3. Intra-aortic Balloon Pump Use With Extra Corporeal Membrane Oxygenation—A Mock Circulation Loop Study

- Abstract
Venoarterial extracorporeal membrane oxygenation (ECMO) is used in cardiogenic shock refractory to inotropic support and intra-aortic balloon pump (IABP) support. Peripheral ECMO can lead to ventricular distention, and IABP can be used to mitigate these effects. The aim of this study was to quantify the effects of IABP concomitant with ECMO, under different simulated hemodynamic conditions in a mock circulatory loop. Different simulated states of isolated left ventricular (LV) failure and biventricular failure with graded LV failure severities were supported with ECMO and ECMO with IABP. The impact on left ventricular end-diastolic pressure (LVEDP), volume (LVEDV), coronary flow rate, and cerebral flow rate were evaluated. Left ventricular volumes and pressures increased from the heart failure states with the addition of ECMO. The IABP provided between 3% and 7% reductions in LVEDP and between 1% and 10% reductions in LVEDV. The addition of IABP had minimal effect on cerebral blood flow (0% to 7%), but the variable impact on coronary blood flow with increased diastolic coronary flow of 23% to 50%, but the reduction in mean coronary flow by up to 30%. The efficacy of the IABP was strongly related to ventricular contractility. This study demonstrates the need for careful IABP selection concomitant with ECMO.

5. The Silk Vista Baby – The UK experience

Background The Silk Vista Baby (SVB) flow diverter (FDS) is the only FDS deliverable via a 0.017 inch microcatheter and is specifically designed for the distal vasculature. We sought to evaluate the safety and efficacy of the SVB.Materials and Methods We performed a retrospective review to identify SVB cases at 4 tertiary neurosurgical centres within the U.K. Clinical, procedural, angiographic and follow-up data were collected.Results We identified 60 patients (35 female, 58%) of average age 54 ± 10.5 (range 30–72) with 61 aneurysms, 50 (81.9%) located in the anterior circulation. The majority of the aneurysms treated were unruptured (46, 75.4%) and saccular (46, 75.4%). Dome size was 6.2 ± 6.2 mm (range 1–36mm) and parent vessel diameter was 2.3 ± 0.4 mm (range 1.2-3.3 mm). An average number of 1.07 devices were implanted. Coils or other devices were implanted in 14 aneurysms (23.3%). At last angiographic follow-up (n = 55), 7.5 ± 4.2 months post-procedure, 32 aneurysms (57.1%) were graded as RRC I, 7 (12.5%) RRC II, and 17 RRC III (30.4%). Clinical complications, excluding death, were seen in 4 patients (6.8%) including 1 delayed aneurysm rupture and 3 symptomatic ischaemic events. Only one patient had permanent morbidity (mRS 1). 3 patients died during follow-up (5.1%); 2 deaths were related to the aneurysms (3.4%) – one ruptured dissecting MCA aneurysm, and one giant partially thrombosed posterior circulation aneurysm. 93% of patients were mRS ≤ 2 at last follow-up.Conclusion The SVB has high rates of technical success and an acceptable safety profile. Distal aneurysms may occlude slower due to relative oversizing of the devices.

