Le lébrikizumab (LEB) est un nouvel anticorps monoclonal pour la dermatite atopique (DA) modérée à sévère de haute affinité pour l’interleukine (IL)-13. L’efficacité du traitement de la DA est évaluée par l’amélioration des signes, des symptômes et de la qualité de vie (QoL), tel que recommandé par le comité HOME (Harmonizing Outcome Measures in Eczema). Une réponse cliniquement significative du LEB chez les patients adultes traités en monothérapie après 16 semaines (S) a déjà été rapportée. Ici, nous décrivons le maintien de cette réponse après 52S chez des patients déjà répondeurs à S16 lors des essais ADvocate1 (NCT04146363) et ADvocate2 (NCT04178967) (données groupées).

Abstract: We present a 15-year-old patient diagnosed with peripheral neurofibromatosis (NF-1), who was admitted with paraparesis caused by a large intrathoracic tumor with an intracanalicular component that affected the spinal cord. After surgery his condition improved, but a year later he suffered a relapse and died. Histologically the tumor was diagnosed as malignant with neurogenic and myogenic differentiation (”malignant triton tumor”). Malignant triton tumors (MTT) are infrequent; those found in the head and neck and the upper or lower extremities have a better prognosis than those in the retroperitoneum, buttock, or trunk. It is not clear whether this variation is due to a difference in tumor grade, stage, or resectability, or whether it is a consequence of therapy.

The fungal micro-organism Trichoderma reesei was grown in batch culture with excess glucose at pH values between 2.7 and 4.5 and temperatures between 25°C and 35°C. A maximum lipid concentration of 16.9% of the cell dry weight was achieved at pH 3.2 and a temperature of 27°C. Lipid concentration was shown to be correlated with a calculated maximum specific growth rate (µmc) and the maximum lipid value occurred at µmc = 0.10 h-1. Fatty acid analysis was carried out and found to change with changing pH and temperature. Palmitic (16:0) acid and unusually high proportions of stearic acid (18:0) were commonly present. A conversion of fatty acids to palmitoleic acid (16:1) occurred following an unidentified nutrient limitation other than nitrogen depletion after 70 h of culture.

The intestine has a high glycolytic activity, but its metabolic role could be altered in diabetes mellitus. The aim of the present work was to investigate in vivo the glucose retained and the lactate produced by the intestine of normal and diabetic rats and in vitro the effect of different arterial glucose concentrations on glucose utilization and lactate, alanine, and pyruvate production in normal and diabetic rats when the glucose is supplied to the intestine exclusively via the vascular route. In vivo, the normal and diabetic rats retained similar percentages of the arterially supplied glucose (14.7±3.2 and 12.6±2.4, respectively). In vitro, when the preparations were perfused under hyperglycemic conditions, the glucose consumed, as a fraction of the quantity infused, was significantly lower (P<0.05) in the diabetic (247.0±22.8 µmol/mmol infused glucose) than in normal (315.0±16.3 µmol/mmol infused glucose) rats. The lactate produced was significantly higher in diabetic than in normal rats whether the preparations were perfused under isoglycemic (P<0.01; 1916.4±124.0 vs 1284±67.7 µmol/mmol consumed glucose) or hyperglycemic (P<0.05; 1356.4±199.7 vs 898.0±87.3 µmol/mmol consumed glucose) conditions. There was significantly (P<0.05) greater alanine release from the diabetic (123.7±21.8 µmol/mmol consumed glucose) than from the normal (40.7±10.3 µmol/mmol consumed glucose) rat preparations perfused under isoglycemic conditions.

L’objectif de cette analyse en sous-groupe était d’évaluer l’efficacité de l’ixékizumab par rapport au placebo, aux semaines 12 et 60, chez des patients atteints de psoriasis en plaques modéré à sévère exposés ou non à un traitement biologique préalable.
