysed samples water pH was equal or higher (≥9) than the maximum allowable value for summer fish ponds. CO32- + HCO3- dominated among the anions and Na++K+ - among the cations in the water of Romanian ponds and two ponds from the Republic of Moldova. In other four ponds the dominance of SO42- among anions and of Na++K+- among cations was recorded. Increased mineralization of water in the studied Moldovan ponds (from 2.14 to 6.11 g/L, depending on the pond and sampling time) offers the opportunity to cultivate fish species that live in saline waters, for example, the pelengas mullet (Mugil soiuy). Even the investigated ponds are located in areas with similar altitude and climate conditions, their water quality revealed high differences, the main factors being the type of water supply, presence or absence of water exchange and the applied management practices. 5. Electromagnetics-Aware Path for UAV-Based Near-Field Antenna Measurements Implemented With a High Dynamic Range SDR

Abstract: An electromagnetics-aware path for drone-based near-field (NF) antenna measurements is implemented in an outdoor experimentation. This technique allows us to significantly reduce the scan duration with respect to a standard half-wavelength NF scanning, using knowledge of the antenna under test (AUT) electrical size only. The validity of our approach is corroborated with planar outdoor NF measurements performed with an untethered drone transmitting at 750 MHz. To deal with the absence of phase coherence, the phase information is retrieved from a reference antenna, while a software-defined radio (SDR) having four super-heterodyne receiving channels is used to properly sample the high-dynamic signals resulting from the large scanning surface.

8. UAV-Based Measurement of Sharp Spectral Resonances in Mutually Coupled SKA-Low Elements

Abstract: Strong mutual coupling between antennas can have detrimental effects in very sensitive radio telescopes based on aperture arrays. For the low-frequency instrument of the square kilometer array, narrowband perturbations in the radiation patterns of the inner elements have been predicted through electromagnetic simulations. This phenomenon deteriorates the spectral smoothness properties and disturbs the station calibration at very specific frequencies. In this letter, we present a new unmanned aerial vehicle (UAV)-based methodology to experimentally characterize such narrowband phenomena with a high-frequency resolution by means of independently-sweeping transmitting and receiving equipment. This solution does not add complexity to the system (e.g., by tethering the UAV). Results and predictions are in good agreement confirming both the presence of spurious resonances and the validity of the simulation approach.

9. Sleep Disturbance and SARS–CoV‐2 Vaccinations in Patients With Chronic Inflammatory Disease

Abstract: Immunocompromised patients with chronic inflammatory disease (CID) may have experienced additional psychosocial burden during the COVID‐19 pandemic due to their immunocompromised status. This study was undertaken to determine if vaccination would result in improved patient‐reported outcomes longitudinally among individuals with CID undergoing SARS–CoV‐2 vaccination regardless of baseline anxiety. Data are from a cohort of individuals with CID from 2 sites who underwent SARS–CoV‐2 vaccination. Participants completed 3 study visits before and after 2 messenger RNA vaccine doses in the initial vaccination series when clinical data were collected. Patient‐reported outcomes were measured using the Patient‐Reported Outcomes Measurement Information System 29‐item Health Profile and expressed as T scores, with 2 groups stratified by high and low baseline anxiety. Mixed‐effects models were used to examine longitudinal changes, adjusting for age, sex, and study site. A total of 72% of the cohort was female with a mean ± SD age of 48.1 ± 15.5 years. Overall, sleep disturbance improved following both doses of SARS–CoV‐2 vaccinations, and anxiety decreased after the second dose. Physical function scores worsened but did not meet the minimally important difference threshold. When stratifying by baseline anxiety, improvement in anxiety, fatigue, and social participation were greater in the high anxiety group. Physical function worsened slightly in both groups, and sleep disturbance improved significantly in the high anxiety group. Sleep disturbance decreased in a significant and meaningful way in patients with CID upon vaccination. In patients with higher baseline anxiety, social participation increased, and anxiety, fatigue, and sleep disturbance decreased. Overall, results suggest that SARS–CoV‐2 vaccination may improve mental health and well‐being, particularly among those with greater anxiety.