Supplemental Digital Content is Available in the Text. Background: Pelvic floor dysfunctions (PFD) affect women, men, and children globally. While physical therapy (PT) has been shown to improve PFDs, overall access to PT is limited because of provider shortage and expertise. We hypothesize telerehabilitation (TR) is a novel approach to address access and expertise care for PFD. Study Design: Case series. Case Descriptions: Three pelvic health patients (1 male and 2 females) volunteered to receive initial and follow-up care via TR. The male patient was treated for premature ejaculation, and the female patients were seen for postpartum recovery, stress urinary incontinence, and diastasis recti. A total of 9 visits were completed. Outcomes: Using a HIPAA-compliant, cloud-based, synchronous mobile app, a 14-question survey, and the Premature Ejaculation Diagnostic Tool (for the male health participant), participants rated their experience with TR. The 3 participants rated the TR system as high quality (2/3) and very high quality (1/3). Using a mobile phone was the preferred digital device (2/3) compared with laptop use. All 3 participants felt their needs were met and that they received good care during the session. Summary: Telerehabilitation has the potential to deliver quality care with improved access for patients with PFD for both initial and follow-up visits. - Abstract
Background and Purpose: Stroke, predominantly a condition of older age, is a major cause of acquired disability in the global population and puts an increasing burden on health care resources. Clear evidence for the importance of intensity of therapy in optimizing functional outcomes is found in animal models, supported by neuroimaging and behavioral research, and strengthened by recent metaanalyses from multiple clinical trials. However, providing intensive therapy using conventional treatment paradigms is expensive and sometimes not feasible because of social and environmental factors. This article addresses the need for cost-effective increased intensity of practice and suggests potential benefits of telehealth (TH) as an innovative model of care in physical therapy. Summary of Key Points:We provide an overview of TH and present evidence that a web-supported program, used in conjunction with constraint-induced therapy (CIT), can increase intensity and adherence to a rehabilitation regimen. The design and feasibility testing of this web-based program, "LifeCIT," is presented. We describe how wearable sensors can monitor activity and provide feedback to patients and therapists. The methodology for the development of a wearable device with embedded inertial and mechanomyographic sensors, algorithms to classify functional movement, and a graphical user interface to presentmeaningful data to patients to support a home exercise program is explained. Recommendations for Clinical Practice:We propose that wearable sensor technologies and TH programs have the potential to provide most-effective, intensive, home-based stroke rehabilitation. AbstractThe increasing demands on renewable energy nowadays caused the development of marine current turbine industry. In order to improve the current design of marine current turbines, studies were conducted to analyse their hydrodynamic performances during operation. Since most of the time marine current turbines operate in arrays, it is important to understand the interactions between the turbines in order to design the optimum turbine farm. OpenFOAM®was used to simulate the turbine interactions of conventional and biomimetic marine current turbines in tandem configuration. The conventional marine current turbines were referred to Pinon et al. (2012) and Mycek et al. (2013) while the biomimetic marine current turbine was adopted from Chu (2016). The numerical simulations were conducted with turbines in different inter-device distances, A/D. The percentage differences of ‘‘efficiency’’, ηbetween the IFREMER-LOMC and the biomimetic turbine case of inter-device distances, A/D= 4, 6, 8 and 10 are 14.3%, 6.4%, 3% and 1.92% respectively. The results show that the power produced by the biomimetic turbines in tandem is comparable with the IFREMER-LOMC turbines when A/D> 4. The biomimetic marine current turbines can be a fair choice due to their potential to have alternative fabrication method of their sheet-like turbine blades.

16. Comparison of three methods of penile vibratory stimulation for semen retrieval in men with spinal cord injury

Study design:Prospective, three-way crossover design.Objective:Penile vibratory stimulation (PVS) is recommended as the first line of treatment for semen retrieval in anejaculatory men with spinal cord injury (SCI). This study compared ejaculatory success rates and patient preference for three methods of PVS within the same group of men with SCI.Setting:Major medical university.Methods:Fifteen men with SCI each received three methods of PVS. Method 1 (M1): applying one FertiCare Personal device to the dorsum or frenulum of the glans penis; Method 2 (M2): ‘sandwiching’ the glans penis between two FertiCare devices; Method 3 (M3): sandwiching the glans penis between the two vibrating surfaces of the Viberect-X3 device. The order of M1, M2 and M3 was varied to control for sequencing effects. Following each PVS trial, subjects rated their experience on a questionnaire with scaled responses.Results:Ejaculation success rates were high for each method; however, ejaculation latency was significantly longer with M3 compared with M1 or M2. In survey questions about patient preference, there were no significant differences between M1 and M2. In contrast, M3 was rated lower than M1 and M2 in patient preference. Semen collection may be more difficult with the Viberect device.Conclusions:On the basis of these findings, we recommend attempting PVS with one FertiCare device. If that fails, use two FertiCare devices. Although the Viberect-X3 was preferred less by patients, it had similar efficacy as the Ferticare vibrator(s) and may be suitable for home use by some patients.