11. Fast Antenna Characterization With Numerical Expansion Functions Built With Partial Knowledge of the Antenna and Accelerated Asymptotic Methods

Abstract: We describe a method to use numerical simulations concurrently with measurements (near-field data) to characterize the radiation properties of an antenna (far-field) with a strongly reduced number of samples. Both the simulations and experimental data are dealt with as sources of information toward obtaining the desired characterization. Simulations compensate for data scarceness, allowing to obtain an accurate characterization with fewer data than with measurements only, and the few measured data allow the comprehensive numerical proxy to get tuned to the true antenna under test (AUT). The method uses partial knowledge of the antenna and leverages accelerated asymptotic numerical methods to perform the numerical simulations needed by the algorithm. We report experimental tests of performance, both in terms of accuracy and speed-up with respect to the classical methods, with reflector antennas up to 27 wavelength. It is important to note that good results are obtained both when samples are taken on irregular optimized grids and on regular grids usually resulting from the conventional measurement rigs.

Abstract: Little is known regarding the reactogenicity and related SARS–CoV‐2 vaccine response in patients with chronic inflammatory disease (CID). Our objective was to characterize the adverse event profile of CID patients following SARS–CoV‐2 vaccination and understand the relationship between reactogenicity and immunogenicity of SARS–CoV‐2 vaccines. CID patients and healthy controls eligible to receive messenger RNA (mRNA) SARS–CoV‐2 vaccines participated in 3 study visits (pre‐vaccine, after dose 1, and after dose 2) in which blood and clinical data were collected. Assessment of adverse events were solicited within 7 days of receiving each dose. Serum anti–SARS–CoV‐2 spike IgG ± antibody titers were quantified following vaccination. Statistical analysis was performed utilizing mixed models and tobit regressions, with adjustment for covariates. The present study included 441 participants (322 CID patients and 119 control subjects). Compared to controls, CID patients reported greater symptom severity after dose 1 (P= 0.0001), including more myalgia and fatigue (P< 0.05). For immunogenicity, a higher symptom severity after dose 1 and a higher number of symptoms after dose 2 was associated with higher antibody titers (P≤ 0.05). Each increase of 1 symptom was associated with a 15.1% increase in antibody titer. Symptom association was strongest with site pain after dose 1 (105%; P= 0.03) and fatigue after dose 2 (113%; P= 0.004). Patients with CID have a distinct reactogenicity profile following SARS–CoV‐2 vaccination compared to controls. Furthermore, there is an association between increased reactogenicity and increased vaccine response. This finding may speak to the more variable immunogenicity in CID patients and may be an important indicator of vaccine response to the novel SARS–CoV‐2 vaccines.

Abstract: Purpose: to assess the level and degree of formation of swimming skills among swimmers-prize-winners in the distances of sports and marathon swimming at the Olympic Games at the I-III stages of their holding from 1896 to 2021.Subject of research: monitoring of the dynamics of the results of the formation of swimming skills by the level of achievements of swimmers-medalists at all Olympic Games at distances of sports swimming at the IIII stages of their holding. Material: the official protocols of competitions at all past Olympic Games at I-III stages of holding, respectively: 1896-1948, 1952-1992, 1996-2021 (among 193+420+387=1000 men and 75+388+387=850 women). Results. At the first stage of the Olympic Games from 1896-1948, the age of swimmers-medalists (men and women: 22-19 years) was determined. The overall difference in the average speed of swimming at all distances in different ways was 0.17 m/s (1.35-1.18). At the second stage of the Olympic Games from 1952-1992, the age of the swimmers-medalists was revealed: the difference between men and women was 1-4 years (21-19 years). The overall difference in the average speed of swimming at all distances in different ways was 0,17 m/s (1,70-1,53). At the third stage of the Olympic Games from 1996-2021, the difference of age of the swimmers-medalists was 1-3 years (24-23 years). The overall difference in the average swimming speed was 0.16 m/s (1,82-1,66). The smallest difference of speed in marathon swimming at 10000 m - 0,11 m/s, in stayer freestyle swimming - 400-800-1500 m at the level - 0,12 m/s, in complex swimming 200-400 m - 0,15 m /s, in relay swimming - 0,15 m/s, breaststroke at 100-200 m to - 0,17 m/s, in backstroke swimming up to - 0,18 m/s at 100-200 m, as well as in swimming 100-200 m butterfly - 0,19 m/s. The largest speed difference in swimming at 50-100-200 m freestyle was noted - 0,21 m/s. The overall difference of speed in all the results of swimmers-prize-winners in the formation of swimming skills in competitions from 1996-2021 was revealed at the level - 0,16 m/s (men - 1,82; women - 1,66; p<0,05). Abstract: Purpose: The International University Sports Federation, which includes swimming in its Summer World University Games, has received the University Games World Championship title since 1959. Swimming has been presented at the 1st Universiade since 1959, to date 30 have been held. The purpose of this study is to determine the indicators of swimming skills among men and women of different ages at 14 World Swimming Universities in Stage I (1959-1991), and among 14 facts Swimming World Universiade in Stage II (1993-2019). Material: The results of the swimmers-finalists were summarized and analyzed for Stage I among 538 men and 456 women and for Stage II, among 828 men and 816 women. Results: The formation dynamics of swimming athletic skills for various marathon distances for different age groups were identify for men and women and the total significant difference in average speed indicators for all distances for men and women were determined. The formation dynamics of physical swimming skills for various marathon distances for different age groups are presented in detail and significant differences in the average swimming speed for all marathon distances are determined. Conclusions: The obtained results will make a significant scientific and methodological contribution to the further improvement of physical education and sports in a gender approach in the process of training and improvement evaluation of youth in the physical education and sport. This can motivate the youth to achieve better swimming result at prestigious competitions, which are the summer swimming marathons at all World University Games, Stage I and II. This study presents the details of the past and present state development of trends in athlete skills formation among swimmers of all ages and gender over a long period of time: from birth, development and stabilization of the ability to properly and reliably improve to the level of prestigious competitions. 17. Smell and taste dysfunction during the COVID-19 pandemic: an infodemiological analysis.