- Abstract
Identification of material properties is one of the key issues in composite materials research. The mechanical properties of composite materials depend on diverse factors such as configuration of the laminates, constituent materials used and production method adopted. Conventional testing approach tends to be time-consuming, expensive and destructive. As an alternative, a rapid, inexpensive, hybrid and non-destructive evaluation approach which utilises experimental modal analysis and finite element analysis is proposed. Experimental modal data which consist of natural frequencies and mode shapes of an orthotropic composite plate are utilised for correlation purpose with its finite element model. This finite element model of the composite plate is continuously updated and achieves less than 5% in difference of natural frequencies and over 70% in modal assurance criterion. Material properties such as Young's moduli, in-plane shear modulus and Poisson ratio of the composite plate are then successfully determined using the well-correlated FE model. [ABSTRACT FROM AUTHOR]

- Abstract
Failure of the duplex stainless steel piping system in oil and gas industry can have disastrous effects. In this study, a novel method of failure analysis of flow-induced vibration problem of in-serviced duplex stainless steel piping system is proposed. The proposed non-destructive technique is able to determine a suitable operating condition for continuous operation without failure. The technique relies on the combined operation of operational modal analysis, operating deflection shape analysis and linear elastic finite element analysis. The effect of different operating conditions for two distinct valve opening cases (i.e. fully opened and partially opened) on the dynamic stress is examined, and they are utilised for forecasting purpose in failure analysis. The result shows that maximum operating conditions are 360 and 400 mmscfd for fully opened and partially opened flow control valves, respectively. Business-To-Business electronic marketplace (B2B e-Marketplace), an electronic platform for buyers and sellers, provides a new dimension in facilitating the marketers to work more effectively when making critical marketing decisions. However, small to medium sized enterprises (SMEs) who are keen to compete in the electronic environment remain concerned as how their businesses can benefit from B2B e-Marketplace. We therefore developed a conceptual framework in which multiple facet of e-Marketing services derived from B2B e-Marketplace are linked to e-Business performance. We will use an adjusted NK simulation to test how Demographic Characteristics, Perceived Risk and Online Data Security influence the relationships between B2B e-Marketplace adoption and e-Business performance. The proposed framework will provide a guideline for academics and practitioners and highlights the significant role of each factor in developing and sustaining effective B2B e-Commerce practice for SMEs. Furthermore, SME managers also can derive a better understanding and measurement of marketing activities that appropriately balance between traditional and B2B e-Commerce practice. - Abstract
Common birthmarks can be an indicator of underlying genetic disease but are often overlooked. Mongolian blue spots (dermal melanocytosis) are usually localized and transient, but they can be extensive, permanent, and associated with extracutaneous abnormalities. Co-occurrence with vascular birthmarks defines a subtype of phakomatosis pigmentovascularis, a group of syndromes associated with neurovascular, ophthalmological, overgrowth, and malignant complications. Here, we discover that extensive dermal melanocytosis and phakomatosis pigmentovascularis are associated with activating mutations in GNA11and GNAQ, genes that encode Gα subunits of heterotrimeric G proteins. The mutations were detected at very low levels in affected tissues but were undetectable in the blood, indicating that these conditions are postzygotic mosaic disorders. In vitro expression of mutant GNA11R183Cand GNA11Q209Lin human cell lines demonstrated activation of the downstream p38 MAPK signaling pathway and the p38, JNK, and ERK pathways, respectively. Transgenic mosaic zebrafish models expressing mutant GNA11R183Cunder promoter mitfadeveloped extensive dermal melanocytosis recapitulating the human phenotype. Phakomatosis pigmentovascularis and extensive dermal melanocytosis are therefore diagnoses in the group of mosaic heterotrimeric G-protein disorders, joining McCune-Albright and Sturge-Weber syndromes. These findings will allow accurate clinical and molecular diagnosis of this subset of common birthmarks, thereby identifying infants at risk for serious complications, and provide novel therapeutic opportunities.

24. Z-scan studies of the nonlinear optical properties of gold nanoparticles prepared by electron beam deposition

This paper details the fabrication process for placing single-layer gold (Au) nanoparticles on a planar substrate, and investigation of the resulting optical properties that can be exploited for nonlinear optics applications. Preparation of Au nanoparticles on the substrate involved electron beam deposition and subsequent thermal dewetting. The obtained thin films of Au had a variation in thicknesses related to the controllable deposition time during the electron beam deposition process. These samples were then subjected to thermal annealing at 600°C to produce a randomly distributed layer of Au nanoparticles. Observation from field-effect scanning electron microscope (FESEM) images indicated the size of Au nanoparticles ranges from ∼13 to ∼48 nm. Details of the optical properties related to peak absorption of localized surface plasmon resonance (LSPR) of the nanoparticle were revealed by use of UV–Vis spectroscopy. The Z-scan technique was used to measure the nonlinear effects on the fabricated Au nanoparticle layers where it strongly relates LSPR and nonlinear optical properties.