Abstract: OBJECTIVE: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can cause smell and taste dysfunction. We aimed to investigate the general community's interest in smell dysfunction (SD) and taste dysfunction (TD) using Google Trends to compare results with more common symptoms associated with SARS-CoV-2 infection, such as fever and cough. MATERIALS AND METHODS: Relative Search Volumes (RSVs) for the English terms "Smell", "Taste", "Fever" and "Cough", filtered by the category "Health", were collected from 2018 through 2020. Moreover, RSVs using synonyms of "Taste" and "Smell" in 5 European languages were analyzed. RESULTS: The worldwide mean RSVs for "Fever", "Cough", "Smell", and "Taste" during 2020 were 49%, 34%, 8% and 9%, respectively. RSVs associated with the search terms "Fever" and "Cough" showed a peak between February and March 2020, as did "Smell" and "Taste". Even though RSVs were much lower, they were highly correlated (r=0.890). RSVs obtained from "Smell" and "Taste" in five European languages (German, English, French, Italian and Spanish) had similar temporal trends. CONCLUSIONS: O ur findings s how the level of the general population's interest for early symptoms, suggesting that their interest in SARS-CoV-2 infection symptoms, such as SD and TD, was scarce but peaked during the pandemic outbreak. 23. Immunity, immunotherapy, and rectal cancer: A clinical and translational science review.

Abstract: Rectal cancer remains a challenging disease to treat. Therapy for locally advanced rectal cancer (LARC), the most frequent presentation, has evolved to include a multimodal approach of radiation, chemotherapy, and surgery. While this approach improves local disease control, the distant recurrence rate is nearly 30% and treatment-related morbidity is substantial, thus underscoring the need for new therapeutic approaches with better efficacy and lower side effects. Immunotherapy could potentially fill this need, but its promise is not yet realized in rectal cancer. In this translational science review, we address what is known about how cytotoxic therapies shape rectal cancer immunity and potentially prime the tumor microenvironment for response to immune checkpoint inhibitors and other immunotherapies. We also address the role of current immunotherapies in colorectal cancer and highlight where novel immunotherapy approaches are currently being evaluated in LARC. Finally, we address important future directions in LARC immunotherapy including the need to define optimal therapeutic sequencing, predictive biomarkers, strategies to limit treatment-related side effects and the potential of gut microbiome manipulation to improve outcomes. Abstract: During the entire 18th century and during the first part of the next one Transylvania was confronted with repeated food crises (shortages due to poor crops and yields, weather conditions, sometimes with intervals of famine, etc.). The difficulties in purchasing food were frequently amplified by the high prices at which food was sold, which made life difficult for the poor people. The reaction of a part of the inhabitants of the Transylvanian area to this situation was that of leaving the province (temporarily or permanently) in search of places that could ensure their livelihood. Most left for Moldavia and the Wallachia, but there were cases of crossings to the Hungarian plains, to the Russian area, etc. There were situations in which their emigration was determined only by the scarcity or lack of available food. In many others, the triggering factors of the emigration trend were more complex, as religious, social, economic and other factors, along with the food shortages, combined to increase the phenomenon. Our study aims to analyse the relationship between the food crises in this area and the emigration trend, making an inventory of them and outlining how the authorities, guided by populist principles, related to this challenge. 30. Risk of Major Adverse Cardiovascular Events in Immune-Mediated Inflammatory Disorders on Biologics and Small Molecules: Network Meta-Analysis.

Abstract: Recent studies raise concern for increased risk of major adverse cardiovascular events (MACE) with Janus kinase (JAK) inhibitors used to treat immune-mediated inflammatory disorders (IMIDs). We aimed to examine MACE risk with licensed biologics and small molecules used commonly between IMIDs: inflammatory bowel disease, rheumatoid arthritis, psoriasis/psoriatic arthritis, and ankylosing spondylitis. Data were obtained from systematic searches (from inception to May 31, 2022) in PubMed, Embase, Ovid Medline, Scopus, Cochrane Central, and ClinicalTrials.gov. Studies that assessed a predefined MACE (myocardial infarction, cerebrovascular accident, unstable angina, cardiovascular death, or heart failure) risk in those ≥18 years of age with IMIDs treated with anti-interleukin (IL)-23 antibodies, anti-IL-12/23, anti-tumor necrosis factor α antibodies (anti-TNF-α), or JAK inhibitors were included in a network meta-analysis using a random-effects model with pooled odds ratios (ORs) reported with 95% credible intervals (CrIs) by drug class and disease state. Among 3528 studies identified, 40 (36 randomized controlled trials and 4 cohort studies) were included in the systematic review, comprising 126,961 patients with IMIDs. Based on network meta-analysis of randomized controlled trials, regardless of disease state, anti-TNF-α (OR, 2.49; 95% CrI, 1.14–5.62), JAK inhibitors (OR, 2.64; 95% CrI, 1.26–5.99), and anti-IL-12/23 (OR, 3.15; 95% CrI, 1.01–13.35) were associated with increased MACE risk compared with placebo. There was no significant difference in the magnitude of the MACE risk between classes or based on IMID type. Anti-IL-12/23, JAK inhibitors, and anti-TNF-α were associated with higher risk of MACE compared with placebo. The magnitude of the increased MACE risk was not different by IMID type. 31. Laryngeal Taping as a Supportive Tool to Relieve Phonasthenia in Singers: A Preliminary Report.

Abstract: This report aims to perform a preliminary evaluation of the role of taping in the improvement of phonasthenia among professional voice users. The larynx is a vital organ but also a work instrument for many people. Prevention of erroneous use of the vocal mechanism is essential for an optimal voice performance. Nineteen singers complaining voice fatigue underwent taping application in peri-laryngeal areas of the anterior neck for 10 days. Taping effects were estimated by the evaluation of voice analysis parameters (jitter, shimmer, noise to harmonic ratio and singing power ratio) performed in M1 and M2 mechanisms with PRAAT software before and after treatment. In addition, a subjective assessment of phonasthenia was performed using a self-administrated questionnaire. Statistically significant reduction of Jitter% in M1 (P = 0.021) was reported at the end of treatment; also, an improvement of NHR in M2 was found immediately after the application (P = 0.012) and after 10 days (P = 0.002). These outcomes suggest possible beneficial effects of laryngeal taping on relieving voice fatigue, and therefore improving voice quality. Self-assessment results are consistent with this finding. Since the application of muscular taping in phoniatrics is still in its early stage, further studies and in particular a standardized protocol of application, could facilitate the spread of this technique, also offering help in comparing results. Abstract: Objective: To investigate the association between genetic polymorphisms and sudden sensorineural hearing loss (SSNHL). Most of the SSNHL cases still remain idiopathic, and several etiopathogenetic hypotheses, including a genetic predisposition, have been proposed.Methods: A literature review was conducted using different databases: Medline/PubMed, EMBASE, and CINAHL, according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. All databases have been searched from May 2016 to April 2020.Results: Genetic susceptibility could represent a key element in the pathogenesis of SSNHL. A number of genetic polymorphisms related to (1) inner ear microvascular disease and endothelial dysfunction and (2) to inner ear oxidative stress and inflammation have been addressed in the current literature.Conclusions: The potential identification of a genetic profile related to SSNHL could provide a more accurate prognostic evidence of idiopathic SSNHL (ISSNHL), offering to the patients not only early-prevention strategies but eventually information on various inheritance modalities.