- Abstract
Deliberate inflicted trauma to children has irrefutably occurred for centuries but the notion of non-accidental injury (NAI) as a distinct clinical entity has only relatively recently been described. Awareness and understanding of this syndrome of abuse has increased greatly in recent decades, although many alternative names have been given to the condition and uncertainty remains as to what exactly comprises the syndrome. What is not in question, however, is the role of imaging; the radiologist is often at the front line in terms of raising the spectre of NAI and in assessing the probability given the objective imaging features available. Non-accidental head injury (NAHI) encompasses a broad spectrum of manifestations, ranging from trivial superficial injuries to potentially fatal severe brain trauma. In this review, we aim to introduce the epidemiological, historical and legal aspects of NAI. Focussing specifically on NAHI, current biomechanical theories and neuropathological aspects will be discussed. 26. Selective Inhibition of SIRT2 Improves Outcomes in a Lethal Septic Model

- Abstract
Background: Seven isoforms of histone deacetylase Class III have been reported - Sirtuin (SIRT) 1-7. We recently demonstrated that EX-527, an inhibitor of SIRT1, reduces mortality in a mouse model of lethal-cecal-ligationand- puncture (CLP)-induced septic shock. Our present study was aimed at determining whether selective inhibition of SIRT2, with AGK2, would decrease animal death and attenuate the inflammatory response in a septic model. Methods: Experiment I: C57BL/6J mice were intraperitoneally given either AGK2 (82 mg/kg) in dimethyl sulfoxide (DMSO) or DMSO alone, and 2 h later subjected to CLP. Survival was monitored for 240 hours. Experiment II: mice treated the same way as Experiment I, were grouped into (i) DMSO vehicle, and (ii) AGK2, with sham mice (operating but without any treatment) serving as controls. Peritoneal fluid and peripheral blood were examined at 24 and 48 hours for cytokine production. Samples of blood at 48 h were also allocated to assess coagulability using Thrombelastography (TEG). Morphological changes of bone marrow were evaluated from long bones (femurs and tibias) with hematoxylin and eosin (H&E) staining. Bone marrow atrophy was quantified by a blinded pathologist. Experiment III: cytokines in supernatant of the cultured normal primary splenocytes were measured after the cells were stimulated by lipopolysaccharide and treated with or without AGK2 (10 µM) for 6 hours. Results: AGK2 significantly reduced mortality and decreased levels of cytokines in blood (TNF-?: 298.3±24.6 vs 26.8±2.8 pg/ml, p=0.0034; IL-6: 633.4±82.8 vs 232.6±133.0 pg/ml, p=0.0344) and peritoneal fluid (IL-6: 704.8±67.7 vs 391.4±98.5 pg/ml, p=0.033) compared to vehicle control. Also, AGK2 suppressed the TNF-? and IL-6 production in the cultured splenocytes (TNF-a: 68.1±6.4 vs 23.9±2.8 pg/ml, p=0.0009; IL-6: 73.1±4.2 vs 49.6±3.0 pg/ml; p=0.0051). The TEG data showed that the mice subjected to CLP displayed prolonged fibrin formation and fibrin cross-linkage time, slower clot formation, decreased platelet function, and clot rigidity. AGK2 treatment was associated with dramatic improvements in fibrin cross-linkage and clot formation times, without a significant impact on the clot initiation parameters or platelet function. Additionally, AGK2 significantly attenuated the bone marrow atrophy (58.3±6.5 vs 30.0±8.2%, p=0.0262). Conclusion: Selective inhibition of SIRT2 significantly improves survival, and attenuates sepsis-associated “cytokine storm”, coagulopathy, and bone marrow atrophy in a mouse model of lethal septic shock.

- Abstract
This paper proposes an approach based on an optical imaging technique for the period measurement of fiber Bragg gratings (FBG). The simple, direct technique involves a differential interface contrast (DIC) microscope and a high-resolution CCD camera. Image processing is performed on the microscope images to obtain low-noise grating profiles and then the grating periods. Adopting a large image sample size in the image processing can reduce uncertainty. During the investigation, FBGs of different grating periods are fabricated by prestraining the photosensitive fibers during the UV-writing process. A good linearity between the measured Bragg wavelengths and grating periods is observed and the measured strain—optics coefficient was found to be in agreement with reported literature.

AbstractThis paper presents a high-conversion gain, low-power, folded CMOS mixer for ZigBee application in 2.4 GHz of user bandwidth. The proposed mixer adapts current reuse technique to increase the conversion gain while substantially reducing the DC power dissipation. The current from LO stage is reused at the transconductance stage to reduce the power consumption. This mixer is verified in 0.13 μm standard CMOS technology. The simulation result exhibits a high-conversion gain performance (CG) of 10 dB, 1 dB compression point (P1 dB) of -13.43 dBm, third-order intercept point (IIP3) of -4.3 dBm, and a noise figure of 16.67 dB. The circuit draws 675 μA current from the 1.2 V of supply voltage headroom.

34. Norovirus: a challenging pathogen

- Abstract
Noroviruses account for over 90% of all viral gastroenteritis cases and ~50% of all outbreaks worldwide. Each year in Australia, there are an estimated 1.8million cases. Cases may be sporadic or part of outbreaks, occurring in either the community or healthcare setting. Outbreaks are associated with significant morbidity and some mortality. They incur substantial costs and can be difficult to control in healthcare institutions or other closed settings. Multiple factors (related to virus biological properties, human immune responses or inadequate management modalities) make it a challenging pathogen to control. They include: multiple transmission routes, low infectious dose, environmental survival, spread and persistence, diagnostic difficulty, hand hygiene controversies, imperfect immunity and immune evasion, asymptomatic and prolonged shedding, lack of vaccine and lack of antiviral treatment. The purpose of this article is to promote a better understanding of these factors in order that health professionals may be better equipped to manage the problems posed by noroviruses. Until large-scale effective vaccination and specific treatments become available, the safeguarding of food and water supplies and the rigorous and timely application of outbreak management and infection control measures will remain the key to norovirus disease prevention and control.

35. Long-term survival outcome following Staphylococcus aureusbacteraemia

- Abstract
Aims To describe long-term survival (beyond 200 days) following Staphylococcus aureus bacteraemia (SAB) and to determine if certain patient subgroups had poorer long-term survival outcomes. Methods A single-centre, retrospective, cohort study of all SAB cases at The Canberra Hospital, a tertiary referral centre, from January 1998 to December 2007 was performed. Clinical and demographic data were obtained from a pre-existing prospectively collected database. Patients were followed-up for a minimum of 9 months. Subsets within the cohort were analysed for differences in long-term survival. The main outcome measure was death from any cause. Results During the 3889-day study observation period, 439 patients had SAB and were followed for a total of 546360 person-days. The overall median survival was 3169 days. The mortality rates were 9.6%, 17%, 24%, 29% and 32% at 7, 30, 90, 180 and 365 days respectively. A total of 188 (43%) patients died. Of the 188 deaths, 22%, 40%, 55%, 67% and 75% occurred within the first 7, 30, 90, 180 and 365 days respectively after their SAB episode. Initial analysis showed poorer long-term survival in those patients with older age, MRSA, with an unknown focus of infection, and who were not admitted under the Infectious Diseases team. However, on multivariate analysis, the only independent risk factors for poorer survival were older age, unknown focus of infection and not being admitted under the Infectious Diseases team. MRSA, sex, surgical vs non-surgical admitting team and an association with an intravascular device were not associated with poorer long-term survival. Conclusions High rates of death continue for many months after patients have an episode of SAB. Short-term follow-up studies (30 days or less) may miss large numbers of SAB-associated deaths. If accurate data on SAB-associated mortality is needed, then follow-up of these patients will be needed for at least 90 days, ideally performed prospectively with a matched control group consisting of hospitalised patients without SAB.

38. Developmental Venous Anomalies (DVA): What Are They Really?

Purpose: The aim of this paper is to analyse the pathophysiology of 3 DVA cases from our institution, review the literature and propose a classification of these lesions. Materials & Methods: The pathophysiology of DVAs were analysed with CT perfusion (CTP), 4 dimensional dynamic computed tomography angiography (4D CTA) and catheter digital subtraction angiography. Results: Symptomatic DVAs may be caused by associated lesions and compression of neural structures by the DVAs. The imbalance between the inflow and outflow of these lesions, including venous ischaemia, has also been postulated as a cause. Our analysis showed that increased cerebral blood flow (CBF) and cerebral blood volume (CBV) and decreased mean transit time (MTT) and time to peak (TTP) were found in DVAs with micro arteriovenous shunting. DVAs without shunting had raised MTT and TTP instead. Conclusion: We postulate that the arteriovenous shunting resulted in arterial steal and chronic hypoxia which could be a pathophysiological mechanism for symptomatic DVAs. CTP and 4D CTA are effective non invasive tools to study DVAs. A classification is proposed.