33. Rehabilitation of Severe to Profound Sensorineural Hearing Loss in Adults: Audiological Outcomes

Abstract: The aim of this article is to describe the audiological patterns of 71 adult patients presenting severe to profound sensorineural hearing loss, who were rehabilitated by cochlear implants (CIs) and hearing aids. This is a retrospective study in a university setting, where the clinical records of 71 adult patients were reviewed and processed. Speech intelligibility was evaluated at one aided ear (CI) or at both aided ears (double CI or a combination of CI and hearing aid [HA]). Patients with a bilateral CI or with a bimodal hearing setup (CI and HA) performed better than those with a single CI; data from the phonetic matrices test showed that there was a statistically significant difference among patients aided by a single CI versus binaural setup (double CI or CI + HA). In particular, patients aided by a bilateral CI, or by a CI and HA, showed an improvement in the functional results of the speech tests, compared to patients using a single CI. Binaural hearing (either with a bilateral CI or bimodal) allows an improvement in the functional results at the speech tests, compared to the use of a CI only.

34. Frequency of Usher gene mutations in non-syndromic hearing loss: higher variability of the Usher phenotype

Abstract: Non-syndromic hearing loss (NSHL) is characterized by a vast genetic heterogeneity; some syndromic forms as Usher syndrome (USH) have onset as isolated deafness and then evolve later in life. We developed an NGS targeted gene-panel containing 59 genes and a customized bioinformatic pipeline for the analysis of DNA samples from clinically highly selected subjects with sensorineural hearing loss, previously resulted negative for GJB2mutations/GJB6deletions. Among the 217 tested subjects, 24 (11.1%) were found to carry mutations in genes involved both in NSHL and USH. For 6 out of 24 patients a diagnosis of USH was performed. Eleven subjects out of 24 had hearing loss without vestibular or ocular dysfunction and, due to their young age, it was not possible to establish whether their phenotype could be NSHL or USH. Seven subjects were diagnosed with NSHL, due to their age and phenotype. A total of 41 likely pathogenic/pathogenic mutations were identified, among which 17 novel ones. We report a high frequency of mutations in genes involved both in NSHL and in USH in a cohort of individuals tested for seemingly isolated deafness. Our data also highlight a wider than expected phenotypic variability in the USH phenotype.

35. Dynamic immunoglobulin responses to gut bacteria during inflammatory bowel disease

Abstract: ABSTRACTAberrant immune responses against gut microbiota are thought to be key drivers of inflammatory bowel disease (IBD) pathogenesis. However, the extent and targets of immunoglobulin (Ig) A versus IgG responses to gut bacteria in IBD and its association with IBD severity is not well understood. Here, we address this by analyzing fecal samples from Crohn’s disease (CD), ulcerative colitis (UC), and Non-IBD patients by flow cytometry for the frequency of bacteria that were endogenously bound with IgA and/or IgG. Assessment of IBD patients from two geographically distinct cohorts revealed increased percentages of IgA- and IgG-bound fecal bacteria compared to non-IBD controls. Notably, the two major subsets of IBD showed distinct patterns of Ig-bound bacteria, with CD activity associated with increases in both IgA and IgG-bound bacteria, whereas UC activity correlated only with increases in IgG-bound bacteria. Analysis of the flow sorted Ig-bound bacterial repertoire by 16S rDNA sequencing revealed taxa that were Ig-bound specifically in IBD. Notably, this included bacteria that are also thought to reside in the oral pharynx, including Gemella, Peptostreptococcus, and Streptococcusspecies. These data show that the pattern of IgA and IgG binding to fecal bacteria is distinct in UC and CD. In addition, the frequency of Ig-bound fecal bacteria may have potential as a non-invasive biomarker for disease activity. Finally, our results support the hypothesis that immune responses to oral pharyngeal bacteria may play an important role in the pathogenesis of IBD.

Abstract: • This is the first clinical study evaluating the role of WMLs in subjects affected by SSNHL. • Cerebral MRI, usually performed in all SSNHL patients, could also have a prognostic role in the management of SSNHL. • According to the results of this study, the presence of WMLs can be linked to a poorer hearing recovery rate. Aim of this paper is to investigate the presence of White Matter Lesions (WMLs) in subjects affected by Sudden Sensorineural Hearing Loss (SSNHL) and possibly to evaluate the significance of WMLs in SSNHL patients. A total of 64 patients (cases) affected by SSNHL were included in this case-control study. Hearing tests were performed at SSNHL onset, after 7 days and after 30 days. Cerebral MRI sequences were performed to rule out retrocochlear pathology, and WMLs were evaluated if present. MRI control group included 32 subjects, without hearing loss, affected by pituitary adenoma, who underwent cerebral MRI of follow-up. WML presence in those affected by SSNHL resulted having a similar distribution to that of the control group; however, we observed complete hearing recovery in 42,9% of patients without WML and in 11,6% of patients with WML >1 (p = 0,017*). The incidence of WML in patients with SSNHL was not different compared to that of the control group; however, MRI could have a prognostic role for SSNHL patients, as the presence of WMLs can been linked to a poorer hearing recovery rate. 38. Anti-Tumour Necrosis Factor Therapy for Inflammatory Bowel Diseases Do Not Impact Serious Infections after Arthroplasty.