- Abstract
Thiazolidinediones (TZD) are peroxisome proliferator-activated receptor γ (PPARγ) agonists that may reduce hepatic steatosis through their effects in adipose tissue and therefore have been assessed as potential therapies to treat nonalcoholic fatty liver disease (NAFLD) in humans. However, some studies suggest that expression and activation of hepatocyte PPARγ promotes steatosis and that would limit the benefits of TZD as a NAFLD therapy. To further explore this possibility, we examined the impact of short-term rosiglitazone maleate treatment after the development of moderate or severe diet-induced obesity, in both control and adult-onset hepatocyte-specific PPARγ knockout (PpargΔHep) mice. Independent of the level of obesity and hepatic PPARγ expression, the TZD treatment enhanced insulin sensitivity, associated with an increase in white adipose tissue (WAT) fat accumulation, consistent with clinical observations. However, TZD treatment increased hepatic triglyceride content only in control mice with severe obesity. Under these conditions, PpargΔHepreduced diet-induced steatosis and prevented the steatogenic effects of short-term TZD treatment. In these mice, subcutaneous WAT was enlarged and associated with increased levels of adiponectin, while hepatic levels of phosphorylated adenosine 5′-monophosphate–activated protein kinase were also increased. In addition, in mice with severe obesity, the expression of hepatic Cd36, Cidea, Cidec, Fabp4, Fasn, and Scd-1was increased by TZD in a PPARγ-dependent manner. Taken together, these results demonstrate that hepatocyte PPARγ expression offsets the antisteatogenic actions of TZD in mice with severe obesity. Therefore, in obese and insulin resistant humans, TZD-mediated activation of hepatocyte PPARγ may limit the therapeutic potential of TZD to treat NAFLD.

40. Management of Posterior Fossa Dissecting Aneurysms

- Abstract
Treatment and prognosis of 14 patients of posterior fossa arterial dissections (AD) and dissecting aneurysms (DA) in one institution was reviewed. Internal trapping of aneurysm was performed for six patients presenting with SAH (three Vertebral, one posterior cerebral, one posterior inferior cerebellar, one anterior inferior cerebellar DA). The patency of the parent arteries was preserved in four DA patients with SAH (two Vertebral, two Basilar DA), 1 incidental vertebral DA, and one DA patient with brainstem infarction using stents and coils (four patients) or coils only (two patient). Proximal occlusion of parent artery was performed in a vertebral DA with SAH. One patient with a superior cerebellar DA presented with a midbrain infarct developed SAH with spontaneous occlusion of the aneurysm two weeks later. Of the 14 cases, ten were angiographically stable or cured during a follow up period of four to 70 months. one spontaneously resolved and two recurred. There was one death.

Objective: The association between iodine status and the prevalence of goiter and thyroid nodules has been well established but the extent to which different iodine intake levels influence the incidence of goiter and thyroid nodules is unclear. The aim of the study was to determine the incidence of goiter and thyroid nodules in 3 regions with different iodine intake levels: mildly deficient, more than adequate, and excessive. Design, patients and measurements: Of the 3385 un-selected subjects enrolled in 1999 in Panshan, Zhangwu, and Huanghua where median urinary iodine excretion (UIE) was 83.5 μg/l, 242.9 μg/l, and 650.9 μg/l, respectively, 2708 (80.0%) participated in the follow-up study in 2004. The examinations of thyroid ultrasonography, thyroid function, thyroid autoantibodies and UIE were performed at baseline and follow-up. Results: The cumulative incidence of diffuse goiter was 7.1 %, 4.4%, and 6.9%, respectively, higher in Panshan and Huanghua than in Zhangwu (p=0.013 and p=0.015) and that of nodular goiter was 5.0%, 2.4%, and 0.8%, respectively, declining with increasing iodine intake levels (p<0.001). Mild iodine deficiency, chronic iodine excess as well as positive thyroid autoantibodies were associated with the occurrence of goiter [Logistic regression: odds ratio (OR)=1.83 (95% confidence interval (CI) 1.26–2.65), OR=1.46 (95% CI 1.01–2.11 ) and OR=1.68 (95% CI 1.14–2.48), respectively]. The cumulative incidence of single nodule was 4.0%, 5.7%, and 5.6%, respectively and that of multiple nodules was 0.4%, 1.2%, and 1.0%, respectively. Conclusions: The relationship between iodine and the risk for the occurrence of diffuse goiter shows a U-shaped curve. Nodular goiters are more prevalent in iodine-deficient areas.