Abstract: Background and Aims There is a paucity of data on the safety of joint replacement surgery in patients with inflammatory bowel disease [IBD], including those on tumour necrosis factor-alpha inhibitors [anti-TNF]. We explored the risk of serious infections in this population. Methods A retrospective case-control study [2006–2014] was performed using the MarketScan Database. All patients aged 18–64 years with an International Classification of Diseases code for IBD and an IBD-specific medication, with ≥ 6 months of enrollment prior to hip, knee or shoulder replacement surgery, were included. Ten non-IBD controls were frequency-matched to each case on length of enrollment, year and the joint replaced. Primary outcome was serious infection [composite of joint infection, surgical site infection, pneumonia, sepsis] within 90 days of the operation. Cox proportional hazards models were used to assess the association of IBD and IBD medications with serious infection. Results More patients with IBD [ N = 1455] had serious infections than controls [3.2% vs 2.3%, p = 0.04], but not after controlling for comorbidities (hazard ratio [HR], 1.3; 95% confidence interval [CI], 0.95–1.76). Among IBD patients, corticosteroids were associated with increased risk of serious infection [HR, 4.6; 95% CI, 2.2–9.8; p < 0.01] while anti-TNFs were not. Opioids were also associated with increased risk of infection [HR, 1.5; 95% CI, 1.2–1.8; p < 0.01]. Conclusions After controlling for comorbidities, IBD patients were not at increased risk of serious infection following joint replacement. 39. Abnormal Small Intestinal Epithelial Microvilli in Patients With Crohn's Disease.

Abstract: Background & Aims Crohn disease (CD) presents as chronic and often progressive intestinal inflammation, but the contributing pathogenic mechanisms are unclear. We aimed to identify alterations in intestinal cells that could contribute to the chronic and progressive course of CD. Methods We took an unbiased system-wide approach by performing sequence analysis of RNA extracted from formalin-fixed paraffin-embedded ileal tissue sections from patients with CD (n = 36) and without CD (controls; n = 32). We selected relatively uninflamed samples, based on histology, before gene expression profiling; validation studies were performed using adjacent serial tissue sections. A separate set of samples (3 control and 4 CD samples) was analyzed by transmission electron microscopy. We developed methods to visualize an overlapping modular network of genes dysregulated in the CD samples. We validated our findings using biopsy samples (110 CD samples for gene expression analysis and 54 for histologic analysis) from the UNITI-2 phase 3 trial of ustekinumab for patients with CD and healthy individuals (26 samples used in gene expression analysis). Results We identified gene clusters that were altered in nearly all CD samples. One cluster encoded genes associated with the enterocyte brush border, leading us to investigate microvilli. In ileal tissues from patients with CD, the microvilli were of decreased length and had ultrastructural defects compared with tissues from controls. Microvilli length correlated with expression of genes that regulate microvilli structure and function. Network analysis linked the microvilli cluster to several other down-regulated clusters associated with altered intracellular trafficking and cellular metabolism. Enrichment of a core microvilli gene set also was lower in the UNITI-2 trial CD samples compared with controls; expression of microvilli genes was correlated with microvilli length and endoscopy score and was associated with response to treatment. Conclusions In a transcriptome analysis of formalin-fixed and paraffin-embedded ileal tissues from patients with CD and controls, we associated transcriptional alterations with histologic alterations, such as differences in microvilli length. Decreased microvilli length and decreased expression of the microvilli gene set might contribute to epithelial malfunction and the chronic and progressive disease course in patients with CD. 40. Prophylactic probiotics for cancer therapy-induced diarrhoea: a meta-analysis.