Human cathepsin K, a cysteine proteinase of the papain family, has been recognized as a potential drug target for the treatment of osteoporosis. The predominant expression of cathepsin K in osteoclasts has rendered the enzyme into a major target for the development of novel antiresorptive drugs. Now, we report the pharmacological properties of OST-4077 [furan-2-carboxylic acid (1-{1-[4-fluoro-2-(2-oxo-pyrrolidin-1-yl)-phenyl]-3-oxo-piperidin-4-ylcarbamoyl}-cyclohexyl)-amide] as a novel selective cathepsin K inhibitor. Human and rat cathepsin K were inhibited in vitro by OST-4077 with the IC(50) values of 11 and 427 nM, respectively. OST-4077 suppressed bone resorption induced by rabbit osteoclasts (IC(50), 37 nM) but did not affect bone mineralization or cellular alkaline phosphatase activity in MC3T3-E1 cells. Parathyroid hormone-induced bone resorption was inhibited in a dose-dependent manner in thyroparathyroidectomized rats gavaged with a single dose of OST-4077 (ED(50), 69 mg/kg). When given orally twice daily for 4 weeks to 3-month-old ovariectomized (OVX) rats, OST-4077 dose-dependently prevented bone loss, as monitored by bone densitometry, ash content, and urinary excretion of deoxypyridinoline. No change in serum osteocalcin in the OVX rats by OST-4077 suggested that bone formation might not be affected by the agent. In summary, OST-4077 selectively inhibited bone resorbing activities of osteoclasts and prevented bone loss induced by estrogen deficiency but did not affect bone formation. OST-4077, an orally active selective human cathepsin K inhibitor, may have the therapeutic potential for the treatment of diseases characterized by excessive bone loss including osteoporosis.

Although the prevalence of seizures in children with sickle cell disease (SCD) is 10 times that of the general population, there are few prospectively collected data on mechanism. With transcranial Doppler and magnetic resonance imaging (MRI) and angiography, we evaluated 76 patients with sickle cell disease, 29 asymptomatic and 47 with neurological complications (seizures, stroke, transient ischemic attack, learning difficulty, headaches, or abnormal transcranial Doppler), who also underwent bolus‐tracking perfusion MRI. The six patients with recent seizures also had electroencephalography. Group comparisons (seizure, nonseizure, and asymptomatic) indicated that abnormal transcranial Doppler was more common in the seizure (4/6; 67%) and nonseizure (26/41; 63%) groups than in the asymptomatic (10/29; 34%) group (χ2; p= 0.045), but abnormal structural MRI (χ2; p= 0.7) or magnetic resonance angiography (χ2; p= 0.2) were not. Relativedecreased cerebral perfusion was found in all seizure patients and in 16 of 32 of the remaining patients with successful perfusion MRI (p= 0.03). In the seizure patients, the perfusion abnormalities in five were ipsilateral to electroencephalographic abnormalities; one had normal electroencephalogram results. These findings suggest that vasculopathy and focal hypoperfusion may be factors in the development of sickle cell disease–associated seizures. Ann Neurol 2005;58:290–302

44. Brain morphometry and IQ measurements in preterm children

Although IQ is thought to remain relatively stable in the normal population, a decline in IQ has been noted in children born preterm. It is not clear, however, to what extent the inclusion of children with clear neurological damage has influenced these findings. We examined IQ scores obtained in childhood and then again in adolescence from a group of children born at 30 weeks gestation or less who had been classified as neurologically normal at 7.5–8 years. They showed a significant decline in mean IQ scores over time. MRI scans obtained from a subset of children at adolescence were read as normal in ∼50% of cases and, in the others, there were no consistent relationships between radiological abnormalities and IQ results. Such children can, however, have relatively subtle brain abnormalities that are not seen on conventional MRI, and we hypothesized that these would be related to declines in IQ. Voxel-based morphometry (VBM) analyses of the MRI scans revealed that absolute IQ scores were related to areas in both the parietal and temporal lobes. The analyses also showed that frontal and temporal lobe regions were associated with the decline in VIQ, while occipital and temporal lobe regions (including the hippocampi) were associated with the decline in PIQ. Hippocampal volume measurements were consistent with the VBM findings. We concluded that preterm children are at risk of declining IQ over time even if they have not suffered obvious neurological damage and that the decline is associated with specific neural regions. Whether this is true of children born at >30 weeks gestation and what other factors predispose to this decline have yet to be determined.