Abstract: Purpose of review Strong preclinical data support prophylactic probiotics as an effective preventive strategy for diarrhoea secondary to anticancer therapies. To determine the composite evidence that this approach translates to the clinic, we performed a meta-analysis of randomized controlled trials (RCTs) of prophylactic probiotics for the prevention of cancer therapy-induced diarrhoea. Recent findings A three-step search strategy was used to identify relevant studies (1 June 2000--1 June 2017) investigating probiotic intervention for diarrhoea secondary to any cancer therapy (cytotoxic, targeted and immunotherapies). RCTs across PubMed, Embase, CINAHL and CENTRAL were assessed for eligibility and assessed using RevMan 5.3 (The Cochrane Collaboration). Seven trials with a total of 1091 patients were included in this meta-analysis. Compared with placebo, prophylactic probiotics did not prevent or reduce the overall incidence of diarrhoea or severe CTCAE Grade at least 3 diarrhoea [relative risk (RR)¼0.81, 95% confidence interval (95% CI)¼0.60--1.09, Z¼1.41, P¼0.16; RR¼0.54, 95% CI¼0.25--1.16, Z¼1.58, P¼0.11], nor did it influence the use of rescue medication (RR¼0.93, 95% CI¼0.53--1.65, Z¼0.24, P¼0.81). 41. De-novo Inflammatory Bowel Disease After Bariatric Surgery: A Large Case Series.

Abstract: Background: Case reports of inflammatory bowel diseases [IBD] have been reported in patients with a history of bariatric surgery. Our aim was to characterize patients who were diagnosed with IBD after having undergone bariatric surgery. Methods: Electronic medical records were reviewed at two institutions to identify patients who developed de-novo Crohn's disease or ulcerative colitis [UC] after bariatric surgery. Data on demographics, type of bariatric surgical procedure, IBD subtype, phenotype and medication usage were obtained. The incidence rate of de-novo IBD after bariatric surgery [per 100 000 personyears] and standardized incidence ratio [SIR] were estimated from a prospective bariatric surgery database. Results: A total of 44 patients with de-novo IBD after bariatric surgery were identified [31 Crohn's disease, 12 UC, one IBD unclassified]. Most patients were female [88.6%], with median age at IBD onset of 44 years [IQR, 37-52] and median time to IBD diagnosis after bariatric surgery of 7 years [IQR, 3-10]. Sixty-eight per cent underwent Roux-en-Y gastric bypass. In the prospective database, the incidence of IBD in patients who underwent bariatric surgery was 26.7 per 100 000 person-years [4.5 for UC and 22.3 for Crohn's disease]. The age-adjusted SIR ranged from 3.56 in the 40-49 year age group to 4.73 in the 30-39 year age group. Conclusion: We described a case series of patients developing de-novo IBD after bariatric surgery. There appears to be a numerically higher incidence of Crohn's disease in this population. 42. Differentiated physical training within the framework of a yearly training cycle of young footballers specialized on the position of goalkeeper.

Abstract: Purpose: The main aim of this study was to find out the increase level of the differentiated special physical training for different age categories of footballers depending on their positions on the field, especially goalkeepers. Material: To achieve this objective we have carried out a complex testing of motor skills specific to the position of goalkeeper in number of 12, divided into the experimental group (n = 6) and the control group (n = 6). In the ascertaining experiment, complied with the requirements of CTYF " Zimbru", it was determined the physical growth (height and weight) and there were applied five motor tests for evaluating the physical training indices such as: sprint 10m, sprint 30m, long jump from standing, alternative speed running on the distance of 180m and resistance running 6 min. Results: Comparing the effects of applying the experimental program through the report of average results to the final testing in the experimental group and the control group, we can mention that the increase of the results of young goalkeepers is within the limits of the marks "good" for the experimental group and "medium" for the control group. Analyzing the results of junior goalkeepers, we see that the final results for testing the sprint 30m are significantly higher compared with the initial results (P<0,01). After processing the results of pedagogical experiment for testing force-speed, both the experimental group and the control group achieved significantly better results compared with the initial ones. Testing the goalkeepers' resistance, we could notice essential improvement of the final results compared with the results of the initial test (P<0,05). The reason is that goalkeepers are those players who carry a heavy workload on the field and it requests an appropriate motor resistance. Conclusions: Following the review of the literature in the field, the teachers' observations during the experiment and relying on survey data, there has been prioritized the development of physical qualities specific to young players on different game positions of goalkeepers, the ways of their manifestations, determining the specific differentiated physical training level in the course of one-year training cycle, which, in their turn, extend and complement the existing layout in the theory and methodology of sports training in football. Abstract: Three different glucagon-like peptide-1 (GLP-1) receptor agonists reduce cardiovascular outcomes in people with type 2 diabetes at high cardiovascular risk with high glycated haemoglobin A1c(HbA1c) concentrations. We assessed the effect of the GLP-1 receptor agonist dulaglutide on major adverse cardiovascular events when added to the existing antihyperglycaemic regimens of individuals with type 2 diabetes with and without previous cardiovascular disease and a wide range of glycaemic control.