45. The synthesis and characterization of polymer coated iron oxide microspheres

- Abstract
For biomedical applications drug carrying polymers are coated around magnetic iron oxide particles to form microspheres. In the present study, the iron oxide powder was ball milled. Microspheres were then synthesized by solvent evaporation, resulting in iron oxide particles encapsulated in a polymer and drug coating. Various parameters, such as the duration of milling and agitation speed as well as the polymer concentration were varied. A milling time of 72 h was found to yield a small size and narrow size distribution of particles; the average particle size was about 600 nm. Measurements of the change in grain size and the magnetic properties of the powder with milling time were performed. It was determined that the size of the microspheres was not sensitive to the initial particle size, but it could be decreased by variation of agitation speed or polymer concentration. The agitation speed and polymer concentration of 400 rpm and 0.04 g poly(l-lactic acid) in 8 g dicholoromethane, respectively, was found to yield small, spherical microspheres with a narrow size distribution. The surface morphology and magnetic properties of the microspheres was also analyzed.

47. Clinical outcomes of hemispherectomy for epilepsy in childhood and adolescence.

- Abstract
Hemispherectomy has been performed in the treatment of epilepsy in association with hemiplegia for over 50 years. However, the optimal timing of surgery with respect to age at presentation and the influence of underlying pathology on outcome is only slowly emerging. This study reports on the clinical course and outcomes of 33 children who underwent hemispherectomy at Great Ormond Street Hospital, London, between 1991 and 1997. Age at surgery was 0.33-17 years (median 4.25) with 1-8 years follow-up (median 3.4). The underlying pathology was developmental in 16 (10 hemimegalencephaly, two polymicrogyria, two focal cortical dysplasia, one diffuse cortical dysplasia and one microdysgenesis), acquired in 11 (six middle cerebral artery infarct, three post encephalitis/trauma, and one each of hemiconvulsion-hemiplegia epilepsy and perinatal ischaemic insult) and progressive in six children (four Rasmussen encephalitis, two Sturge-Weber syndrome). At follow-up, 52% were seizure free, 9% experienced rare seizures, 30% showed >75% reduction in seizures and 9% showed <75% seizure reduction or no improvement. Seizure freedom was highest in those with acquired pathology (82%), followed by those with progressive pathology (50%) and those with developmental pathology (31%). However, seizure freedom, rare seizures or >75% reduction in seizures occurred in 100% of those with progressive pathology, 91% of those with acquired and 88% of those with developmental pathology, indicating a worthwhile seizure outcome in all groups. Hemiplegia remained unchanged following surgery in 22 out of 33 children, improved in five and was worse in six. No significant cognitive deterioration or loss of language occurred, and four children showed significant cognitive improvement. Behavioural improvement was reported in 92% of those who had behaviour problems pre-operatively.

The nature of the relationships between status epilepticus, acute hippocampal injury, mesial temporal sclerosis (MTS) and temporal lobe epilepsy remains unclear. The aim of this study was to investigate whether generalized status epilepticus is associated with brain abnormalities, especially in the mesial temporal lobe, within 5 days of the acute event. Such changes may be the first part of a causative pathophysiological sequence relating status epilepticus and MTS. Thirty‐five children with a history of status epilepticus, including 21 with a history of prolonged febrile convulsion (PFC), underwent qualitative and quantitative MRI investigations within 5 days of the acute episode. Quantitative assessments of the hippocampus included T2 relaxometry and hippocampal volumetry. Hippocampal volumes were large in patients with PFC when compared with controls. In addition, T2 relaxation time was elevated in patients with PFC compared with control subjects during the first 2 days of the acute event. No difference was observed in patients examined 3–5 days after the event. Patients with afebrile status epilepticus had a variety of imaging abnormalities including elevated hippocampal T2 values, but no evidence of hippocampal enlargement. PFC is associated with hippocampal abnormalities, consistent with hippocampal oedema, whilst non‐febrile status epilepticus is not. A systematic longitudinal study is required to characterize the evolution of these abnormalities and to determine whether any patient develops MTS.