46. LINE-1 global DNA methylation, iron homeostasis genes, sex and age in sudden sensorineural hearing loss (SSNHL)

Abstract: Background: Sudden sensorineural hearing loss (SSNHL) is an abrupt loss of hearing, still idiopathic in most of cases. Several mechanisms have been proposed including genetic and epigenetic interrelationships also considering iron homeostasis genes, ferroptosis and cellular stressors such as iron excess and dysfunctional mitochondrial superoxide dismutase activity. Results: We investigated 206 SSNHL patients and 420 healthy controls for the following genetic variants in the iron pathway: SLC40A1− 8CG (ferroportin; FPN1), HAMP− 582AG (hepcidin; HEPC), HFEC282Y and H63D (homeostatic iron regulator), TFP570S (transferrin) and SOD2A16V in the mitochondrial superoxide dismutase-2 gene. Among patients, SLC40A1− 8GG homozygotes were overrepresented (8.25% vs 2.62%; P= 0.0015) as well SOD216VV genotype (32.0% vs 24.3%; P= 0.037) accounting for increased SSNHL risk (OR = 3.34; 1.54–7.29 and OR = 1.47; 1.02–2.12, respectively). Moreover, LINE-1 methylation was inversely related (r2= 0.042; P= 0.001) with hearing loss score assessed as pure tone average (PTA, dB HL), and the trend was maintained after SLC40A1− 8CG and HAMP− 582AG genotype stratification (ΔSLC40A1= + 8.99 dB HL and ΔHAMP= − 6.07 dB HL). In multivariate investigations, principal component analysis (PCA) yielded PC1 (PTA, age, LINE-1, HAMP, SLC40A1) and PC2 (sex, HFEC282Y, SOD2, HAMP) among the five generated PCs, and logistic regression analysis ascribed to PC1 an inverse association with moderate/severe/profound HL (OR = 0.60; 0.42–0.86; P= 0.0006) and with severe/profound HL (OR = 0.52; 0.35–0.76; P= 0.001). Conclusion: Recognizing genetic and epigenetic biomarkers and their mutual interactions in SSNHL is of great value and can help pharmacy science to design by pharmacogenomic data classical or advanced molecules, such as epidrugs, to target new pathways for a better prognosis and treatment of SSNHL.

Abstract: Nonalcoholic fatty liver disease (NAFLD) commonly coexists with Crohn's disease (CD); however, it remains unclear if it is more prevalent than would be expected as ultrasound surveys of CD patients report a very wide range of prevalence (9%–40%).1–3 To address this uncertainty, we performed a prospective, cross-sectional survey of NAFLD in CD patients by generating magnetic resonance proton density fat fraction (MR-PDFF) maps as compared with 2 control populations. 48. Lactobacillus rhamnosusGG protects the intestinal epithelium from radiation injury through release of lipoteichoic acid, macrophage activation and the migration of mesenchymal stem cells

Abstract: ObjectiveLactobacillus rhamnosusGG (LGG), a probiotic, given by gavage is radioprotective of the mouse intestine. LGG-induced radioprotection is toll-like receptor 2 (TLR2) and cyclooxygenase-2 (COX-2)-dependent and is associated with the migration of COX-2+mesenchymal stem cells (MSCs) from the lamina propria of the villus to the lamina propria near the crypt epithelial stem cells. Our goals were to define the mechanism of LGG radioprotection including identification of the TLR2 agonist, and the mechanism of the MSC migration and to determine the safety and efficacy of this approach in models relevant to clinical radiation therapy.DesignIntestinal radioprotection was modelled in vitro with cell lines and enteroids as well as in vivo by assaying clinical outcomes and crypt survival. Fractionated abdominal and single dose radiation were used along with syngeneic CT26 colon tumour grafts to assess tumour radioprotection.ResultsLGG with a mutation in the processing of lipoteichoic acid (LTA), a TLR2 agonist, was not radioprotective, while LTA agonist and native LGG were. An agonist of CXCR4 blocked LGG-induced MSC migration and LGG-induced radioprotection. LGG given by gavage induced expression of CXCL12, a CXCR4 agonist, in pericryptal macrophages and depletion of macrophages by clodronate liposomes blocked LGG-induced MSC migration and radioprotection. LTA effectively protected the normal intestinal crypt, but not tumours in fractionated radiation regimens.ConclusionsLGG acts as a ‘time-release capsule’ releasing radioprotective LTA. LTA then primes the epithelial stem cell niche to protect epithelial stem cells by triggering a multicellular, adaptive immune signalling cascade involving macrophages and PGE2 secreting MSCs.Trial registration numberNCT01790035; Pre-results.
